Localized or solitary fibrous tumor (SFT) of the pleura has been classified as a type of mesothelioma, arising from the submesothelial connective tissue cells. The preoperative diagnosis of the tumor at the cytologic or histologic level is very important for the proper handling of the lesion. This preoperative diagnosis is now possible by means of the advance in the transthoracic fine needle aspiration biopsy (FNA) techniques and in the very experience of the cytopathologists. We describe FNA cytologic feature of two cases of SFT arising from the pleura. Cytologic, histologic, immunohistochemical, and electron microscopic characteristics of pleural SFT are discussed. The tumor cells of SFT are spindle or oval in shape with a variable amount of cytoplasm. They are arranged in irregular trabeculae intimately associated with capillaries. A unique cytologic feature observed in this tumor is that thick, eosinophilic, amorphous collagen bundles are scattered between tumor cells.

Fine needle aspiration cytology(FNAC) has been used effectively as the initial modality in evaluating various thyroid lesions. We correlated cytologic and histopathologic features to investigate the diagnostic pitfalls of FNAC of the thyroid. A total of 1,593 FNACs of the thyroid were diagnosed at the Department of Pathology, Hanyang University Hospital, from January 1993 to December 1997 There were 963 cytologically benign cases(60.5%), 97 suspicious cases(6.1%), and 75 malignant cases(4.71%). The remaining 458 cases(28.8%) were unsatisfactory. Subsequent surgical resection was done in 192 cases. Seventy-two cases(37.5%) were cytologically diagnosed as benign, 45 cases(23.4%) suspicious, 56 cases(29.2%) malignant, and 19 cases(9.9%) unsatisfactory. Histopathologically, 101 cases were benign(11 thyroidites, 52 adenomatous hyperplasias, 34 follicular adenomas, and four Hurthle cell adenomas), and 91 cases malignant(72 papillary carcinomas, 16 follicular carcinomas, one medullary carcinoma, one anaplastic carcinoma, and one granular cell tumor). After excluding 19 unsatisfactory cases, 63 were misdiagnosed. They included 17 benign(three thyroidites and 14 adenomatous hyperplasias), 27 suspicious(10 follicular adenomas, four Hurthle cell adenomas, and seven follicular carcinomas), and 19 malignant(16 papillary carcinoma, one medullary carcinoma, one anaplastic carcinoma. and one granular cell tumor) lesions. The accuracy rates in the benign, suspicious, and malignant categories were 54.9%, 49.8%, & 92.8%, respectively. The cytological pitfalls were as follows: (1) background, (2) crowded follicular cell clusters indistinguishable between follicular neoplasia and adenomatous hyperplasia, (3) papillary structure, irregular nuclear membrane and pleomorphism mimicking those of papillary carcinoma, (4) indistinct eosinophilia in follicular epithelial cells, (5) unusual cellular components not commonly seen in FNACS of the thyroid.

Hemangioendotheliomas are vascular neoplasms characterized by histologic appearance that are intermediate between hemangiomas and angiosarcomas first described by Weiss and Enzinger in 1982. They are classified into at least 3 subgroups, including epithelioid hemangioendothelioma, spindle cell hemangioendothelioma, and malignant endovascular angioendothelioma, and have been reported principally in soft tissues of the extremities, lung, liver, and bone. The cases involving the central nervous system reported are very rare. We report an intraspinal epithelioid hemangioendothelioma occurring in a 52-year-old man. To our knowledge, it is first case of spinal hemangioendothelioma in Korea.

Although peritonitis is an unusual form of tuberculosis, it is one of the most important diseases of the peritoneum in this country. Chylous ascites is developed by the various etiologies, but it is rarely caused by tuberculosis. We experienced a case with chylous ascites following tuberculous peritonitis with lymphadenitis which had not responded to a longterm antituberculous medication. In the findings of the second laparoscopy after six months` treatment, milky-turbid chylous ascites has still resided in the peritoneal cavity with some nodules newly developed over the hepatic surface. Significantly elevated triglyceride level in ascites was identified. The patient was much improved with conservative treatment. So we report it with a review of the literature.(Korean J Gastroenterol 1997;29:847-852)

