N/A The immunogenecity and safty of heat-inactivated hepatitis B vaccine, containing 3 pg HBsAg, were studied in 43 subjects of healthy human volunteers during five months after the first injection. The volunteers received intramuscularly three doses of the vaccine(Hepaccine-B) at intervals of one month. Blood specimens to be examined for the serological markers of B virus and the aminotransferase activity were drawn at one, two, three, four hepatitis and five months after the first injection. Serum HBsAg, anti-HBs, anti-HBc, HBeAg and anti-HBe were detected by solid-phase radioimmunoassay. The aminotransferase activity was measured by automatic kinetic methods. The results obtained were as follows: 1) The anti-HBs response to the vaccine was seen in 34.9Yo after one month after the first injection, 69.0Yo after two months, 85,4Y after three months, increased to 94.7Yo after five months. 2) The main side effects were soreness, swelling or redness at the injection site and mild general symptoms; such as fatigue, headache or dizzness during one to three days. No hepatitis B virus and other infections in any of the vaccinees were observed. On the basis of these findings during five months observation period after the first injection, the immunogeneciy and safty of this vaccine seemed to be excellent.
감염증 환자에서 vancomycin, teicoplanin 투여례에 대한 연구 - 항균제 투여의 적응증 평가 -

Background : Glycopeptide antibiotics are the only drugs for treatment of infections due to beta-lactam-resistant Gram-positive bacteria. As the incidence of infection and colonization with vancomycin-resistant enterococci(VRE) rapidly increases, the hospital infection control practices advisory committee(HICPAC) recommends prudent vancomycin use to detect, prevent and control infection and colonization with VRE. Methods : The inpatients admitted from September to December, 1996 in Pusan National University Hospital, with Gram-positive bacterial infections were evaluated retrospectively to see whether the administrations of glycopeptide antibiotics were appropriate or not, upon comparison with the recommendations for preventing the spread of vancomycin resistance by HICPAC. Results : Teicoplanin has been chosen more frequently than vancomycin of the glycopeptide antibiotics. The indications of administration of glycopeptides in patients with pneumonia, wound infections, sepsis, and in febrile or neutropenic patients with malignancies were appropriate, but the use of glycopeptides for elimination of merely colonized bacteria in the oral cavity could not be excluded. Inappropriate use of glycopeptides was 10.6%, and inappropriately long-term use without positive culture for beta-lactam-resistant Gram-positive organisms was about 40% of total days of drug use. Conclusion : It seems essential for the quality assurance committee to make a plan in teaching the HICPAC recommendations to the medical practitioners who prescribed the glycopeptides inappropriately or used for irrelevantly long to his patient, monitor and survey their use of glycopeptides prospectively and periodically, and if there are repeated inappropriate prescriptions, a certain penalty would be given to the practitioners.
가열 비활성화한 B 형 간염백신에 대한 혈청 Anti - HBs Response 의 장기 관찰

N/A In order to clarify the serum anti-HBs response and persistence, and prophylactic effects to hepatitis B (HB) vaccination, 43 human healthy young adult volunteers who were seronegative for 5 serological markers of hepatitis B virus (HBV) and received intramuscularly HB vaccine (Hepaccine- B) at 0, 1 and 2 months were followed up for 36 months after the first injection. The heat-inactivated HB vaccine was prepared by Cheil Sugar Co. And contained 3 ug of HBs Ag. Blood specimens to be examined for the 5 serological markers of HBV and the aminotransferase activity were drawn at 1, 2, 3, 4, 5, 6, 12, 17, 24 and 36 months after the first injection. The five serological markers of HBV were detected by radioimmunoassay and the aminotransferase activity was measured by automatic kinetic method. The results obtained were as follows: 1) Anti-HBs response to the HB vaccine was noted in 17 of 19 (89,3%) male vaccinated subjects and all of 19 (100%) female vaccinated subjects at 4 months after the first injection, and were higher in females than in males. 2) Anti-HB response tc HB vaccine were 94.7% at the 4 months after the first injection and gradually decreased thereafter to 92.1% at 6 months and 86.8% at 24 months, and increased again to 94.4% at 36 months. 3) Negative seroconversion rate of anti-HBs was 8.3%(3/36) during the period of 24 months after the first injection. 4) Mean anti-HBs titers reached at peak at 9 months after the first injection in male subjects and 6 months in female subjects, and were higher in females than in males. 5) Non responders were 5.3% (2/38), low responders 18.4,o (7/38) and good responder 76.3% (29/39) during the period of 24 months after the first injection. 6) Anti-HBs titers of 11 vaccinated subjects reached at peak during the period of 5-6 months after the first injection and gradually decreased during 24 months and increased again at 36 months. It suggqsted that this may be due to booster results by natural infection of HBV. 7) No infection of HBV were noted in all of 38 vaccinated subjects during 24 months after the first injection and in all of 11 vaccinated subjects who were during 36 months respectively. On the basis of this findings the immunogenecity, persistence and prophylactic effect of this Hepaccine-B seem to be excellent during the period of 36 months after the first vaccination.

