        Facial nerve reconstruction using a split hypoglossal nerve in treating unilateral facial palsy after acoustic neuroma removal

        Two patients underwent split hypoglossal-facial nerve anastomosis(anastomosis of a split hypoglossal nerve to the facial nerve) for treatment of unilateral facial palsy. All patients previously had undergone removals of a large acoustic neuroma. Facial nerve had been resected and immediately reconstructed in one patient. In the other patient, facial nerve had been anatomically preserved at the first operation and the interval between tumor resection and reconstruction was 10 months. Postoperative recovery of facial movement was good and the degree of hypoglossal nerve atrophy on the operated side was graded mild or moderate in all cases during an average follow-up period of 1.2 years. It was concluded that split hypoglossal-facial nerve anastomosis results in good facial reanimation and may reduce the degree of hemiglossal atrophy in comparison with the classic hypoglossal-facial nerve anastomosis.

        Relation of Enteric α-Synuclein to Gastrointestinal Dysfunction in Patients With Parkinson’s Disease and in Neurologically Intact Subjects

        Background/Aims α-Synucleinopathy in the brain is the neuropathological hallmark of Parkinson’s disease (PD). However, the functional impact of α-synucleinopathy in the enteric nervous system remains unknown. We aim to evaluate the association between gastrointestinal (GI) dysfunction and α-synuclein (αSYN) pathology in the stomach and colon of PD patients and controls, as well as to investigate the association between the αSYN pathology in GI tract and future PD risk. Methods A total of 35 PD patients and 52 neurologically intact subjects were enrolled in this study. Endoscopic biopsies were performed, and then immunohistochemical staining for αSYN was performed. All subjects completed the validated Rome III questionnaire for the assessment of GI symptoms. The association between GI symptoms and the αSYN pathology in GI mucosa was evaluated. Incident PD cases were assessed during a median follow-up of 46 months. Results The proportion of self-reported constipation and functional constipation through the Rome III questionnaire was significantly higher in PD patients than in controls (P < 0.001 and P = 0.015). However, no significant association was found between the αSYN pathology in the stomach and colon mucosa and constipation, as well as other GI symptoms including dyspepsia symptoms and abdominal discomfort or pain, regardless of the presence or absence of clinical PD (P > 0.05). No incident PD cases were diagnosed during study period. Conclusions Our present study suggests that the deposition of αSYN in the mucosal enteric nervous system may not be reflected by functional impairment of the affected segment of the gut.         This paper proposes a controller to compensate the DC-Link voltage unbalance in the BLDC motor drive system based on the four-switch and half-bridge boost converter. In this paper, we can see that the proposed feed?forward controller is useful for control of the neutral point voltage unbalance. The comparative experimental results show that the system became more robust by the feed-forward control of the neutral point voltage.

        <P>Asymmetric Schottky contact thin-film-transistors (ASC-TFTs) with an amorphous- In<SUB>2</SUB>Ga<SUB>2</SUB>ZnO<SUB>7</SUB> channel were fabricated, and their operation characteristics were examined. Ti, Ni, and Pt were evaluated as source/drain metal, and the variations in the device performance were analyzed in terms of energy level and bias polarity, which were carefully simulated to understand the influence of the contact properties on the device performance. The contact nature largely influenced the distribution of potential under the given gate and drain biases, as well as the accompanying carrier accumulation layer and current path formation. Schottky-type contact induced conduction path formation even on the back surface of the channel when drain voltage was high even with sufficiently high gate bias being applied. Based on these results, by applying different metal for each source and drain metal, ASC-TFTs integrating TFTs and Schottky diodes were fabricated, which showed a rectification ratio of drain current higher than 10<SUP>8</SUP> according to the bias direction. In addition, the transfer and output characteristics of ASC-TFTs were evaluated for various operation regimes, and the roles of the Schottky junction in device operation were studied in detail.</P>

        Transarterial Chemoembolization Can Be Safely Performed in Patients with Hepatocellular Carcinoma Invading the Main Portal Vein and May Improve the Overall Survival.

