        병원 약제부업무와 병원약국경영

        약학인은 수요자인 국민을 먼저 생각하는 즉, 국민복지 차원에서 순수약학의 학문적 양심과 순수성을 갖는 것이 대단히 중요하다. 이러한 점에서 볼 때 우리나라는 우선 국민을 위한 정부차원의 복지정책 문제점의 해결이 시급하다. 현재의 의료체계는 정부가 단시일내에 도입된 전 국민 의료보험의 실시로 제도적으로는 선진국형이나 실제로는 과도기적 단계에서 발전하지 못하고 생명을 중요시하는 환자진료라는 측면에서 수정, 개선되어야 할 점이 너무나 많은 실정이다. 약학대학은 현시점을 직시하고 21세기를 향하는 교육개편으로 대학이 약학의 주체가 될 수 있고, 약사가 의료인으로서의 자질을 갖출 수 있도록 마음을 합하여 매진하여야 할 것이다. 이러한 완벽한 틀 속에서 약사가 맡은바 사명을 다할 수 있다면 이것이 바로 국민이 원하는 의료서비스일 것이다. 우리는 국민이 있기에, 수요자가 있기에 정부기관도 있고, 대학도 있고, 약사란 직업도 있다는 것을 명심하여야 할 것이다. 우리 자신들이 중요하다면 국민에게 최선을 다하는 것은 너무도 당연한 일이다. 이러한 당연한 일을 놓고 왜 논해야 하는지를 깊이 반성하고 우리 모두 힘을 합하는 것이 바로 의료서비스인 것이다. 정부관련기관, 약학대학, 전문분야에 있는 약사들의 삼자는 반드시 한마음이 되어야 하며, 권위주의적이어서는 안되고 겸허한 마음으로 국민에게 최선을 다하는 것만이 결국 국민보건의 질을 높이는데 큰 역할을 하게 될 것이다.

        Filgrastim의 약물사용 평가

        A retrospective drug use evaluation (DUE) study on filgrastim was conducted by reviewing patients charts following criteria for drug use evaluation established by American Society Hospital Pharmacy and add to the standard indicators of ministry of Health & Wellfare by Korean Government. Total 33 charts of inpatients treated with filgrastim in Korea University Anam Hospital from jan. 1, 1995 to July 30, 1995 were retrospectively reviewed. As a results, 33cases(100%) met with the criteria for the justification of use. In analysis indicators including allergy test and complete blood count(CBC) monitoring showed the accepted levels 93.9%, 97%, respectively. In appropriated dose, 21.2% was documented 5mcg/㎏/day of the standard indicators of Am. S. Hosp. Pharm. And 48.5% to 50㎎/㎡/ day of the standard indicators of Ministry of Health & Wellfare by Korean Government. Indicator of filgrastim continued until patient achives absolute neutrophil count (ANC) of≥10,000cell/㎣by Am. S. Hosp. Pharm. was documented 15.2% and started with ANC of <500㎣ and discontinued to ≥1,000㎣ with Ministry of Health & Wellfare was documented 15.2%. Bone pain of filgrastim complication was 12.1%. In Outcome analysis, decreased incidence and severity of infection was 75.8% and antineoplastics given 100% scheduled. Reduction in duration or severity of ANC nadir was documented 62%. To be used effectively and safely, filgrastim is recommended to be adjusted to a standard level of dosage and an use limit by Korean Government.

      • 4,5-Dihydro-6-[3-nitro-4-methoxyphenyl]-5-methyl-3-[2H]-pyridazinone과 Rolipram의 혈소판응집 억제작용

        The inhibitory effects of two types of phosphodiesterase(PDE) inhibitors(DMP and rolipram) on thrombin-stimulated platelet aggregation were investigated using rabbit platelets. The effect of these PDE inhibitors on ATP secretion, intracellular Ca^2+ concentration and the contents of cAMP, cGMP, and IP_3 in the platelets were also studied in order to elucidate the action mechanisms of these PDE inhibitors. 50% Inhibitory concentrations(IC_50) of DMP and rolipram on aggregation were 2.21x10^-7 and 8.11x10^-4M, IC_50 of DMP and rolipram on ATP secretion were 2.01x10^-7, 7.35x10^-4, IC_50 of DMP and rolipram on intracellular Ca^2+ concentration were 2.21x10^-7 and 8.11x10^-4M, DMP sigificnatly increased cAMP content, but did not affect cGMP and IP_3 contents. Rolipram increased cGMP content significantly, but did not affect cAMP content. These results suggest that the increased intracellular IP_3 content caused by thrombin-stimulated ATP release and thereby resulted in platelet aggregation. On the other hand, DMP, a new PDE Ⅲ inhibitor is believed to activate protein kinase A by increasing cAMP content and thereby either to release or store the intracellular free Ca^2+ concentration is believed to decrease ATP release and thereby to inhibit platelet aggregation.

