      • 중증 재생불량성 빈혈 환자의 동종골수이식 후에 병발한 간정맥폐색질환(Hepatic Veno-Occlusive Disease)의 임상 경과 및 혈액응고 인자들의 변화

        연구배경: 재생불량성 빈혈 경우에는 조혈모세포 이식 후 VOD의 발생에 관한 보고가 거의 없는 실정이다. 저자 등은 cyclophosphamide와 antithymocyte globulin으로 전처치 후에 동종골수이식을 시행한 재생불량성 빈혈 환자들을 대상으로 하여 VOD의 발생과 임상 양상, 혈액응고 인자들의 변화에 관하여 분석하여 보았다. 방법: 재생불량성 빈혈로 동종골수이식을 시행 받은 13예 환자를 대상으로 하였다. 임상적 기준에 따라 VOD를 진단하였으며 임상 경과에 따라서 경중을 분류하였다. Protein C, protein S, antithrombin Ⅲ, tPA, PAI-1 등의 혈액응고 인자들을 전처치 투여전, 골수 주입당일, 이식 후 제7, 14, 21일에 측정하였다. 결과: 7예(53.8%)에서 VOD가 발생하였으며, 경증 5예, 중등도 2예였다. VOD군에서 혈소판치가 20.000/㎕ 이상을 유지하는 데까지 걸리는 시간이 더 길었다(p=0.026). 또한 VOD군에서 적혈구 및 혈소판의 수혈 요구량이 각각 유의하게 많았다(p=0.025, 0.015). 혈액응고 인자들의 변화를 보면, 제 0일에 VOD군에서 PAI-1의 혈중 농도가 유의하게 더 높았다.(p=0.034). 결론: 재생불량성 빈혈환자에서도 조혈모세포 이식 후에 VOD의 높은 발생률을 관찰하였으며, 대부분이 경증이었다. VOD 환자에서의 혈액응고 인자들의 변화는 PAL-1이 VOD의 감별 진단을 위하여 이용될 수 있음을 시사하고 있다. Background: There are few reports about the occurrence of hepatic veno-occlusive disease(VOD) after bone marrow transplantation (BMT) for severe aplastic anemia (SAA). We analyzed the incidence and clinical outcomes of VOD and changes of coagulation factors in patients with SAA who were conditioned with cyclophosphamide plus ATG and underwent allogeneic BMT. Methods: Thirteen patients with SAA who underwent allogeneic BMT were included. A diagnosis of VOD was made according to clinical criteria, and severity of VOD was classified according to clinical course. Plasma levels of protein C, protein S, antithrombin Ⅲ, t-PA, and PAI-1 were determined before preparative chemotherapy, in the day of marrow infusion, and days 7, 14, and 21. Results: VOD occurred in 7/13(53.8%). Five were mild VOD and two were moderate VOD. Time to platelet ≥20,000/L was slower (p=0.026) and red cell and platelet transfusion requirements were higher (p=0.025,0.015) in patients with VOD . The mean plasma level of PAI-1 at 0 were higher in patients with VOD (p=0.034). Conclusion: We observed the high incidence of VOD after allogeneic BMT in patients with SAA. A majority of cases of VOD were mild. The changes of PAI-1 after BMT may be useful marker of hepatic VOD

