        두경부암에서 방사면역치료의 역할

        Radioimmunotherapy (RIT) is a therapy that takes advantage of the “cross-fire” effect ofemitted radiation by radionuclides conjugated to tumor-directed monoclonal antibodies (mAb)(including those fragments) or peptides. While RIT has been successfully employed for thetreatment of lymphoma, mostly with radiolabeled antibodies against CD20 [90yttrium (90Y)-ibritumomab tiuxetan; Zevalin® and 131iodine (131I)-tositumomab; Bexxar®], its use in solid tumorsis more challenging, so far. Immuno-PET, a tool for tracking and quantification of mAbswith PET in vivo, is an exciting novel option to improve diagnostic imaging and guide mAbbasedtherapy. RIT in solid tumors including head and neck cancer may be an alternative treatmentwith advances in various biological, chemical, and treatment procedures, and it may helpto reduce unnecessary exposure and enhance the therapeutic efficacy. Also, immuno-PET basedon RIT might play an important role in cancer staging, in patients or targets selection of targetedtherapeutics and in monitoring the response of targeted therapeutics as precision medicine. In this review, fundamentals of RIT/immune-PET and current knowledge of the preclinical/clinical trials in RIT for solid tumor including head and neck cancer are reviewed.

        동적 외비밸브 허탈의 진단과 치료: 비익강화이식과 Alar Flaring Suture의 활용

        In contrast to Caucasians, dynamic collapse of external nasal valve is not a common cause of nasal obstruction in Asians. Nevertheless, Asians with thin skin and weak nasal cartilages can have such problems. To diagnose these patients, visual inspection of external nose during forced inspiration is most important, which physicians frequently neglect. If dynamic collapse is suspected, physicians should locate the maximal site of collapse through modified cottle maneuver. The mainstay of treatment is proper reinforcement of main site of collapse with cartilage graft or various suture techniques. This article presents diagnosis and successful surgical treatments of two cases of dynamic external valve collapse with alar batten graft and alar flaring suture technique.         Comparative Study of Unilateral Axillo-Breast Approach with Gas Insufflation Versus Gasless Transaxillary Approach for Endoscopic Thyroidectomy in a Single Institute

        Background and Objectives There have been various approaches introduced for endoscopic thyroidectomy. This study evaluates and compares the surgical outcomes of two such approaches: the unilateral axillo-breast approach (UABA) with gas and the gasless transaxillary approach (TA). Subjects and Method We retrospectively analyzed 279 patients who underwent endoscopic thyroidectomy via UABA with gas or gasless TA from March 2008 to August 2012. Studied variables were clinicopathologic data, surgical outcomes, complications, and cosmetic satisfactions. Results Of the 279 patients, 195 (69.9%) underwent UABA with gas and 84 (30.1%) underwent gasless TA. All of the variables related to clinicopathologic characteristics showed no significant differences between the two groups. The mean operation time was significantly shorter in the UABA with gas group (131.76±44.37 min) than in the gasless TA group (191.01± 55.90 min) (p<0.001). The mean postoperative pain Visual Analogue Scale scores in the UABA with gas group were 2.61±0.96 and 1.85±0.79, respectively, and those in the gasless TA group were 3.12±1.02 and 2.17±0.76, respectively, at 1 and 3 days after surgery. Incidences of postoperative complications were similar except for the higher rate of seroma in the gasless TA group. Cosmetic satisfaction scores of UABA with gas were higher than those of the gasless TA. Conclusion UABA with gas may be a good option for endoscopic thyroidectomy because this approach is less invasive than the gasless TA is. Korean J Otorhinolaryngol-Head Neck Surg 2016;59(12):848-55

        Simultaneous Occurrence of Chromophobe Renal Cell Carcinoma and Urothelial Carcinoma in the Same Kidney

        The simultaneous occurrence of a renal cell carcinoma and a urothelial carcinoma in the same kidney is uncommon. Here we report the case of a 79-year-old woman with ipsilateral synchronous renal cell carcinoma and urothelial carcinoma. She was referred to our hospital for gross hematuria and right flank pain. A computed tomography scan showed a 15x20 mm enhanced lesion on the upper calyx and a 12x15 mm mass on the lateral aspect of the right kidney. We thus suspected a renal pelvis tumor and performed right hand assisted laparoscopic nephroureterectomy with bladder cuff excision (HALSNU). Gross findings were multiple, pale yellowish papillary masses on the upper and lower major calices, of which the largest one measured 16x20 mm. A separated solid mass measuring 12x16 mm was also noted on the anterior midportion of the kidney. The former was a urothelial carcinoma and the latter was a chromophobe renal cell carcinoma. We present a rare case of a chromophobe renal cell carcinoma and a urothelial carcinoma in the same kidney.

