      • Cerivastatin과 Cyclosporine을 병용 투약한 신이식 환자에서의 횡문근융해증 1예

        Cerivastatin, a kind of statins, is a recently used lipid-lowering drug, which inhibits of hydroxymethylglutarate coenzyme A (HMG-CoA) reductase. The serum concentrations of statins in healthy volunteers are increased by the simultaneous administration of cyclosporine, itraconazole, as well as erythromycin and verapamil and increased risk of myalgia and rhabdomyolysis has been reported after concomitant use of HMG-CoA reductase inhibitors with these drugs. A 57 year-old female kidney transplant recipient admitted because of a dark urine, myalgia and weakness of extremities. Six weeks before admission, the patient has taken cerivastatin 0.3 mg daily. Cyclosporine, prednisolone and mycophenolate mofetil were maintained for recent two years. Initial laboratory data showed serum creatinine 1.1 mg/dl, AST 235 IU/L, LDH 2,465 IU/L, CPK 1600 IU/L, myoglobin >3,000 ng/ml and cyclosporine 288ng/ml (TDX). Medication of cerivastatin was discontinued, and the patient recovered rapidly with supportive therapy. After eighteen weeks of cerivastatin withdrawal, the drug was represcribed to control the increased lipid levels. The patient showed the same clinical manifestations suggesting rhabdomyolysis following eight weeks of cerivastatin administration. We report a case of rhabdomyolysis associated with cerivastatin and cyclosporine combination therapy in a kidney transplant recipient.

      • 도로노면 온도측정 시스템 개발과 지형에 따른 도로노면 온도 특성

        도로교통안전관리공단의 교통사고 통계에 의하면 최근 2년간(2004~2005년) 노면상태별 교통사고는 건조한 노면상태에서 연평균 183,643건으로 가장 높게 발생하였다. 그러나 사고 100건당 사망자 수인 치사율의 경우 노면상태가 습기인 경우가 3.59로 가장 높으며, 다음으로 결빙 2.97, 건조 2.90, 적설 2.01의 순으로 나타났다. 사고는 건조한 노면 상태에서 가장 많이 발생하지만 사고발생시 사망자는 노면상태가 습기이거나 결빙상태일 때 가장 많이 발생하므로 결빙상태인 노면에서의 사고는 운전자에게 치명적일 수 있다. 최근 국민소득 증가로 겨울철 레포츠의 활성화와 격주 근무 및 주 5일제 근무 확산으로 겨울철 유동인구가 증가하면서 도로의 활용도는 높아져 노면 결빙으로 인한 사고는 더욱 증가할 수 있다.

        물 부족 해결을 위한 도로노면 유출수 처리 및 빗물 재이용 방안 연구

        In this study, a pilot plant of Back-washing Sponge Media, Vegetation Filtration, and Multi–Layer Filtration process was installed to treatent and re-use road runoff during rainfall. In order to evaluate the efficiency of the unit process of the Pilot Plant, the experiment was conducted by changing the concentration of the artificial rainfall. Experiments were carried out by artificial rainfall runoff considering the rainfall runoff around the Seoul area roads. In addition, the process recovery was evaluated by estimating the recovery rate through the backwash to control not only the water quality, but also the clogging due to high concentration of particulate matter inflow and long–term operation. CODcr and TP in the raw water of organic matter and nutrients of high concentration were 41%, 10%, and 40% in the pretreated sponge media pretreatment, and 94% and 44% in the vegetation filtration process and 98% in the multi-layer filtration process, respectively. Experiments on only multi-layered process showed that heavy metals tend to increase somewhat after decreasing processing concentration. The treatment efficiencies of Cd, Cr and Pb were 25~27% and the efficiency of Zn was 96%. Finally, through backwashing, it was confirmed that the amount of permeate recovered up to 78-85% through backwashing as a result of the experiment with high concentration of particulate matter and pollutants in order to confirm the permeation amount and recovery amount.

