        불안정성 콜레스 골절에서 외고정장치 시술과 경피성 핀고정술 치료결과 비교

        The twenty-three cases of unstable Colles fracture were treated from Jan. 1994 to May 1998 at the department of orthopaedic surgery of Seoul Red Cross Hospital. Among them, the sixteen cases were treated with closed reduction with percutaneous pinning, others were treated with external fixator(Ace colles quadrilateral devices, USA). A retrospective study was made and evaluated using the Modification of Gartland and Werley's scoring system. The result of this study were as follow: 1. The ratio of male to female was 9 cases(39%) to 14 cases(61%). 2. The causes were falling down from a height 12 cases(50%), slip down 8 cases(35%) and traffic accident 3 cases(13%). 3. The reduction loss did not occur with the patients treated external fixation using Ace colles qredrilateral device, however three cases of the reduction loss have occurred with the patients using percuteneous pinning. 4. According to the Modification of Gartland and Werley's scoring system, the results of external fixation were excellent 4 cases(58%), good 1 case(14%), fair 1 case(14%) and poor 1 case(14%). and for percuteneous pinning, excellent 6 cases(38%), good 4 cases(25%), fair 2 cases(12%) and poor 4 cases(25%). 5. The complications of cases using external fixation were pin site infection 1 case and wrist stiffness 1 case, but for percutaneous pinning, reduction loss 3 cases. pin site infection 2 cases, wrist stiffness 2 cases, and decreased external rotation of forearm 3 cases.

        견봉쇄골 관절 분리 및 오구쇄골 인대의 부분파열과 동반된 오구돌기 골절 - 1례 보고 -

        A coracoid process fracture associated with acromioclavicular dislocation has been reported infrequently. A coracoid process fracture associated with coracoclavicular ligament rupture has been reported only in two cases. Two separated injury mechanisms seem to be involved in this instance; one is direct trauma to the shoulder girdle causing dislocation of acromioclavicular joint, and the other is sudden avulsive pull on coracoid process by the conjoined tendons of short head of biceps brachialis and coracobrachialis, and pectoralis minor muscle causing fracture of the coracoid process. We reported one case of coracoid process fracture associated with acromioclavicular dislocation and partial rupture of coracoclavicular ligament. The lesion was treated by open reduction and internal fixation using two Kirschner - wires for the acromioclavicular joint and a cortical screw for coracoid process, and the result was satisfactory without complication.

        국내 운항선박 내항성능해석을 위한 우리나라 해역의 파랑특성 연구

        The hull structural rule is applied to domestic voyage vessels and is almost the same as the class hull rule. The class hull rule was developed based on the North Atlantic and is also applied to small vessels with length not greater than 90m. It is more reasonable to apply wave loads developed based on South Korea to domestic voyage vessels. However, there is no information to calculate a design wave for a domestically operated ship because an analysis of the design wave conditions around South Korea has not been done yet. In this study, a design wave condition for application to domestic navigation ships was selected using a short and long-term prediction method. The wave heights corresponding to 1-year and 20-year return periods were estimated to be about 17.98m(amplitude 8.99m) and 20.68m(amplitude 10.34m). 국내 항해에 종사하는 선박의 선체구조규칙은 대부분 북대서양을 기준으로 개발된 선급 규정을 따르도록 되어있으며, 더욱이 90m 이하인 소형선박에도 이러한 가혹한 규정이 적용되고 있어 이에 대한 개정이 요구된다. 따라서 우리나라 주변만을 항행하는 선박에 대해서는 우리나라 해역을 기준으로 개발된 파랑하중을 적용할 필요가 있다. 그러나 아직까지 우리나라 해역에 대한 설계 파랑조건에 대한 분석이 이뤄지지 않아, 설계파를 산정하는데 필요한 정보가 없는 실정이다. 본 연구에서는 국내 항행선박에 적용하기 위한 설계파랑 조건을 선정하고자 하였다. 이를 위해서 파도 산포도를 바탕으로 단장기 예측을 통해 설계파를 산정하였다. 그 결과 1년과 20년에 1회 발현하는 파의 파고는 17.98 m(진폭 8.99 m), 20.68 m(진폭 10.34 m)인 것으로 산정되었다.