        병원약사의 의식구조에 관한 조사

        This study was designed to analyze the way of thinking of hospital pharmacists. The purpose of the study was to investigate professional sense and difficulties of hospital pharmacists, and suggest for an improvement. This study was carried out by making up a questionnaire to hospital pharmacists who worked on university hospitals and general hospitals with more than a hundred beds in Seoul, Pusan, Incheon, Daegu, Daejeon, and Kwangju, The questionnaires were returned from 839 hospital pharmacists and analyzed. The results were as follows. Only 13.2% of hospital pharmacists were satisfied with their hospitals. The main cause of unsatisfaction was their small pay (60.3%). So 74.1% of them demanded a raise of salay and the improvement of labor conditions. Nevertheless, 66.5% of them preferred to resign the right rather than to push on their demand to the last. Moreover, only 25% of them joined a labor union. 73.8% of them ere not attached to their job at all and did not want to become a pharmacist if they have a chance to choose a job again. Only 26.3% of them thought their positions as a lifelong job. About a half of them wanted to go a postgraduate course in future and pointed out to accumulate a store of knowledgement and information via activation of Drug Information Center. However, many of them usually had not joined an seminar, SIG and other academic meetings. In conclusion, hospital pharmacists were depressed with a low pay and bad labor conditions such as overwork. We suggest the Korean Society of Hospital Pharmacists have to make an effort to improve the right of hospital pharmacists and make programs to assure and elevate the quality of them. Additionally, the hospital pharmacists themselves should make an effort to improve the their right and cultivate their ability.

        Erythropoietin 약물사용평가

        Erythropoietin is hematopoietic agent that principally influences erythropoiesis. In this research, we investigated the drug use through chart reviewing of Korea University Medical Center Anam Hospital's patients and used "Erythropoietin drug use evaluation criteria" of American Society Hospital Pharmacy. The results are as follows, 59 cases (100%) of testees met with the criteria for the justification of use. As to critical indication, 52.5% of patients with hypertention begin with the situation of modest blood pressure. Initial dosage of erythropoietin was proved to be appropriate for 74.6% of patients. The main complication is blood pressure increasing which was found in 29 testees (49.2%), 15 patients (51.7%) of them reduced erythropoietin dosage or discontinued until blood pressure was controlled. As to outcome measures, testees whose hematocrit went back to 30-33% was only 3 cases (6.1%), and 31 testees (52.5%) increased their red blood cell or hemoglobin so that they decreased need for transfusions. In this research, we could conclude that, in order to supply erythropoietin enough to keep hematocrit at 30-33%, the economic supports to patients with chronic renal failure by Korean government should be enlarged.

      • 포스포디에스테라제 Ⅲ의 저해물인 KR-30075의 흰쥐에서의 약물속도론

        A procedure for the determination of KR-30075 and its metabolites in plasma and urine by high performance liquid chromatography is described. For the study of pharmacokinetic properties of KR-30075, a new PDE Ⅲ inhibitor, the plasma concentration and urinary excretion after an oral administration of KR-30075 (4 ㎎/㎏) in the male at rat (Sprague Dawley) were determined by high performance liquid chromatography. The best extraction efficiency of KR-30075 and KR-30072 is obtained with ethyl ether adjusted to pH 4.0. Retention times of both KR-30072 and KR-30075 were within 5 min and resolution was complete at the flow rate of 1.0 ㎖/min. The sensitivity and specificity of this HPLC assay appears to be satisfactory for the pharmacokinetic study of KR-30075 and its metabolites. One-compartment open model with first-order absorption was applied to evaluate the pharmacohinetic parameters of KR-30075 according to Minimum AIC Estimation.T_max was 1 hr, C_max was 0.789±0.31 ㎍/㎖ and elimination half T_(1/2) was 6.3l min after oral administration of 4 ㎎/㎏ KR-30075 to male rats.
