        컴퓨터 응용 인공고관절 모사 시술 방법 연구

        현재 인공고관절 전치환 수술 과정의 높은 난이도 때문에 숙련된 의사의 경험에 상당히 의존하고 있는 실정이다. 비구컵과 대퇴 임플란트의 위치 선정은 수술의 성공 여부와 밀접한 관계가 있지만 아직까지는 의사의 눈대중에 의해 이루어지는 경우가 많다. 이러한 이유로 인하여 인공삽입물(비구컵, 대퇴시스템) 삽입의 정확성이 낮아서 재치환술을 해야 하는 경우가 자주 발생한다. 본 연구에서는 환자 골반골(pelvis)과 대퇴골(femur)의 3차원적인 고유 기하학적 정보를 이용하는 모사 시술을 통하여 비구컵과 대퇴 임플란트의 정확한 위치를 선정하는 방법을 결정하였다. 이를 위하여 우선 비구컵의 위치 변화에 따른 대퇴 전염각 및 대퇴 경간각의 변화 양상을 살펴보았고 또한 대퇴 임플란트의 삽입 위치에 따른 대퇴스템 경부 길이 및 대퇴스템각의 변화 양상을 조사하였다. 본 연구에서 다양한 모사 시술을 통한 기하학적 정보의 분석 결과 비구컵과 대퇴 임플란트의 정확한 위치 선정이 가능함을 확인하였다. 본 연구의 모사 시술 방법을 이용하여 의사가 인공고관절 수술을 선행한다면 수술의 정확도와 숙련도를 향상시킬 수 있을 것으로 기대된다. Total hip arthroplasty(THA) considerably depends on high-experienced doctors because of high difficulty of the operation. Selection of acetabular cup's and femoral implant's position is closely related with success or failure of THA. Nevertheless the selection has usually depended on doctor's eye measurement, which makes the position accuracy of artificial joint lower after THA, often resulting in revision of THA. The present study determined a method to select accurately the position of acetabular cup and femoral implant through surgical simulation with 3D characteristic geometrical information of patient's pelvis and femur. We examined the change of femoral anteversion angle and neck-shaft angle accompanied by the change of acetabular cup's position and the insertion position of femoral implant. As result of analyzing geometrical information through different surgical simulations, we found that it was possible to select the accurate position of acetabular cup and femoral implant. It is expected to help doctors get experienced in THA operation through repetitive surgical simulations using the method suggested in the study.

        어린이 성장판 영상화를 위한 초음파와 X-선 방식의 비교 평가

        본 연구에서는 진단 방법의 정확성이 아직까지 구체적으로 규명되지 않고 있는 어린이의 성장 발육을 체계적이고 과학적으로 측정할 수 있는 방법을 제시하고자 시도하였다. 현재 많이 이용되고 있는 성장 진단 방법은 X-ray 영상을 이용한 각 관절 부위의 성장판의 개폐 분석이다. 그러나 X-ray 방식은 성장 진단에서 요구되는 주기적이고 반복적인 측정을 하지 못함과 동시에 방사선 노출이라는 단점을 가지고 있다. 따라서 인체에 무해한 초음파 영상을 이용한 성장판 개폐 분석 대체 가능성을 평가하기 위해 종골의 초음파 광역 감쇄 영상을 이용한 프로파일 분석 및 성장판 영상 분석 알고리즘을 제시하였다. 본 연구에서는 7세~16세의 어린이 269명에 대해서 X-ray를 이용하여 무릎, 손가락, 종골에서의 성장판 영상을 얻었다. 또한 초음파 방식을 이용하여 종골의 성장판 영상을 얻었다. 각 관절에서의 성장판 개폐 유무 시기는 각 개인별로 측정 방법에 따라 조금의 차이는 있으나 전반적으로 일치하였다. 또한. X-ray방식과 초음파 방식에 의한 성장판 영상을 비교한 결과 상호 높은 연관성을 나타내었다. 이와 같이 성장판을 정확하게 측정하기 위해 도입된 초음파 프로파일 분석 알고리즘은 기존의 X-ray 영상 방식에 의한 성장판의 측정을 대체할 수 있을 것으로 사료된다. The purpose of this study was to suggest a systematic and scientific method for measurement of children's growth development, in which the accuracy of existing diagnosis method has not been concretely examined yet. The most popular method for diagnosis of children's growth is to analyze the opening degree of growth plate in each joint by X-ray image. However, X-ray method has some disadvantages; it is impossible to measure the diagnosis of growth periodically and repeatedly due to the radiation problem. Hence, this study introduced a profile analysis and the algorithm of analyzing the image of growth plate with the BUA(Broadband Ultrasound Attenuation) of calcaneus, to verify the possibility of alternative ultrasonic method harmless to human body. We obtained the images of growth plate in proximal tibiae, phalanges, and calcanei of 269 children (7∼16 years old) with X-ray. And the image of growth plate in calcanei was also obtained front those children with ultrasound. The results showed that the time of the opening degree of growth plate in each joint was almost consistent between X-ray and ultrasonic images. Also, the images of growth plate measured by X-ray and ultrasound showed the high correlation. Therefore, it is expected that the algorithm of ultrasonic profile analysis introduced in this study can replace the existing X-ray method to measure the growth plate correctly.

