      • Dietary Iron Intake and Body Iron Status of Myocardial Infarction Patients in Chunan Area

        It has been known for some time that elevated body iron could be a risk factor for coronary heart disease. The present study was conducted to determine body iron status and dietary iron intake of patients with myocardial infarction(MI). Seventy five patients from the Chunam area with their first MI history within he past 2 months were recruited. The serum iron concentration, total iron binding capacity(TIBC) and percent transferrin saturation(TS) were selected as indicators of body iron status. Twenty four hour recall was conducted by trained interviewers to asses the dietary intake. Most women (91.3%) showed waist to hip ratio(W/H) greater than 0.85 while 17.3% of men were assessed to have a tendency of abdominal obesity(W/H>0.95). The average BMI of women was 25.80 and that of men was 23.98. The average diet intake of participants was below the recommended dietary allowances (RDA) for most nutrients. He average dietary iron intake was 10.03 mg/day for all subjects while women's iron intake was significantly lower than men's. However, a great proportion of participants (77%) showed a tendency to have normal iron status. About 9% of the participants were assessed as iron deficient and 14% had an iron overload. The mean serum iron concentration was 125 g/dl ranging from 13.3 to 280.6 g/dl. Iron intake from animal sources were significantly associated with body iron status (r=0.257, p=0.026) when TIBC was used as an iron status indicator. When iron status was assessed with TS, it was directly associated with iron intake from animal sources(r=0.278, p=0.05) for he subjects in the normal iron status group. He results of the present study showed that the nutrient intake of Mi patients in Chunan was not quite adequate while iron status was mostly in the normal range. Further studies are needed to investigated whether there is a possible difference in iron metabolism of the MI patients.

        젊은 여성의 혈청 셀레늄 농도 및 체격에 따른 체내 철 수준

        본 연구는 젊은 여성인 여대생의 철 영양상태를 살펴보고, 체격과 체내 셀레늄 수준과의 관계를 평가하고자 하였다. 연구 대상자들의 평균 나이는 21.9세이며, 키, 체중 및 체질량지수는 각각 164 ㎝와 60.1 kg, 22.2 ㎏/㎡를 나타 내었고, 체질량지수는 17.9 ㎏/㎡에서 35.9 ㎏/㎡까지의 다양한 수준의 체격 분포를 보였다. 본 연구 여대생들의 철결핍과 빈혈율은 각각 27.1%, 8.6%이지만, 철 결핍성 빈 혈율은 1.4%에 불과하였다. 체질량지수에 따른 철 영양상태를 보면, 체질량지수가 가장 낮은 집단에서 혈청 페리틴 수준과 혈청 철 수준이 낮았지만, 체중 증가에 따라 유의적으로 증가하였다. 헤모글로빈 수준이나 적혈구 용적비는 체격에 따른 차이를 전혀 보이지 않았으며, 적혈구수는 저체중군에서 중체중군으로 체질량지수가 증가하였으나 고체중군에서 유의적으로 낮았다. 본 연구 대상자들의 평균 혈청 셀레늄 수준은 12.0 ㎍/㎗를 나타내었다. 혈청 셀레늄 수준에 따른 철 지표를 보면 혈청 페리틴 수준은 혈청 셀레늄 농도 증가에 따른 차이를 보이지 않지만, 혈청 철 수준과 혈액의 적혈구수, 적혈구 용적비는 평균 셀레늄 수준이 9.6 ㎍/㎗에서 혈청 셀레늄 11.4 ㎍/㎗으로 증가하는 경우 유의적으로 증가하여 낮은 셀레늄군에 비해 중셀레늄군 및 고셀레늄군에서 증가하였다. 혈청 페리틴 수준은 체질량지수에 의해 약 25.0%를 설명할 수 있으며, 적혈구수는 혈청 셀레늄 수준과 체지방율에 의해 변이의 26.2%를 설명할 수 있었다. 그러나 헤모글로 빈이나 적혈구 용적비 등 다른 철 지표들은 쳬질량지수나 혈청 셀레늄 수준에 의해 설명이 되지 않았다. 결론적으로 본 연구는 체내 철 수준은 혈청 셀레늄수준, 체격인자 중 체질량지수 및 체지방율과 관련성을 제시하였다. Se and Fe are trace minerals acting as antioxidant scavenging free radicals. Iron deficiency is the most frequently reported nutritional deficiency in females. Body iron status are known to be dependent not only upon dietary iron intake, but also upon micro-mineral nutrition and obesity. Antioxidants such as selenium are reported to play an important role on the regulation of erythropoiesis by protecting RBC membrane from antioxidative damage. In this study, iron status in young females and its relationships with selenium status and physique were examined. Serum selenium and iron concentrations were measured by HANARO research reactor using neutron activation analysis method (NAA-method). The proportion with iron deficiency and anemia were 27.1% and 8.6%, respectively in young females, but the proportion with iron deficient anemia was 1.4%. The mean serum selenium level was 12.0 ㎍/㎗ and in normal range in the young women. The study participants were tertiled according to BMI and serum selenium levels. Serum ferritin and iron levels inclined with increasing BMI tertiles. Serum iron and RBC count were higher in middle selenium group than low selenium group. Individuals had significantly lower hematocrit level in the lowest tertile for their serum selenium levels compared with the highest tertile. The serum ferritin level was predicted 25% by BMI and RBC count 26.2% by the serum selenium level and body fat%. In conclusion, this study shows that body iron status in young adult females are influenced by obesity and body selenium status.

