        하인두 편평상피암종의 경부 림프절 전이의 유형 및 의의

        Background and Objectives The aim of this study was to evaluate the incidence and prognostic significance of cervical lymph node metastasis in squamous cell carcinoma (SCC) of the hypopharynx. Subjects and Method A retrospective review of the 64 patients who were previously untreated for SCC of the hypopharynx and underwent surgery was performed from October 1993 to June 2008. Fifty-six patients had simultaneous bilateral neck dissection, whereas eight had unilateral neck dissection. Results The median age was 61.0 years (range, 34-75 years) for the study group consisting of 62 males and two females. Evaluating according to the N stages, there were 15 (23.4%), 10 (15.6%), 37 (57.8%), and 2 (3.1%) cases with N0, N1, N2 and N3, respectively. Forty-nine patients (76.6%) had pathologically proven cervical metastasis. Contralateral occult lymph node metastasis occurred in 20.5%. Ipsilateral and contralateral occult metastasis rates for clinically node negative patients were 41.2% and 11.8%, respectively. The most frequent sites for positive neck nodes occuring at each level were as follows: II (48.5%), level III (40.6%), level IV (26.6%), paratracheal node (21.4%), level V (9.4%), levels I (7.8%), and retropharyngeal node (6.3%). The 5-year overall survival and disease-specific survival rates were 50% and 53%, respectively. Cervical nodal metastasis (p=0.044) was statistically significant prognostic factors for disease-specific survival. Conclusion Metastasis to the cervical lymph node group is very frequent and has an impact on survival in patients with hypopharyngeal SCC. Therefore, we advocate bilateral neck dissection in patients with hypopharyngeal SCC with clinically positive metastasis. Ipsilateral elective neck dissection may be needed for clinically node negative patients.

        Eigenface를 이용한 인간의 감정인식 시스템

        Emotions recognition is a topic on which little research has been done to date. This paper proposes a new method that can recognize the human's emotion from facial image by using eigenspace. To do so, first, we get the face image by using the skin color from the original color image acquired by CCD color camera. Second, we get the vector image which is projected the obtained face image into eigenspace. And then, we propose the method for finding out each person's identification and emotion from the weight of vector image. Finally, we show the practical application possibility of the proposed method through the experiment. 본 논문에서는 다양한 환경하에서 인간의 식별과 감정을 인식할 수 있는 감정 인식 알고리즘을 제안한다. 제안된 알고리즘을 구현하기 위해, 먼저, CCD 칼라 카메라에 의해 획득한 원 영상으로부터 피부색을 이용해 얼굴영상을 얻는 과정을 거친다. 그 다음, 주요 요소분석을 기본으로 하는 얼굴인식기술인 Eigenface를 사용하여 이미지들을 고차원의 픽셀공간으로부터 저차원공간으로의 변환하는 과정을 거친다. 제안된 개인에 대한 식별과 감성인식은 사용한 특징벡터들의 추출로 인한 Eigenface의 가중치와 상관관계를 통해 이루어진다. 즉, 영상의 가중치로부터 개인에 대한 식별과 감성정보를 찾는 방법을 제안한다. 마지막으로, 실험을 통해 제안된 방법의 응용가능성을 보인다.

        양측 안면 신경 마비를 동반한 Wegener씨 육아종 1예

        Wegeners granulomatosis is a granulomatous inflamatory proces with necrotizing vasculitis. It can involve any organ in body, granulomatosis presenting as bilateral facial paralysis. A 54-year-old man was sufering from nasal stufiness and rhinorrhea and treated with antibiotics. In spite of the treatment, the nasal symptoms were agravated and left facial paralysis was developed suddenly with left hearing impairment. Diagnosis as Wegeners granulomatosis was suported by biopsy from nasal cavity. He right facial paralysis was emerged abruptly. At 6 months after discharge, his facial nerve continued to function at a House-Brackmann grade II level. (Korean J Otorhinolaryngol-Head Neck Surg 2007 ;50 :822-5)

        갑상선에서 발생한 편평상피세포-유두상 동시 암종 1예

        The simultaneous occurrence of two distinct neoplasms derived from different cells of origin is sometimes recognized. In the thyroid gland, two such distinct tumors could consist of a concurrent squamous cell carcinoma and well-differentiated carcinoma showing follicular epithelial cell differentiation. Because the squamous cell carcinoma is characterized by rapidly progressive clinical course in spite of its differentiated morphologic feature, the tumor should be treated more vigorously at its initial stage. We report a rare, interesting case of concurrent squamous cell and papillary carcinoma of the thyroid gland. A 69-year-old male underwent a total thyroidectomy, central neck dissection and bilaterally modified radical neck dissection, which was diagnosed as papillary carcinoma by ultrasonography-guided aspiration cytology. The pathological report of the surgical specimen of the thyroid gland revealed synchronous squamous cell carcinoma and papillary carcinoma. He received local radiotherapy but died of pneumonia eight months after surgery. (Korean J Otorhinolaryngol-Head Neck Surg 2008;51:1065-7)

        인터넷 기반 원격제어를 위한 임의의 시간지연을 갖는 지능형 제어기의 설계

        본 논문은 인터넷 상에서 임의로 변화하는 입력지연을 갖는 불확실 퍼지 시스템의 지능형 강인 퍼지 제어기 설계를 논의한다. 임의로 변화하는 입력지연은 유한개의 상태를 갖는 마코프 확률과정으로 표현된다. 디지털 안정화기를 설계하기 위하여 연속시간 Takagi-Sugeno 퍼지 시스템을 이산화하며 제어기의 입출력단에 영차의 샘플/홀드 함수를 가정한다.이산화된 시스템은 확률적 과정에 따라 변화하는 도약 시스템으로 표현된다. 확률적 강인 안정가능성 조건은 선형 행렬 부등식의 형태로 표현된다. This paper discusses a design of intelligent controller with time delay for Internet-based remote control. The finite Markovian process is adopted to model the input delay of the overall control system. It is assumed that the zero and hold devices are used for control input. The Takagi-Sugeno (T-S) fuzzy system with uncertain input delay is utilized to represent nonlinear plant. The continuous-time T-S fuzzy system with the Markovian input delay is discretized for easy handling delay, accordingly, the discretized T-S fuzzy system is represented by a discrete-time T-S fuzzy system with jumping parameters. The robust stochastic stabilizibility of the jump T-S fuzzy system is derived and formulated in terms of linear matrix inequalities (LMIs). An experimental results is provided to visualize the feasibility of the proposed method.

        땅속작물 수확기의 지능형 제어 시스템 개발

        In this paper, we study the system of intelligent underground crop harvester with various control functions. Current underground crop harvester machines are a one-way control system, in which all the operating principle depends entirely on the PTO power coming from the tractor, so there is a limit to the source that can be controlled. Intelligent underground crop harvester development research is to integrate electronic control technology into existing unidirectional mechanical structure, and to implement various functions such as the digging, sorting and collecting function according to the need of the user. If the various controls can be realized in real time according to the environment and crops, it will be possible to improve productivity and convenience in dry-field farming and to develop unmanned agricultural machinery and automatic control in the long term.
