        경추 골절 및 탈구의 관혈적 정복술 직후 발생한 사지마비 - 1례 보고 -

        Open reduction and internal fixation of the cervical spine is a commonly performed method of treatment for acute cervical fracture dislocation. A sudden or gradual worsening of the neurological status of a patient during reduction should alert the physician to the presence of high grade compression of the spinal canal. Loss of neurological function during or after manipulation or open reduction and internal fixation should raise the suspicion of compression of the spinal cord from a lesion occupying the canal, such as a herniated disc, buckling of the ligamentum flavum, an epidural hematoma or bone fragments. Magnetic resonance imaging or myelogram are the most helpful diagnostic means and should be used initially if suspected. Treatment is anterior decompression and autogenous strut bone graft. Causes of our case include ruptured disc, vertebral end plate and posterior longitudinal ligament We experienced a case of immediate quadriparesis after posterior decompression and sublaminar wiring for cervical fracture dislocation which was resulted from ruptured disc, vertebral end plate and posterior longitudinal ligament.

        세 가지 역충전재의 근단부 폐쇄 효과에 관한 전기화학적 연구

        The purpose of this study was to evaluate the apical sealing ability of Super-EBA, MTA and Dyract-flow as retrofilling materials. Forty-eight extracted human teeth with straight and single root canal were used in this study. The root canals were prepared to a #40 apical canal size and obturated with gutter-percha. Apicoectomies were performed and root end cavities were prepared to a depth of 3mm using an ultrasonic device. The root end cavities were filled with Super-EBA, MTA or Dyract-flow. Leakage was measured using an electrochemical technique for 4 weeks. According to this study, the results were as follows. 1. Increasing leakage with time was observed in all groups. 2. No significant difference was noted among the 3 groups with time (p = 0.216). 3. No significant difference was noted among the 3 groups when measured within the same time interval (p = 0.814). The results of this study suggest that the sealing ability of Dyract-flow is equal to that of Super-EBA and MTA, and Dyract-flow may be an alternative to other materials for root-end filling. . 본 연구의 목적은 Super-EBA, MTA, Dyract-flow를 근관 역충전재로 사용하였을 때의 근단부 폐쇄 능력을 전기화학적 방법을 통해 알아보고자 하는 것이다. 48개의 단근치를 Profile을 사용하여 근관형성한 후, Gutter-percha cone으로 수직가압 충전을 시행하였다. 근단부에서 3mm 되는 부위를 bur로 절단한 후, ultrasonic device로 3mm 깊이의 역충전 와동을 형성하였다. 각 실험군은 Super-EBA, MTA, 혹은 Dyract-flow로 역충전하고 4주에 걸쳐 전기화학적인 방법으로 치근단 누출의 변화를 측정한 후, Repeated measures of ANOVA로 통계분석하였다. 그 결과 모든 시편에서 시간에 따른 치근단 누출의 증가가 관찰되었으나, 각 실험군 간에 누출의 차이는 나타나지 않았다. 이러한 결과는 Dyract-flow의 치근단 역충전재로써의 사용 가능성을 시사한다.

        멀티미디어 응용을 위한 수송 계층에서의 유연한 오류 제어 모델

        미래의 멀티미디어 응용 환경은 각 매체와 응용의 요구 사항들을 효율적으로 충족시킬 수 있는 유연한 오류 제어 모델을 필요로 하고 있으나, 지금까지의 오류 제어는 단일 매체를 처리하기 위한 제한된 유연성만이 제공되어왔다. 이를 해결하기 위하여 본 논문에서는 매체의 신뢰도 기준인 오류 허용률(ETL: Error Tolerance Level)과 응용의 실시간성 기준인 지연(Delay)을 고려하여 수송 계층에서 매체별로 유연하게 오류 제어 방법을 적용하는 모델을 제안한다. 이 모델에서는 매체별로 오류 제어 방법을 선정하기 위하여 오류 혀용률과 등시성과 같은 매체의 속성, 응용의 속성인 지연, 그리고 망에서의 데이터 손실율, 망의 전송 형태, 응용의 연결 모드와 같은 환경 파라미터가 영향 인자로써 고려되었다. Emerging service classes need an error model which efficiently satisfies the requirements for each media and application. However limited flexibility in the error control has been provided. Therefore we propose a new error control model which applies error control scheme per media flexibly with respect to ETL(Error Tolerance Leverl) and delay. In order to select the error control scheme in this model, the attributes of media(i.e., ETL, isochronism), the attributes of application(i.e., delay) and environment parameters(i.e., packet loss ration, network types, connection modes of the application) are considered as effect factors.