        전립샘암 세포주에서 β-Catenin 및 Matrix Metalloproteinase-7의 발현에 대한 Decursin의 효과

        Purpose: Alterations in the Wnt/β-catenin pathway are associated with the development and progression of human prostate cancer. Decursin can attenuate the Wnt/β-catenin pathway. We investigated the relationship between the Wnt/β-catenin pathway and decursin in prostate cancer cells. Materials and Methods: PC-3 and LNCaP cell lines were used. Cell viability was measured with methyl-thiazole tetrazolium bromide(MTT) assays, and cell apoptosis analysis was performed by FACScan. The amount of β-catenin protein after treatment with decursin was measured by Western blot analysis. Expression of MMP-7 mRNA was detected by real-time polymerase chain reaction(RT-PCR). Results: Death and apoptosis were increased after treatment with decursin 0.5-100μM in PC-3 and LNCaP cells. This was revealed dose and time-dependent increase of cancer cell death on 24, 48 and 72 hours. FACScan showed an increment of apoptosis on 24, 48 hours. Expression of intracellular β-catenin protein was decreased dose-dependently in both of prostate cancer cell lines. Decursin reduced MMP-7 mRNA expression on 6, 12, 24, 48 hours dose-dependently. Conclusions: Decursin affects the viability of prostate cancer cells. Increased cancer cell death was associated with increased apoptosis. This study suggests that decursin may play a role in the treatment of prostate cancer.         Novel Information on Anatomic Factors Causing Grasp Reflex in Frontal Lobe Infarction: A Case Report

        We report a patient with a severe limitation of function in the right hand resulting from grasp reflex following a stroke affecting the left anterior cerebral artery region. We describe, using diffusion tensor tractography (DTT), a disconnection between the bilateral frontal lobes via the corpus callosum. The patient could not control his right hand at all, even though his bilateral corticospinal tracts were intact. We noted that over the infarcted lesion on DTT, the white matter was invisible from the corpus callosum to the prefrontal cortex. These findings reflected a unique pattern of white-matter disconnection between the ipsilateral medial frontal lobe and ipsilateral and contralateral frontal cortex causing hand function deterioration in the form of severe grasp reflex.

        ADI-FDTD 방법을 이용한 3차원 인터커넥트 모델링

        본 논문에서는 3차원 인터커넥트(3-D interconnect) 구조를 해석하기 위하여 ADI-유한차분시간영역(ADI-FDTD, Alternating Direction Implicit Finite Difference Time Domain)방법으로 맥스웰 회전 방정식(Maxwell's curl equation)을 계산하는 수치 해석 모델을 개발하였다. 3차원 인터커넥트 모델내의 전자기파 문제를 해석하기 위하여 맥스웰 회전 방정식을 ADI-유한차분시간영역방법으로 이산화 하였으며, ADI-유한차분시간영역의 경계에서 발생하는 반사파를 해결하기 위하여 흡수 경계 조건인 완전 정합 층 방법(PML, Perfectly Matched Layer)을 도입하였다. 개발한 ADI-유한차분시간영역방법 및 완전 정합 층의 수치 모델을 검증하기 위하여 3차원 마이크로스트립 전송선(microstrip transmission line) 구조를 3차원 그리드(grid) 구조로 모델링한 후, 시간영역에서 전계 분포를 컴퓨터로 모의 실험하였다. 그리고 본 논문에서 제안한 ADI-유한차분시간영역방법과 종래의 스탠다드 유한차분시간영역방법의 수치적 성능을 정량적으로 비교, 분석하였다. In this paper, we developed a numerical analysis model by using ADI-FDTD method to analyze three-dimensional interconnect structure. We discretized maxwell's curl equation by using ADI-FDTD. We introduced PML(Perfectly Matched Layer) absorbing boundary condition to solve the effect of the reflected wave at the interface. Evaluting the numerical model of PML and ADI-FDTD, we simulated the electric field distribution in time domain. We compare standard FDTD with ADI-FDTD, and analysis the result.