        <P>Purpose: To determine the efficacy and safety of transarterial chemoembolization (TACE) in patients with hepatocellular carcinoma (HCC) and main portal vein (MPV) invasion. Materials and Methods: This study was approved by the institutional review board, and the requirement to obtain informed consent was waived. The authors retrospectively assessed the electronic medical records of patients in whom HCC with MPV invasion was newly diagnosed from January 2004 to December 2007 at a single tertiary medical center. Patients with decompensated hepatic function were excluded. Outcomes of patients treated with TACE were compared with those of patients given supportive care according to Child-Pugh class. Results: One hundred twenty-five patients (104 men and 21 women; mean age, 55.7 years; age range, 33.4-83.0 years) were included. The median overall survival was 3.7 months (range, 0.2-33.3 months). Eighty-three of the 125 patients (66.4%) were treated with TACE and 42 (33.6%) received supportive care. Repeated TACE showed significant survival benefits compared with supportive care in patients with Child-Pugh class A (median survival, 7.4 months vs 2.6 months, respectively; P < .001) and class B (median survival, 2.8 months vs 1.9 months, respectively; P = .002) disease. Results of multivariate analysis showed that treatment with TACE (hazard ratio, 0.263; 95% confidence interval [CI]: 0.164, 0.424; P < .001) and Child-Pugh class A status (hazard ratio, 0.550; 95% CI: 0.368, 0.822; P = .004) were independent predictive factors of a favorable outcome. There were no procedure-related deaths within 4 weeks after TACE, and patient morbidity was 28.9% (24 of 83 patients). Conclusion: TACE can be performed safely and may improve the overall survival of patients with HCC and MPV invasion. © RSNA, 2011.</P>

        Berberine Decreases Cell Growth but Increases the Side Population Fraction of H460 Lung Cancer Cells

        Berberine has been reported to inhibit tumor growth in lung cancer. Thus, the effects of berberine on cancer cells as well as the cancer stem cell (side population; SP) fraction were investigated in the H460 lung cancer cell line, and the effects of berberine treatment on cell growth, cell cycle, and cell death were evaluated. Changes in the SP fraction were examined after treatment with berberine, 5-fluorouracil (5-FU), and co-treatment. Berberine inhibited cancer cell growth in a dose-dependent manner. Treatment of the cells with berberine resulted in a 4% increase in cell death and an 8% increase in the number of cells of $G_0/G_1$ phase, compared to the untreated control. To examine the relationship between berberine and cancer stem cells, the SP fraction was analyzed. Surprisingly, the SP cell fraction was increased upon berberine treatment and further increased after co-treatment with 5-FU. These results are in contrast to the study of Kim et al. (2008) with MCF-7 breast cancer cells, in which berberine inhibited the growth of both cancer cells and the corresponding cancer stem cells. Results of the present study suggest that berberine should be used with caution in the treatment of various cancers, despite its positive effect on cancer cell growth inhibition.

        Case Reports : A Case of Pulmonary Vein Tumor Presenting as a Left Atrial Mass

        Primary cardiac tumors are extremely rare and can originate within the heart or be the result of tumor spread from other sites. We report a female patient with a pulmonary vein tumor extending into the left atrium that had a suspicious primary malignant origin with a sacral metastatic carcinoma. The patient was admitted complaining of pain in her buttock area as a result of a sacral tumor. It was believed that the sacral tumor was a metastasis from the imaging study and clinical manifestation. The primary malignant origin was evaluated. The chest CT showed a left atrium thrombus-like lesion without a pulmonary abnormality. After a transesophageal echocardiogram, the patient was diagnosed with a pulmonary vein tumor extending to the left atrium. The patient was given palliative radiotherapy for the sacral pain. Initially, the clinical impression was a metastatic sacral tumor with a thromboembolism of the left atrium. However, this patient was finally diagnosed with a pulmonary vein tumor with a left atrium extension by a transesophageal echocardiogram.