        저출력 레이저가 기계적 노출치수에 미치는 영향에 관한 연구

        The purpose of this study was to investigate the bio-stimulating effect of low power density laser radiation on the mechanically exposed pulp. Class V cavities on dog's teeth were prepared and the pulps were mechanically exposed with a round bur. In control group, the exposed pulps were capped with $Ca(OH)_2$ powder and the cavities were sealed with Z.O.E.. In experimental group A, the pulps were irradiated with GaA1As laser for 5 minutes and then they were treated the same as control group. In experimental group B, the exposed pulps were covered by aluminum foil and sealed with Z.O.E. after they were irradiated with the laser as the experimental group A. In the all groups, the pulps were histopathologically observed at the time intervals of 1, 2 and 3 week after experiment and the results were statistically evaluated. The results were as follows: 1) In control and experimental groups, mild vascular congestion and bleeding was found in most of the specimens and for the new formation of dentin bridge, experimental group A had the most cases. The dentin bridge had discontinuous osteodentin like appearance without any dentinal tubules. Inflammatory cell infiltration consisted of acute and chronic inflammatory cell, and the formation of microabscess was also observed. 2) The degree of inflammatory cell infiltration was not significantly different among control group and experimental groups at 1 week, 2 week and 3 week. 3) The formation of new dentin bridge was not significantly different between control group and experimental group A at 1 week, but at 2 week and 3 week, experimental group A showed significantly more cases of new dentin formation than control groups. (P < 0.05). 4) Between control group and experimental group B, there was no significant difference in formation of the new dentin bridge at 1, 2 and 3 week. (P> 0.05). 5) There was no significant difference in formation of the new dentin bridge at 1 and 2 week between experimental group A and experimental group B, but at 3 week, the former significantly had more cases of new dentin bridge formation than the latter.(P < 0.05).

        일본 나가노현 노자와온센 지역의 온천공유자원 관리방식에 관한 연구

        In the Japanese community, traditional CPRs (common pool resources or commons) have been managed by the Irai group. An important feature of these commons is that they have maintained a long–term autonomous management system and that the traditional way continues to be maintained. The hot springs and forests, which occupy most of the commons of Nozawa Onsen Village, were transferred to the traditional village before the regional merger in the late 19th century. At that time, forests that were organized as traditional villages were converted into the property of the residents, and were not transferred to local governments through the subsequent reform of the local system, remaining in the possession of the Irii group. This phenomenon is found all over Japan. Because the common law is more dominant than the modern civil law in the management of these commons, the traditional organization of the community enables the role of strong governance. This paper examines the history and current status of traditional organizations in a Japanese community in pursuit of endogenous development through community practice with residents and local governments. 일본의 지역사회에는 지역공동체가 관리하는 전통적인 공유자원이 존재해 왔다. 이들 공유자원의 중요한 특징은 자치적 관리방식을 오랜 기간 동안 지속해 왔으며 오늘날에도 전통적인 방식이 유지된다는 점이다. 노자와온센무라의 공유재산의 대부분을 차지하는 온천과 임야는 19세기 말 지역합병 이전의 자연촌 단위에 연원한다. 당시 새로운 행정 구역으로 재산을 이전하지 않고 자연촌을단위로 재산구를 구성하거나 주민의 공유로 전환된 재산은 이후의 지방제도 개혁을 통해서도 지역 집단의 소유로 남았다. 이런 현상은 일본 전국에 걸쳐서 발견된다. 이러한 공유자원의 관리에는 근대적 민법보다 관습법이 우선시되기 때문에 지역사회의 전통조직이 관리의주체가 된다. 이 논문에서는 한 지역사회의 전통조직에서 이루어지는 공유자원 관리방식의실천을 통하여 공동체의 구체적 모습을 드러내 보이고자 한다.
