N/A Diabetes mellitus can be divided into insulin-dependent (IDDM) and non-insulin-dependent diabetes mellitus (NIDDM) by NDDG criteria largely based on clinical features. Recently the development of a radioimmunoassay for human C-peptide and of ICSA assay have provided insight into the pathogenesis of diabetes mellitus. We observed the characteristics of 105 diabetics under 40 years of age and classified these patients by their clinical features, ICSA and 24 hour urine C-peptide. The results were as follows: 1) Diabetic ketoacidosis developed in R patients (3%). of these, patients belonged to the 1st decade (8 patients), which had the highest incidence of diabetic ketoacidosis. 2) Considering of family history and obesity, the cases with typical NIDDM, which had family history and were obese, were between 4% and 29%. The cases with typical IDDM, which had no family history and were not obese, were between 45%, and 60%. 3) Considering family history, obesity and insulin secretory capacity, the cases with typical NIDDM, which had family history and normal or increased insulin secretory capacity and were obese, were 15%. The cases with typical IDDM, which had not family history and decreased insulin secretory capacity and were not obese, were 14. 4) In ICSA assay, 12.5% of the patients were positive. All these patients had family history, various obesity and increased insulin secretory capacity. In conclusion, in Korean diabetics between 15 and 40 years of age, 15 percent of the patients were considered typical NIDDM and 14 percent were considered typical IDDM, The remaining patients could not be classified accurately and turther detailed study were needed.

Corrosive esophagitis is caused by ingestion of strong acids or strong alkalis, incidentally or with suicidal motivation. It produces severe inflammation and necrosis on mucosa of upper gastrointestinal tract, and in consequence causes severe esophageal stricture which require surgical repairment. But, recently nonoperative dilatation techniques such as bougination, balloon dilatation, endoscopic laser therapy, endoscopic electrocoagulation, and endoscopic microwave coagulation have been applied at these esophageal stricture and have had more excellent results as compared with surgical repair. Esophageal endoscopic endaprosthesis have been also used widely in malignant esophageal stricture, but not in benign esophageal stricture. We experienced a case of 61 year-old man with severe corrosive esophageal stricture which is succesefully treated by esophageal endoscopic endoprosthesis.

Precocious puberty is generally defined as the onset of scecondary sexual characteristics before the age of 9 year in boys or 8 year in girls. In girls the most common final diagnosis is idipathic precocious puberty. While the medroxyprogesterone acetate and cyproterone acetate have been used in decreasing secondary sexual development, they have been less effective in slowing bone age advancement and height velocity. Recently LHRH agonists have been used widely for treatment of true precocious puberty. We experienced a case of 9 year-old girl with true precocious puberty which is successfully treated by LHRH analogue (Triptorelin) (J Kor Soc Endocrinol 7:71~75, 1992)

Skeletal pain, abnormal bone morphology, and an increased risk of fracture are well-known manifestations of elevated parathyroid hormone levels in patients with primary hyperparathyroidism. Several studies have shown that primary hyperparathyroidism is associated with a significantly reduced bone density and these bone changes begine to heal after successful surgery. During recent years, several new techniques for measuring bone mass have been developed and become used for characterization of various disorder affecting skeletal mineralization. Of these, dual energy X-ray absorptiometry (DEXA) has been found to satisfy the requirement of sufficient precision and accuracy at a relatively low cost and limited radiation exposure. In 22 year-old male patient, we measured the bone mineral density before and after surgical correction of primary hyperparathyroidism by the use of DEXA. This patient had a reduced bone mineral density before surgery and successful parathyroidectomy increased bone mass by about 39% within 1 year. (J Kor Sor Endocrinol 7:76~80, 1992)
