        기초지방자치단체 보건의료 지출수준의 결정요인

        Background: The purpose of this study was to analyze the related factors which decide the percentage of health care expenditure of the total fiscal expenditure of local governments and to provide the basic data to contribute for the efficient allotment of healthcare budget. Methods: This study was conducted by the percentage of health care expenditure for 3 years by classifying a total of 230 local governments into the metropolitan cities (gu, 69), the counties (si, 75), and the boroughs (gun, 86) all over the country. With the collected data, the general characteristics of independent variables and the dependent variable were analysed using SPSS ver. 18.0, The correlation analysis and multivariate regression analysis were conducted for the characteristics of variables according to regions by year. Results: In correlation between health care expenditure by year and other variables, there was a significant positive correlation with unemployment rate, metropolitan cities (gu) and other regions, the percentage of health center personnel, health care expenditure in last year as a independent factors. On the other hand there was no correlation with social assistance recipients and the percentage of aging population, financial self-reliance, industrialization rate, suicide rate, cardiac disease mortality, cerebrovascular mortality on health care expenditure. Conclusion: The study clearly shows that health care expenditure of local governance was not correlated with health care need factors comparing social welfare expenditure.

      • 부품국산화 개발 활성화를 위한 제도 개선 방안

        As most of the parts of the weapon system operated in Korea are procured through overseas purchases, a huge budget is leaked abroad every year. Accordingly, the Ministry of National Defense and each military are making great efforts to localize weapons system parts. However, privatel companies are having a lot of difficulties participating in the localization of military equipment. Therefore, in this study, the improvement methods were proposed to overcome these limitations and activate the parts localization development project. The first is to actively include the items economical viable items when filing requirements. Second, it is necessary to revise the selection criteria for selection of eligible companies for localization. The third is to improve the system so that the company can support the initial investment cost. By improving thc problems of the current system in order to promote the localization of parts for military equipment as described above, it is expected that more domestic companies will participate in the parts localization development business. 우리나라에서 운영하는 무기체계의 부품은 대부분이 해외 구매를 통해 조달됨에 따라 매년 막대한 예산이 국외로 유출되고 있다. 이에 따라 국방부 및 각 군에서는 무기체계 부품의 국산화를 위해 많은 노력을기울이고 있으나, 민간 업체에서는 군 장비의 국산화 개발 사업에 참여하는 데 많은 어려움을 겪고 있다. 따라서 본 연구에서는 민간 업체 측면에서 군의 부품국산화 개발 사업에 참여하는 데 있어서의 제한 사항들을 살펴보고 부품국산화 개발 활성화를 위한 몇 가지 제도 개선 방안을 제시하였다. 첫째는 소요제기 시 경제성 있는 품목을 적극적으로 포함하는 것이고, 둘째는 국산화 개발 적격업체 선정 평가 기준을 개정하는 것이며, 셋째는 업체의 초기 투자 비용을 지원해 주도록 제도를 개선하는 것이다. 이상과 같이 방산 장비 부품국산화 활성화를 위하여 현행 제도의 제한사항을 업체 중심으로 개선하여 더 많은 국내 기업들이 부품국산화개발 사업에 참여하길 기대한다

        The connection system of steel pile and RC footing is an important structure because the total load of upper construction should be transferred through the joint construction of different two materials-steel and RC-with strongly changed section area. Although many connection systems have been developed, their structural and economical efficiency and workability are often insufficient. Therefore, a new connecting system was developed to improve the above mentioned problems of current systems. The divided arc plate could improve the workability and economical efficiency, structural efficiency could be reached by welding. The main purpose of the research is to evaluate the structural behavior of the new designed connection system through experiments.

        흰쥐에서 심혈관 조절기전에 대한 척수의 Adenosine A2 수용체의 역할

        The present study was designed to assess the role of spinal adenosine A2 receptor in the regulation of cardiovascular functions such as mean arterial pressure (MAP) and heart rate (HR) in male Sprague-Dawley rats. Rats (250∼300 g) were anesthetized with urethane and paralyzed with d-tubocurarine and artificially ventilated. blood pressure and HR were continuously monitored via a femoral catheter connected to a pressure transducer and a polygraph. Drugs were administered intrathecally using injection cannula through guide cannula which was inserted inthrathecally at lower thoracic level through a puncture of an atlantooccipital membrane. Intrathecal injection of an adenosine A₂ receptor agonist, 5`-(N-cyclopropyl)-carboxamaidoadenosine (CPCA; 1, 2 and 3 nmol, respectively), produced a dose-dependent decrease in MAP and HR. Pretreatment with N^G-nitro-L-arginine methyl ester (L-NAME), a nitric oxide synthase inhibitor or 10 nmol of MDL-12,330, an adenylate cyclase inhibitor blocked significantly the depressor and bradycardic effect of 2 nmol of CPCA. But, Pretreatment with 3 nmol of bicuculline, gamma-aminobutyric acid A (GABA_A) receptor antagonist, or 50 nmol of 5-aminovaleric acid, GABA_B receptor antagonist did not inhibit the depressor and bradycardic effect of 2 nmol of CPCA. These results indicate that adenosine A₂ receptor in the spinal cord plays an inhibitory role in the regulation of cardiovascular function and that the depressor and bradycardic action of adenosine A₂ receptor are mediated via the synthesis of nitric oxide and the activation of adenylate cyclase in the spinal cord of rats.

