        Combined Treatment with Low Concentrations of Aqueous and Gaseous Chlorine Dioxide Inactivates Escherichia coli O157:H7 and Salmonella Typhimurium Inoculated on Paprika

        Combined treatment with gaseous and aqueous chlorine dioxide (ClO<sub>2</sub>) was performed to improve the microbiological safety and quality of paprika. A single treatment of 50 ppmv ClO<sub>2</sub> gas for 30 min decreased the populations of Escherichia coli O157:H7 and Salmonella Typhimurium by 2.33 and 2.91 log CFU/g, respectively. In addition, a single treatment of aqueous ClO<sub>2</sub> (50 ppm) for 5 min decreased these populations by 1.86 and 1.37, respectively. The most dramatic effects were achieved by combined treatment of 50 ppm aqueous and gaseous ClO<sub>2</sub> for 30 min, which decreased populations of E. coli O157:H7 and S. Typhimurium by 4.11 and 3.61 log CFU/g, respectively. With regard to the qualities of paprika, no adverse effects were elicited by the combined treatment. Thus, combined treatment with aqueous and gaseous ClO<sub>2</sub> is a suitable approach that can be used to improve the microbial safety and quality of paprika.

        채소류 오염 미생물 제거를 위한 안식향산과 초음파의 병용처리

        Purpose: This study was conducted to determine an efficient combination of ultrasound and benzoic acid treatment for microbial sterilization. The combined treatment was applied against two bacteria that frequently contaminate fresh vegetables, Escherichia coli and Pseudomonas spp., and was compared with individual treatment with either ultrasound or benzoic acid. Methods: The combination of ultrasound and benzoic acid was applied to a mixture of three Escherichia coli strains and Pseudomonas fluorescens B7 strain isolated from the floret surfaces of broccoli. Benzoic acid was applied for 30 min at different concentrations after which concentrations resulting in at least a 1 log reduction in the number of viable cells were selected for the combination treatment. Ultrasound was applied for 5-60 min in both single and combined treatments. Results: The 0.10% and 0.01% benzoic acid were selected for combined treatment for the E. coli mixture and P. fluorescens B7, respectively. Combined treatment for over 5 min significantly reduced the number of viable cells when compared to indivudual treatment with either ultrasound or benzoic acid under the same conditions. Conclusion: Combined treatment with ultrasound and benzoic acid was more effective than individual treatment for sterilization of fresh vegetables in vitro; however, further study is needed to determine if combined treatment is effective in vivo.

        관절ㆍ류마티스 질환의 한ㆍ양방 협진에 관한 임상적 고찰

        Objectives : To evaluate the current status of East-West combination treatment in joint disorders. Methods : The medical records of patients who visited the Joints & Rheumatism Center at the Kyung Hee East-West Neo Medical Center from April 2006 to June 2009 were evaluated. The general characteristics of patients who underwent combination treatment, trend in number of cross-system referrals, and disorders and involved body regions of patients referred to the Eastern medical hospital from the Western medical hospital were initially assessed. 6 major disorders were found from the initial scanning. The trend in number of cross-hospital referrals, number of visits to the Eastern medical hospital, current status of combination treatment, treatment modality, and reason for cross-system referral was evaluated. Results : 1. 1510 patients were referred from the Eastern medical hospital to the Western medical hospital, and 1065 patients were referred from the Western medical hospital to the Eastern medical hospital. First visit patients reached a peak at the second quarter of 2007 and fourth quarter of 2006 respectively, and have steadily decreased from then on. Referrals of female patients were twice as common as male patient referrals. Patients in their sixth or seventh decade of life were most commonly referred, and more outpatients were referred compared to inpatients. 2. Patients with knee joint disorders were most commonly referred from the Western medical hospital to the Eastern medical hospital, followed by hip, shoulder, ankle, wrist, and elbow joint disorders. The most common disorders for each of the above regions in referred patients were knee osteoarthritis, avascular necrosis of the hip, adhesive capsulitis, and ankle strain and sprain. The generalized disorders rheumatoid arthritis and ankylosing spondylitis followed. 3. Patients referred to the Eastern hospital received approximately 3 to 10 Eastern medical treatment sessions. 45 percent remained on constant combination treatment, and 98 percent of referred patients received acupuncture treatment. Conclusions : In regard to the number of patients and duration of combination treatment, combination treatment was successfully performed for knee osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis, while it was not so for avascular necrosis of the hip, adhesive capsulitis, and ankle strain and sprain. Further research on this subject is required.