        當歸의 種類에 따른 四物湯이 白鼠의 胸部大動脈 血管弛緩에 미치는 影響

        We have examined the relaxational reponse to the water extract of Angelica gigas Nakai (AG), A. sinensis(Oliv.) Diels (AS), A. acutiloba Kitagawa (AA), and Four-Substance Decoctions (AG-FSD, AS-FSD, AA-FSD) in isolated thoracic aorta from sprague dawley(SD) rat in the presence and absence of endothelium. Rat thoracic aorta was investigated in vessel segments suspended for isometric tension recording by polygraph. Responses to AG, AS, AA and AG-FSD, AS-FSD, AA-FSD were investigated in vessels precontracted with 5-hydroxytryptamine(5-HT) were compared in vasodilation effect. We found that the thoracic aorta segments responded to AG, AS, AA and AG-FSD, AS-FSD, AA-FSD with a dose-dependent vasodilation. The 5-hydroxytryptamine induced contraction at 10-4M were inhibited by 26.3%, 2.7%, 2.1% and 24.6%, 1.1%, 3.2% after addition of the 0.1 g/㎖ water extract of AG, AS, AA and AG-FSD, AS-FSD, AA-FSD. The 5-hydroxytrptamine induced contraction at 10^-4M with and without endothelium were inhibited by 24.6% and 6.9% after addition of the 0.1 g/㎖ water extract of AG-FSD. In conclusion, AG, AS, AA and AG-FSD, AS-FSD, AA-FSD-induced relaxation in the isolated rat thoracic aorta were composed of dose-dependent relaxation. AG-FSD has very potent vasodilation.

        One of protection methods for computer internal data is to pursue data flow in program and to control its flow. In this thesis, we propose a mechanism which guarantees secure information flow in a program. By investigating security classes of all the variables in a statement and judging necessary condition for secure information flow, proposed model descriminates the program whether it is secure or not. Out model can be implemnted at language translation time, particularly semantic analysis phase, by pursuing security class transfer of each statement.

        Response Prediction after Neoadjuvant Chemotherapy for Colon Cancer Using CT Tumor Regression Grade: A Preliminary Study

        Purpose To investigate whether CT-based tumor regression grade (ctTRG) can be used to predict the response to neoadjuvant chemotherapy (NAC) in colon cancer. Materials and Methods A total of 53 patients were enrolled. Two radiologists independently assessed the ctTRG using the length, thickness, layer pattern, and luminal and extraluminal appearance of the tumor. Changes in tumor volume were also analyzed using the 3D Slicer software. We evaluated the association between pathologic TRG (pTRG) and ctTRG. Patients with Rödel’s TRG of 2, 3, or 4 were classified as responders. In terms of predicting responder and pathologic complete remission (pCR), receiver operating characteristic was compared between ctTRG and tumor volume change. Results There was a moderate correlation between ctTRG and pTRG (ρ = -0.540, p < 0.001), and the interobserver agreement was substantial (weighted к = 0.672). In the prediction of responder, there was no significant difference between ctTRG and volumetry (Az = 0.749, criterion: ctTRG ≤ 3 for ct- TRG, Az = 0.794, criterion: ≤ -27.1% for volume, p = 0.53). Moreover, there was no significant difference between the two methods in predicting pCR (p = 0.447). Conclusion ctTRG might predict the response to NAC in colon cancer. The diagnostic performance of ctTRG was comparable to that of CT volumetry.