        지난 100년간 세계 기온은 0.74°C 상승하였으 며, 그 상승 속도는 날로 증가하여 금세기 말에는 6.4°C가 상승할 것으로 예상된다. 지구온난화와 관련한 기후변화는 지구촌을 심각하게 위협하고 있으며, 국제적 위기상황을 반영하듯 세계는 ‘녹색 성장’이라는 주제에 국가적 역량을 집중하고 있다. 이러한 국제적 위기상황을 공동대처하기 위해 1992년에 유엔 기후변화협약(UNFCC)이 체결 된 이후, 국가간 이행 협약인 교토의정서가 정식 으로 발효되면서 온실가스 감축은 선택이 아닌 의 무가 되었다. 이에 주요 선진국들은 투자지원, 세 금 감면 등의 혜택을 통해 친환경 차량 및 바이오 연료 개발과 같은 환경친화적 신기술 개발을 장려 하는 한편, 첨단기술을 활용한 교통관리방안과 직⋅간접적으로 통행혼잡에 의한 환경적, 경제적 외부 발생비용을 부담하도록 하여 수요자체를 감 소시키는 정책도 병행하여 추진하고 있다.

        Nitric Oxide-Induced Autophagy in MC3T3-E1 Cells is Associated with Cytoprotection via AMPK Activation

        Nitric oxide (NO) is important in the regulation of bone remodeling, whereas high concentration of NO promotes cell death of osteoblast. However, it is not clear yet whether NO-induced autophagy is implicated in cell death or survival of osteoblast. The present study is aimed to examine the role of NO-induced autophagy in the MC3T3-E1 cells and their underlying molecular mechanism. The effect of sodium nitroprusside (SNP), an NO donor, on the cytotoxicity of the MC3T3-E1 cells was determined by MTT assay and expression of apoptosis or autophagy associated molecules was evaluated by western blot analysis. The morphological observation of autophagy and apoptosis by acridine orange stain and TUNEL assay were performed, respectively. Treatment of SNP decreased the cell viability of the MC3T3-E1 cells in dose- and time-dependent manner. SNP increased expression levels of p62, ATG7, Beclin-1 and LC3-II, as typical autophagic markers and augmented acidic autophagolysosomal vacuoles, detected by acridine orange staining. However, pretreatment with 3-methyladenine (3MA), the specific inhibitor for autophagy, decreased cell viability, whereas increased the cleavage of PARP and caspase-3 in the SNP-treated MC3T3-E1 cells. AMP-activated protein kinase (AMPK), a major autophagy regulatory kinase, was activated in SNP-treated MC3T3-E1 cells. In addition, pretreatment with compound C, an inhibitor of AMPK, decreased cell viability, whereas increased the number of apoptotic cells, cleaved PARP and caspase-3 levels compared to those of SNP-treated MC3T3-E1 cells. Taken together, it is speculated that NO-induced autophagy functions as a survival mechanism via AMPK activation against apoptosis in the MC3T3-E1 cells.

        집단미술치료가 외상 후 스트레스를 경험한 응급구조사의 회복탄력성과 정서 조절 곤란에 미치는 효과

        The purpose of this study was to investigate the effects of group art therapy on the resilience and emotional regulation difficulties of emergency medical technicians who experienced PTSD. The subjects were 7 emergency medical technicians working at the fire station located in the city D who experienced PTSD. The program of group art therapy was conducted once a week for 90 minutes per session, from May 1st, 2023, to August 31st, 2023, with a total of 14 sessions. As research tools, we quantitatively analyzed the pre- and post-changes using resilience scales and emotion regulation difficulties scales. Additionally, we qualitatively analyzed the changes in major content for each session using the Person in the Rain (PITR) drawing tests. The results of this study were as follows. Firstly, group art therapy had a positive effect on the resilience of emergency medical technicians who experienced PTSD. Secondly, group art therapy had a positive effect on the emotion regulation difficulties of emergency medical technicians who experienced PTSD. Thirdly, in the Person in the Rain (PITR) drawing tests, changes in emotional regulation, reduction of emotional instability, the establishment of a positive self-image, and improvement of stress-coping ability appeared. These research results lead to the conclusion that group art therapy has a positive effect on the resilience and emotion regulation difficulties of emergency medical technicians who experience PTSD. This suggests its potential utility in counseling and art therapy settings for emergency medical technicians.

        Autophagy-Related Protein MAP1LC3C Plays a Crucial Role in Odontogenic Differentiation of Human Dental Pulp Cells

        Background: Autophagy plays important roles in odontogenic differentiation of dental pulp cells (DPCs) in the developmental stage of tooth bud. Few studies have reported the role of autophagy during reparative dentin formation process. The objective of this study was to discover gene expression pattern correlated to autophagy and their role during odontogenic differentiation process in DPCs. Methods: After tooth cavities were prepared on the mesial surface of lower first molar crown of rats. Odontogenic differentiation and reparative dentin formation were assessed based on detection of morphology change with hematoxylin and eosin staining. Results: After tooth cavities were prepared on the mesial surface of lower first molar crown of rats, odontogenic differentiation and reparative dentin formation were assessed based on detection of morphology change with hematoxylin and eosin staining and dentin sialophosphoprotein (DSPP), whereas autophagy inhibitor 3-methyladenine (3MA) reversed. Results of quantitative polymerized chain reaction array of autophagosome formation related genes revealed that GABARAPL2 was prominently upregulated while expression of other ATG8 family members were moderately increased after tooth cavity preparation. In addition, human DPCs incubated in differentiation medium predominantly upregulated MAP1LC3C, which selectively decreased by 3MA but not by autophagy enhancer trehalose. Knock-down of MAP1LC3C using shRNA resulted in strong downregulation of dentin matrix protein 1 and DSPP as well-known odontogenic marker compared to knock-down of MAP1LC3B during odontogenic differentiation process of human DPCs. Conclusion: Our results suggest that MAP1LC3C plays a crucial role in odontogenic differentiation of human DPCs via regulating autophagic flux.
