In order to examine the mutagenicity of cadmium dichloride the author studied the induction of sister chromatid exchanges in chinese hamster ovary $K_1$ cells which treated with cadmium dichloride at various concentrations. The results obtained were as follows : 1. In cells treated with $10^{-4}M$ cadmium dichloride, a small number of cells were viable but no mitosis was bound. 2. The frequencies of sister chromatid exchanges in cells treated with $10^{-5}M\;and\;10^{-6}M$ cadmium dichloride as $10.7{\pm}1.9\;and\;8.3{\pm}2.1$, respectively, were significantly increased for control ($6.0{\pm}2.3$). (P<0.05). 3. There were dose-dependent relationship between the concentration of cadmium dichloride and frequency of sister chromatid exchanges in cells treated with cadmium dichloride at concentration ranging from $10^{-5}\;to\;10^{-7}M$.

방사선취급 병원근무자들의 염색체이상 및 자매염색분체교환 빈도

In order to evaluate the cytogenetic hazard among hospital workers potentially exposed to low dose of radiation, the analysis of chromosome aberrations(CA) and sister chromatid exchanges(SCE) in lymphocytes were performed in 79 hospital workers and 79 non-exposed workers. The mean frequency of chromosomal exchange and deletion(respectively, $0.20\times10^{-2}/cell\;and\;0.39\times10^{-2}/cell$) in the exposed group were significantly higher than those$(0.07\times10^{-2}/cell\;and\;0.23\times10^{-2}/cell)$ in control group. The frequency of sister chromatid exchanges was 5.04/cell in the control vs. 6.57/cell in the exposed group. There were also significant differences in the mean frequencies of CA and SCE adjusted for age, sex, smoking, drinking between two groups. There were no evidence of significant increase of CA and SCE according to the department or duration of employment. But the frequency of cells having chromosome aberration was significantly higher in the exposed group than in the control group related to duration of employment. There was no dose-effect relationship between the cumulative doses and the frequency of CA and SCE. But in the case of last 1 yr cumulative dose, there were evidence of significant dose-dependant increase of chromosome type CA and percentage of cells with aberration. The result suggest that there is cytogenetic hazard in risk group like hospital workers handling low dose radiation. And the analysis CA and SCE are useful biological indicators for the exposure of low dose level of radiation.

부비동염이후 발생한 경막하 농양 1례

부비동염은 흔하고 일반적으로 소아에서는 양성질환인 경우가 많다. 경험적으로 사용하고 있는 항생제로 인해 소아의 부비동염의 두 개강 내 합병증은 드물지만, 발생할 경우 생명에 위협적일 수 있다. 두 개강 내 합병증은 정맥동 혈전, 안와 합병증, 뇌수막염, 뇌수막하 농양 등이 있다. 이러한 합병증은 소아에서 심각한 예후를 보일 수 있다. 따라서 뇌수막하 농양을 초기에 정확하게 진단하여 치료를 시작하는 것이 중요하다. 저자들은, 급성 부비동염 치료 중이던 15세 남아가 3일간의 발열, 두통을 동반한 편마비 증상으로 내원하여 뇌 전산화 단층촬영에서 뇌수막하 농양으로 진단되어 항생제 투여 및 신경학적 수술로 치료된 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. The current paper reports on a case of subdural empyema secondary to frontal sinusitis in an otherwise healthy child. Sinusitis is a common and benign condition in most pediatric cases. Because of the widespread use of antibiotics, intracranial extension of pediatric sinusitis is rarely seen today; however, complications (e.g., cavernous sinus thrombosis, orbital infection, meningitis, and subdural empyema) are potentially life threatening. A 15-year-old right-handed male presented with a 3-day history of fever, headache, and left-sided palsy. Computed tomography revealed right-sided subdural empyema with right frontal sinusitis and maxillary sinusitis. A postoperative inpatient neurological consultation was requested 2 months post-surgery due to motor function deficits. The results suggested that early and accurate diagnosis of subdural empyema leads to prompt treatment and a favorable outcome for the patient.

