      • VIP를 분비하며 신경절신경종의 분화를 가지는 갈색 세포종 1예

        갈색 세포종은 신경내분비종양의 하나로 VIP를 포함한 다양한 신경내분비 펩타이드를 분비할 수 있다. 저자들은 만성 설사와 체중 감소를 주소로 내원한 환자에서 좌측 부신에 VIP를 생성하는 갈색 세포종을 진단하여 성공적으로 제거한 후 증상이 모두 호전된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Vasoactive intestinal peptide (VIP) is a 28-amino acid peptide, which may cause secretory diarrhea by stimulating the production of adenylate cyclase. Neuroendocrine tumors, secreting vasoactive intestinal peptide (VIP), are almost always of a pancreatic in origin. However, a pheochromocytoma may produce several neuropeptides, containing VIP, as they are considered to be neuroendocrine tumors. A 57-year-old woman, who presented with chronic watery diarrhea, hypokalemia, weight loss and a left adrenal mass, is described. Histologically, the tumor was diagnosed as a pheoch-romocytoma, with ganglioneuronal differentiation, and was histochemically confirmed to produce a vasoactive intestinal polypeptide. A left adrenal VIP-producing pheochromocytoma was successfully resected. After surgery, her diarrhea subsided and the electrolytes, affected neuroendocrine hormone levels, blood pressure and blood sugar level were normalized (J Kor SOC Endocrinol 18:227-231, 2003).

        Review : Protein Delivery System based on Various Polysaccharides

        Delivery of therapeutic protein drugs is a hot issue in the clinical application, because protein drugs have low side effects and highly therapeutic effects compared with chemical drugs. Despite their prominent advantages, protein drugs have high risk for human therapy such as their easy degradation by proteolytic enzymes, renal filtration and immune response. Over the past few decades, a large number of polysaccharides as vehicles for the protein delivery system have been developed to overcome the problems. This review presents the studies on protein delivery based on polysaccharides used as stabilizer and vehicles comprising nano-or microspheres to overcome inherent limitations of therapeutic proteins.

        연구논문 : 한반도 주요 산정의 식물종 분포와 기후변화 취약종

        This work aims to select the potentially vulnerable plant species against climate change at alpine and subalpine belts of Mts. Sorak, Jiri, and Halla, from central, southern, southern insular high mountains of the Korean Peninsula, respectively. The selection of global warming related vulnerable plants were performed by adapting various criteria, such as flora, endemicity, rarity, floristically specific and valuable species, species composition at mountain summits, horizontal and vertical ranges of individual species, and their distributional pattern in the Korean Peninsula. Line and quadrat field surveys along the major trails from all directions at height above 1,500 meters above sea level of Mts, Sorak, Jiri and Halla were conducted each year during spring, summer, and autumn from 2010 to 2011. Based upon above mentioned eight criteria, high level of climate change related potentially vulnerable arboral plants, such as Rhododendron aureum, Taxus caespitosa, Pinus pumila, Oplopanax elatus, Vaccinium uliginosum, and Thuja koraiensis are noticed from at subalpine belt of Mt. Sorak. Species of Abies koreana, Rhododendron tschonoskii, Oplopanax elatus, Taxus cuspidata, Picea jezoensis, and Juniperus chinensis var. sargentii belong to climate change concerned vulnerable species at subalpine belt of Mt. Jiri. High level of climate change related species vulnerability is found at alpine and subalpine belts of Mt. Halla from Diapensia lapponica var. obovata, Salix blinii, Empetrum nigrum var. japonicum, Vaccinium uliginosum, Juniperus chinensis var. sargentii, Taxus cuspidata, Rhamnus taquetii, Abies koreana, Hugeria japonica, Prunus buergeriana, and Berberis amurensis var. quelpartensis. Countermeasures to save the global warming vulnerable plants in situ are required.

