        Lumbar Interbody Fusion and Osteobiologics for Lumbar Fusion

        Lumbar interbody fusion (LIF) is an excellent treatment option for a number of lumbar diseases. LIF can be performed through posterior, transforaminal, anterior, and lateral or oblique approaches. Each technique has its own pearls and pitfalls. Through LIF, segmental stabilization, neural decompression, and deformity correction can be achieved. Minimally invasive surgery has recently gained popularity and each LIF procedure can be performed using minimally invasive techniques to reduce surgery-related complications and improve early postoperative recovery. Despite advances in surgical technology, surgery-related complications after LIF, such as pseudoarthrosis, have not yet been overcome. Although autogenous iliac crest bone graft is the gold standard for spinal fusion, other bone substitutes are available to enhance fusion rate and reduce complications associated with bone harvest. This article reviews the surgical procedures and characteristics of each LIF and the osteobiologics utilized in LIF based on the available evidence.

        나노펄스 플라즈마를 이용한 탈황 시스템의 H<sub>2</sub>O 및 NH<sub>3</sub>, 펄스 인가전압에 따른 입자변환 분석

        Nano-pulse plasma technology has great potential as the process simplicity, high efficiency and low energy consumption for SO<sub>2</sub> removal. The research on the gas-to-particle conversion is required to achieve higher efficiency of SO<sub>2</sub> gas removal. Thus, we studied the effect of the relative humidity, NH<sub>3</sub> concentration and applied voltage of the nano-pulse plasma system in the gas to particle conversion of SO<sub>2</sub>. The particles from the conversions were increased from 10 to 100 nm in diameter as relative humidity, NH<sub>3</sub> concentration, applied voltage increases. With these results, nano-pulse plasma system can be used to more efficient removal of SO<sub>2</sub> gas by controlling above parameters.

        수종의 근관세척액과 Listerine<sup>(R)</sup>의 항균성 비교 연구

        본 연구는 여러 가지 근관세척액(NaOCl, CHX, EDTA)과 구강세정제로 사용되는 $Listerine^{(R)}$을 근관감염균주인 Porphyromonas gingivalis와 Enterococcus faecalis를 상대로 항균효과를 비교하고 $Listerine^{(R)}$이 근관세척액으로 사용가능한지를 확인하고자 시행하였다. 본 연구에서는 Porphyromonas gingivalis ATCC 3327과 Enterococcus faecalis ATCC 29212의 표준균주를 사용하였다. 실험을 위한 근관세척제로 0.1%, 0.2%, 1%, 2% Chlorhexidine(CHX)과 0.5%, 1%, 2.5%, 5.25% NaOCl, 0.5M EDTA(18.6% EDTA). 그리고 $Listerine^{(R)}$원액을 이용하였다. 항균효과의 비교는 액체배지상에서 균주의 혼탁도와 한천 확산법을 이용한 억제대 비교로 하였다. 모든 실험군은 대조군과 비교시 근관내 균주에 항균성을 나타냈다(p < 0.001). 모든 농도의 NaOCl, CHX, 그리고 EDTA는 실험균주에서 높은 항균성을 보였다. 모든 실험에서 $Listerine^{(R)}$은 다른 근관세척제에 비해 낮은 항균성을 보였다. 결론적으로, $Listerine^{(R)}$이 E. faecalis와 P. gingivalis에 대해 항균성을 보이나, 일반적으로 사용되는 근관세척 액에 비해서는 현저히 낮은 항균성을 나타냄으로 근관세척 액으로 사용은 적합하지 않음을 나타냈다. The purpose of this study is to compare the antibacterial effect of $Listerine^{(R)}$ on two microorganisms (P. gingivalis and E. faecalis) with various root canal irrigants (NaOCl, CHX, EDTA) and to identify possibility of using $Listerine^{(R)}$ as a root canal irrigant. Porphyromonas gingivalis ATCC 3327 and Enterococcus faecalis ATCC 29212 were used in this experiment. For the test irrigants, 0.5%, 1%, 2.5%, 5.25% NaOCl, 0.1%, 0.2%, 1%, 2% CHX, 0.5M EDTA (18.6% EDTA) and $Listerine^{(R)}$ were prepared. Distiled water was used as control. Two methods-1) Comparison of turbidity in broth and 2) Agar diffusion test-were used to determine the extent of antibacterial effect of $Listerine^{(R)}$ and to compare it with that of NaOCl, CHX, and EDTA. All solutions tested were effective against two bacterial strains compared with control (p < 0.001). Any concentration of NaOCl, CHX, and EDTA showed similarly high effectiveness against all bacterial strains. In all experiment, $Listerine^{(R)}$ showed significantly low antibacterial effect compared with the other root canal irrigants (p < 0.05). In conclusion, the results reflect remarkably low antibacterial effect of $Listerine^{(R)}$ as compared with root canal irrigants in general so it is not suitable for the root canal irrigant.

