정신분열병 환자의 자기효용성 증진을 위한 정신사회재활치료 프로그램의 개발과 효과성 연구

본 연구는 정신분열병 환자의 자기효용성 증진을 위한 정신사회재활치료 프로그램을 개발하고 실시하여 그 효과성을 검증하는데 목적이 있다. 본 연구의 효과성 검증은 본 프로그램에 참가한 대상자 15명 중 총 결석회수가 3회 이하인 환자 8명을 대상으로 분석하였으며, 프로그램구성은 집단활동 28회기와 콘서트발표로 하였다. 그리고 본 프로그램 효과성을 측정하기 위하여 사용한 측정도구로는 Barbara Mcdermott(1955)가 개발한 ‘자기효용성’척도를 번안하여 사용하였다. 본 프로그램 실시 후, 통계처리 결과에 따르면 본 프로그램 개입 후에 자기효용성이 유의한 차이를 나타내어 프로그램 효과성을 입증 할 수 있었다. 그리고 운영과정에서 나타난 보완점을 제언하고 앞으로 더 많은 포괄적인 정신사회재활치료 프로그램이 개발 될 수 있기를 기대하였다. The purposes of this study were to develop psycho-social rehabilitation program and to evaluate the effectiveness of that. This study tried to promote program participants self-efficacy level, who have been suffered from schizophrenia. Eight schizophrenics, who have had absented less than four times during the program procedure, were studied with both quantitative and qualitative research methods. Quantitatively, measuring the self-efficacy level of the program participants with the Self-Efficacy scale, which was developed by Barbara McDermott(1995), has had been used. Qualitatively, individual case recordings have had been carefully analyzed. The program was composed of 28 sessions of group activities including one concert presentation. It was observed that there are some statistically significant differences in program participants overall self-efficacy level at the time of program terminated. According to these results, the positive effectiveness of this program would be verified. Especially, this community-based psycho-social rehabilitation program was effective in promoting the schizophrenics social skills.

Rosiglitazone metabolites in rat plasma were analyzed after intraperitoneal and oral administration to rats. Seven metabolites (M1-M7) were detected in rat plasma (IP and PO), and the structures were confirmed using liquid chromatography-triple time of flight (TOF) mass spectrometry; as a result, the most abundant metabolite was M5, a de-methylated rosiglitazone. Other minor in vivo metabolites were driven from monooxygenation and demethylation (M2), thiazolidinedione ring-opening (M1, M3), mono-oxygenation (M4, M7), and mono-oxygenation followed by sulfation (M6). Among them, M1 was found to be a 3-{p-[2-(N-methyl- N-2-pyridylamino)ethoxy]phenyl}-2-(methylsulfinyl)propionamide, which is a novel metabolite of rosiglitazone. There was no significant difference in the metabolic profiles resulting from the two administrations. The findings of this study provide the first comparison of circulating metabolite profiles of rosiglitazone in rat after IP and PO administration and a novel metabolite of rosiglitazone in rat plasma.

노래를 매개로 한 집단활동이 재가 정신분열장애인의 음성증상 변화에 미치는 영향

회복지실천이 관심을 두는 다른 욕구집단 즉, 빈곤계층이나, 일반적 장애인, 혹은 범죄인 등과 마찬가지로 정신장애인들의 탈시설화가 곧 그들의 지역사회통합을 보장해 주는 것은 아니다. 오랜 기간 병원과 같은 폐쇄적인 시설에 격리되어 있던 정신장애인이 지역사회로 돌아와 통합되기 위해서는 그들의 지역사회 적응을 원조해 주기 위한 적절한 서비스가 지역사회 내에서 지속적으로 제공되어야 한다. 이러한 기능을 수행하도록 기대되고 있는 것이 바로 지역사회정신보건센터일 것이다. 2004년 6월 말 현재 우리나라에는 전국적으로 115개소의 지역사회정신보건센터가 설치되어 운영되고 있으며, 시설 이용 연 인원은 전체 등록 정신장애인의 절반에 조금 못 미치는 25,181명에 이른다. 그러나 이들은 대부분 지역사회에 온전히 통합․정착하지 못하고 시설과 지역사회를 전전하며 떠돌고 있는 실정이다. 본 연구는 이러한 현상이 효과적인 지역사회정신보건 프로그램의 부재로 인한 것이라는 문제인식으로부터 계획되었다. 정신장애인의 상당부분을 차지하고 있는 정신분열장애인의 지역사회에의 적응과 통합을 저해하는 요인을 찾아내어 적절한 해결을 도와주어야 할 필요성의 인식으로부터 본 연구는 시행되었다. 본 연구에서는 정신분열이 갖는 병리적 증상 중 사회적응능력을 결정적인 영향을 미치는 것으로 알려져 있는 음성증상과 정신장애에 대한 지역사회의 왜곡된 인식을 정신분열장애인의 지역사회통합을 저해하는 요인으로 보았다. 따라서 본 연구는 정신분열장애인의 음성증상 완화를 도울 수 있고 또 동시에 지역사회의 인식을 개선할 수 있는 프로그램을 개발하고 시행함으로써, 정신분열장애인의 지역사회통합을 위해 필요한 개인의 변화와 환경의 변화를 추구하고자 하는 목적을 설정하였으며, 이의 실천 과정 및 목적 달성 여부를 양적․질적으로 평가함으로써 향후 지역사회내에서 정신장애인들을 대상으로 계획되고 또 제공될 프로그램의 방향을 제시하고자 하였다.