        Improving the Microbial Safety of Fresh-Cut Endive with a Combined Treatment of Cinnamon Leaf Oil Emulsion Containing Cationic Surfactants and Ultrasound

        Endive is widely consumed in a fresh-cut form owing to its rich nutritional content. However, fresh-cut vegetables are susceptible to contamination by pathogenic bacteria. This study investigated the antibacterial activities of the combined treatment of cinnamon leaf oil emulsion containing cetylpyridinium chloride or benzalkonium chloride (CLC and CLB, respectively) as a cationic surfactant and ultrasound (US) against Listeria monocytogenes and Escherichia coli O157:H7 on endive. The combined treatment of CLC or CLB with US reduced the population of L. monocytogenes by 1.58 and 1.47 log colony forming units (CFU)/g, respectively, and that of E. coli O157:H7 by 1.60 and 1.46 log CFU/g, respectively, as compared with water washing treatment. The reduction levels of both pathogens were higher than those observed with 0.2 mg/ml sodium hypochlorite. In addition, the combined treatment showed no effect on the quality of the fresh-cut endive (FCE). In particular, the degree of browning in FCE was less for the treatment group than for the control and water washing treatment groups. Thus, cationic surfactant-based cinnamon leaf oil emulsions combined with US may be an effective washing treatment for the microbial safety of FCE.

        국내 증례 보고를 통해 본 대상포진의 한방 단독 치료와 한·양방 협진 치료의 효과 분석

        Objectives : The aim of this article is to compare the effect of the korean medicine alone with korean-western combination treatment to herpes zoster. Methods : Domestic databases(OASIS, NDSL, RISS, KISS, KTKP, KMBASE, DBPIA, KoreaMed, National Assembly Library) were used to search case study papers related to herpes zoster and korean medicine treatment. We searched them using the term 'Herpes zoster' or 'Ramsey Hunt' combinated with 'Acupuntcure', 'Herbal medicine' or 'Korean medicine'. Results : A total of 36 studies were selected and analyzed. 25 studies treated with korean medicine and 11 studies treated with korean-western combination treatment which include any western medication. Of these 36 papers, there were a total of 24 cases in which a VAS test was undertaken. According to 36 published papers, no statistically significant difference in the periods of treatment with korean medicine alone from those of korean-western combination treatment was found. Also, no statistical difference in the VAS changes per period with korean medicine alone from those with korean-western combination treatment was found. Conclusions : According to the results, These findings suggest that korean medicine treatment can be effective for the herpes zoster the same as korean-western combination treatment.

        Effect of Combined Traditional Acupuncture, Pharmacopuncture and Applied Kinesiology on Lumbar Diseases of Resident Patients

        Objectives: Recently several Korean medical doctors have begun practicing applied kinesiology (AK). Although the efficacy of combining traditional acupuncture (TA) and pharmacopuncture (PP) on lumbar diseases such as lumbar spinal stenosis (LSS) and lumbar herniation of intervertebral disk (LHID) has been examined, the possible benefits of combining TA, PP and AK approaches have not been examined. Therefore the aim of this study was to develop effective treatment for lumbar disorders by combining TA, PP, and AK treatments. Methods: Twenty-four patients hospitalized at Samse Korean Traditional Medicine Hospital between March and September 2018 with L5 or S1 root radiculopathy associated with LSS and LHID were included in this study. They were treated for 10 days with TA, PP and AK approaches that included category block, manipulation and strain/counterstrain treatments. The primary outcomes were mainly assessed using Japanese Orthopedic Association lumbar scores (JOALS). JOALS were determined before the start of treatment as well as five and 10 days after treatment started. Results: The treatments improved the lumbar condition of the patients based on JOA pain score. Conclusion: The combined TA, PP and AK treatments were effective in treating spinal diseases of resident patients. Prospective, controlled, and relevant protocols using multimodal strategies to define the role of TA, PP and AK are needed.