주사슬에 나프탈렌고리와 테트라메틸렌 및 헥사메틸렌 유연격자를 가지면서 트라이어드 메소겐을 갖는 새로운 열방성 방향족 액정 폴리에스테르(TLCP)를 용액중합법에 의하여 합성하였다. TLCP의 함량을 달리하는 TLCP/PEN in situ 복합재료를 용융방사하여 연신비가 각각 다른 단섬유를 제조하고 그들의 열적, 기계적 특성 및 모폴로지를 조사하였다. 합성된 TLCP는 네마틱 액정중합체였으며 고체상에서 액정상으로의 전이 온도는 249 ℃였다. 블렌드내의 TLCP 도메인들은 매트릭스 고분자인 PEN에 잘 분산 되었으며 어떠한 거대 상 분리 현상도 보여 주지 않았다. TLCP 함량의 증가에 따른 블렌드내 PEN의 cold crystallization 온도가 낮아진 것으로 보아서 TLCP가 PEN에 대한 조핵제 역할을 하였음을 알 수 있었다. 블렌드 섬유내의 TLCP의 도메인 크기는 대략 40 ~ 50 nm정도의 미세한 크기였으며 매트릭스와의 사이에 좋은 계면접착력을 보였다. 또한 cold 및 hot-drawing 과정을 거친 낮은 draw ratio(DR)에서는 거의 fibril이 형성되지 않았지만, 높은 DR에서는 잘 발달된 fibril들을 보여 주었다. TLCP의 강화효과로 인하여 10 wt% TLCP/ PEN 블렌드 섬유의 초기 모듈러스는 270 %, 인장강도는 235 %의 증가를 보여주었다. 반면에 신장률은 DR의 증가와 함께 감소 하였다. A thermotropic liquid crystalline polymer(TLCP) which has flexible butylene/hexylene spacers in the main chain and a triad aromatic ester type mesogenic unit containing a naphthyl group was prepared by solution polycondensation. The in-situ composites based on poly(ethylene 2,6-naphthalate)(PEN) and a thermotropic liquid crystalline polymer(TLCP) were prepared and melt spun at different TLCP contents and different draw ratios to produce monofilaments. Blends of the TLCP with PEN were investigated in terms of thermal, mechanical properties and morphology. The TLCP synthesized showed nematic mesophasic behavior and its transition temperature to isotropic melt from mesophase was 249℃. The blends showed well dispersed TLCP phases in the PEN matrix without macroscopic phase separation. Inclusion of TLCP in theblends decreased the cold crystallization temperature of PEN in the blend, therefore, the TLCP acts as a nucleating agent in the blend and showed good interfacial adhesion between the dispersed LCP phases and PEN matrix with domain sizes 40~50 nm in diameter and well developed fibrillation in the monofilaments. The TLCP acted effectively as a reinforcing material in the PEN matrix at the 10wt% level, it led to an increase of initial modulus up to 270% and tensile strength by 235%, while the elongation rate increasing with higher draw ratios.

Background: To standardize the histocompatibility test, we have performed a proficiency survey by compare our results with other reports from nationwide HLA laboratories in Korea. Methods: During a three-year periods, 6 trials of proficiency survey were performed with 35 laboratories. Test number and items included in each survey were 3 HLA DNA typings including HLA -A -B -C and -DR, and 6 HLA crossmatch tests. HLA A-B-DR haplotype frequencies were estimated by the maximum likelihood method using the computer program developed at the 11th International Histocompatibility Workshop. Results: High consensus (>90%) was obtaiend for HLA-DNA antigen typings. And low consensus was obtaiend for 19 of 30 trails in HLA crossmatch but not unacceptable. A total of 39 A-B-DR haplotypes with frequencies ≥0.5% were identified in Koreans, among which 17 haplotypes showed frequencies≥ 1.0%, which were similar results with other reports. Conclusions: This study demonstrates that there are high proficiency in HLA laboratory of PNUH but high resolution program will be necessary for improvement of HLA crossmatch.