        Walled-off Pancreatic necrosis in a Dog

        A 7-year-old, castrated, male Maltese dog presented with hyporexia and depression for 3 days. Elevatedserum amylase, lipase activities, and liver enzyme values were found upon blood examination. An abdominal masswas seen on radiographs caudal to the gastric body in the left middle abdomen. In the left middle abdomen, abdominalultrasonography also revealed a massive, irregularly marginated, heterogeneous mass of unknown origin, and in theright cranial abdomen, heterogeneously hypoechoic pancreatic tissue and hyperechoic change of adjacent mesentericfat were observed. Contrast-enhanced computed tomography showed an irregular contour of the left pancreatic limbas well as heterogeneously enhanced parenchyma. A low-attenuating peripancreatic fluid collection with a thin andirregular wall was also seen. Based on these findings, an atypical pancreatic abscess with necrotizing pancreatitis whichmanifested as walled-off necrosis was suspected. The mass was excised, and the pancreatic abscess was confirmedby histopathologic examination. No complications were found in the patient after two months of follow-up examination.

        Double Outlet Right Ventricle in a Cat

        A 10-month-old intact male Scottish Fold was presented with cardiomegaly. The cat showed exerciseintolerance after birth. Radiographs showed cardiomegaly with bulging of the main pulmonary artery and dilation ofpulmonary arteries. Echocardiogram revealed abnormally arisen aortic root toward right ventricle with left-to-rightshunted perimembraneous ventricular septal defect located underneath the aortic root. Based on imaging studies, thecat was diagnosed as subaortic type of double outlet right ventricle (DORV) without concurrent abnormalities.

      • Preparation of dual curable acryl/epoxy resin using various acrylic monomer

        The acryl/epoxy dual curing system is a good way to improve the adhesive strength. In the acryl / epoxy dual curing system, the acrylate polymer chain is formed through the primary UV curing, and the additional curing reaction between the functional group of the acrylate and the epoxy occurs through the secondary thermal curing. The adhesion properties of the dual curable adhesive can be adjusted by the type of acrylic monomer. Butyl acrylate and 2-ethyl hexyl acrylate are used for the enhancement of adhesion. Isobornyl methacrylate and cyclo hexyl acrylate are used to improve heat resistance. Acrylic acid, 2-hydroxy ethyl acrylate, and hydroxyl propyl acrylate are used for the thermal curing with epoxy. The UV and thermal curing behaviors of the dual curable adhesives were investigated using photo-differential scanning calorimetry, fourier transform infrared spectrophotometer, and the determination of the gel fraction, pendulum hardness and adhesion strength.

        Ultrasonographic and Computed Tomographic Appearance of Spontaneous Cutaneous Fistula Resulted from Retained Surgical Gauze in a Dog

        A 6-year-old, spayed female Maltese was presented with the condition of a chronic recurrent abscess formation in the left flank region. Despite the antibiotics and drainage therapy given to the dog, the lesion formed a continued serosanguineous to the point that a purulent discharge was evident. In the meantime, an abdominal ultrasound revealed the presence of a well-defined mass with a hypoechoic outer margin, and a hyperechoic inner rim in the cranial of the kidney. A fistula was noted as being present with a connection between the subcutaneous lesion of the left flank and the abdominal mass. It is emphasized that CT scans revealed the existence of a soft tissue dense mass with low attenuation area, as seen in some internal areas and also a peripheral contrast enhancement was noted within a nonenhancing central region. There was additional nonenhancing fluid found dorsal to the inflammatory tract passing under the epaxial muscles and at the peritoneum. Likewise, the tract exited the skin surface in the left flank. A tentative diagnosis of an abdominal abscess with spontaneous cutaneous fistula was made based on the ultrasonographic and CT appearances. A foreign body such as surgical gauze should always be considered a potential cause of draining tract in small animals, as was considered to be the problem in this case.

        Freight Demand Forecasting Considering Economic Growth Factors

        This work proposes a methodology to project future freight demand for all commodity types that begin and end in each geographical region and the amount of freight that moves between all origin and destination pairs. Following the traditional four-step demand forecasting framework, the procedure corresponds to trip generation and trip distribution analysis for interregional freight demand. Using future economic growth factors from macroeconomic and input-output models, the amounts of freight production and attraction in each analysis zone are forecasted and taken as given. Subsequently, an iterative matrix balancing method is applied to determine the estimated freight shipment demand for all origin and destination zone pairs. The proposed algorithm is applied to generate predicted future freight demand within the United States from 2010-2050 in five-year increments based on the national freight demand data from 2007. Four different scenarios are proposed that consider variations in both global economic growth and environmental regulation. This study will assist transportation planners and decision makers in public and private sectors to assess how future freight delivery demand on the national scale considering various future global economic growth and environmental policy scenarios will affect various issues such as air quality and human health problems.