        임신말 모체.제대혈 및 태반의 철분함량과 신생아체중

        The purpose of this study is to assess the maternal iron status during pregnancy and to evaluate the relationships bet-ween the iron indices of maternal, umbilical cord serum, placenta and pregnancy outcomes. Venous bloods samples were drawn from 54 pregnant women just before delivery and cord bloods of their newborn babies were collected immediately after birth. And also, placental tissues were extracted. We investigated the difference of the iron status indices of maternal, umbilical cord serum and placental tissue between two gestational age group (PT group, NT group : preform delivery and normal term delivery at 34.9wk and 39.0wk of mean gestational length, respectively) and also assessed correlations of iron status indices of maternal, umbilical cord serum and placenta tissue. And lastly, we related between birth weight and iron status indices of maternal, umbilical cord serum and placental tissue. The concentrations of maternal serum ferritin and of placental iron were significantly higher in NT group (32.1 $\pm$ 21.1 ng/ml, 68.5 $\pm$ 16.7 $\mu$g/g), than those of NT group (20.8 $\pm$ 11.6 ng/ml, 53.2 $\pm$ 17.4 $\mu$/g) respectively (p<0.001). However the serum ferritin of umbilical cord were significantly higher in NT group (PT : 109.4 $\pm$ 65.7 ng/ml, NT : 147.0 $\pm$ 56.8 ng/ml) than those of PT group (p<0.05). Our results showed that a negative association between birth weight (r=-0.361) and maternal serum ferritin and that a positive association between birth weight and umbilical cord serum ferritin (r=0.261). Despite not a significant difference, there was tendency that highest concentration of maternal serum ferritin was associated with the lowest birth weight. These findings indicate that birth weight of newborn is dependent of multiple factors such as maternal iron status during pre-pregnancy, body size, general nutritional status. Although for women who enter pregnancy with low iron stores, enough intakes of iron during pregnancy could produce undesirable pregnancy outcome. Therefore we suggest for successful pregnancy outcome and delivery differential iron supplementation program will be carried out individual pregnant women on the basis of pre-pregnancy nutritional status.

        임신기간과 분만 후 모체의 철 및 엽산 영양상태의 종단적 변화

        Anemia in women during pregnancy and after delivery has been known to affect the mother, the fetus, and the infant's growth and health status. Studies examining, changes in iron and folate status associated with anemia during pregnancy and during pregnancy, and those supplements are stopped after postpartum. However, the effects of those have not been clearly determined in pregnant and lactating Korea women. Therefore, this study was performed to determine the changes in maternal iron and folate status during pregnancy and six months after delivery longitudinally in six pregnant women who consumed supplements from 20 wk to delivery. We concluded that the iron status deteriorated during pregnancy and especially was weak in the third trimester, but had a tendency to recovery after delivery. On the other hand, the folate status deteriorated in the first and second trimester and was good in the third trimester, but had a tendency to decrease after delivery. These results suggested that the iron status was not improved despite consuming total iron supplements of 50 mg/day through diets and supplements during the second half of the pregnancy. On the other hand, the folate status improved at the end of pregnancy by consuming folate supplements of a total of 800 mg/day through diets and supplements. However, folate status was poor in the first half of the pregnancy, and the tendency of folate status to decrease during postpartum was advanced. At the point in which iron and therefore supplementation is essential. However, the effects of supplement intake time and intake dosage need to be verified and the nutritional status changes of postpartum women should be carefully monitored.