        美 국방 혁신 생태계 분석을 중심으로 한 국내 국방기술 R&D 체계 발전 방안 연구

        최근 4차 산업혁명의 가속화로 인해 AI, 로봇 등으로 대표되는 첨단 신기술 분야의 기술 발전이 급격하게 이루어지고 있다. 이로 인해 미래 전장 환경도 획기적으로 변화할 것으로 예측되어 우리 군은 국방혁신 4.0을 통한 AI 과학기술강군 육성이라는 국정과제를 수행하고 있다. 그러나 현재 우리나라의 국방 무기체계 획득 체계 하에서는 민간 영역에서 발전하고 있는 첨단 신기술을 무기체계에 적용하기 쉽지 않다. 국방기술 R&D 사업 예산이 증가하고 있음에도 불구하고 국방기술 R&D의 폐쇄성과 획득체계의 경직성으로 인해 민간의 유인이 제한되는 현실이 있다. 한편, 미국은 4차산업혁명과 같은 환경변화에 적극적으로 대응하여 민간 첨단 신기술의 신속 도입을 위한 국방 혁신을 추진하고 있다. 제3차 상쇄전략, 미중 기술패권 경쟁 등에서 알 수 있듯이 미국은 기술적인 혁신을 최우선 가치로 두고 있으며, 민간에서 개발된 신기술을 국방 영역에 적용하기 위한 혁신 생태계를 구성하고 있다. 본 논문에서는 민간에서 급격히 발전하고 있는 첨단 신기술을 국방 영역에 신속하게 적용하기 위해 노력하고 있는 미 국방부와 각 군 내 국방 혁신 조직들의 역할과 임무를 분석하고, 이를 바탕으로 우리나라 국방기술 R&D 사업 체계와 비교 · 분석하며 기존 R&D 사업 체계의 발전방안과 함께 민간과 군 관련 기관 간의 협력을 강화하여 우수한 민간 신기술의 신속한 스핀온을 통해 국방 혁신을 가속화할 수 있는 방안을 제안하고자 한다. Recently, with the acceleration of the Fourth Industrial Revolution, there has been a rapid advancement in cutting-edge technologies such as artificial intelligence and robotics. As a result, it is predicted that the future battlefield environment will undergo a revolutionary transformation. In response to this, the South Korean military is undertaking the national task of Fostering an AI Scientific and Technological Power through Defense Innovation 4.0. However, under the current South Korean defense acquisition system, incorporating advanced civilian technologies from the private sector into the defense weapon systems proves challenging. Despite an increase in the research and development (R&D) budget for defense technology, the closed nature and rigid structure of defense R&D limit incentives for private-sector involvement. In contrast, the United States proactively responds to environmental changes like the Fourth Industrial Revolution and drives defense innovation. They prioritize technological advancements and consider technological competition like the Third Offset Strategy as a top priority. Additionally, they have established an innovation community to apply cutting-edge civilian technologies developed in the private sector to defense. The aim of this paper was to analyze the roles and missions of defense innovation organizations within the US Department of Defense and each branch of the military, which actively integrate advanced civilian technologies into defense. Based on this analysis, ways to advance the South Korean defense technology R&D system are proposed to foster collaboration between the private sector and defense agencies. By strengthening cooperation and facilitating the swift spin-on of exceptional civilian technologies, we aim to accelerate defense innovation in South Korea.

      • Prevalence of Systemic Lupus Erythematosus in South Korea: An Administrative Database Study

        <P><B>Background</B></P><P>Systemic lupus erythematosus (SLE) is a rare autoimmune disease for which a population-based survey on the prevalence of the disease in South Korea has not yet been conducted. Our goal was to estimate the nationwide prevalence of SLE.</P><P><B>Methods</B></P><P>The International Classification of Diseases, Tenth Revision (ICD-10) code for SLE diagnosis—M32—was tentatively given when patients were suspected to have SLE before 2009. As such, the positive predictive value (PPV) of the M32 code shown in medical bills reflecting true SLE was uncertain. We attempted to estimate the prevalence of SLE in South Korea using national administrative database data from 2004–2006. We approximated the actual number of SLE patients by analyzing a list of SLE-coded patients provided by the National Health Insurance (NHI) and Health Insurance Review and Assessment Service. Prevalence was estimated by multiplying the PPV of the M32 diagnostic code by the number of patients receiving the code. The PPV was determined by three methods: direct investigation of the medical records of patients randomly selected from the SLE-coded patients list; assessment of all SLE patients treated at 56 selected hospitals in South Korea; and extrapolation from sub-groups at a single institute to the sub-groups of the national NHI data.</P><P><B>Results</B></P><P>The estimated number of national SLE cases was between 9000 and 11 000, depending on the method of ascertainment, corresponding to a prevalence of 18.8–21.7 per 100 000 people.</P><P><B>Conclusions</B></P><P>This is the first report of a nationwide prevalence survey of SLE in South Korea. National databases may serve as a resource for epidemiologic studies of rare autoimmune diseases like SLE.</P>

      • Mitochondrial ATP synthase is a target for TNBS-induced protein carbonylation in XS-106 dendritic cells

