정상인과 편마비 환자의 심박변동신호의 특성에 관한 연구

In this paper, the power spectral analysis and the fractal analysis of heart rate variability(HRV) were performed to evaluate the effects of brain lesion on cardiovascular system and autonomic function for 24 normal subjects and 22 hemiplegic patients. The ECG and respiration signals were recorded at tilt angles of $0^{\circ}$ and $70^{\circ}$ for 5 and 6 minutes successively under the condition of frequency controlled respiration (0.25Hz). For normal subjects, HR, LF component, HF component and fractral dimension of HRV were distinctly changed after orthostatic stress, whereas, for hemiplegic patients, those were little changed. Complexity and variability of heart rate of patients were smaller than those of normal subjects. Sympathetic tone of patients was higher than that in normal subjects. All of these results support that autonomic disorder and cardiovascular disturbance accompanied by brain lesion could be assessed by the power spectral analysis and fractal analysis of HRV.

Objective : The effects of electroacupuncture(EA) stimulation on heart rate variability(HRV) for healthy adults are investigated with power spectrum analysis(PSA) of HRV. Methods : The HRVs of every 10 minute for 22 healthy volunteers(13 men and 9 women) were measured for an hour with SA-3000P(Medicore Co., Ltd., Korea). The median age with arithmetic range of 13 men and 9 women was 26.00 years with 22.75~27.00 years. The initial 20 minutes were defined as baseline period(pre-EA period), the following 20 minutes as the EA period and the last 20 minutes as the post-EA period. In each EA periods, volunteers received EA(2Hz, 0.6~0.8ms duration, maximal tolerated stimulation without discomfort) on the right Zusanli(ST36), Shangjuxu(ST37) acupoints while supine, resting. Results : Heart rate of volunteers at post-EA period was significantly decreased compare to that of pre-EA period, while SDNN at post-EA period was significantly increased. Heart rate at EA period was significantly decreased compare to that of pre-EA period, but Ln(LF) and LF/HF at EA period were significantly increased. Ln(HF) at post-EA period was significantly decreased compare to that of EA period, while the other variables were not significantly changed. Conclusions : The results suggest that EA in healthy adults is associated with changed activity in the sympathetic and parasympathetic nervous system. Further study is needed for investigating the effects of EA on HRV and autonomic nervous system.

연구배경: 대사증후군에서 심혈관 질환의 위험이 증가하는 것은 잘 알려져 있으며 최근 심장 자율신경계의 이상과 심혈관 질환의 관계가 제기되고 있다. 이 연구에서는 대사증후군을 이루고 있는 요소들이 각각 심박동 변이와 어떤 관계가 있는지와 대사증후군 요소의 개수에 따른 심박동 변이의 차이를 알아보고자 하였다. 방법: 일개 대학병원에서 2002년 2월에서 2004년 10월까지 건강검진센터를 방문한 수진자 2,919명 중심박동 변이분석과 설문정보가 완성되고 심박동 변이 검사상 배제기준을 통과한 총 592명 (남자309명, 여자 283명)을 최종 연구대상으로 하여 대사증후군 각 요소의 개수와 심박동 변이 지표와의 관계 및 대사증후군의 존재여부에 따른 심박동 변이 지표의 차이를 분석하였다. 결과: 대사증후군 구성 요소의 개수는 SDNN (standard deviation of all normal RR intervals)과는 유의한 음의 상관관계가 있었으며 (P for linear trend=0.03) LF/HF (low frequency/high frequency)와는 유의한 양의 상관관계가 있었다(P for quadratic trend=0.01). 대사증후군 각 요소별 분석상 남자에서 비만이 있는 군에서 비만이 없는 군에 비해 LF/HF가 의미있게 높았고 (P=0.0002), 여자에서는 고중성지방혈증이 있는 군에서 없는 군에 비해 SDNN이 의미있게 낮은 결과를 볼 수 있었다 (P=0.04). 또한 여자에서 대사증후군이 있는 군에서 없는 군에 비해 SDNN이 의미있게 낮았다 (P=0.01). 결론: 대사증후군의 구성 요소의 개수가 많아질수록 SDNN으로 대표되는 전체 자율신경반응은 감소하고 LF/HF로 대표되는 교감신경/부교감신경 비율은 증가하였다. 특히 남자에서는 비만과, 여자에서는 고중성지방혈증이 있을 때 심박동변이가 감소하였다. Background: Metabolic syndrome (MS) increases the risk of cardiovascular events. Heart rate variability (HRV) represents autonomic function and reduced HRV significantly increases cardiovascular mortality. In this study, we investigated whether each of the MS component is related to HRV indices. Methods: We analyzed 592 subjects who participated in routine health check up from February 2002 to October 2004. We measured their metabolic components and evaluated the relationships of MS and HRV indices. Results: Entire sample was divided into 5 group according to the number of metabolic compononets. All results were adjusted for gender, age and mean heart rate. As increasing the number of metabolic components, SDNN (standard deviation of all normal RR intervals) was decreased (P for linear trend = 0.03) and LF/HF (low frequency/high frequency)was increased (P for quadratic trend = 0.01). Paticularly in men, obese group has statistically higher LF/HF than non-obese group (P = 0.0002). In women, hyperTG (Triglycerides) group has lower SDNN comparing to normal TG group (P=0.04). Conclusions: Metabolic syndrome adversely affects cardiac autonomic control and a reduced cardiac autonomic control may contribute to the increased risk of subsequent cardiovascular events in individuals who exhibit metabolic syndrome.