        라식수술에서 발생한 유리절편을 절편회전술로 치료한 1예

        목적: 레이저각막절삭가공성형술(Laser-assisted in situ keratomileusis, LASIK) 중 발생한 유리절편으로 인해 생긴 불규칙난시를 절편회전술로 치료한 1예를 보고하고자 한다. 증례요약: 21세 여자 환자의 라식수술 중 의도치 않게 우안 유리절편이 발생하였다. 절편의 표시가 지워진 상태여서 임의의 위치에 놓은 후 수술을 종료하였다. 술 후 3개월째 우안 난시는 -3.00D, 좌안 난시는 -0.75D로, 우안 나안시력은 0.4, 좌안 나안시력은 1.0이 었다. 기존의 위치로 돌리기 위해 각막형태검사를 분석하여 절편회전술을 시행하여 불규칙난시를 치료해 우안 나안시력 1.0으로 호전 되었다. 결론: 라식수술 중 합병증으로 발생할 수 있는 유리절편 발생과 동시에 기존 수술 전 표식을 식별할 수 없는 경우에는 술 후 각막형태 검사 분석을 통해 2차적으로 유리절편회전술을 시행하여 기존의 위치로 돌려놓는 것이 난시 교정 및 나안시력 향상에 도움이 된다. <대한안과학회지 2018;59(5):484-490> Purpose: To report a case of irregular astigmatism caused by a free flap during laser-assisted in situ keratomileusis (LASIK) surgery that was treated with a flap rotation based on postoperative topography. Case summary: A 21-year-old female underwent LASIK, which was complicated by a free cap on her right eye. Because the gentian violet markings were no longer present, the exact orientation of the cap was unknown. At 3 months after surgery, the astigmatism of the right eye was -3.00 diopters (D) with an uncorrected visual acuity (UCVA) of 0.4, and the astigmatism of the left eye was -0.75 D with an UCVA of 1.0. The corneal topography was analyzed in order to return to the existing position. Free cap repositioning was performed and irregular astigmatism was corrected to improve the UCVA to 1.0. Conclusions: If the preoperative markings cannot be identified on a free flap during LASIK, secondary postoperative corneal topographic analysis can be performed to restore the corneal free flap to its original position to minimize astigmatism with good visual outcomes.         This work aims to establish the landscape ecological countermeasures to improve the sustainability of urban mountain forest and to enrich the biodiversity in twenty mountains of the Seoul Metropolitan City. Landscape ecological analysis was conducted based upon various criteria, such as area and elevation of mountains, flora, naturalized plants, vegetation, forest types, mountain trails, and public facilities. Various measures were suggested for the better maintenances of floristic diversity, introduced plants, vegetation and forest types of urban mountain forests. Sustainable recommendations for land use adminstration were developed for the preparations of a climate change-adapted urban mountain forests, as well as rational managements of ecosystem, walking trails, and public facilities within mountain forest. Rich and diverse natural vegetation as well as high floristic diversity were reported at mountains with higher elevations, larger area size, and rich landscape diversity. Mountains with low elevation, small size with easy public access, however, might require an urgent action to decrease the burdens by anthropogenic disturbances and urban development.

        Safety Evaluation of Paclitaxel-Eluting Biliary Metal Stent with Sodium Caprate in Porcine Biliary Tract

        Background/Aims: Metallic stents designed to relieve malignant biliary obstruction are susceptible to occlusive tumor ingrowth or overgrowth. In a previous report, we described metallic stents covered with paclitaxel-incorporated membrane (MSCPM-I, II) to prevent occlusion from tumor ingrowth via antitumor effect. This new generation paclitaxeleluting biliary stent is further endowed with sodium caprate (MSCPM-III) for enhanced drug delivery. The purpose of this study is to examine the safety of its drug delivery system in the porcine biliary tract. Methods: MSCPM-III (10% [wt/vol] paclitaxel) and covered metal stents (CMSs) were endoscopically inserted in porcine bile ducts in vivo. Histologic biliary changes, levels of paclitaxel released, and various serum analytes (albumin, alkaline phosphate, aspartate transaminase, alanine transaminase, total protein, total bilirubin, and direct bilirubin) were assessed. Results: Based on the intensity of reactive inflammation and fibrosis, changes in porcine biliary epithelium secondary to implanted MSCPMIII were deemed acceptable (i.e., safe). Histologic features in the MSCPM-III and CMS groups did not differ significantly. In a related serum analysis, paclitaxel release from MSCPMIII stents was below the limit of detection for 28 days. Biochemical analyses were also similar for the two groups, and no evidence of hepatic or renal toxicity was found in animals receiving MSCPM-III stents. Conclusions: In a prototypic porcine trial, this newly devised metal biliary stent incorporating both paclitaxel and sodium caprate appears to be safe in the porcine bile duct.

        <P>High-grade gliomas are closely related to the mesenchymal phenotype which might be explained by unorthodox differentiation of glioma cancer stem cells (gCSCs). We reasoned that other non-neural stem cells, especially mesenchymal stem cells (MSCs), might play a role in expressing mesenchymal phenotype of high-grade gliomas. Thus we hypothesized that cells resembling MSCs exist in glioma specimens.</P>

        일대일 웹 마케팅을 위한 디지털콘텐트 추천 시스템

        Various studies to increase customer satisfaction of a web based system are performed actively. Also in recent days an interest about the personalization that supporting a order type service on customer's viewpoint was raised. So the studies supporting the personalization is required in a web-based marketing system. In this study, we designed an intelligent recommendation system which supporting one to one web marketing using cross selling. The proposed system used an intelligent data mining method as a concurrent cross selling and a sequential cross selling. Also, In experiment on the prototype, we show a proposed system was usable in an practical system applying the mining result. 디지털콘텐트는 복제가 용이하고 원본과 복사본이 동일하다는 특성 때문에 불법적인 복제와 유통의 방지를 통한 저작권의 보호에 어려움이 있다. 근래에는 웹을 기반으로 한 각종 디지털콘텐트 서비스 시스템이 상용화되고 있으며, 이것이 안정된 수익 모델로서 발전하기 위하여 적절한 저작권 보호 기술이 요구된다. 일반적으로 웹 기반의 저작권 보호를 위해서는 디지털 콘텐트의 암호화를 통한 안전한 전송 방법을 사용한다. 이때 암호화된 디지털 콘텐트의 크기는 증가하여 실행과정에 필요한 시간을 증가시킨다. 따라서 실행시간과 안전성을 고려한 시스템의 설계가 필요하다. 본 연구에서는, 디지털콘텐트의 저작권 관리 기술을 기반으로 부분 암호화를 통해 수행시간과 안전성을 고려한 디지털콘텐트 전송 시스템을 설계하였다. 또한 분석을 통해 제안시스템의 성능을 평가하였다.