        초음파 골밀도 측정에서 재현성 향상 방법에 관한 연구

        It is very important to achieve a high reproducibility in the ultrasonic measurement of bone mineral density. In this study, we examined number of sampling waveform, control of temperature, diameter of region of interest as factors to improve reproducibility. We decided the optimal number of waveforms to be converted to frequency domain as period of 1. We have minimized the effects of variable temperature and constrained generation of micro bubble by keeping temperature within a range of $32\pm0.5^{\circ}C$ with a precise temperature controlling algorithm. We also found the optimal diameter of region of interest to be 13mm. In this paper, we demonstrated the improved reproducibility by controlling various factors affecting the ultrasonic measurement of bone mineral density.

        내부 펌프의 새로운 로버 설계에 관한 연구

        A internal lobe pump is suitable for oil hydraulics of machine tools, automotive engines, compressors, constructions and other various applications. In particular, the pump is an essential machine element of an automotive engine to feed lubricant oil. The subject of this paper is the theoretical analysis of internal lobe pump whose the main components are the rotors: usually the outer one is characterized by lobes with circular shape, while the inner rotor profile is determined as conjugate to the other. The topic of this paper is the design of a new rotor, which is based on specific performance as different types depending on the shape of the lobe of the outer rotor. First, the design of internal lobe pumps with circular, elliptical, and their combined lobe profiles is considered. The latter is a new type of lobe profile with special shape whose curvature follows a definite function. Then we introduce the performance indexes used for the comparison. Some of these indexes, such as flow rate and flow rate irregularity, are commonly used for the comparison, while specific slipping is particularly suitable in this case. It is possible to notice that the circular and elliptical type is comparable to the circular one or the elliptical one in terms of flow rate irregularity, but has improved performance in terms of specific slipping. Results obtained from the analysis enable the designer and manufacturer of oil pump to be more efficient in this field.

        There are many methods to assemble various parts of a product, and one of them is the bolting system widely used in a industrial field due to the merits; easiness to obtain strong tightening force, simplicity of assemble or disassemble in order to repair, substitution or cleaning, and so on. But this bolting system needs attachments not to let a bolt loose and they are an important factor to cost a great deal. In this study, some equations are suggested and FE analyses are carried out to verify the cause of the bolt looseness occurring in the tightening process. And because the number of bolts in the bolting system has been decided by empirical know-how of designers in the field, safety rate in the plate type heat exchanger is often too high. Therefore the equations to decrease in quantity are suggested in consideration of the relationship between a critical shearing force acting on the screw and a normal force acting on the cooling plate by the working fluid.

        원저 : 저신장과 비만도의 상관성 분석 -저신장을 주소로 내원한 환아 명을 대상으로-

        Objectives : The purpose of this study was to investigate correlation between short stature and obese degree. And we also wanted to know the characteristics of patients visited clinic in the name of ``short stature``. Methods : Height, body weight, BMI, fat mass, lean body mass, percent body fat were taken intended for 236 patients in the name of short stature. To all the patient questionnaire for growth clinic was drawn up, and ultrasound scan was taken through calcaneous of the right foot. The patients were classified to three groups - short, average, tall group - according to relative position of mid-parental height. It was analysed that the differences between groups in obese degree. Also was investigated correlation between position of short stature and obese degree, and between obese degree and bony maturity. Results & Conclusion : 1. The average ages of patients in the name of ``short stature`` were 12.69±3.93 years old in boys, 10.66±3.67 years old in girls. And it seemed to be just before second rapid maturing period. 2. The average BMI were 20.58±4.07kg/m2 in boys, 18.65±2.85kg/m2in girls, and average percent body fat were 21.99±7.35% in boys, 26.01±6.35% in girls. 3. The numbers of obese children were 34(31.2%) in boys, 19(14.9%) in girls on the basis of BMI. And the numbers were 39(35.8%) in boys, 53(41.7%) in girls on the basis of percent body fat. There was a big difference in case of girls. 4. The numbers of AG(average group) were 48(44.0%) in boys, 60(47.2%) in girls, the numbers of SG(short group) were 35(32.1%) in boys, 31(24.4%) in girls, and numbers of TG(tall group) were 26(23.9%) in boys, 35(27.6%) in girls. 5. There were no significant differences among the groups in BMI, lean body mass, fat mass, percent body fat. Only significant difference in DI(disease index). 6. There were no significant correlation between PH(percent height) and BMI, leanbody mass, fat mass, percent body fat. Only significant correlation in DI(disease index). 7. There were no significant differences between DA(difference between bone age and chronorogical age) and BMI, leanbody mass, percent body fat. Only significant correlation in fat mass.

        저신장을 주소로 내원한 환자들의 모발내 중금속 함량분석
