신경섬유종증 환자에서 상완신경총 병변의 소견을 보인 척추 신경섬유종

본 증례는 50세 여성의 제 1형 신경섬유종 환자로서 척추 신경섬유종에 의해 초기 상완신경총 병변 소견을 보였다. 약 4개월 동안 후경부통, 좌측 견관절, 좌측 상완의 마비를 보였으며, 초기 전기진단학적 검사상 경수 5번과 6번 신경근을 중심으로 쇄골상부 신경절전성 상완신경총 병변이 관찰되었다. 그러나, 액와 자기공명영상, 초음파검사, 단순 방사선검사, 골주사 등의 영상학적 검사에서는 심한 흉추 척추 전-측만증과 경수 뇌경막에 확장 소견을 제외하고는 상완신경총 병변의 소견은 관찰되지 않았다. 7개월경과 후, 좌측 상지 및 양측 하지의 근력 저하, 경수 4번이하의 감각 저하, 경도의 흉통을 동반한 호흡곤란이 진행되었다. 응급으로 시행된 경수 자기공명영상에서 좌측 경수 3번과 4번에 경막내 액와-속질의 신경종양이 진단되었다. 수술적 처치를 시행하였으며, 거시적 관찰상 척수 신경섬유종이 진단되었다. 급격히 진행하는 척수 신경섬유종은 초기에 상완신경총의 신경근 병변의 소견을 보일 수 있으므로, 신경섬유종증 환자에서 상완신경총 병변의 진단에 세심한 주의를 요한다. We observed a patient with symptom similar to brachial plexus lesion caused by spinal neurofibroma in a 50-year-old female with a history of neurofibromatosis-1(NF-1). She presented with posterior neck, left shoulder and left upper arm paralysis of 4 months duration. The initial electrodiagnostic study(EDS) revealed supraclavicular preganglionic brachial plexus lesion of mainly involved left C5 and C6 roots. However, radiological imagings including axillar MRI, ultrasonography, cervical plain radiography and bone scintigraphy showed no definitive brachial plexus disease except cervical dural dilation and severe thoracic scolio-Iordosis. On 7 months after the first admission, she had aggravated muscle weakness on the left upper arm and both lower extremities, sensory impairment below C4 and respiration difficulty with the chest pain. Emergency MRI showed intradural axillar-medullary neurogenic tumor at the left C3-C4. Subsequently, the patient was treated with surgical resection. Grossly specimen examination displayed the presence of a spinal neurofibroma. In rapidly progressing spinal neurofibroma represented firstly as root lesion of brachial plexus in EDS, the physician should pay scrupulous care to evaluate the cause.

Background: This study aimed to identify the clinical characteristics and prognosis of primary gastric lymphoma detected by screening upper endoscopy in high prevalence area of Helicobacter pylori ( H. pylori) infection. Methods: Between October 2003 and May 2013, consecutive subjects who were diagnosed with primary gastric lymphoma by screening upper endoscopy were retrospectively enrolled at Seoul National University Hospital Healthcare System Gangnam Center, Seoul, Korea. Results: During the study period, a total of 101,103 subjects received 207,114 screening upper endoscopy. Among them, primary gastric lymphoma was detected in 49subjects. They were 53.0 ± 10.5 years of age, and 72.9% (35/49) were female. Histologic type of primary gastric lymphoma was predominantly (98.0%, 48/49) extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma), and one remaining case (2.0%) was diffuse large B-cell lymphoma. Initial stage was IEa in 95.9% (47/49), and H. pylori was positive in 91.8% (43/49). Among 41 H. pylori-positive patients with gastric MALT lymphoma stage IEa, H. pylori eradication achieved complete remission in 95.1% (39/41). There were three cases of recurrence with (n = 2) or without (n = 1) reinfection of H. pylori. Conclusions: Upper endoscopy as a screening for upper gastrointestinal malignancymay detect gastric MALT lymphoma in early stage, which can be successfully treated with H. pylori eradication, especially in H. pylori endemic area. For those who achieved complete remission, it is needed to undertake surveillance for recurrence.
Prevalence of serum allergen-specific immunoglobulin E for canine atopic dermatitis in Korea

Canine atopic dermatitis (CAD) is an allergic skin disease with characteristic clinical features associated with immunoglobulin E (IgE) antibodies. Identification of the causative allergens is the diagnostic goal, which is essential to treat and manage CAD patients. CAD is commonly associated with environmental allergens surrounding the patients. For this reason, it is important for diagnostic tests to select allergens that are related to the environment of each country and each province. There are two main allergen-specific tests, serological IgE test (SAT) and intradermal skin test (IDT). SAT did not show direct cutaneous reaction but did show serological reaction against allergens. However, SAT is simpler and more convenient than IDT in small animal practice. In this study, we selected domestically prevalent allergens for SAT, including 60 food allergens and 60 inhalant allergens, and tested eight dogs tentatively diagnosed with CAD based on Favrot’s criteria. Furthermore, IDT was performed on four dogs from the SAT group for comparison of SAT and IDT, and the results were very similar. In SAT, four types of mites (Bloomia tropicalis, Glycophagus domesticus, Euroglyphus maynei, and mite mixture 1 Korea; house dust mites), four types of molds (Botrytis cinerea, Alternaria alternata, mold fungi mixture 11, mold fungi mixture), and one type of pollen (tree pollen mix 3 Korea) induced a reaction in more than half of dogs tested. In IDT, all four dogs reacted positively to Dermatophagoides farinae, and three reacted positively to Dermatophagoides pteronyssinus and house dust. The mean agreement rate between SAT and IDT in this study was 76.3%. This is the first trial to apply local allergens for SAT in Korean veterinary medicine, and it might play an important role for diagnoses and management of animal allergic diseases.

<P>GPR119 belongs to the G protein-coupled receptor family and exhibits dual modes of action upon ligand-dependent activation: pancreatic secretion of insulin in a glucose-dependent manner and intestinal secretion of incretins. Hence, GPR119 has emerged as a promising target for treating type 2 diabetes mellitus without causing hypoglycaemia. However, despite continuous efforts by many major pharmaceutical companies, no synthetic GPR119 ligand has been approved as a new class of anti-diabetic agents thus far, nor has any passed beyond phase II clinical studies. Herein, we summarize recent advances in research concerning the physiological/pharmacological effects of GPR119 and its synthetic ligands on the regulation of energy metabolism, and we speculate on future applications of GPR119 ligands for the treatment of metabolic diseases, focusing on non-alcoholic fatty liver disease.</P>
