The separation characteristics of the carboxymethylcellulose(CMC) membrane for organic aqueous solution were determined by vacuum pervaporation. The apparatus used in this experiment can forecast the concentration distribution and/or permeating phenomenon of organic solvent in the membrane, unlike the conventional apparatus. The purpose of this study is to investigate the permeation and separation characteristics of the homologous series of organic solvents through selected hydrophilic polymer membranes. CMC membrane is suitable for the separation of water/organic solvent mixtures due to the high separation factor and the large amount of permeation. The separation analysis was carried out with a Hewlett Packard, HP5890 Series Gas Chromatograph. The permeating concentration is generally increased at the beginning stage, but led to equilibrium permeate according to experiment time. It is desirable that the separation characteristics should be evaluated of equilibrium state.

A total hip replacement arthroplasty has become an established procedure in the management of painful arthntic hip in past two decades. And there are many reports that are concerned in a follow-up study of the total hip replacement arthroplasty. This is a follow-up study in 56 total hip replacement arthroplasties in 44 patients of 98 total hip replacement arthroplasties in 70 patients, performed at the Department of Orthopedic Surgery, Kosin Medical College, between January 1981 and March 1988. The author tries to find which type of implant is better and hopes that this report will provied guide to the surgeon who will be going to do total hip replacement arthroplasty. Our results concludes the followings : 1. There were 28 males(63.6%) and 16 females(36.4%). The peak age incidence was 6th decade, which comprised 31.8% of total cases. 2. A most common cause was post-traumatic or idiopathic avascular necrosis of the head, comprised 35 cases.(62.5%) 3. Variable prosthesis were used; Muller type, 15 cases, Charnley type, 12 cases; cementless PM type, 13 cases; and Harns/Galante types, 16 cases. 4. Postoperative complications were as follows : 4 cases of wound problem including 2 early deep infections, 2 dislocations, 1 femoral nerve palsy, 1 acetabular perforation, 1 stem perforation, 3 femoral shaft fractures, 4 cases of trochanteric wire breakasges, 6 ectopic ossifications, 4 cases of loosening including 3 femoral and 1 both femoral and acetabular components loosening. 5. Postoperative pain relief was better in Harris/Galante type(average 41.1 points). 6. The functional results was better in Harri/Galante type by average 43.3 points. 7. The range of motion(flexion) improve from 86.7 deg. to 104 deg in Harris/Galante type. 8. Postoperative total Harris hip scores were 92.7 points in Harris/Galante type, 85.5 points in Muller type, 79.7 points in Charnley type and 76.8 points in cementless PM type.
수핵성형술(Nucleoplasty)을 이용한 디스크 내장증(Internal Disc Disruption)의 치료 연구들의 의학적 논거에 대한 비판

Nucleoplasty is a type of percutaneous disc decompression that has been developed to treat herniated intervertebral discs. Currently, in some clinics, researchers have also applied this procedure to patients with internal disc disruption, apart from the originally intended usage on herniated intervertebral discs. The purpose of this study is to evaluate the feasibility of this extended use based on medical logic. To achieve this, the author analyzed case studies on performing nucleoplasty on patients with internal disc disruption. The main points of the analysis are, first, the validity of the treatment evidence presented by the researchers and, second, the relevance of the patient selection criteria. As a result, it is judged that the therapeutic rationale of existing papers applying nucleoplasty to the treatment of internal disc disruption is unclear or inconsistent with general medical logic, and in the process of patient screening, discs that may be deemed inappropriate for percutaneous decompression are included. Therefore, the author believes that existing studies applying nucleoplasty to the treatment of internal disc disruption have the nature of somewhat adventurous experiments that are unnecessary or can cause potential side effects. In order to uphold patients' rights and improve the completeness of the study in the research process on this topic, the author thinks that it is essential to establish clearer therapeutic evidence than the current level of understanding and to have an elaborate patient screening process based on it. 수핵성형술은 경피적 디스크 감압술의 한 종류로 디스크 탈출증의 치료를 위해 개발된 시술법이다. 현재 몇몇 임상 현장에선 이 기술을 원래의 적응증인 디스크 탈출증을 지닌 환자들에 대해서 뿐 아니라, 디스크 내장증을 지닌 환자들에게도 사용하고 있다. 이 연구의 목적은 이와 같은 임상 현장에서의 수핵성형술의 적응증의 확대가 타당한 의학적 논리 위에서 수행된 것인지 평가하는 것이다. 그것을 위해 논자는 수핵성형술을 디스크 내장증 환자들에게 적용한 논문들을 분석하였다. 분석의 요점은 첫째, 연구자들이 제시하는 치료 근거의 타당성과 둘째, 환자 선별 기준의 적절성이다. 그 결과 수핵성형술을 디스크 내장증의 치료에 적용한 기존 논문들이 제시하는 치료 근거는 불분명하거나 일반적인 의학의 논리에 부합하지 않는 측면이 있는 것으로 판단되며, 환자 선별의 과정에선 경피적 감압술이 기능을 발휘할 수 있는 조건을 지닌 디스크를 찾기 위한 절차를 엄밀히 수행하지 않거나 경피적 감압술에 부적합한 것으로 판단될 수 있는 디스크도 시술 대상에 포함시킨 것으로 평가된다. 따라서 논자는 수핵성형술을 디스크 내장증 치료에 적용한 기존 연구들은 불필요하거나 잠재적 부작용을 발생시킬 수 있는 다소 모험적인 실험의 성격을 갖고 있다고 판단한다. 이 주제에 대한 연구 과정에서 환자의 권익을 지키고 연구의 완성도를 높이기 위해선 현재의 이해 수준보다 선명한 치료적 근거의 확립과 이에 기반한 정교한 환자 선별 과정이 필수적으로 갖추어져야 할 것이라 생각한다.