        PointNet을 이용한 3D 한국어 단어 인식 Lip-Reading

        기존의 립리딩은 주로 영상을 통해 영어를 인식하는 연구가 많다. 본 논문은 10가지의 한글 단어 데이터 를 통해 인식하는 립리딩 방법을 제안하고, 영상만이 아니라 ToF Sensor를 이용한 3차원 립리딩 방법을 제안한 다. 본 논문에서 사용한 데이터는 RGB 영상 데이터와 Point Cloud 형식의 3차원 데이터로 각각 학습을 진행하고 결과를 보여준다. 학습에 사용된 네트워크는 RGB 영상 데이터를 사용하기 위한 VGGNet 기반의 네트워크와. Point Cloud 데이터를 학습시키기 위한 PointNet 기반의 네트워크를 사용했다. 각각의 네트워크는 사용된 데이터 에 맞게 하이퍼 파라미터와 신경망 튜닝을 진행했다. 3차원의 데이터를 사용한 네트워크의 결과가 제일 높게 나왔 다는 것을 통해서 립리딩에 깊이 정보가 중요하게 작용했음을 알 수 있다. There are many studies that recognize English mainly through images in the existing lip reading. This paper proposes a lip reading method that recognizes through 10 Korean word data, and proposes a 3D lip reading method using ToF Sensor as well as an image. The data used in this paper are RGB image data and 3D data in the form of Point Cloud, respectively, and the learning is conducted and the results are shown. The networks used for learning used a VGGNet-based network to use RGB image data and a PointNet-based network to learn Point Cloud data. Each network tuned hyperparameters and neural networks to match the data used. It can be seen that depth information played an important role in lip reading from the fact that the result value of the network using 3D data was the highest.

        화학적 산화공정을 이용하여 소변의 색도 및 유기물 처리를 통한 재이용 기술 연구

        This study was conducted to solve the water shortage problem by reclaiming urine from homes or public places and using it as cleaning water for toilets. The process used in this experiment is a chemical oxidation process combining ozone, hydrogen peroxide, and UV. We set the key substance that is to be removed as chromaticity and conducted the experiment to remove it. If the quantity or concentration of injected ozone, UV, and hydrogen peroxide is insufficient, then the chromaticity will initially increase due to low oxidizing power, and will later decrease. In addition, the efficiency of removing chromaticity appeared to be higher, depending on the quantity of ozone injected, for medium concentrated urine than highly concentrated urine. However, the absolute quantity of removed chromaticity was about 68% higher for highly concentrated urine, when 16 g/hr of ozone was injected. The higher the pH level, the reaction time and efficiency of removing chromaticity were higher, and in normal conditions, in reference to a pH of 8.55, there was a 6% difference in efficiency between a pH level of 5.05 and a pH level of 10.12. Finally, when processing urine through an ozone-only process, COD decreased steadily over time, but DOC did not decrease. This is because ozone reacts selectively with organic matter.

        Frequency and Outcome of Neuroleptic Rotation in the Management of Delirium in Patients with Advanced Cancer

        Purpose The response to haloperidol as a first-line neuroleptic and the pattern of neuroleptic rotation after haloperidol failure have not been well defined in palliative care. The purpose of this study was to determine the efficacy of haloperidol as a first-line neuroleptic and the predictors associated with the need to rotate to a second neuroleptic. Materials and Methods We conducted a retrospective review of the charts of advanced cancer patients admitted to our acute palliative care unit between January 2012 and March 2013. Inclusion criteria were a diagnosis of delirium and first-line treatment with haloperidol. Results Among 167 patients with delirium, 128 (77%) received only haloperidol and 39 (23%) received a second neuroleptic. Ninety-one patients (71%) who received haloperidol alone improved and were discharged alive. The median initial haloperidol dose was 5 mg (interquartile ranges [IQR], 3 to 7 mg) and the median duration was 5 days (IQR, 3 to 7 days). The median final haloperidol dose was 6 mg (IQR, 5 to 7 mg). A lack of treatment efficacy was the most common reason for neuroleptic rotation (87%). Significant factors associated with neuroleptic rotation were inpatient mortality (59% vs. 29%, p=0.001), and being Caucasian (87% vs. 62%, p=0.014). Chlorpromazine was administered to 37 patients (95%) who were not treated successfully by haloperidol. The median initial chlorpromazine dose was 150 mg (IQR, 100 to 150 mg) and the median duration was 3 days (IQR, 2 to 6 days). Thirteen patients (33%) showed reduced symptoms after the second neuroleptic. Conclusion Neuroleptic rotation from haloperidol was only required in 23% of patients with delirium and was associated with inpatient mortality and white race.
