        내측광근 강화운동을 이용한 슬개-대퇴 동통 증후군 치험 2례

        슬개-대퇴 통증 증후군 환자 2례에 내측광근 강화 운동을 이용하여 치료한 결과, Knee X-ray(Skyline view) 상의 일치각 및 NRS, WOMAC Index의 호전을 확인하였다. 이에 저자는 본 증례를 보고하는 바이다. Objectives: The purpose of this case report is to investigate the effect of exercise for vastus medialis to knee pain patients with patello-femoral pain syndrome. Methods: Two patients with patello-femoral pain syndrome were treated by exercise for vastus medialis. To measure the outcome of the patients' improvement, we observed the X-ray of both knee sky-line view, Numerical Rating Scale(NRS), and Western Ontario and McMaster Universities(WOMAC) index. Results: After being treated by exercise for vastus medialis, the patients' X-ray of both knee sky-line view, Numerical Rating Scale(NRS), and Western Ontario and McMaster Universities(WOMAC) index were significantly improved. Conclusions: These results suggest that exercise for vastus medialis is effective treatment to patients with patello-femoral pain syndrome.

        Arthroscopic Reduction of Irreducible Posterolateral Knee Dislocation with Interposition of the Vastus Medialis: A Case Report

        Irreducible traumatic knee dislocation is rare. The knee dislocation is classified depending on the incarcerated structures. Complete reduction is achieved by extracting the incarcerated structure. Several reports introduce the reduction of irreducible traumatic knee dislocation by open surgery or arthroscopy. This case describes irreducible posterolateral knee dislocation with interposition of the vastus medialis. Closed reduction failed in the emergency room, and complete reduction was attained by arthroscopically sectioning the muscle and fascia of the vastus medialis in the intercondylar notch.         스쿼트 운동 시 적용된 중주파 전기자극이 안쪽빗넓은근과 가쪽넓은근의 근활성도 및 근활성비에 미치는 영향

        Purpose : The purpose of this study is to investigate the effect of mid frequency electrical stimulation during squat exercise on the muscle activity and muscle activity ratio of vastus medialis oblique and vastus lateralis, and to prepare scientific basic data for exercise intervention using mid frequency electrical stimulation. Methods : This study was conducted with students from University C located in Busan, and among a total of 123 subjects, 12 subjects who complained of knee joint dysfunction between 80 and 90 points using the Kujala patellofemoral score (KPS) were used. All subjects participated in the experiment for 3 days, and MVIC values were measured for normalization of muscle activity values on the first day. For the two days, participants participated in the experiment and performed squat exercise or squat exercise receiving mid-frequency electrical stimulation in random order. Measurements were taken in the squat position immediately after the squat exercise, and muscle activities of vastus medialis oblique and vastus lateralis were measured. The measured data were compared through the dependent t test, and the statistical significance level was set to .05. Results : According to the results of this study, in the case of applying mid-frequency electrical stimulation together in the ratio of vastus medialis oblique and vastus lateralis muscle activity during squat exercise, higher values were observed compared to the case of not applying mid-frequency electrical stimulation together, and statistically significant. Also, when mid-frequency electrical stimulation was applied to both vastus medialis oblique and vastus lateralis activities during the squat exercise, higher values were shown compared to the case where the mid-frequency electrical stimulation was not applied together, but there was no statistically significant difference. Conclusion : The results of this study reported that mid-frequency electrical stimulation provided to vastus medialis oblique increased the muscle activity ratio of vastus medialis oblique and vastus lateralis. Therefore, the improvement of coordination due to the enhancement of the activity of vastus medialis oblique through mid-frequency electrical stimulation will be more helpful in the treatment of patellofemoral pain syndrome patient. In addition, it is hoped that the electrical stimulation method applied to exercise will be widely used.

