Internal derangement of the temporomandibular joint(TMJ) is defined as an abnormal relationship of the articular disc to the condyle. Mandibular manipulation is one of the conservative treatments to be considered first to manage the patients with anterior disc displacement without reduction. Mandibular manipulation is used to increase articular mobility and to restore the displaced disc into an anatomically normal position. While Farrar's technique has been popularly used, Minagi et al., Mongini and Suarez introduced the manipulation technique conducted by the patients themselves. But there is no study on the efficacy of self-manipulation technique, comparing with conventional one. The aim of this study was to investigate the efficacy of the conventional and self-manipulation technique, which was modified to complement the previously described technique by Minagi et al., in the treatment of patients with anterior disc displacement without reduction. TMD patients, who visited Department of Oral Medicine of Seoul National University Dental Hospital from December, 2002 to November, 2004 and were diagnosed as anterior disc displacement without reduction by TMJ magnetic resonance imaging (MRI) were enrolled. Conservative treatments including physical therapy, exercise, behavioral therapy, stabilization splint therapy, and manipulation therapy were done to every single patient until the symptomsimproved enough to discharge the patient. The charts were reviewed retrospectively according to the type of manipulation. In the results, patients whose maximum mouth opening was more than 40 mm was higher in the self-manipulation group(69.9%) than in the conventional manipulation group(42.9%). But difference between two groups was not significant. According to the fact that we decided to discharge the patients whentheir mouth opening increased to more than 40 mm and subjective symptoms such as pain and discomfort were improved as well, treatment period of discharged patients was significantly shorter in the self-manipulation group($29.2{\pm}12.3$ weeks) than in the conventional manipulation group ($61.0{\pm}38.0$ weeks) (p<0.01). In conclusion, in the treatment of TMD patients with anterior disc displacement without reduction, the self-manipulation technique which is performed by patients themselves is an effective treatment modality for increasing the range of mouth opening and shortening the total treatment period.
Effect of a Mouthwash Containing Cetylpyridinium and Zinc Chloride on Oral Malodor

The aim of this study is to clinically evaluate the effect of a mouthwash containing cetylpyridinium and zinc chloride on reduction of morning oral malodor in healthy subjects measured by organoleptic measurement and a portable sulfide monitor. A total of 8 healthy non-smoking male volunteers were enrolled in this study of crossover design consisting of two experimental phases. The subjects were instructed to rinse the mouth with 10 mL of the experimental mouthwash containing cetylpyridinium and zinc chloride for 30 seconds at the first phase. At the second phase after a one-week washout period, each subject rinsed with distilled water as a control. All experiments were conducted at around 8:30 a.m. and oral malodor was measured using organoleptic measurement and a portable sulfide monitor just before rinsing with the experimental mouthwash or control (baseline), 1 hr, 2 hrs, and 3 hrs after rinsing. The mouthwash containing cetylpyridinium and zinc chloride reduced morning oral malodor up to 3 hrs after rinsing. Organoleptic score and concentrations of volatile sulfur compounds after use of the experimental mouthwash significantly decreased with time, and the decreases were significantly different between the mouthwash and control. In conclusion, the mouthwash containing cetylpyridinium and zinc chloride is significantly effective on reduction of morning oral malodor in healthy subjects by 3 hrs.

본 논문에서는 시간지연을 갖는 고차모델을 저차모델로 간소화하기 위한 주파수 전달함수 합성법을 제안한다. 모델축소는 축소시스템의 분자다항식에 의해 가중된 오차함수를 최소화하는 것에 기반을 두고 있다. 제안된 방법은 보다 우수한 저주파수 적합도를 제공한다. 그리고 네 개의 예제가 제안된 방식의 유용성을 나타내기 위해서 주어진다. This paper proposes a frequency transfer function synthesis for simplifying a high-order model with time delay to a low-order model. The model reduction is based on minimizing the m function weighted by the numerator polynomial of reduced systems. The proposed methods provide a better fitness within low frequency. Four examples are given to illustrate the feasibilities of the suggested schemes.

본 논문에서는 블루투스를 이용하여 신생아를 중앙 감시하는 장치를 제안하고 구현하였다. 기존의 신생아 중앙 감시 장치는 부피가 크기 때문에 신생아에 대한 부착이 불편하고 유선으로 연결되어 측정 및 감시 시에 휴대가 어려워서 공간의 제약을 가지는 단점이 있다. 이러한 단점을 해소하기 위해서 블루투스를 이용한 신생아 보육장치를 제안하였다. 제안된 시스템은 수신부와 송신부로 구성된다. 송신부는 온도센서, 습도 센서, ECG 측정 장치 및 블루투스 모듈로 구성되며 인류베이터의 온도, 습도 및 ECG 데이터를 측정한 후 블루투스를 이용하여 송신하는 기능을 수행한다. 수신 시스템은 송신 시스템에서 전송한 신호를 블루투스 수신 모듈을 통해 수신하여 인큐베이터 별 송신 시스템의 고유한 식별부호와 심전도 및 온습도 신호를 분리하여 각 신호들을 화면에 표시함으로써 중앙 감시가 효과적으로 이루어지도록 한다. 또한 수신부의 시스템 관리자는 신생아 보육장치와 신생아의 상태를 중앙 감시할 수 있도록 하는 기능을 한다. 제안된 시스템은 다양한 환경에서 신생아 보육장치의 원격 진단에 유용하게 쓰일 수 있을 것으로 생각된다. In this paper, we present a centralized monitoring system for infant incubator using Bluetooth. Conventional monitoring systems for incubators require large space and wire connection, which causes the spatial restrictions. To overcome this disadvantage, centralized monitoring system is proposed for infant incubators using Bluetooth. The implemented system consists mainly of transmission systems and receiver systems. There are temperature sensors, humidity sensors, ECG measurement units and Bluetooth modules in the transmission systems. For temperature, humidity and ECG data, the transmission systems acquire them from the measuring modules in the incubator and transmits the signals using Bluetooth. In the receiver system, users can see the status of the infant by accessing the central monitoring host computer. That is, one can monitor the information on the temperature and the humidity in the incubator and Infant's ECG without dependence to a conventional bulky system. Also, the system manager in the receiver system can maintain centralized monitoring of the situations in all incubators and infant. The developed system will be useful in remote diagnosis of infant incubator In various environments.
