In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

Hepatitis B virus replicates through 3.5kb pregenomic RNA intermediate which is regulated by core promoter. Pathogenesis of hepatitis B virus has been bilieved the result of host immune response. But recently many studies have reported that high level of viral replication caused by mutation in core promoter might result in severs hepatitis. But these studies were performed in vitro, not in vivo. So there is yet debate about which factor, viral of host factor, is more important in pathogenesis of hepatitis B virus. So we measured real viral replication level in 204 chronic hepatitis B patients by quantifying HBV DNA from sera by our novel PCR-based more sensitive method, and compared these results with ALT level measured from same sera, which indicates liver cell damage. Surprisingly there are no significant correlation between HBV DNA quantity and ALT level. Then we cloned core promoter region. In SSCP, we found that many viral mutants coexist in one patient. Base on SSCP result, we chose main viral core promoter type in each patients, which is thought to determine overall viral replication level in this patient. Main type of core promoter region of each 41 patients were directly sequenced. And with these we measured promoter activity by luciferase assay system and compared promoter activity with on another. We found tha there were some differences in promoter activity according to core promoter sequences. And we constructed replication-competent viral constructs with core promoter from 41 patients and Transfected these into HepG2 cell and measured HBV DNA by southern blot. There were also differences in HBV DNA quantity according to core promoter sequences. On these all results we investigated correlation between the effect of HBV core promoter on viral replication in vitro and HBN DNA quantity, ALT level from sera of each patients. We found there is no significant correlation among them. As a result, we concluded that in determining severity chronic hepatitis B patients, host factors of each patient is more important rather than replicative activity of virus itself.

This research is to point out problems and improvements about the present system of environmental impact assessment in Korea. Environmental impact assessment is an integral part of a decision-making process for a development project. But There are some problems in this system. So this paper presents a number of policy alternatives which environmental impact assessment can be improved First, an Environmental Impact assessment report should be prepared by the fair and objective person not developer. Second, Environmental Impact assessment system is not enough, so screening system and seeping system should be added to it. It need to reinvented so as to include a practical alternative investigation. Third, comprehensive environmental information system on the regional and national levels should be established ton provide necessary information prior to Environmental Impact Assessment. Fourth, Environmental Impact assessment time and participation of regional residents is not enough.

천추 갈색종양(Brown tumor)으로 발현된 부갑상선 기능항진증 1예

부갑상선 기능항진증에 의한 골병변으로 갈색종양이 있다. 갈색종양은 주로 장골을 침범하고 척추골에서의 발생은 매우 드물다. 국내의 보고에서 갈색종양의 장골 침범은 있었으나, 척추골에서의 발생은 보고된바 없다 .저자들은 부갑상선 기능항진증으로 인하여 척추골의 천추에 발생된 갈색종양 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. The bony involvement in primary hyperparathyroidism present as browm tumor or osteitis fibrosa cystica and lead to pathologic fracture. Brown tumor commonly involve long bone such as femur, tibia, fibula, humerus, and radius. Brown tumor of axial skeleton is a rare entity. Previously reported case that involve axial skeleton still not present. We report a 47-year-old male patient that he has a hyperparathyroidism presenting as brown tumor involving sacrum.

It was for temperature and humid. It was high-priced goods. made in foreign contra. So the purposes of the Heat Controller for agriculture are to settle the breakdown-troubles and regulate temperature for agricultural products.