A bioequivalence study of LANIDIEM® tablet 4 mg (Samil. Co., Ltd.) to Vaxar® tablet 4 mg (GlaxoSmithKline Co., Ltd.) was conducted according to the guidelines of Korea Food and Drug Administration (KFDA). Forty healthy male Korean volunteers were enrolled in the study and thirty six volunteers completed the study according to the protocol. Thirty six volunteers received each medicine at the lacidipine dose of 4 mg in a 2×2 crossover study. There was one week wash-out period between the doses. Plasma concentrations of lacidipine were monitored by a high performance liquid chromatography – tandem mass spectrometry (LC-MS/MS) for over a period of 24 hours after drug administration. AUCt (the area under the plasma concentration-time curve from time zero to 24 hr) was calculated by the linear trapezoidal rule method. Cmax (maximum plasma drug concentration) and Tmax (time to reach Cmax) were compiled from the plasma concentration-time data. Analysis of variance was carried out using logarithmically transformed AUCt and Cmax. No significant sequence effect was found for all of the bioavailability parameters indicating that the crossover design was properly performed. The 90% confidence intervals of the AUCt ratio and the Cmax ratio for LANIDIEM® / Vaxar® were log 0.8102~log 1.0417 and log 0.8493~log 1.1439, respectively. These values were within the acceptable bioequivalence intervals of log 0.80~log 1.25. Thus, our study demonstrated the bioequivalence of LANIDIEM® tablet 4 mg and Vaxar® tablet 4 mg with respect to the rate and extent of absorption.

정신분열병 환자와 일반인의 고독 및 자기표현 평정과의 관계

본 연구는 2005년 5월 9일부터 5월 18일까지 S정신 병원과 P정신 요양원 재활병동에서 재활치료를 받고 있는 만성정신분열병 환자들과 일반인을 대상으로 사회·인구학적 특징과 고독 및 자기표현 평정을 조사하여 비교해 봄으로써 정신사회 재활치료의 효과적 개입의 기초 자료로 삼고자 하였다. 환자군은 DSM-Ⅳ진단기준에 의한 환자 128명이고, 일반인군은 N지역에 살고 있으면서 환자군과 교육수준ㆍ나이ㆍ성별 등이 비슷한 123명으로 하였다. 사회·인구학적 특징에 관한 조사는 6문항으로 직접 만들어 사용하였고, 고독에 대한 평가는 김교헌(1991)이 번안한 Russell 등 (1980)에 의해 개발된 개정판 고독척도(The Revised UCLA Loneliness Scale : 이하 UCLA) 총 20문항을 정신분열병 환자들이 쉽게 이해 할 수 있는 용어로 재해석하여 사용하였다. 그리고 자기표현 평정 평가는 변창진․김성희(1981)가 번안한 Rakos & Shroeder(1980)에 의해 개발된 자기표현 평정 척도(Self-Directed Assertiveness Training : 이하 SDAT) 총 20문항을 환자군의 실제 이해수준을 고려하여 약간 수정하여 사용 하였다. 사회·인구학적 특징과 UCLA, SDAT 제반 척도간의 통계처리는 SPSS PC<sup>⁺</sup> Ver 11.0을 이용하여, Frequencies, one way ANOVA, t-검증으로 통계적 유의성을 비교하였다. 그 결과 정신분열병 환자군이 일반인군에 비해 고독을 더 많이 느끼고 있었고, 자기표현평정을 잘못하고 있었다(p< .001). 본 연구 결과 증상이 안정된 정신분열병 환자의 재활치료 개입 시 고독과 자기표현 평정이 전반적 사회·심리적인 기능을 예측하는 한 인자가 될 수 있음을 알 수 있었다. This study collects basic data for effective intervention of rehabilitation treatment by investigating and analyzing social․demographic characteristics and solitude․ self-expression serenity through chronic schizophrenia patients and non-patients as subjects of the research on May. The patients are selected from rehabilitation ward in S psychiatric hospital and P mental recuperation ward. The experimental group is consisted of 128 patients by DSM-IV diagnosis standard, and the control group is consisted of 123 non-patients who live in n city and have similar educational level, age, and gender to patients. I made 6 inquiries myself and used it for an investigation of social․ demographic characteristics, and I chose total 20 inquiries from The revised UCLA Loneliness Scale : below UCLA developed by Russell et al. (1980) and translated by Kim, Gyo Heon(1991) for a valuation of solitude and reused them by interpreting to easy terms patients can understand. I also selected total 20 inquiries from Self-Directed Assertivenss Traning : below SDAT developed by Rakos & Shroeder(1980) and translated by Byeon, Chang Jin·Kim, Seong Hui(1981) and partially modified them for the actual understanding level of patients. I investigated statistic significance by frequencies, t-test one way ANOVA, based on SPSS PC<sup>⁺</sup> Ver 11.0 for social·demographic characteristics, statistic treatment of UCLA, and SDAT various criterions. The conclusion appeared that patients got more damaged significantly in solitude and self-expression serenity than non-patients. This study suggests that the rehabilitation intervention for the relieved schizophrenia patients needs to take account of solitude and self-expression by them.