        강릉대 일부 여대생의 철분영양상태에 관한 연구

        To evaluate iron nutritional status of female college students, fasting blood samples were taken from 76 female students of Kangnung National University. Hemoglobin(Hb), hematocrit(Hct), serum iron(Fe), total iron binding capacity(TIBC) and serum ferritin concentrations were measured and transferrin saturation was calculated. Mean values for Hb, Hct, Fe, TIBC, TS and serum freeitin were 13.64$\pm$1.42g/dl, 40.99$\pm$4.31%, 103.0$\pm$33.3$\mu\textrm{g}$/이, 395.3$\pm$9.07$\mu\textrm{g}$/dl, 26.58$\pm$9.07$\%$and 26.76$\pm$17.5ng/ml, respectively. Prevalence of iron deficiency greatly varied by indices from 6.8% when judged by Hct to 26.0$\%$ by serum ferritin concentration. The Hb concentration was positively correlated with hematocrit (r=0.5402), serum iron(r=0.2819) and transferrin saturation(r=0.2777)(p<0.05). on the other hand, serum ferritin concentration showed significantly negative correlation with TIBC(r=-0.3196). Two-day dietary intake records were collected from subjects to estimate mean daily iron intake and bioavailability of dietary iron. Mean daily intake of iron was 13.15mg and heme iron intake was 0.83mg which was 6.4% of total iron intake. Total absorbable iron calculated by the method of Monsen was 1.27mg and bioavailability of dietary iron was 9.6%. In the light of high prevalence of iron deficiency based of serum ferritin concentration and low bioavailability of iron in the diet, guidelines about diet should be made to increase the content and bioavailability of iron in the diet if female college students.

        전주지역 노인의 철분영양상태

        The purpose of this study was to assess the nutritional status of iron of the elderly residing in the Jeonju area. The anthropometric parameters, nutrient intake and biochemical status of iron, were measured for 60 korean elderly(23 elderly men and 37 elderly women aged 60 - 79 years old). The level of hemoglobin(Hb), hematocrint(Hct), serum iron(Fe), total iron binding capacity(TIBC) and serum ferritin(Ferritin) were measured and transferrin saturation(TFsaturation) was calculated. Mean values of Hb, Hct, Fe, TIBC, TFsaturation and Ferritin were 14.49$\pm$0.93g/이, 42.47$\pm$2.59%, 125.48$\pm$52,.46$\mu\textrm{g}$/dl, 338.13$\pm$45.92$\mu\textrm{g}$/dl, 193.00$\pm$125.03$\mu\textrm{g}$/l in elderly men and 12.82$\pm$0.99$\mu\textrm{g}$/dl, 37.66$\pm$2.90%, 100.08$\pm$42.82$\mu\textrm{g}$/dl, 356.41$\pm$54.65$\mu\textrm{g}$/dl, and 99.35$\pm$117.22$\mu\textrm{g}$/l in elderly women, respectively. Prevalence of iron deficiency varied greatly with biochemical indices of iron. It was 13.0% when judged by Fe(60 $\mu\textrm{g}$/dl) and TFsaturation(15%) whereas 34.78% by Hct(41%) in elderly men. However 13.5% of the elderly women showed iron depletion(Ferritin<20$\mu\textrm{g}$/l) and 43.2% showed suppressed erythropoiesis with iron deficiency(TIBC>360$\mu\textrm{g}$/dl). The anemic subjects assessed with TFsaturation(<15%) represented 13.5% of the elderly women, whereas 18.9% of the subjects possessed less than 12g/dl of Hb. The Hb concentration was positively correlated with Hct(r=0.980, p<0.001), Fe(r=0.384, p<0.01) and TFsaturation(r=0.349, p<0.01). On the other hand, Ferritin concentration showed a significantly negative correlation with TIBC(r=0.349, p<0.05) and a positive correlation with TFsaturation(r=0.362, p<0.01). Major food groups of iron intake in the elderly were vegetables, cereals, and fish. The mean daily intake of iron was not significantly different between elderly men and women(12.82mg vs 10.35mg). Intake of heme iron however, was significantly higher(p<0.01) in elderly men(1.03mg) than women(0.42mg). Total absorbable iron caculated by the method of Monsen was 0.55mg, 0.40mg in elderly men and women, respectively and bioavailability of dietary iron 4.29% and 3.87%.