언어의 분절범주의 모델에 관하여

In den Modellen der Kategorisierung, die im Rahmen der kognitiven Semantik bekannt sind, gibt es die Familiena¨hnlichkeit, die notwendigen und hinreichenden Bedingungen und den Prototyp. G. Kleiber nach sucht die Standardtheorie der Prototypensemantik das Prinzip der Kategorisierung in dem Prototyp, und deren erweiterte Version in der Familiena¨hnlichkeit, wobei Versionen gemeinsam das Modell der notwendigen und hinreichenden Bedingungen verweigern. Es handelt sich bei dem Modell der Familiena¨hnlichkeit um Eigenschaften, die nicht bei allen Mitgliedern auftregen mu¨ssen, aber mindestens bei zweien. Sie brauchen natu¨rlich nicht bei allen u¨brigbleibenden aufzutreten. Der entscheidende Mangel der Auffassung liegt darin, dass derartige Verbindung durch Assoziationen in jeder Sprache ohne Unterbrechung zu finden ist. Es gibt dann in ihr nur eine einzige u¨bergeordnete Kategorie - diese ist letzen Endes jede einzelne Sprache an sich -, und alle Wo¨rter in ihr sind Unterkategorien, die unmittelbar zu ihr geho¨ren. Wir wissen jedoch schon, dass es in ihr unheimlich viele u¨ber - und untergeordnete Kategorien gibt. Das Modell der Familiena¨hnlichkeit kann daher keine angemessene Erkla¨rung fu¨r die sprachlichen Gliederung geben. Der kognitiven Semantik nach soll die Kategorisierung im Modell der notwendigen und hinreichenden Bedingungen auf der Grundlage gemeinsamer Eigenschagften erfolgen. Obwohl sie derartige Kategorisierung verneine will, dennoch gibt es offenbar Kategorien wie Zahlreihe, Leistungsbewertung, milita¨rischer Titel, Verwandtschaft vor allem im Koreanischen sowohl in der fast ku¨nstlichen als auch in der natu¨rlichen Sprache. Dem Prototypenmodell nach besitzt die Kategorie eine prototypische Struktur mit einem Prototyp im Zentrum und anderen Exemplaren,die je nach ihrer A¨hnlichkeit mit dem Prototyp mehr oder weniger weit entfernt vom Zentrum angeordnet sind. Derartige gegliederte Kategorien sind in der Sprache leicht zu finden. Der Prototyp ist jedoch keine neue wissenschaftliche Erkenntnis, sondern eine Entwicklung auf der Grundlage der Wiederentdeckung, weil die wichtigen Eigenschaften des Prototyps bereits von der sprachlichen Feldlehre aufgekla¨rt sind. Wir gelangen somit zum Ergebnis, dass die gegliederten Kategorien einer Sprache aufgrund der Felder bzw. der Prototypen gebildet werden, und dass die Modelle der notwendigen und hinreichenden Bedingungen und, wenn u¨berhaupt, der Familiena¨hnlichkeit Sonderfa¨lle des Feld-bzw. Prototypenmodells sind.

하부 요로 폐쇄에 대한 요류 역학적 연구

요류 측정술을 이용하여 전립선 비대증 환자 40례에서 경요도 전립선 절제술후 치료 효과를 평가하여 보고한다. Uroflowmetry has been proposed as a non-invasive screening test and may be used to reveal lower urinary tract dysfunctions, such as benign prostatic hyperplasia, urethral stricture and neurogenic bladder. We tested 40 patient of benign prostatic hyperplasia (BPH) diagnosed with clinical manifestations, IVP and cystoscopy. Group 1 including 15 patients performed both preoperative and postoperative uroflowmetry. Group 2 including 25 patients shown acute urinary retention preoperatively, performed only postoperative uroflowmetry. The results were obtained as follows. 1.In group 1, average preoperative maximum flow rate (MFR) was 7.2 cc/sec and postoperative MFR was 20.1 cc/sec (P<0.01). On flow rate normogram, preoperative MFR of all patients was below -2 standard deviations (SD) and average MFR was -3.1 SD. In postoperative MFR, below -2 SD values showed in 3 patient, -1 SD ~-2 SD in 6, -1 SD ~ mean in 4, above mean values in 2 and average MFR was -1.3 SD. 2.In Group 2, postoperative average MFR was 22.3cc/sec, below -2 SD values showed in 6 patients. -2 SD∼ -1 SD in 5, -1 SD - mean in 8, above mean values in 6 and average MFR was -0.93 SD. 3.MFR values of group 1 increased an average of 2.8 SD in each patient and 80% of patients were improved in uroflowmetry after transurethral resection of prostate (TURP). 4.If MFR of acute urinary retention was below -2 SD in group 2, 76% of patients were improved in uroflowmetry after TURP.
