      • 내당능장애 및 제 2형 당뇨병 환자에서 인슐린 분비능과 인슐린저항성의 변화

        연구배경: 인슐린 저항성과 인슐린 분비능을 일상적인 상황에서는 측정하기가 쉽지 않으며 내당능장애나 당뇨병의 발생에 미치는 역할에 대해서도 많은 논란이 있어 왔다 저자 등은 성인에서 경구당부하 검사를 통하여 분류된 내당능장애와 제 2형 당뇨병 환자들에서 인슐린 분비능과 인슐린 저항성 지수를 구하여 정상군, 내당능장애군, 그리고 당뇨병 환자군 사이에 차이를 비교하고, 내당능장애와 당뇨병의 발생과 이들 지수와의 관계를 알아보고자 하였다. 방법: 40세 이상 성인 105명의 75g 경구당 부하검사 결과를 인슐린 측정치와 함께 분석하였으며 정상대조군(n=26) 내당능장애군(n=28), 제 2형 당뇨병 환자군(n=51)으로 분류하여 인슐린 분비능과 인슐린 저항성을 평가하였다 초기 인슐린 분비반응은 당부하 30분에 혈당 증가치에 대한 인슐린 증가치의 비 △ Ins(30)/△ Glu(30)]로 평가하였고, 전체 인슐린 분비반응은 2시간 및 3시간 동안의 평균 인슐린농도로 평가하였다 인슐린 저항성은homeostasis model assessment (HOMA)를 적용하여 구하였다(HOMA IR). 결과: 1). 당부하 후 정상대조군에 비해서 내당 능장애와 당뇨병 환자군에서 인슐린 분비반응이 지연되고 초기 인슐린 분비반응을 나타내는 △ Ins(30)/△ Glu(30) 비도 의의 있게 감소되어 있었다. 2) 당부하 후 전체 인슐린 분비반응을 알아보기 위해 측정한 2시간 동안의 평균 인슐린 농도는 세 군간에 유의한 차이가 없었으나 3시간 동안의 평균 인슐린 농도는 정상대조군에 비해서 내당능장애군에서 유의하게 증가되어 있었다. 3) 인슐린 저항성(HOMA IR)은 당뇨병환자 군에서 정상대조군에 비해서 유의하게 높았고 대조군과 내당능장애 사이에는 차이가 없었다 4) 상관분석에서는 공복혈당과 △ Ins(30)/△ Glu(30) 비 및 HOMA IR은 비슷한 상관 관계를 보였으며 당부하 후 2시간 혈당과의 상관계수는 △ Ins(30)/△ Glu(30)비가 HOMA IR에 비하여 높았다. 5) 로지스틱 회기분석 결과 △ Ins(30)/△ Glu(30). HOMA IR. 그리고 체질량지수가 당뇨병의 발생위험도와 유의한 관계가 있는 것으로 나타났다 결론: 정상인에 비해서 내당능 장애군과 경도의 당뇨병 환자군에서 전체 인슐린 분비능은 감소되지 않고 초기 인슐린 분비반응이 감소된 소견과 인슐린 저항성이 당뇨병 환자군에서만 의의 있게 존재하는 것으로 보아 초기인슐린 분비반응이 내당능에 미치는 영향에 대한 고려와 이에 대한 연구가 더 필요할 것으로 사료된다. Background: Glucose intolerance depends on a complex interaction between insulin secretion and insulin resistance. So the purpose of the present study was to report the results of assessment of insulin resistance and insulin secretory response in adults with normal glucose tolerance and in patients with impaired glucose tolerance(IGT) or type 2 diabetes mellitus(Type 2 DM). Methods: A total of 105 subjects was selected who were given 75-g oral glucose tolerance test(OGTT) with simultaneous measurement of serum insulin. They were classified as normal (n = 26), IGT(n = 28), or Type 2 DM(n = 51) by WHO criteria. The ratio of incremental insulin response at 30 min to incremental glucose response at 30min〔△ Ins(30)/△ Glu(30)〕 was used to assess the early-phase insulin secretion, and mean insulin concentration for- 2 hours or 3 hours after glucose load was regarded as total insulin response. The homeostasis model assesment(HOMA) was used to assess insulin resistance(HOMA IR). Results: Subjects with IGT or Type 2 DM were characterized by delayed peak insulin responses and lower levels of △ Ins(30)/△ Glu(30). Total insulin responses for 2 hours after glucose load were not different among the groups but those reponses for 3 hours were significantly increased in subjects with NGT, Subjects with Type 2 DM showed higher HOMA IR when compared with subjects with NGT. There was no difference between normal controls and subjects with IGT in the values of HOMA IR. Significant relationship was revealed between fasting blood glucose and Llns(30)/,LGlu(30) or HOMA IR. Compared with HOMA IR, △ Ins(30)/△ Glu(30) was more correlated with blood glucose at 2 hr during the OGTT. Logistic regression analyses showed that △ Ins(30)/△ Glu(30), HOMA IR, and body mass index are related with increased odds ratio of development of Type 2 DM. Conclusions: Above results showed that although total insulin response to glucose load is not decreased in subject with glucose intolerance, early-phase insulin response is decreased. Considering the level of insulin resistance in our subjects, it is necessary to clarify the role of defect in early-phase insulin secretion in the development of IGT or Type 2 DM in Korean.

