Purpose: Surgery for thyroid gland requires skin incisions that can result in postsurgical neck scar. To overcome this, many surgeons performed a endoscopic thyroidectomy. But, this approach had a some problems. One of postoperative problems, iatrogenic cervical dystonia(CD) may be occurred. At common, CD is defined as a syndrome characterized by prolonged muscle contraction causing twisting, repetitive movements or abnormal posture. Botulinum toxin A(BTA, Botox^Ⓡ, Allergan, Irvine, CA, USA) is well known treatment agent in the treatment of CD. So, the authors applied BTA injection in rare case with iatrogenic CD resulting in endoscopic thyroidectomy. Methods: A 43 - year - old female had operated endoscopic subtotal thyroidectomy in 3 years ago. She had symptoms such as progressive cervical pain, abnormal neck posture, depression, and sleep difficulty. About 1 year later, the patient who had previous myomectomy of the clavicular head of sternocleidomastoid muscle, however, symptoms were not improved. And then the patient received BTA therapy in our department. The 2 units per 0.1 ㎖ solution was administered in a 1 ㎖ tuberculin syringe. Results: The dose of BTA used in the patient was 36 units for vertical platysmal bands, superficially, and 10 units for ipsilateral sternocleidomastoid muscle, intramuscularly. After 2 weeks, additional the dose of BTA used in the patient was 5 points for remained scar bands, superficially. Complications related to injection such as significant swallowing difficulties, neck muscle weakness, or sensory change were not observed. In 9 months follow - up, the patient had a good result was obtained by the method of BTA injection alone. Conclusion: The basic concept is selective denervation for the hyperactive individual muscles and scar bands. We conclude that BTA is an effective and safe treatment for CD despite the iatrogenic and complex presentation of this complication.

Combined Subbrow Excision with Upper Blepharoplasty Using a Tadpole Shaped Excision for Blepharochalasis

BackgroundIn efforts to solve blepharochalasis, induced by aging process, various surgicalmethods have been developed over the years. Among them, classical upper blepharoplasty has a problem that severe lateral hooding may be restricted in making correction, cause of difference in thickness between the pretarsal skin and subbrow skin. And also subbrow excision (SE) blepharoplasty cannot be easily corrected in severedrooping of the medial skin. In an effort to make up for the disadvantages of thesemethods, we attempted SE–tadpole shaped excision blepharoplasty (TSEB). MethodsFifteen patients underwent SE-TSEB from January 2013 to November 2014. SE-TSEB was performed the following cases: 1) patients who showed moderate to severe blepharochasis with severe lateral wrinkles; 2) patients with a history of pre-existent SE blepharoplasty; 3) patients with a tattoo on the eyebrows, who want to makerevision; and 4) patients with eyelid skins that are thick or have overly protuberant fatpads of the retro-orbicularis oculi fat (ROOF), and have a plan to extirpate them. ResultsDuring the follow-up observation period, most patients showed amicable correctionof visibility difficulty and were aesthetically satisfied with the postoperative resultswith respect to correction of the lateral wrinkles. Scars formed in the subbrow incision line were within the scope of management. ConclusionsSE-TSEB is a treatment modality that overcomes the disadvantages and limitations of classical upper blepharoplasty and SE blepharoplasty. Furthermore, maximizing the advantage of each surgical approach is another innovative approach for correction of changes in the upper eyelid, induced by aging.