        절화 백합 잎마름병 방제 및 품질 보존을 위한 감마선 150 Gy와 Sodium Dichloroisocyanurate 병용처리

        For the purpose of practical application of gamma irradiation for export lily without the ionic energy damage, 150 Gy gamma ray was suggested as the safe level on cut lilies, the oriental cultivar Siberia (white) and Sorbonne (pink). The combined treatments of 150 Gy gamma ray and sodium dichloroiso-cyanurate (NaDCC) at 0, 40, 100, and 200 μg/l, were tried to control lily leaf blight caused by Botrytis elliptica. Incidences of petal blight on the two oriental lilies at control were 93%–95%, whereas those at the combined treatments of 150 Gy and 100 μg/l or higher of NaDCC were 76%–83%. However, severity of petal blight was not reduced by the combined treatments. Gamma irradiation at 150 Gy did not change the cut lily quality such as improve cut lily longevity of vase-life compare with the control. In addition, fresh weight, chlorophyll contents, and the color of pink petals of Sorbonne were not changed by 150 Gy of gamma irradiation. Therefore, we recommend the combined treatment of 150 Gy gamma irradiation and 100 μg/l NaDCC for the standard operating procedure on cut lily for export quarantine to control leaf blight.

        Treatment Outcomes of Combination Therapy with 1,064-nm Neodymium-doped Yttrium Aluminum Garnet Laser and Efinaconazole 10% Solution for Big Toenail Onychomycosis: a Retrospective Study

        Background: Laser therapy can be used as an alternative treatment for onychomycosis; however, there are some limitations to its efficacy as a single agent. Objective: To evaluate the effectiveness of combination therapy with 1,064-nm neodymium-doped yttrium aluminum garnet (Nd:YAG) laser and topical efinaconazole in onychomycosis treatment and identify factors influencing the therapeutic outcomes of combination treatment. Methods: Big toenails with onychomycosis were treated by 1,064-nm Nd:YAG laser at 4-week intervals with daily application of topical efinaconazole. Therapeutic response was assessed through onychomycosis severity index (OSI) and percentage of nail infected (PNI), and its association with a variety of factors that may affect treatment outcome was evaluated. Results: One hundred big toenails were included in the study and significant clinical improvements were observed after treatment (OSI improvement score = 76.68 ± 28.83, PNI improvement score = 72.37 ± 30.37). There was no difference in treatment response according to the number of laser treatments, onychomycosis clinical type, or initial severity. However, patient age was negatively correlated with clinical improvement (p = 0.019). Also, female patients had better therapeutic responses than male patients. Conclusion: Combined treatment with Nd:YAG laser and topical efinaconazole has a significant therapeutic effect on onychomycosis. A randomized controlled trial is warranted in the future.

        금형재료용 주철강의 복합열처리 특성

        Currently, there are two main issues regarding the development of core technologies in the automotive industry: the development of environmentally friendly vehicles and securing a high level of safety in the event of an accident. As part of the efforts to address these issues, research into alternative materials and new car body manufacturing and assembly technologies is necessary, and this has been carried out mainly by the automotive industries. Large press molds for producing car body parts are made of cast iron. With the increase of automobile production and various changes of design, the press forming process of car body parts has become more difficult. In the case of large press molds, high hardness and abrasive resistance are needed. To overcome these problems, we attempted to develop a combined heat treatment process consisting of local laser heat treatment followed by plasma nitriding, and evaluated the characteristics of the proposed heat treatment method. From the results of the experiments, it has been shown that the maximum surface hardness is 864 Hv by the laser heat treatment, 953 Hv by the plasma nitriding, and 1,094 Hv by the combined heat treatment. It is anticipated that the suggested combined heat treatment can be used to evaluate the durability of press mold.