균형성과표를 이용한 응급의료센터 핵심성과지표 개발

현재 우리나라 응급의료센터를 대상으로 균형성과표를 적용하거나 도입을 검토한 사례는 없다. 이 연구의 목적은 균형성과표(Balanced Score Card, BSC)를 활용하여 응급의료센터의 핵심성과지표(Key Performance Indicator, KPI)를 개발하는 데 있다. 이를 위해 문헌고찰과 실무자회의를 거쳐 응급의료센터를 위한 30개의 KPI 후보군을 선정하였으며, 선정된 KPI의 타당성 검증과 연구모형의 변수 간 관련성 검증을 위해 전문가를 대상으로 총 232부의 설문지를 회수하여 분석하였다. 분석은 AMOS 7.0의 통계패키지를 활용한 구조방정식 모형을 통해 수행하였다. 연구 결과 재무 관점 5개, 고객 관점 7개, 학습·성장 관점 8개, 내부 프로세스 관점 6개 등 총 26개의 KPI가 최종 선정되었다. BSC 관점별 비중이 상대적으로 높은 KPI는 재무 관점에서는 전문의 1인당 의료수입, 고객 관점은 고객 배려와 의사의 전문성, 학습·성장 관점은 의사의 연수교육 이수와 의료진의 논문발표 수였다. 내부 프로세스 관점에서는 6개의 KPI가 유사한 비중을 보였다. 또한, 연구모형에서 제시한 관련성을 검증한 결과 대부분 유의한 관계를 보였으나 재무적 관점의 관련성 중 일부는 유효하지 않았다. 이 연구에서 개발된 BSC 관점별 KPI는 개발 단계에서부터 연구자뿐만 아니라 실무자도 참여하였고, 나아가 응급의료전문가를 대상으로 설문조사를 실시하여 타당성 검증을 하였기 때문에 유사한 환경에 있는 응급의료센터에 일반화하여 적용 가능하며, 응급의료서비스 질 향상에 긍정적인 영향을 미칠 것으로 기대된다. A research in evaluating performance of emergency medical center with application of Balanced Score Card (BSC) is in the beginning stage. The purpose of this study is to develop Key Performance Indicators (KPI) for emergency medical centers adopting BSC. Through literature review and brown-bag meetings with staffs at K hospital, the study refined 30 KPIs for emergency medical center. The study confirmed the validity of KPIs by analyzing 232 copies of questionnaires collected for the analysis. Based on the analysis, 26 KPIs has been finally chosen, 5 KPIs in financial, 7 KPIs in customers, 8 KPIs in learning & growth, and 6 KPIs in internal business processes perspective. Also, the study showed KPIs with higher weights in each dimension of BSC. Finally, the study tested a statistical significance of hypothesized relationship through an structural equations model by utilizing the statistical package of AMOS 7.0. 13 among 22 hypothesized relationship were accepted. The study proposes an effective performance indicators for professionals at emergency medical centers and it is expected to be likely to have positive influence upon enhancing services of the center.

부산지역 거주 노인의 인지기능장애 및 치매 유병률

Objectives : To investigate the prevalence of cognitive impairment and dementia in elderly people, aged 65 or above, residing in Busan Metropolitan City. Methods : Total of 1,101 old people, aged 65 or above, living in Busan as of December 31, 2001 were selected using stratified three-stage cluster sampling. Cognitive impairment was determined from the MMSE-K score, and dementia confirmed from five psychometric measures and the Barthel index. The crude prevalence, sex-age adjusted for the Korean population, were obtained. Results : With the cut-off point for cognitive impairment was set at 24 points, or below, on the MMSE-K scale, the crude rate of cognitive impairment was 29.3% (15.7% for men and 37.5% for women), and the sex-age adjusted prevalence was 30.5% (17.5% for men and 37.0% for women). When the cut-off point for cognitive impairment was set at 20 points, or below, on the MMSE-K scale, the crude rate of cognitive impairments were 10.0% (4.1% for men and 13.5% for women), and 10.6% (4.7% for men and 13.1% for women), respectively. The crude dementia, and the sex-age adjusted rates were 7.4% (2.4% for men and 10.5% for women), and 8.0% (2.7% for men and 10.0% for women), respectively. Conclusions : The prevalence of dementia in this study was somewhat lower than that reported by other domestic and foreign studies. Our results related to the difference in time and space, diagnostic tools, response rates, and distribution of male and female subjects, etc.

복합만성질환의 흔한 유형과 의료비에 미치는 영향

Background: Current trends in Korea population aging with advances in public health and clinical medicine foretell rises in the prevalence of not only chronic diseases but also patients with multimorbidity. One important aspect in analyzing multimorbidity is to define the list of chronic diseases included when calculating multimorbidity index. The objective of this study is to describing the effect of multimorbidity on healthcare cost in Korea using US Office of the Assistant Secretary of Health (OASH) list. Methods: We analyzed the Korea Health Panel Data representing non-institutionalized Korean adult populations aged 20 and more. We calculated multimorbidity index based on OASH list and estimated the prevalence and healthcare cost for each OASH chronic disease. Results: In 2011, 15.2 million (39.6%) Koreans aged 20 and more were living with chronic condition. The health care cost due to chronic diseases, accounted for 80.2% of the overall healthcare costs and the prevalence of chronic conditions, the prevalence of multimorbidity and healthcare cost increased with ages. In the analysis using OASH list, 40% of the adult population over the age of 20 and 66.7% of the population over the age of 65 was affected with multimorbidity. In most of diseases in OASH list, prevalence of mulitmorbidity was high and healthcare cost increased with multimorbidity. Conclusion: OASH chronic disease list that accounts for 72.4% of prevalence and 86.7% of healthcare cost of persons with chronic conditions in Korea. OASH chronic disease list would be a useful and representative indicator for studying multimorbidity.

The aim of our study was to investigate the chemical composition of the Asian ginseng seed (Panax ginseng C.A. Meyer) and the American ginseng seed (Panax quinquifolium L.) grown in Korea (3 years, KGS3; 4 years, KGS4), China (4 years, CGS4), and USA (4 years, AGS4). AGS had the heaviest 100-seed weight (4.21±0.31 g). The approximate compositions of the ginseng seeds were 13.66-17.00% crude protein, 2.21-8.65% crude ash, 19.06-24.06% crude lipid, and 43.21-47.49% crude fiber. The mineral contents of the ginseng seeds were greater in order of K>P>Ca>Mg>Fe>Na>Zn >Cu. The unsaturated fatty acid content was 96.71-96.94%, and the major fatty acids oleic acid and linoleic acid were present. Total sugar content was 15.00-26.17 ㎎ glucose/g. The acidic polysaccharide content was 0.56-0.80 ㎎ β-Dgalacturonic acid/g. These results showed the differences in the physicochemical characteristics of ginseng seeds with respect to cultivation location, cultivation year, and species.
