        정신병의 수용전념치료

        There has been increasing interest in the psychosocial treatment of psychosis based on Acceptance and Commitment Therapy (ACT), which has the primary aim of helping people more effectively engage in their lives guided by their values. The approach includes various techniques to change one’s relationship to private experiences, such as hallucination and delusion, rather than rectifying the contents of these experiences. Using the six core processes of the ACT model (hexaflex) tailored to the characteristics of people with psychosis may help alleviate their distress from the symptoms and shape pragmatic value-consistent behaviors. This review introduces the ACT model of psychosis and illustrates the specific ways of applying ACT to people with psychotic experiences.

        This review underscores the integration of cognitive behavioral therapy (CBT) as a critical component of psychiatry residency programs essential for the comprehensive training of residents. The core competencies psychiatry residents should develop as CBT therapists, including specific skills and knowledge domains, are outlined. In addition, the review addresses the necessary attitudes and techniques for faculty engaged in CBT supervision, providing insight into effective educational and supervisory strategies. The application of these principles in clinical training settings is discussed, highlighting their potential to improve the quality and effectiveness of CBT training in psychiatry residency programs.

        Acceptance and Commitment Therapy (ACT) is a functional contextual intervention approach based on the behavioral theory on human language, which views human suffering as originating in psychological inflexibility fostered by cognitive fusion and experiential avoidance. Thus, the goal of ACT is to enhance psychological flexibility using six core processes including acceptance, cognitive defusion, self-as-context, contact with present moment, values clarification, and committed action. Recent clinical trials have suggested the efficacy for ACT in the treatment of various mental illness and psychological distress. The aim of this review is to offer more knowledge and better understanding of ACT by presenting its underlying principle and an overview of the research field.

        Distress in patients with digestive cancers can considerably impact not only their treatmentadherence but also their health-related quality of life. Early distress recognition in patients, appropriate interventions by the oncology team, or referral to mental health professionals, canenhance the treatment efficiency. This review aims to provide an overview of distress evaluationand its management in patients with cancer, specifically digestive cancers, and to address thepsychological responses and distress that occur in those patients during treatment. Furthermore, the review will introduce possible psychological interventions for patients with digestivecancers who experience significant distress, depending on the duration and planning of thetreatment.

        중독질환의 수용전념치료

        Acceptance and Commitment Therapy (ACT) is one of the mostwell-known third wave cognitive behavioral therapies (CBTs)based on relational frame theory (RFT) and functional contextualism.From an ACT perspective, human suffering and psychopathologyresult from psychological inflexibility, which is drivenby experiential avoidance and cognitive fusion. Within the context,addiction is conceived as a component of the learned repertoireof behavior driven by psychological flexibility. Addictive behaviorsare remarkably effective, over the short term, for peopleto avoid their unwanted private events such as negative thoughts,affects, and physical sensations but also to make coherent consequencesin line with their verbal rules about behaviors andthemselves. However, addictive behaviors can cause great difficultieswith their values and disconnect from their life areas inthe long term. ACT can help people with addiction by fosteringpsychological flexibility and moving in the direction of a valuedlife. This review aims to introduce the pathology of addiction fromthe perspective of ACT and to discuss the possibility of use ofACT as a novel therapeutic approach to treating addiction with itskey features of addiction treatment processes in clinical settings.

        금연치료의 장애물인 알코올 의존의 접근 방법

        Smoking continues to be as great a hazard to global health as alcohol use. Tobacco (nicotine) and alcohol dependence share a high degree of co-morbidity and interact across many levels of analyses involving potentiated reinforcement. The co-use of these two addictive substances accelerates ‘synergistic adverse effects’, such as the development of various types of cancer and cognitive impairment. Accordingly, critical steps in the process of smoking cessation treatment may require an under-standing of the interaction between alcohol and tobacco dependence. This review discusses how to utilize knowledge of alcohol and tobacco interactions to encourage smoking cessation in alcohol-dependent smokers.

        Global consumption of energy drinks has prompted significant concerns regarding its associated risks. Adolescence marked by heightened consumption of these beverages coincides with a crucial period of brain development, raising apprehensions about the adverse impact of psychostimulants such as caffeine on this process. Moreover, emerging evidence underscores the link between energy drink use and mental health issues, compounded by heightened risk when combined with alcohol. This review adopts an addiction psychiatry lens to scrutinize the prevalence and implications of high-caffeine energy drink consumption among adolescents, offering insights into clinical practice responses.

        Smartphones have significantly changed our lives and become a digital lifestyle hub. However, uncontrolled smartphone usage has created a new issue in global health, including a range of mental health concerns. Previous studies have proposed that excess use of a smartphone that interferes with daily life should be called “smartphone addiction”, which has similar core features to substance- related disorders (substance addiction) and known behavioral addictions. The aim of this review was to add to our understanding of smartphone addiction by presenting an overview of the research field. (Korean J Med 2016;91:245-249)

        조현병 환자에서 항정신병약물 유지치료 시 치료결과에 대한 연구

        Objectives : Relapse prevention is a major therapeutic goal in the treatment of schizophrenia. However, many patients experience multiple functional impairments and treatment resistance due to recurrence. This study was designed to investigate the follow-up of patients with using antipsychotic drugs and to compare the total treatment failure rate, withdrawal reasons, and duration period of antipsychotic drugs. Methods :IThe subjects were 1963 patients who taking antipsychotic drugs under the diagnosis of schizophrenia. We selected 1836 patients using 10 antipsychotic drugs according to frequency of using. The rate of total treatment failure of them was divided into 6-month, 1-year, 2-year, 3-year, and 5-year according to the time of drug withdrawal. We compared the total treatment failure rate at 1 and 3-year between 10 antipsychotic drugs. Results : The total treatment failure rate of clozapine was lowest compared with the other 9 antipsychotic drugs in all the surveyed periods. When evaluating actual number of subjects, olanzapine, sulpiride, risperidone, aripiprazole, amisulpride, and haloperidol were lower significantly compared with ziprasidone at 1-year in the total treatment failure rate, but there was no significant difference between them except clozapine at 3-year. The results of the analysis based on the number of prescriptions showed that the total treatment failure rate of the atypical antipsychotic drug was lower than that of the typical antipsychotic drug at 1-year, but the difference was decreased over time except quetiapine and ziprasidone. Conclusion : In conclusion, although there is some controversy about which drug to prescribe to the patient, the clinician needs a proper prescription considering various factors such as efficacy, side effects, price, and formulations of each drug.
