        Recently, visiting of the President activates the trade between South-North Korea. But a study of North Korea, specially in building code, was much insufficient. The purpose of this comparison was to understand the characteristics of building codes and to examine effects in Building industry in North Korea. The results of this comparison were as follows : 1) The characteristics of building code in North Korea are to regulate only common principles and political tendencies and building code is focused on development of the National economy 2) In North Korea, building constructions were controled by National Planning committee and made a point of convenience and economy.

        급성 호흡근 마비를 보인 Sj?gren 증후군 1예

        Sjögren syndrome is a chronic autoimmune disease characterized by lymphocytic infiltration of exocrine glands resulting in dry mouth and eyes. Approximately one-third of patients present with systemic manifestations, but respiratory muscle involvements have been rarely reported. We report a case of acute respiratory failure complicated by primary Sjögren syndrome. Muscle biopsy revealed perivascular lymphocytic infiltrations. Corticosteroid therapy improved respiratory muscle weakness. Sjögren syndrome should be considered as one of the underlying diseases causing acute respiratory failure.

        Facilitating Stroke Management using Modern Information Technology

        Background and Purpose Information technology and mobile devices may be beneficial and useful in many aspects of stroke management, including recognition of stroke, transport and triage of patients, emergent stroke evaluation at the hospital, and rehabilitation. In this review, we address the contributions of information technology and mobile health to stroke management. Summary of Issues Rapid detection and triage are essential for effective thrombolytic treatment. Awareness of stroke warning signs and responses to stroke could be enhanced by using mobile applications. Furthermore, prehospital assessment and notification could be streamlined for use in telemedicine and teleradiology. A mobile telemedicine system for assessing the National Institutes of Health Stroke Scale scores has shown higher correlation and fast assessment comparing with face-to-face method. Because the benefits of thrombolytic treatment are time-dependent, treatment should be initiated as quickly as possible. In-hospital communication between multidisciplinary team members can be enhanced using information technology. A computerized in-hospital alert system using computerized physician-order entry was shown to be effective in reducing the time intervals from hospital arrival to medical evaluations and thrombolytic treatment. Mobile devices can also be used as supplementary tools for neurologic examination and clinical decision-making. In post-stroke rehabilitation, virtual reality and telerehabilitation are helpful. Mobile applications might be useful for public awareness, lifestyle modification, and education/training of healthcare professionals. Conclusions Information technology and mobile health are useful tools for management of stroke patients from the acute period to rehabilitation. Further improvement of technology will change and enhance stroke prevention and treatment.

        재하방법에 따른 사질토 지반의 연직응력 특성

        This study was carried out to evaluate the vertical stress properties in sandy soil according to changes of loading type in soil bin compacted three layers. The following conclusions and comparisons have been made based on careful analysis from theoretical and experimental methods. : When sandy soil subjected to cycle-loading, compression of foundation and diffusion of vertical stress increment(${\Delta}{\sigma}_2$) were influenced by magnitude of loading plate. When sandy soil subjected to reloading after removing of pre-loading, the distribution of ${\Delta}{\sigma}_2$ depth at one time of loading plate width was different from its distribution at more deep point cause of load hysteresis, so in case of design of structure, the effect of ${\Delta}{\sigma}_2$ as depth must be considered. The increment of vertical stress will be different as loading condition and foundation depth, the loading condition must be considered in case of structure design.

        Development of Smartphone Application That Aids Stroke Screening and Identifying Nearby Acute Stroke Care Hospitals

        Purpose: The benefits of thrombolytic treatment are time-dependent. We developed a smartphone application that aids stroke patient self-screening and hospital selection, and may also decrease hospital arrival time. Materials and Methods: The application was developed for iPhone and Android smartphones. Map data for the application were adopted from the open map. For hospital registration, a web page (http://stroke119.org) was developed using PHP and MySQL. Results: The Stroke 119 application includes a stroke screening tool and real-time information on nearby hospitals that provide thrombolytic treatment. It also provides information on stroke symptoms, thrombolytic treatment, and prescribed actions when stroke is suspected. The stroke screening tool was adopted from the Cincinnati Prehospital Stroke Scale and is displayed in a cartoon format. If the user taps a cartoon image that represents abnormal findings, a pop-up window shows that the user may be having a stroke, informs the user what to do, and directs the user to call emergency services. Information on nearby hospitals is provided in map and list views, incorporating proximity to the user’s location using a Global Positioning System (a built-in function of smartphones). Users can search for a hospital according to specialty and treatment levels. We also developed a web page for hospitals to register in the system. Neurology training hospitals and hospitals that provide acute stroke care in Korea were invited to register. Seventy-seven hospitals had completed registration. Conclusion: This application may be useful for reducing hospital arrival times for thrombolytic candidates.