        운동심리학 : 엘리트 체조선수의 철분영양상태 및 혈중 철분지표에 관한 연구

        This study was conducted with the total 28 national male and female gymnasts to evaluate physical activity level, iron nutritional status, to analysis hematological status. From the data the study was to analysis the players iron nutrition and to suggest prescrip scientific nutrition for the increase the ability. The average daily energy intake was 2,182.7±367.13 ㎉ for the male players, 1,417.9±305.45 ㎉ for the female players and was 86%, 68% of Korean Recommended Daily Allowance (RDA). The rate of the 3 major nutrition (C : F : P) was 52 : 33:15%, 55 : 27 : 13%. It showed that low carbohydrate and high fat intake rate. Also the female players was taking less than 50% of iron intake rate and intake less nutritions compare with the daily suggestion except vitamin C. The rate for the intake animal and vegetable iron of the male and female players is 37 : 63, 34 : 66 which showed higher vegetable iron intake rate. From hematological status showed female players had low Hb,. RBC, WBC, Hct than male players (p<.001). The relationship for the hematologic anemia shows both players had positive relationship (p<.01) of Hb level and hematocrit of RBC and between Hb and hematocrit. However, female players show negative relationship between TIBC(total iron binding capacity) (p<.05).Therefore, the study suggest will guide basic data for standard iron intake allowance for the each players of the national team to improve the athletic performance, To prevent anemia for the players there will be scheduled check up and find out the correct standard blood iron study.

      • Cross-Sectional Study on Iron Status of Asan Residents and Regional Comparison

        Iron deficiency and anemia are severe nutrition problems in most of Korea. Iron intake, especially iron with better bioavailability is insufficient over a total age group. Recent changes in diet and life style of Koreans have been repeatedly suggested problems caused by excess nutrient intake rather than under intake. Despite the changes in diet patterns, iron deficient anemia is still prevalent in many parts of Korea. Eight hundred and fifty subjects (323 male and 527 female subjects) in Asan were recruited from farming, factory and urban area. Each subject was interviewed to assess nutrients intakes according to a 24hr-recall method. Twelve hour fasting blood samples were collected to vacutainer with EDTA for hemoglobin (Hb) and separate the tubes for serum iron (SI) and total iron binding capacity (TIBC). The mean serum iron value of female subjects in the factory area was significantly higher (p < 0.05) than that of the female subjects in the urban area although subjects in urban area showed significantly higher the dietary iron intake for both the men and woman (p < 0.05). Dietary iron intake for the younger women was lowest in the farming area and those in the urban area showed the highest dietary iron intake (p < 0.05). When the dietary iron intake was compared by different the age groups, dietary iron intake of the older women from animal sources was less than that of younger women in the urban area (p < 0.05). Dietary iron intake of Asan residents was not sufficient regardless of age, sex and regions and intake of heme iron was especially lower than nonheme iron. (J Community Nutrition 5(1) : 37∼43, 2003)

        일부 지역 농촌 노인의 철분 영양상태에 관한 연구

        The purpose of this study was to examine nutritional status of iron in 27 rural elderly subjects. The daily intake, urinary excretion and serum level of iron were determined by 24-hr food collection method, and 24-hr urine and serum collection for 3 days. The results were summarized as follows. 1. The mean age, height, weight, and blood pressure were 67.7 years, 163.2㎝, 55.9㎏, and 121.1/80.0㎜Hg in men and 69.9 years, 146.5㎝, 47.6㎏, and 108.9/71.1㎜Hg in women, respectively. 2. The daily intake of energy, protein, vitamin A, vitamin B₁, vitamin B₂, niacin, vitamin C, and iron were lower than Korean recommended dietary allowances. 3. The daily iron intake was 7.82㎎. The proportion of heme iron and nonheme iron to the total iron intake were 8.05% and 91.94%, respectively. Major iron sources of the rural elderly were cooked rice, Ko Chu Jang, beef blood, anchovy, soybean paste, and leafy radish. 4. The hemoglobin, hematocrit, serum iron, and total iron binding capacity were 14.3g/dl, 42.9%, 79.4 ㎍/dl, and 268.4 ㎍/dl in men and 12.9g/dl, 38.8%, 92.4 ㎍/dl, and 277.9 ㎍/dl in women, respectively. 5. There was a significant positive correlation between dietary intake and urinary excretion of iron.