        Pemetrexed-Erlotinib, Pemetrexed Alone, or Erlotinib Alone as Second-Line Treatment for East Asian and Non-East Asian Never-Smokers with Locally Advanced or Metastatic Nonsquamous Non-small Cell Lung Cancer: Exploratory Subgroup Analysis of a Phase II Tr

        Purpose This subgroup analysis of a phase II trial was conducted to assess possible ethnicity-basedtrends in efficacy and safety in East Asian (EA) and non-EA populations with nonsquamousnon-small cell lung cancer (NSCLC). Materials and MethodsNever-smoker patients (n=240) with locally advanced or metastatic nonsquamous NSCLCincluded 133 EA patients randomized to pemetrexed supplemented with dexamethasone,folic acid, and vitamin B12 plus erlotinib (pemetrexed-erlotinib) (n=41), erlotinib (n=49), orpemetrexed (n=43), and 107 non-EA patients randomized to pemetrexed-erlotinib (n=37),erlotinib (n=33), or pemetrexed (n=37). The primary endpoint, progression-free survival(PFS), was analyzed using a multivariate Cox model. ResultsConsistent with the results of the overall study, a statistically significant difference in PFSamong the three arms was noted in the EA population favoring pemetrexed-erlotinib (overallp=0.003) as compared with either single-agent arm (hazard ratio [HR], 0.48; 95%confidence interval [CI], 0.29 to 0.79; p=0.004 vs. erlotinib; HR, 0.40; 95% CI, 0.23 to 0.70;p=0.001 vs. pemetrexed). The EA patients treated with pemetrexed-erlotinib achieved alonger median PFS (7.4 months) compared with erlotinib (4.5 months) and pemetrexed(4.0 months). The PFS results also numerically favored pemetrexed-erlotinib in the non-EApopulation (overall p=0.210) (HR, 0.62; 95% CI, 0.37 to 1.05; p=0.078 vs. erlotinib; HR,0.75; 95% CI, 0.42 to 1.32; p=0.320 vs. pemetrexed) (median PFS: pemetrexed-erlotinib,6.7 months; erlotinib, 3.0 months; pemetrexed, 4.4 months). ConclusionThe PFS results from this subset analysis in both EA and non-EA populations are consistentwith the results in the overall population. The PFS advantage for pemetrexed-erlotinib issignificant compared with the single agents in EA patients.

      • 기하학적 특징에 기반한 순수 얼굴영역 검출기법

        얼굴 영역의 정확한 위치를 정확히 찾는 것은 얼굴 인식을 위한 핵심적인 전처리 과정이다. 본 논문에서는 조명조건, 표정, 배경의 변화에 무관하게 얼굴의 구성요소를 검출할 수 있는 강건한 기법을 제안한다. 수평 방향의 상대적으로 어두운 화소값을 갖는 눈썹, 눈, 입 둥과 같은 독립된 얼굴 요소를 검출하기 위해 형상 분해 국부 최적 임계치 기법을 적용하며 얼굴을 구성하는 간단한 기하학적 조건을 만족하는 얼굴 요소의 그룹을 검색함으로써 순수 얼굴영역을 검출한다. AR-face 데이타베이스의 영상에 적용한 결과, 두꺼운 안경테에 의해 눈썹이 가리워진 특수한 경우를 제외한 거의 모든 영상에 대해 정확한 얼굴 영역을 검출할 수 있었고, 얼굴의 대칭성을 이용해 회전과 이동 변화를 보상할 수 있으므로 강건한 얼굴 인식의 전처리 과정으로 사용할 수 있다. Locating exact position of facial components is a key preprocessing for realizing highly accurate and reliable face recognition schemes. In this paper, we propose a simple but powerful method for detecting isolated facial components such as eyebrows, eyes, and a mouth, which are horizontally oriented and have relatively dark gray levels. The method is based on the shape-resolving locally optimum thresholding that may guarantee isolated detection of each component. We show that pure facial regions can be determined by grouping facial features satisfying simple geometric constraints on unique facial structure. In the test for over 1000 images in the AR -face database, pure facial regions were detected correctly for each face image without wearing glasses. Very few errors occurred in the face images wearing glasses with a thick frame because of the occluded eyebrow -pairs. The proposed scheme may be best suited for the later stage of classification using either the mappings or a template matching, because of its capability of handling rotational and translational variations.

        Region-based Corner Detection by Radial Projection

        We propose a novel method which detects convex and concave corners using radial projection. The sum of two neighbors’ differences at the local maxima or minima of the radial projection is compared with the angle threshold for detecting corners. In addition, the use of oriented bounding box trees and partial circles makes it possible to detect the corners of complex shapes. The experimental results show that the proposed method can separately detect the convex and concave corners, and that this method is scale invariant.

        Precise Detection of Car License Plates by Locating Main Characters

        We propose a novel method to precisely detect car license plates by locating main characters, which are printed with large font size. The regions of the main characters are directly detected without detecting the plate region boundaries, so that license regions can be detected more precisely than by other existing methods. To generate a binary image, multiple thresholds are applied, and segmented regions are selected from multiple binarized images by a criterion of size and compactness. We do not employ any character matching methods, so that many candidates for main character groups are detected; thus, we use a neural network to reject non-main character groups from the candidates. The relation of the character regions and the intensity statistics are used as the input to the neural network for classification. The detection performance has been investigated on real images captured under various illumination conditions for 1000vehicles. 980 plates were correctly detected, and almost all non-detected plates were so stained that their characters could not be isolated for character recognition. In addition, the processing time is fast enough for a commercial automatic license plate recognition system. Therefore, the proposed method can be used for recognition systems with high performance and fast processing.
