This work was done for 9 patients having experience of a herb medical treatment after being diagnosed as CVA during a year from January, 1996 to December, 1996 by using an ethnographic research method. The summarized results of this research are following. Ⅰ. THE EXPERIENCE OF THE ILLNESS First, the falling-ill phase is the time that they have the first stroke of paralysis and the decision pattern of medical institution' comes out. The emotional experience in the period is something like 'flustration', 'anxiety', 'despair', and 'expectation'. Second, the active-treatment phase is the time that the patients as well as their family or care giver not only show the positive attitude and actively participate in the illness treatment but also show a lot of interest in medical institutions and activities of health recovery. There is a primary factor of the continuation of treatment as an experience of treatment and being crushed and sensitivity as an experience of the illness. Third, the rehabilitation phase is the time that the patients or their family become tired and insensitive to the treatment and recuperation, and then reduce the treatment activity. There is a primary influence factor of the discontinuance of treatment as an experience of treatment and physical experience and emotional experience as an experience of the illness. The physical experience is divided into 'personal-hygiene care', and 'the sphere of activity' The emotional experiences are 'blaming someone', 'contempt' and 'despair' as a negative experience and 'hope' as a positive experience. Ⅱ. COPING STRATEGY There are a physical coping, an emotional and mental coping, a social coping, and a spiritual coping as a coping strategy used for the patients to overcome their illness and adjust themselves to their altered life. First, the physical coping comes out as 8 categories, 'using an auxiliary tool', 'doing exercise', 'protecting', 'improving their diet', 'taking care of something', 'using subsidiary medicines', 'trying a folk remedy', and 'having interest in their health'. Second, for the emotional and mental coping, there are 'accepting' and 'trying' as a positive coping and a failure of control as a negative coping. Third, the social coping is appeared as 'being supported'. Fourth, the spiritual coping is recognized as' recourse to God' and 'preparation of death'. After all, the elderly CVA patients in an agricultural area choose the act of treatment based on the traditional belief and the relationship with a caretaker. A personal health can be maintained by taking care of themselves and controling their mind, and the overcome of the illness is decided on the basis of traditional concepts and cultural principles in which the patients as well as the family, neigbors and take carers should work out together and cooperate with each other in order to achieve that.
두 진단검사의 비교에 대한 민감도와 특이도의 다변량 메타분석법

질병에 대한 새로운 진단검사 방법이 의학 연구자들에 의해 끊임없이 개발되고 있으며, 기존 진단검사 방법과 새로운 진단검사 방법을 비교하는 연구논문이 계속 출간되어 누적되고 있다. 메타분석법으로 다수 연구논문의 결과를 종합하여 정확성이 높은 진단검사에 대해 객관적인 결론을 내리게 된다. 이와같이 출간된 두 진단검사를 비교하는 각 연구논문은 각각 질병을 가진 개체와 질병을 가지지 않은 개체에 두 검사를 모두 실시하여 한 쌍의 민감도와 특이도를 구하여 비교한다. 이러한 연구논문의 결과를 종합하는 메타분석은 동일 개체에 실시한 두 검사로 인해 한 쌍의 민감도간의 연관성과 한 쌍의 특이도 간의 연관성을 고려한 메타분석법을 본 논문에서 제시한다. 논문예제 자료와 모의시험으로 메타분석 검정통계량의 효율성을 평가한다. Researchers are continuously trying to find innovative diagnostic tests and published articles are accumulating at an enormous rate in many medical fields. Meta-analysis enables previously published study results to be reviewed and summarized; therefore, an objective assessment of diagnostic tests can be done with a meta-analysis of sensitivities and specificities. Data obtained by applying two diagnostic tests to a well-defined group of diseased patients produce a pair of sensitivity and by applying the same medical tests to a group of non-diseased subjects produce a pair of specificity. The statistical tests in the meta-analysis need to consider the correlatedness of the results from two diagnostic tests applied to the same diseased and non-diseased subjects. The associations between two diagnostic test results are often found to be unequal for the diseased and non-diseased subjects. In this paper, multivariate meta-analytic methods are studied by taking into account the different associations between correlated variables. On the basis of Monte Carlo simulations, we evaluate the performance of the multivariate meta-analysis methods proposed in this paper.

Fatigue is a common complaint and affects the quality of life in modern people. Physical stress may induce activation of certain immune cells. Fermented porcine placenta (FPP) has been used to alleviate fatigue. Inflammatory cytokines are produced by physical stress and results in symptoms of fatigue. However, the role of FPP on fatigue-associated inflammatory cytokine production has not been elucidated yet. Thus, we estimated the anti-fatigue effect of FPP and its active components, leucine (Leu) and lysine (Lys) in activated RAW264.7 macrophages and forced swimming test (FST) fatigue animal model. Pretreatment with FPP, Leu, or Lys significantly inhibited the lipopolysaccharide (LPS)-induced tumor necrosis factor-α, interleukin (IL)-1β, and IL-6 production without inducing cytotoxicity on LPS-stimulated RAW264.7 macrophages. FPP, Leu, or Lys inhibited the production of nitric oxide and downregulated the expression of inducible nitric oxide synthase on LPS-stimulated RAW264.7 macrophages. Furthermore, caspase-1 activities increased by LPS were significantly reduced by FPP, Leu, or Lys. In the FST, inflammatory cytokine levels of the mice administrated with FPP, Lys, and Leu were significantly reduced compared with the control group at 21 days. Collectively, these results show that anti-fatigue effect of FPP and its active components, Leu and Lys might be derived from the down-regulating of inflammatory mediators.