Marked eosinophilia is found in many kinds of disease state, It usually occur in allergic and parasitic conditions and occ assionally in autoimmune disease, blood dyscrasias, or certain malignancics. Idio- pathic hypereosinophilic syndrome is characterized by markedly elevated peripheral blood eosinophil count without explainable etiology. Eosinophilopoiesis is related with T-lymphocyte activation and cytokine cascade controlling eosinophilic production. It shows tissue infiltration in many organs including endomyocardium, lung, liver, gastrointestinal tract. We have recently experienced a case with idiopathic hypereosinophilic syndrome of a 65-year-old woman who had of complained anorexia, weight loss and diarrhea during the past month and half. Persistantly increased eosinophil count was shown in her peripheral blood, the count averaging 3480/mm'(41% in differential count). Abdominal computer tomography disclosed multiple small relatively ill-defined hypoechoic lesions in the liver. Microscopically, the needle biopsy of the liver revealed heavy infiltration by eosinophilis especially in the portal tracts. On reviewing, previously perfomed endoscopic biopsies from the stomach and terminal ileum also had mild to moderate eosinophilic infiltration, respectively. No evidence of parasitic infection was present on the clinical history and laboratory test. Corticosteroid was administered ard brought marked improvement of clinical manifestations including both hematologic and radiologic abnormalities. (Korean J Gastroenterol 1997; 30:397- 403)

Kinetics of hepatocytes in various precancerous and cancer iesions during hepatocarcinorenesis was evaluated in 38 Sprague-Dawley rats fed with 0.075% 2-acetylaminofluorene diet. The rats were sacrificed after 4, 7, 10 days. 2, 3, 4, 6, 8 weeks, and 3, 4, 5, 6, 7, 8 months of the experiment. The lesions were divided as altered cell lesions (clear cell focus, acidophilic cell focus, and basophilic cell focus). regenerating nodules. neoplastic nodules, and hepatocellular carcinomas. Cell kinetics was studied by intraperitoneal bromodexyuridine(BrdU) injection 2 hours before sacrifice. After detection of the BrdU-incorporated nuclei with monoclonal antibody kit, the BrdU labelling index(BLI) was measured as percentage of BrdU labelled cells among about 1000 cells. The liver cells in the altered cell lesions showed significantly increased BLI(clear cell 1.66: acidophilic 2.39: basophilic 2.41) in comparion with normal control(0. 18) or non-lesional hepatocytic lobules (0. 47), and lower BLI than hepatocellular carcinoma(8.61∼27.07) (P<0.05). Regenerating and neoplastic nodules did not differ in their BLI (1.54 and 1.70), and were significantly lower in their proliferating kinetics than acidophilic or basophilic cell foci (P<0.05). It was suggested from this study that the altered cell lesion should be true premalignant lesion, which was composed of at least initiated cells by the chemical carcinogen. The basophilic and acidophilic foci in altered cell lesions were suggestive of close correlation with the hepatocellular carcinoma in that they had greatest proliferative cell kinetics of all the lesions occurred before cancer developed.
실험적 간암 및 전암병변에서 H-ras, bcl-2 및 p53 종양유전자의 발현에 관한 연구

Various changes in percancerous lesions during the hepatocarcinogenesis of rats fed with 2-acetylaminofluorene have been reported. They include oval cells, altered cell lesions, regenerating nodules and adenomatoous nodules. Recently, studies have been conducted in animals to clarify the role of oncogenes in the precancerous and cancerous lesions during hepatocarcinogenesis. In this study, the authors has evoked hepatocellular cercinoma in Sprague-Dawley rats and performed immunohistochemical stains on various precancerous and hepatocellular carcinoma lesions for H-ras, Bcl-2, and p53 oncogenic proteins. The followings were results: 1. No immunoreaction for three oncoproteins tested was detected in the normal control, perilesional hepatocytes, or oval cells. 2. The positive rates of immunoreactivity in altered cell lesions, regenerating nodules, adenomatous nodules, and hepatocellular carcinomas were 26.7%, 45.8%, 60%, 50% for H-ras, adn 26.7%, 41.7%, 60%, 50% for Bcl-2, respectively. 3. No expression for p53 was noted in all precancerous lesions and hepatocellular carcinoma lesions studied. In conclusion, the p53 tumor suppressor gene appears not involved in the 2-AAF-induced hepatocarcinogenesis. H-ras and bcl-2 oncogene are involoved in the early precancerous lesions, and the oncogenes may play an important role to promote cell proliferation and maintain the number of mutant clones through blocking apoptosis.