        자돈 설사병에 대한 형질전환 당근백신의 방어 효능 평가

        The study evaluated whether a transgenic carrot vaccine could induce a K88-specific immune response in sows and whether the resultant maternal antibody could protect piglets against enterotoxigenic Escherichia coli (ETEC) K88ac infection. Sows (n = 15) selected randomly from a farm in Korea were assigned to three groups (n = 5 per group: control [untreated]), group A (orally inoculated with a nontransgenic and transgenic carrot vaccines at 2 and 4 weeks ante partum, respectively), and group B (conventionally vaccinated according to the manufacturer's instructions). After 7 days of lactation, 5 piglets selected randomly from each group were challenged with $1{\times}10^{10}$ colony forming units/mL ETEC K88ac. Group C had the lowest mean fecal consistency score on post-challenge days 1 and 7. Histiologically, On post-challenge day 7, group C showed an increased duodenum and ileum villus:crypt ratio, compared to group A in the duodenum, with group B displaying the highest ratio. Groups B and C had more increased villus width than group A in the jejunum. Group C displayed the greatest increase in villus width in the ileum. The colostrums and serum from groups B and C displayed higher concentrations of IgA and IgG against ETEC K88, compared to group A. Based on the results, it was concluded that the transgenic carrot vaccine in sow per oral may have an effect on preventing piglet diarrhea as good as commercial recombinant vaccine.

      • 대규모 유전자 상호작용 네트워크 추론을 위한 클라이언트-서버 시스템 구조

        본 논문은 베이지안 네트워크를 기반으로 대규모 유전자 상호작용 네트워크를 추론하기 위한 클라이언트-서버 시스템 구조를 제시한다. 유전체 수준(genome-wide)의 대규모 유전자 상호작용 네트워크를 베이지안 네트워크 형태로 추론하기 위해서는 병렬 서버를 이용하더라도 통상 수십시간이 소요된다. 따라서, 일반적인 대화형(interactive) 독자(standalone) 시스템 구조보다는 배치형(batch) 분산(distributed) 시스템 구조가 적합하다. 본 논문에서는 그와 같은 상황에 적합한 느슨한 연결의 (loosely-coupled) 클라이언트-서버 시스템을 구현할 결과를 기술한다. 유전자 상호작용 네트워크 추론은 크게 두 단계로 나누어진다. 첫째로, 생물주석정보(biological annotation)과 유전자 발현정보(expression data)를 사용하여, 전체 유전자 집단을 서로 중복이 가능한 모듈들로 나누며, 둘째로, 각각의 모듈들에 대해 독립적인 베이지안 학습을 수행하여 추론결과를 얻고, 각 모듈들이 공통으로 포함하는 유전자를 사용하여 각 모듈의 추론결과들을 하나로 통합한다. We present a client-server system architecture for inferring genetic interaction networks based on Bayesian networks. It is typical to take tens of hours when genome-wide large-scale genetic interaction networks are inferred in the form of Bayesian networks. To deal with this situation, batch-style distributed system architectures are preferable to interactive standalone architectures. Thus, we have implemented a loosely coupled client-server system for network inference and user interface. The network inference consists of two stages. Firstly, the proposed method divides a whole gene set into overlapped modules, based on biological annotations and expression data together. Secondly, it infers Bayesian networks for each module, and integrates the learned subnetworks to a global network through common genes across the modules.

        소결방법에 따른 다공성 티타늄 임플란트의 기계적 특성

        Purpose: This study was performed to compare mechanical properties for sintering methods of porous Ti implants. Methods: The specimens of Ti implant were fabricated by several sintering methods. One of them is spark plasma sintering(SPS). Another is electro discharge singering(EDS) and the other is high vacuum sintering(HVS). Mechanical properties of porous Ti implants were evaluated by universal testing machine(UTM) and their fracture surface was examined under a sanning electron microscope(SEM). Results: The tensile strength was in a range of 71 to 230 MPa, and Young's modulus was in a range of 11 to 21 Gpa. It matched with range of cortical bone. Conclusion: Mechanical properties of porous Ti implants were similar to human bone. It was shown that sintering methods of spherical powders can efficiently produce porous Ti implants with various porosities. Porous metals will be commonly used in orthopedic and dental application despite of initial focus has been on bioceramics.