다문화사회의 사회통합 과제와 홍익인간 사상의 의미

The aim of this study is to suggest that theoretical foundation for social integration which is social integration in the multicultural society. In this article, I focused on Honk-Ik-In-gan thought. multiculturalism which has been considered as the ideal type of social integration in a multicultural society exposes it’s limits and contradiction. In Korea, we idealize multiculturalism without discussing it as social issues and considering it’s theoretical basis. As a result, the definition what the multiculturalim is that is in a state of confusion, because multiculture and multlculturalism in the social policy have same meaning in Korea. Hong-Ik-In-Gan thought suggests the alternative in response to the western multiculturalim cause practical problem. especially, Shin-In-Hab-Il thought, Han thought, and equity and equality thought in Hong-Ik-In-Gan idea propose harmony and cohesion in a multicultural society. In conclusion, Hong-Ik-In-Gan thought has the significance, cause it is basic foundation of the theory, korean multiculturalim. 이 글은 다문화사회의 사회통합과정에서 나타나는 사회적 과제를해결하기 위하여 홍익인간사상에 주목한다. 홍익인간사상은 다문화사회의 사회통합을 위한 철학적 이론적 토대를 마련하려는 중요한자원이라 할 수 있다. 다문화사회의 사회통합은 전 세계적인 추세이며 한국사회 역시 당면과제로 맞게 되었다. 다문화주의는 다문화사회의 사회통합에 가장 이상적인 방안으로 간주되었으나 사회통합 실천과정에서 논리적 모순과 한계가 있는 것으로 나타났다. 한국사회는 다문화사회의 사회통합방안으로 과연 무엇이 적절한가에 대한 사회적 차원의 논의와 사회통합의 철학적 기반에 대한 고민은 생략한채 다문화사회의 사회통합을 위한 가장 이상적인 방안으로 서구의다문화주의를 이상화시켜버렸다. 그 결과 ‘다문화’가 무엇인가에 대한 개념이 명확하게 정의되지 않은 상태에서 다문화주의와 다문화가혼동되어 사용되는 등 개념상의 혼란과 오류가 나타나고 있다. 이 글에서 주목하는 홍익인간 사상은 서구 다문화주의에 나타나는 논리적모순과 한계에 대한 대안을 제시해 줄 뿐 아니라 한국적 다문화주의를 위한 이론적 토대가 된다는 점에서 의의가 있다. 무엇보다 홍익인간 사상의 신인합일은 인간과 사회의 관계를 재조명하고 있다. 즉 인간의 자율성을 바탕으로 인간과 사회의 조화로운 관계를 상정하고있다. 또한 홍익인간 사상의 한사상과 천지인 사상은 특수성과 보편성의 조화가 가능함을 함의하고 있다. 다양성과 통합이 가능한 것은이질적인 것들 간의 관계가 상생의 관계를 전제로 하고 있기 때문이다. 마지막으로 홍익인간 사상은 평등과 평화의 원리를 통해 다양성에서 발생할 수 있는 위계와 차별, 분리와 배제의 문제를 해결할 수있는 실마리를 제공해 준다. 즉 공평과 평등의 원리야말로 형평을 의미한다. 결론적으로 홍익인간 사상은 다문화주의의 모순을 보완할뿐 아니라 ‘한국적 다문화주의’를 위한 철학적ㆍ정치이념적 토대를마련해 준다는 점에서 현대사회에 시사하는 바가 크다고 할 수 있다.

한국의 산업화와 여성고용구조의 변화 : 1960-2000년대 from the 1960s to 2000s

Korea that included in the global capital division has accomplished the rapid economic development under the state-led since 1960s. Korean industrialization, based on light industry using women`s labor of the low wage, resulted in feminization of labor` with the beginning of economic development. Women`s labor market relates closely the process of industrialization under the project of economic growth. The purpose of this article is to examine the aspects of such changes in the employment structure of women`s labor. In particular, this article is focused on the changes of women`s labor market, based on the process of the industrial restructuring under the state-led and the global economy. As well, this paper perceives the composition of women`s labor, gendered hierarchies, patriarchy with the capitalism, and the stratified arrangement of women`s labor. In addition, this paper examines how the state has formulated the national labor policy, particularly the labor policy for women, to mobilize women labor for the project of the economic development, and how women workers have coped with difficulties.

Iatrogenic Blepharoptosis after Cataract Surgery

Eyelid complications after cataract surgery can occur including ectropion, entropion and blepharoptosis. Among them, blepharoptosis has been reported at high rate about 13%. Various causes of ptosis after cataract surgery have been reported. We report the case of three patients treated for blepharoptosis after cataract surgery. All these patients received cataract extraction by a local ophthalmology before they visited our clinics. They presented aponeurotic dehiscence, it was graded as partial and underwent surgical correction by levator aponeurosis advancement. The postoperative course was uneventful. Ptosis was corrected and there was no recurrence during the period of follow-up. All three of them were pleased with the final result. Since post-cataract extraction ptosis is frequent but a preventable complication, understanding the causes of ptosis after surgery is very important. That can help a surgeon prevent its occurrence with careful surgical planning.
