N/A Objective : Both bronchial asthma and chronic obstructive pulmonary disease(COPD) are defined as disease entities causing sudden exacerbation of dyspnea; the former is a reversible airflow obstruction and the latter partially reversible and characterized by slowly progressive airflow obstruction. There were several reports suggesting endothelin release during an acute exacerbation of asthma and COPD, however few report showed comparative data of asthma and COPD concomitantly. The author examined the release of endothelin in COPD in comparison with those in asthmatics and healthy controls. Methods: Plasma and 24 hour urine were collected from 33 asthmatics and 13 COPDs during an acute respiratory exacerbation and from 10 healthy controls. Endothelin was determined by radioimmmunoassay. Results: Plasma endothelins were elevated in asthma and COPD and there were no significant changes compared with controls. Endothelin excretion in 24 hour urine was significantly higher in asthma and COPD than in controls. Endothelin concentrations in 24 hour urine showed a negative correlation with FEV1 Conclusion: Endothelins in 24 hour urine were increased in acute exacerbation of asthma and COPD and these increases were negatively correlated with FEV1 significantly. These findings are interpreted that endothelin contributes to the pathogenesis of acute exacerbation in asthma and COPD.

N/A Sympathetic activity is related to the pathogenetic mechanism in essential hypertension. It is now evident that plasma norepinephrine reflects sympathetic activity. However, there are some conflicting results concerning the levels of urine catecholamines and their significance. This study was performed to evaluate the significance of urine catecholamines as an index of sympathetic activity in essential hypertension. The study was done on thirteen patients of established hypertension. Normotensive controls were fourteen healthy individuals. The concentrations of urine norepinephrine and dopamine were detected by employing liquid chromatography-electrochemica1 detector. The results were summarized as follows: 1) Age, body surface area, G.F.R., 24 hours urine excretion of sodium and plasma renin activity were not different between normotensive and hypertensive groups. 2) There was a significant decrease in systolic and diastolic blood pressure after treatment with low salt diet and atenolol. 3) In hypertensive group, the values of urine norepine-phrine and dopamine were significantly higher than normotensive group and both were significantly reduced after treatment with low salt diet and atenolol. 4) In hypertensive group, significant correlation was observed between urine norepinephrine and dopamine levels. Also, significant correlation was observed between the decrements in systolic blood pressure and the changes in urine norepinephrine by the treatment. In conclusion, urine catecholamines, norepinephrine and dopamine, were elevated in essential hypertension, and were significantly reduced after low salt diet and atenolol therapy, and it is suggested that urine catecholamines can be useful index in evaluating sympathetic activity in essential hypertension.

N/A Backgroud: Vasoactive intestinal polypeptide (VIP) is composed of 28amino acid residues. T lymphocyte has receptors for VIP on its surface membrane. It is assumed that the signal transduction for VIP is mediated by an activation of adenylate cyclase, which is much alike to that of stimulation of β-adrenergic receptor. In the present study, therefore, we have investigated the interrelationship of the signal transduction pathways between VIP and β-adrenergic receptor. Methods: The intracellular ATP pool of the T lymphocytes of rat was labelled by the incubation with [32P] orthophosphate-containing media for 3hours. The cells were stimulated with VIP (10(-5) M), isoprotereol (10(-5) M) or forskolin (10(-5)M) with or without pretreatment of propranolol (10(-5)M). Phosphorylated protein fractions were separated by means of two dimensional gel electrophoresis using non-equilibrium pH gradient electrophoresis and sodium dodecyl sulfatepolyacrylamide gel electrophoresis. Gels were dried and phosphorylated proteins were observed by autoradiography. The concentrations of intracellular cAMP and IP, were measured by radioimmunoassay and IP,-binding protein assay, respectively. Results: 1) Stimulation of the lymphocyte with VIP caused intense phosphorylation of nine proteins and the proteins were the same as those obseved by isoproterenol or forskolin treatment. 2) Propranolol, a β-Adrenergic antagonist, blocked the effect of VIP partially, but not that of forskolin. 3) The increase in the intensities of phosphorylation of these proteins were correlated with the increase of cAMP by each drugs; VIP, isoproterenol, and forskolin. 4) IP, was not increased either by isoproterenol, VIP or forskolin. Conclusion: This study suggests that VIP increased phosphorylation of 9 proteins via the activation of adenylate cyclase, and propranolol could inhibit the action of VIP partially by affecting the component between the VIP-receptor and adenylate cyclase.

N/A Objectives: Even resectable case under TNM staging criteria in lung cancer, poor pulmonary function makes surgical candidates inoperable and lung resection in patients with diminished cardiopulmonary reserve function is associated with increased perioperative morbidity and mortality, We planned to examine which can be the most important predicted determinants in asssessing preoperative routine check-up and/or exercise test in prospect of postoperative short-term complications. Methods: Twenty two patients, taken lung resection under the diagnosis of an operable lung cancer were evaluated prospectively about not only the staging work-up, but also oxygen consumption and carbon dioxide production with treadmill test, and stair climbing. Control subjects were selected randomly from the thirty healthy person visiting health care center and examined general check-up together with spirometry and exercise test. The postoperative complications occurred within three months were included followings' arrhythmia requiring therapy, pneumonia, atelectasis, air-leak and/or pneumothorax, and death from the above reasons, Results: Lung cancer group showed a significant decrease in FEV1, Bruce stage on treadmill test, and maximal oxygen consumption compared with normal control group. Comparison within groups according to complication showed significantly increased smoking history and significantly decreased FEV1 and FEV1 ppop in complicated group. In complicated group, the ratio of VO2max/VO2max, predicted was also decreased significantly, while the ICU day increased significantly. In complicated group, three cases among four with normal to mild pulmonary insufficiency on spirometry showed quite low VO2max level, Conclusion: These findings suggest that heavy smoking and diminished preoperative FEV1 in operable lung cancer are regarded as high risk factors, and it needs special concerns about postoperative complications when the ICU days is increasing. VO2max is thought to be an useful measurement in expecting postoperative complications, especially in cases of relatively good pulmonary function and further study will be needed for the more subjects in near future.