        Lipoxygenase Inhibitors from Paeonia lactiflora Seeds

        Previously, the methanolic extract of Paeonia lactiflora seeds was shown to have strong inhibitory activity against soybean lipoxygenase (SLO). Four phenolic compounds were isolated from the seeds by solvent fractionation, Sephadex LH-20 column chromatography and preparative HPLC, and three of them showed strong SLO inhibition and were characterized as trans-resveratrol, ε-viniferin and luteolin by UV, IR, ¹H-NMR, ¹³C-NMR and MS spectrometry. trans-Resveratrol (IC_(50)=1.02 μM), ε-viniferin (IC_(50)=O.81 μM) and luteolin (IC_(50)=10.01 μM), first found in the above seeds, exhibited a potent SLO inhibitory activity although their activity was lower than that of a well-known lipoxygenase inhibitor, nordihydroguaiaretic acid (NDGA) (IC_(50)=0.57 μM). These results suggest that Paeonia lactiflora seeds, now an unused plant seed, may be developed into useful sources of anti-inflammatory drugs.

        Lipoxygenase Inhibition and Antioxidative Activity of Flavonoids from Paeonia moutan Seeds

        Previously, the methanolic extract of Paeonia moutan seeds was found to potently inhibit soybean lipoxygenase (SLO). Hence to isolate SLO inhibitor, the defatted methanolic extract of the seeds was consecutively partitioned with ether, ethyl acetate, n-butanol, and water. The ether souble fraction showing strong inhibitory activity against SLO was further fractionated into a strongly acidic, a weakly acidic, and a neutral fractions. The strongly acidic components of the ether extract were successively subjected to chromatography on a silica gel, Sephadex LH-20, and preparative HPLC. Four phenolic compounds were isolated, and two of them showing a strong SLO inhibition activity were identified as luteolin (IC_(50)=2.32 ㎍/ml) and 5,6,4'-trihydroxy-7,3'-dimethoxyflavone (IC_(50)=0.31 ㎍/ml) by UV, IR, ¹H- & ¹³C-NMR, and MS spectroscopy. In addition, two flavonoids showed significantly anti oxidative activity as strong as that of α-tocopherol (p<0.05) in the autoxidation system of linoleic acid. These results suggest that luteolin and 5,6,4'-trihydroxy-7,3'-dimethoxy-flavone may be used as a potential source of anti-inflammatory agents with antioxidative activity.

        Introduction: Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening complication after solid organ transplantation. HLH has been reported as problem related with kidney and liver transplant, and there are limited reports of HLH after lung transplantation. Case: A 60-year-old man with idiopathic pulmonary fibrosis underwent bilateral lung transplantation. After lung transplantation, acute rejection was suspected and high dose steroid therapy was done. Since postoperative day(POD) 25, thrombocytopenia( platelet 112*1000/uL) and leukopenia(2530/uL)) were presented. The patient complained of intermittent symptom of low-grade fever, chest discomfort and dyspnea. Echocardiography showed stress induced cardiomyopathy and results of peripheral blood smear was nonspeci. c. Pneumonia was developed and patient was treated with antibiotics. Hyperbilirubinemia(total bilirubin 2.1 mg/dL) started to present at POD 50. Results of abdomen sonography was nonspeci. C except for mild splenomegaly (11.4cm). By POD 80, bilirubin was getting higher(total bilirubin 33.3mg/dL, direct bilirubin 25.7mg/dL, gamma glutamyl transpeptidase 260IU/L) and leukopenia and thrombocytopenia was getting aggravated (white blood cell 2080/uL and platelet 57* 1000/uL). The . brinogen was mildly elevated (4210m/dL), triglyceride was normal (127 mg/dL), the ferritin was elevated (4518 ng/mL) and soluble interleukin-2 receptor was elevated (8730U/ml). However, the finding of peripheral blood smear was still nonspeci. c. The cause of pancytopenia, low grade fever and hyperbilirubinemia was unclear, and we conducted bone marrow biopsy on POD 82. The finding showed that histiocytes were frequently seen with occasional hemophagocytes. Taken together, cytopenia, bone marrow hemophagocytes, elevated soluble interleukin-2 receptor and elevated ferritin were positive among laboratory tests listed in diagnostic criteria of HLH. We managed with etoposide and high dose steroid, but patient deteriorated and died on POD 87. Summary: HLH is a significant diagnostic and therapeutic challenge in lung transplantation and is potentially lethal complication. Therefore, clinicians should consider HLH as possible diagnosis in clinical context.