        실험적으로 유발한 가토 VX2 암종의 종양 혈관: 조영 증강 출력 도플러 초음파 및 혈관 조영술 소견과 병리조직학적 소견과의 비교

        목적: 실험적으로 가토의 대퇴부에 유발한 VX2 암종의 종양 혈관상을 조영 증강 출력 도플러초음파 및 디지털 감산 혈관 조영술의 조영 증강 양상으로 평가하고 양자의 소견을 비교하며 조영 증강 양상의 차이를 유발하는 병리 조직학적 근거를 규명하는데 있다. 대상과 방법: 가토의 대퇴부에 이식된 30개의 VX2 암종에 대해 조영 전 출력 도플러 초음파 영상을 얻고 미세기포 초음파 조영제를 정주 하여 조영 증강 출력 도플러 초음파 영상을 얻은 후 디지털 감산 혈관 조영술을 시행하였다. 이후 조직 절편을 얻어 영상 소견과 병리 조직학적 소견을 연관 분석하였다. 조영 전 및 조영 증강 출력 도플러 초음파와 혈관 조영술 영상에서 종양 혈관의 분포 양상을 분석하고 종양내 혈관 밀집도 점수 및 종양의 전체 면적에 대한 혈관 면적의 백분율을 구하였다. 병리조직학적 검사를 통해 괴사정도, 섬유혈관성 간질 및 피막의 발달 정도를 분석한 뒤 이를 영상 소견과 연관시켜 보았다. 결과: VX2 암종의 종양 혈관상은 조영 증강 출력 도플러 초음파검사에서 무혈관성이 9예, 주 변부 분포형이 15예, 미만성 분포형이 6예였다. 혈관 조영술에서는 8예가 무혈관성, 14예가 주변부 혈관 분포, 그리고 나머지 8예는 미만성 혈관 분포를 보였다. 혈관 분포의 양상은 두 검사간에 통계적으로 유의한 상관관계를 보였다. 혈관 밀집도 점수와 혈관 면적 백분율은 조영증강 출력 도플러와 혈관 조영술 사이에서 유의한 상관관계를 보였다.병리 조직학적 검사에서 미만성 혈관분포를 보였던 예들은 종양의 경계가 분명하고 피막과 섬유혈관성 간질이 잘 발달되어 있었다. 괴사정도, 피막 및 섬유성 간질의 발달 정도는 출력 도플러 초음파 및 혈관 조영술의 혈관밀집도 점수, 혈관 면적 백분율과 유의한 상관관계를 보였다. 결론: 가토의 대퇴부에 유발한 VX2 암종은 조영 증강 출력 도플러 초음파와 혈관 조영술에서 무혈관성에서부터 과혈관성에 이르기까지 다양한 종양 혈관상을 보였고 이러한 종양 혈관의 양상은 괴사정도, 섬유성 간질 및 피막의 발달 정도 같은 병리 조직학적 지표들과 유의한 상관관계를 보였다. Purpose: To describe findings of enhanced power Doppler sonography and DSA in experimentally induced VX2 carcinomas in rabbit thigh and to correlate the imaging findings with the histopathologic features. Materials and Methods: A total of 30 VX2 carcinomas were implanted in rabbit thigh, and after conventional and enhanced power Doppler sonography and DSA, histopathologic examination was performed. Enhanced power Doppler sonography and DSA, were used to determine the distribution pattern of tumor vascularity; to assess its grade and the percentage of a tumor area occupied by vessels, conventional and enhanced power Doppler sonography, as well as DSA, were used. The grade of necrosis and the development of fibrovascular stroma and capsule were histopathologically determined. The findings of power Doppler sonography were compared with those of DSA and the imaging features were correlated with the histopathologic features. Results: At enhanced power Doppler sonography, the signal was either avascular (n=9), peripheral (n=15) or diffuse (n=6), while at DSA, the corresponding totals were eight, fourteen and eight. There was statistically significant corelation between enhanced power Doppler sonography and DSA, both in their depiction of the distribution of patterns of tumor vascularity and as regards their findings of grade and percentage of vascular area. As determined by both conventional and enhanced power Doppler sonography, and by DSA, grade of necrosis and the development of fibrovascular stroma and a capsule correlated with grade and the percentage of vascular area. Conclusion: Experimentally induced VX2 carcinomas in rabbit thigh demonstrated various patterns of tumor vascularity, and the findings of enhanced power Doppler sonography correlated with those of DSA. Tumor vascularity, as demonstrated by two imaging modalities, correlated closely with grade of necrosis and the development of fibrovascular stroma and a capsule, as revealed by histopathologic examination.

        Development and Clinical Evaluation of a Rapid Diagnostic Test for Yellow Fever Non-Structural Protein 1

        A rapid diagnostic test (RDT) kit was developed to detect non-structural protein 1 (NS1) of yellow fever virus (YFV) using monoclonal antibody. NS1 protein was purified from the cultured YFV and used to immunize mice. Monoclo- nal antibody to NS1 was selected and conjugated with colloidal gold to produce the YFV NS1 RDT kit. The YFV RDTs were evaluated for sensitivity and specificity using positive and negative samples of monkeys from Brazil and negative human blood samples from Korea. Among monoclonal antibodies, clones 3A11 and 3B7 proved most sensitive, and used for YFV RDT kit. Diagnostic accuracy of YFV RDT was fairly high; Sensitivity was 0.0% and specificity was 100% against Dengue viruses type 2 and 3, Zika, Chikungunya and Mayaro viruses. This YFV RDT kit could be employed as a test of choice for point-of-care diagnosis and large scale surveys of YFV infection under clinical or field conditions in en- demic areas and on the globe.