Therefore, the social development of ancient iron-making technology is based on phase-sensitive. Modern steel making up the monopoly of the country's target under the strict control of production, distribution was. It is essential to produce iron weapons was a threat is because you can keep the throne in the hands of the forces that can cause side effects when I went was to block. This study created a rail Cholbi(iron monument) and the regional distribution pattern of the production, construction background, looked on. Cholbi(iron monument) for the production and recording 『the Annals of the Joseon Dynasty』 often appear in history books and many academic interests, but was off target. Compared to a stone monument that was not generally as well as the Japanese colonial period and over the course of modernization destroyed, damaged a lot of cases the cause may be found in front. Cholbi(iron monument), except for the gravestones of the Joseon Dynasty monument erected in honor of virtue, as an example of content that dominated a packman business, founding of the school and confirmed that a few were built as a special purpose. Based on these discussions changes the identity of the hero monument and production inspector, review of production through the Joseon Dynasty period Cholbi (iron monument) contemplated the significance is reflected in production. 우리사회에서 철을 자유로이 사용하게 된 것은 채 100년이 안 된다. 중세 이전 우리나라의 철 생산은 원시적인 행태를 벗어나지 못했다. 그러던 것이 태종실록의 기록에 따르면 1407년 전국적으로 대규모 철장[철광산] 증설을 시행하였는데, 세종실록지리지에는 이 때 개발된 철장의 수를 전국적으로 78개소였다고 기록하고 있다. 15세기에 편찬된 농사직설에 따르면 철재 농기구는 지주 등 일부만이 소유할 수 있는 중요한 재산이었음을 알 수 있다. 당시 가장 중요한 생산수단 이었던 농기구마저 지주 등 일부에게 한정되어 사용되던 시절이었기에 다른 용도로 철을 사용한다는 것은 많은 제약이 따랐다. 이러한 상황은 16세기 말~17세기 전반기에 거듭된 전쟁으로 관영수공업이 파괴되었으며, 국가로부터 제철업경영권을 위임 받은 ‘별장’이 세금을 내는 조건으로 철소(鐵所)를 사적으로 경영하게 되면서 획기적으로 변화하게 되었다. 조선시대 철비는 크게 현감, 관찰사 등 지방수령의 공덕을 기리기 위한 공덕비와 1684년 제작으로 서당을 운영하기 위하여 창립한 전남 진도 학계(學契)비 등의 사적(史蹟)비, 보부상 들이 세운 송덕비로 크게 나눌 수 있다. 철은 과거 부의 상징이자 나무나 돌에 비해 강하고 영원하다는 믿음의 대상이었다. 때문에 중요한 공덕비 건립이나, 맹세의 상징으로 철비를 건립하게 된 것이다. 뿐만 아니라 철은 동양사상에서 악한 것을 물리치고, 지기(地氣)가 강한 곳을 누른다는 비보풍수의 목적으로 사용되었는데, 철비가 세워진 가문은 최고의 영광이었다고 한다. 철비는 17~18세기 들어 제작이 급격히 증가하는 현상을 보인다. 이것은 선정을 베푼 수령의 증가가 아닌, 역설적으로 원성을 듣던 수령이 직접 세우는 사례가 증가하며, 부를 축적한 중인계층들이 양반으로 신분을 바꾼 후 조상의 정통성을 가공하기 위해 철비를 세우는 경우가 허다했다. 어떠한 이유로 철비를 제작하게 되었는지는 기록으로 남아 있지 않아 확인할 수 없으나, 오행(五行)사상과 관련 깊을 것으로 추정된다. 철은 곧 금(金)이다. 오행에 있어 '금'의 기운을 보면 '금'은 대지를 뜻하며, 그 색은 황금이며 황금은 모든 것을 포용하는 찬란한 휘광이다. 또한 금은 모든 쇠, 또는 철이기도 하기 때문에 음의 기운에 속한다. 금은 단단하고 변함없으며, 절대 부서지지 않는 강인한 기운을 지니고 있다. 이승이 양이라면 저승은 음이다. 이러한 오행사상과 철이 지니는 가치 때문에 철로 비를 제작했을 것으로 생각된다. 뿐만 아니라 조선시대에 들어서면 급격한 생산력의 증대가 이루어진다. 세종대 이후 농업기술의 발달에 기인한 바 크며, 17세기 이후 상업자본의 성숙과 함께 사대부와 견줄만한 재력을 모은 중인계층의 성장이 두드러진다. 이러한 사회여건 속에서 자신들의 권위와 부를 상징하기 위해 당시까지만 하여도 귀했던 철을 소재로 비를 건립한 것으로 판단된다.
