성대 피열사이 유착에 의한 기관내 삽관의 곤란 —증례보고—

Interarytenoid adhesion of the vocal cords, with a triangular anterior opening and a smaller posterior rounded opening, is one of the complications of intubation. The vocal cords are tethered to each other and they are restricted with narrow abduction, causing difficult intubation. A 57-year-old woman was scheduled to undergo lung wedge resection. The patient had experienced a prolonged intubation 15 years previously. Thereafter, she had undergone two operations and intubation was difficult to perform with using small size endotracheal tubes (6.5 and 6.0 mm inner diameter, respectively). Despite this past medical history, anesthesia was performed without further evaluation because of the patient's refusal. After the failure of an initial trial of intubation with a double lumen endobronchial tube, we tried to intubate with a small sized endotracheal tube (5.0 mm). But it was impossible to pass the tube through the vocal cords. Endobronchial endoscopy revealed interarytenoid adhesion of her vocal cords. After tracheostomy, she received wedge resection. Therefore, the possibility of difficult intubation due to interarytenoid adhesion of the vocal cords should be considered for a patient with a past history of intubation.

경찰공무원의 직무만족에 관한 연구 : 전남경찰을 중심으로

경찰공무원의 직무는 국민의 생명과 직결되고 나아가 경찰사기에도 직접적인 영향을 주기 때문에 직무만족에 미치는 영향요인은 매우 중요하다. 2021년 이후 경찰은 자치경찰제 시행, 수사구조 개편, 용산 이태원 참사 사건 등 많은 사건들이 발생하면서 “경찰의 역할은 어디까지 인가? 혹은 국민이 경찰에게 기대하는 경찰의 역할과 수준은 어느 정도일까?” 하는 의문을 가지게 되곤 했다. 위와 같이 국민은 “최고의 복지는 안전이다.”라는 기치 아래 안전에 대한 관심이 많아지면서 경찰의 업무에도 많은 변화를 가져오고 있다. 하지만 경찰공무원이 모두 같은 업무를 수행하는 것은 아니다. 즉, 경찰관 개인 특성과 환경에 따라 그 직무를 수행하면서 느껴지는 직무에 대한 만족감이 다양하게 나타날 것이다. 따라서 이 연구는 전남경찰공무원의 직무만족의 인식을 분석하여 바람직한 경찰공무원의 직무만족 향상 방안을 제시하고자한다. 즉, 성별, 연령, 근무관서, 근무부서, 계급, 총 근무년수, 출․퇴근 거리에 따른 직무만족 인식을 분석하였다. 위와 같은 연구문제의 분석을 통해 개인 특성에 따른 맞춤형 직무만족 개선 정책을 수립하는데 타당한 근거자료를 제시하고자 한다.

2008년 10월부터 2009년 9월까지 유입수계 하천의 수질특성을 파악하고 통계적 처리를 통하여 수질자료를 해석하였다. 순천만 유입수계 중에서 순천서천, 순천동천, 옥천은 환경부고시 하천수질 평가항목 모두에서 I등급으로 비교적 양호한 수질상태를 보였으나, 하천에서 유기물 오염의 지표인 BOD 항목에 대해서 해룡천과 순천만 유입지점에서는 각각 “약간 나쁨”과 “보통”의 결과를 나타내었다. 한편 총질소과 총인의 경우는 하수종말처리장 방류지점과 해룡천에서 다른 하천에 비해서 상대적으로 높은 결과를 나타내었다. 요인분석 결과 수질특성을 설명하는 1요인으로는 인간기원의 오염물질로서 기여율은 53.7%, 2요인으로는 지형적인 인자로서 기여율은 25.0%로 나타났다. 그리고 3요인은 자연환경의해서 기인된 요 인들로서 기여율은 14.2%이며, 누적기여율은 92.8%로 평가되었다. 요인분석 결과 해룡천과 하수종말처리장 방류수로부터 상대적으로 많은 오염물질이 배출되어 순천만으로 유입되고 있어, 순천만의 수질 보존을 위해서는 순천하수종말처리장과 해룡천의 수질오염물질 배출의 보다 철저한 관리와 정화노력이 필요하다고 판단된다.

This research focuses on analyzing the correlation between adolescent self-esteem and labor participation. The findings of this study are as follows: First, when it was analyzed whether there are differences in self-esteem with ages, out of school students were the lowest. and self-esteem level increased with age. Second, when the differences in self-esteem levels between genders were examined, male students showed slighty higher positive self-esteem and total self-esteem, which shows statistically significant difference. Third, the correlation between self-esteem and labor participation is as follows: 50.4% of students experienced part-time jobs in low self-esteem group. And 50.6% of students experienced part-time jobs in high self-esteem group. It shows that self-esteem level doesn’t have relation to part-time job experience. Fourth, when the correlation between part-time job intention and self-esteem was analyzed, 50.5% of students in low self-esteem group responded they would have part-time job, but 49.5% of students in high self-esteem group. Also, 50.9% of low self-esteem group students responded they had no intention of getting part-time jobs, but 49.1% in high self-esteem group. It shows self-esteem level doesn’t influence the decision to get part-time jobs. Last, the correlation between the reason they get part-time jobs and self-esteem level was analyzed. 68.1% of students in low self-esteem group had part-time jobs voluntarily, while 31.9% had to have jobs involuntarily for economic cause. Also 75.2% of students in high self-esteem group had part-time jobs for voluntary motive and 24.8% for economic cause. So high self-esteem group showed more voluntary cause and low involuntary cause for part-time jobs, but there was no statistically significant value. 본 연구는 청소년의 자존감이 노동참여와 어떤 관계가 있는가를 분석하는데 그 목적이 있다. 연구 결과는 다음과 같다. 첫째, 자존감이 학년별로 차이가 있는가를 분석한 결과 자존감(긍정)은 학교 밖 학생들이 가장 낮았고, 재학생은 학년이 높을수록 자존감이 높게 나타났다. 둘째, 자존감에 대한 성별 차이에서 남학생이 여학생보다 자존감(긍정)과 자존감(합계)에서 다소 높게 나타났다. 셋째, 자존감과 노동참여 관계를 분석한 결과 자존감이 낮은 집단에서 아르바이트 경험이 있다고 응답한 비율은 50.4%, 자존감이 높은 집단에서는 50.6%이었다. 자존감의 높고, 낮음이 아르바이트 경험과는 차이가 없는 것으로 나타났다. 넷째, 아르바이트 의향과 자존감과의 관계를 분석한 결과 아르바이트 의향이 있다고 응답한 학생 중에 자존감이 낮은 집단의 비율은 50.5%이고, 자존감이 높은 집단의 비율은 49.5%이었다. 따라서 자존감의 높고, 낮음이 아르바이트 의향과는 차이가 없는 것으로 나타났다. 마지막으로 아르바이트 이유와 자존감과의 관계를 분석한 결과 자존감이 낮은 집단에서 아르바이트를 하는 이유가 경제적 이유가 아닌 자발적 동기인 경우가 68.1%이고, 경제적 비자발적 동기인 경우가 31.9%이었다. 또한 자존감이 높은 집단에서 아르바이트를 하는 이유가 경제적 이유가 아닌 자발적 동기인 경우가 75.2%이고, 경제적 비자발적 동기인 경우가 24.8%이었다. 자존감이 높은 집단이 자존감이 낮은 집단에 비해서 자발적 동기가 높고, 비자발적 동기가 낮게 나타났지만, 통계적으로 유의미한 차이는 나타나지 않았다.