        <P>ROS are produced in dendritic cells (DCs) during antigen presentation in contact hypersensitivity (CHS). As a result, ROS cause a number of nonenzymatic protein modifications, including carbonylation, which is the most widely used marker of oxidative stress. 2,4,6-Trinitrobenzene sulfonic acid (TNBS) is a well-characterized contact allergen that results in the formation of ROS. However, proteins that are carbonylated in DCs in response to TNBS have not been identified. To study ROS-dependent protein carbonylation in response to TNBS, we used the well-established mouse DC line, XS-106. We focused on the effects of TNBS on oxidation by examining selected oxidative markers. We identified TNBS-induced ROS and myeloperoxidase (MPO) proteins and demonstrated that the increase in ROS resulted in IL-12 production. The increase in oxidation was further confirmed by an oxidation-dependent increase in protein modifications, such as carbonylation. In fact, TNBS strongly induced carbonylation of mitochondrial adenosine triphosphate (ATP) synthase in XS-106 DCs, as determined by MALDI-TOF analysis and 2-D Western blotting. ROS production and protein carbonylation were confirmed in human monocyte-derived DCs (Mo-DCs). Furthermore, glutathione (GSH) decreased ROS and protein carbonylation in Mo-DCs. Carbonylation of ATP synthase in DCs may contribute to the pathophysiology of CHS.</P>

        Odontogenic infection involving the secondary fascial space in diabetic and non-diabetic patients: a clinical comparative study

        Objectives: This retrospective study was performed to evaluate the clinical impact of diabetes mellitus on the prognosis in secondary space infection. Materials and Methods: Medical records, radiographic images, computed tomography, and microbial studies of 51 patients (25 diabetic patients and 26 non-diabetic patients) were reviewed. Patients were diagnosed as secondary fascial space infections with odontogenic origin and underwent treatment at Chonnam National University Hospital, in Department of Oral and Maxillofacial Surgery, from January 2007 to February 2009. Results: Compared to patients without diabetes, patients with diabetes were presented with the following characteristics: older age (diabetic patients: 62.9 years, non-diabetic patients, 47.8 years), more spaces involved (diabetic patients, 60%; non-diabetic patients, 27.3%), more intense treatment, longer hospitalization (diabetic patients, 28.9 days; non-diabetic patients, 15.4 days), higher white blood cell and C-reactive protein values, higher incidence of complication (diabetic patients, 40%; non-diabetic patients, 7.7%), and distinctive main causative microorganisms. Conclusion: These results suggest that the prognosis of diabetic patients is poorer than that of non-diabetic patients in secondary space infections since they had greater incidence rates of involved spaces, abnormal hematologic findings, more complications, and additional procedures, such as tracheostomy.

      • Repeated dose 90-day oral toxicity of Kaolin in Spraque-Dawley (SD) rats

        Kaolin is a naturally occurring clay mineral consisting of aluminum silicate. Kaolin is used in cosmetic, toothpaste, paint. People can be exposed to kaolin in the workplace by the powder or from skin or eye contact. In this study, we used the kaolin from South Korea to investigate the potential toxicity of kaolin in Spraque-Dawley(SD) rats. 7-week-old male SD rats were administered by oral gavage, once per day for 90 days. Those were separated into 5 groups (5 female and 5 male for each group) and given four dose of 250, 500, 1000, 2000 mg/kg test article or vehicle for a period of 90 days. We were measured clinical signs, mortality, body weights, food consumption, hematology, serum biochemistry, necropsy, gross findings, histopathology and organ weights. The treated animals survived throughout the study period and did not reveal any treatment related major abnormal clinical signs at the tested dose levels. The overall percent body weight gain in rats treated with the test material was found to be normal during the 90-day observation period and no abnormalities were observed in the necropsy. Hematology and serum biochemistry revealed no evidence of specific toxicity to test material. These results that, NOAEL (No Observed Adverse Effect Level) is considered to be more than 2000 mg/kg/day in both sexes.