      • 당단백질 IIb/IIIa 수용체 길항제의 현재와 미래

        Department of Neurology, Brain Research Institute, Yonsei University College of Medicine, Seoul, Korea1 9 1 8년 스위스의 의사 G l a n z m a n n은 주로 점막출혈을 보이고 심각한 출혈은 잘 일어나지 않는 혈액이상을 보이는 환자를 보고하였으며 후에 Glanzmann thromb a s t h e n i a라는 병명으로 알려지게 되었다. 1974년에 이르러서야 혈소판의 응집( a g g r e g a t i o n )에 당단백질 (glycoprotein, GP) II 및 III 가 관여하고 있으며, 이러한 당단백질의 문제로 혈소판의 응집에 장애가 생기는 것이 이 병의 원인이라고 알려지게 되었다. GP IIb/IIIa는 i n t e g r i n의 한 종류이다. Integrin은 대부분의 세포에 존재하는데 세포-세포, 세포-기질 간의 부착에 관여할 뿐만 아니라 세포의 생존에 필수적인 신호전달을 매개한다. 다른 i n t e g r i n이 여러 세포나 구조에 존재하는 것과 달리 GP IIb/IIIa 수용체는 혈소판과 거핵세포( m e g a k a r yo c y t e )에만 존재한다. GP IIb/IIIa 수용체는 활성화되지 않은 혈소판에서 약 6 0 ~ 8 0 , 0 0 0개 정도 존재하는데. 혈소판이 thrombin, adenosine diphosphate (ADP) 또는 collagen 등의 효현제에 의해서 활성화되면 G PIIb/IIIa 수용체의 수가 늘어난다. 그러나 이러한 수적인변화보다 혈소판 활성화에 의해서 나타나는 가장 현저한 변화는 GP IIb/IIIa 수용체가 불활성화 상태에서 활성화 상태로 구조적인 변화를 하는 것인데, 이는 혈소판의 inside-out 신호전달에 의해 이루어지게 된다[1]. 현재 임상에서 사용되는 대부분의 항혈소판제제들은 이러한 inside-out 신호전달을 차단시키는 약들이다. 하지만 이러한 약제들은 혈소판의 특정 활성경로를 억제시킬 뿐이어서 단독사용 시 그 효과 역시 제한적일 수 밖에 없다. 반면 GP IIb/IIIa 수용체는 혈소판 응집의 최종 공통 경로 (final common pathway)이기 때문에, 이 수용체를 봉쇄할 수 있다면 혈소판 응집을 효과적으로 억제할 수 있을 것이다. GP IIb/IIIa 수용체 길항제가 주목을 받기 시작하고, 실제 임상에 쓰이게 된 것은 전술한대로 GP IIb/IIIa 수용체가 혈소판에만 특이적으로 존재하는 i n t e g r i n이면서 동시에 혈소판 응집의 최종 공통 경로이기 때문이다(Fig. 1). 최근 이 약제가 관상동맥질환에서 뛰어난 효과를 보이면서 뇌졸중으로 활용 범위가 넓혀지고 있다. 본 종설에서는 현재 개발되어 임상에 사용되고 있거나 임상연구가 진행된 약제를 중심으로 임상에서의 유용성과 미래의 쓰임새에 대해 알아보고자 한다.

      • 대동맥 죽상경화를 동반한 급성 뇌경색 환자의 임상적 특징

        Background: Transesophageal echocardiography (TEE) is a useful tool for evaluating aortic plaques. Severalinvestigators have reported that the aortic arch atherosclerosis is a potential source of systemic emboli and anindependent risk factor for ischemic stroke. But the clinical characteristics and neuroimaging findings of thestroke patients with aortic plaques remain unknown. Methods: From Yonsei Stroke Registry, we reviewed 333consecutive patients with acute ischemic stroke, who underwent both TEE and vascular imaging studies. We analyzedthe risk factor profiles, clinical features, neuroimaging findings, and TEE data of these patients. Based onthe TEE findings, the plaques protruding into the lumen≥4 mm, mobile or ulcerated lesions in the proximal aortawere defined as complex aortic plaques (CAP). The possible etiologies of stroke were classified into four groups;CAP only group, potential cardiac sources of embolism (PCSE) only group, relevant artery atherosclerosis (RAA)only group, and more than two etiologies or cryptogenic group (uncertain group). R e s u l t s: Among the 333patients, aortic plaques were found in 105 (31.5%) patients, Fifty nine patients (17.7%) had the CAP. The patientswith CAP were older (p〈0.01) and had more frequent history of cigarette smoking (p=0.01) and ischemic stroke(p=0.04) than those without. Total cholesterol level was also higher (p=0.02). The etiologic evaluations revealedCAP only in 31 (9.3%), PCSE only in 68 (20.4%), RAA only in 59 (17.7%), uncertain mechanism in 175 (52.6%)patients. The CAP only group less often had cortical dysfunctions (9.7%, p〈0.05), but more likely to have a classiclacunar syndrome (54.8%, 〈0.01). Less than 1 cm sized lesions were frequently found in CAP only group(55.6%), when compared with PCSE only (19.4%, p〈0.01), RAA only (29.8%, p=0.03), or uncertain group(25.0%, p〈0.01). C o n c l u s i o n s: Aortic plaques were frequently found in acute stroke patients. The CAPs wereclosely related with old age, smoking, previous stroke, and hypercholesterolemia. The clinical presentations ofCAP patients were characterized by minor stroke symptoms and small lesion sizes.(Korean Journal of Stroke 2005;7(2): 165~171)

        사질토 지반의 상대다짐도 및 토층에 따른 연직지중응력 분포 특성

        This study was carried out to evaluate the vertical stress properties in sandy soil according to changes of foundation condition in soil bin compacted three layers. The following conclusions and comparisons have been made based on careful analysis from theoretical and experimental methods. : When sandy soil subjected to circular uniform load, the vertical stress increments ($\Delta\sigma_z$) was increased as load increasing, the maximum values of $\Delta\sigma_z$ was achieved at the point loading axis, and $\Delta\sigma_z$ was not shown over at a distance of three times of loading plate width (B). The vertical stress increments were achieved largely at 80 % relative compaction (Rc) compared to 95 % relative compaction due to stress concentration in sandy soil. When sandy soil subjected to circular uniform load, the $\Delta\sigma_z$ differences between theoretical and experimental values as load increased were more increased and its maximum differences were achieved at stress axis. When gravel surface macadamized over sandy soil subjected to load, the $\Delta\sigma_z$ was concentrated to load axis as load increasing, so that macadamization will be decreased load transmission.