        Purpose: The purpose of this study was to show the effects of voluntary change of weight bearing on the vastus medialis oblique and the vastus lateralis ratio during squat exercise. Methods: Twenty-four healthy adults were recruited for this study. The subjects performed squat exercise with median weight bearing, lateral weight bearing and non-changed weight bearing. The muscle activities of the vastus medialis oblique and the vastus lateralis were measured during the squat for all three conditions. The measured data were analyzed using one-way ANOVA to investigate the effect of muscle activation on the each condition. The statistical analyses were performed using SPSS ver. 17.0, and a p-value of less than 0.05 was considered significant for all cases. A post-hoc test was performed using Tukey’s test. Results: The study showed that the vastus medialis oblique and the vastus lateralis ratio significantly changed according to the voluntary change of weight bearing during a squat. The vastus medialis oblique and the vastus lateralis ratio significantly decreased under the condition of voluntary lateral weight bearing. Conclusion: We suggest squat exercise on median weight bearing to increase the vastus medialis oblique and the vastus lateralis ratio. If your patient has. .

        Effects of the Early Sensorimotor Training on Vastus Medialis Oblique Muscle Activation in Patients after Partial Medial Meniscectomy

        Background: Early rehabilitation after partial meniscectomy is important to recover the balance of the vastus medialis oblique and vastus lateralis and prevent pathological problems in the lower extremities and the whole body. Objective: To compare muscle activations for patients after partial meniscectomy. Design: Dual-group Pretest-Posttest Design from the Quasi-Experimental Research. Methods: Twenty participants after partial meniscectomy were recruited and were randomly divided into a Q-setting sensorimotor training group (QSMTG) and Q-setting exercise group (QSEG). Muscle activity of the vastus medialis oblique and vastus lateralis was measured before and after intervention. Results: In the two groups, the vastus medialis oblique and vastus lateralis activations increased significantly (P<.05). The Q-setting sensorimotor training group showed more increases than the Q-setting exercise group, and there were significant differences between the groups (P<.05). The activation ratio of the vastus medialis oblique and vastus lateralis had increasingly significant differences in the Q-setting sensorimotor training group (P<.05), and there were no significant differences between the groups (P>.05). Conclusion: Q-setting exercise with sensorimotor training was a useful method that improved the balance of vastus medialis oblique (VMO) and vastus lateralis (VL) activity after meniscectomy.

        가쪽넓은근 억제 테이핑 적용이 넙다리네갈래근의 근수축 특성 변화에 미치는 영향

        The purpose of this study was to investigate the effects of vastus lateralis inhibition taping when using non-elastic tape with the contraction characteristics of the quadriceps muscles. The study included 37 healthy individuals in their 20s. To compare the contraction characteristics of the quadriceps muscles after applying the non-elastic tape to the vastus lateralis, muscle tension and muscle contraction times were measured using tensiomyography. There was a significant difference in muscle tension of both the vastus lateralis and rectus femoris before and after applying the non-elastic tape to the vastus lateralis muscles. There was a significant difference in the contraction characteristics of the vastus medialis muscle on the left side in male participants. Additionally, before and after applying the non-elastic tape, the vastus lateralis/vastus medialis ratio and the rectus femoris/vastus medialis ratio of muscle tension decreased significantly. The muscle contraction times of the three muscles were not affected by applying non-elastic tape to the vastus lateralis. Use of non-elastic tape reduced muscle tension of the quadriceps muscles and rectus femoris muscles, but had no effect on the muscle contraction time.

        후방보행 훈련이 하지 등속성 근력 및 내측광근 표면 근전도에 미치는 영향

        Purpose Backward walking exercise is being recommended for increasing lower extremity strength, especially vastus medialis strength. But little research has been conducted backward walking training to clarifying vastus medialis strength. The purpose of this study is to verify the effectiveness of backward walking training on knee muscle strength especially vastus medialis strength. Subjects and Methods Fourteen healthy male adults were randomly divided into backward walking (BW, n=7) and forward walking (FW, n=7) training groups. Each group took part in muscle strength tests of knee extension and flexion isokinetic peak torque (PT), total work (TW). Vastus medialis (VM) and vastus lateralis (VL) root mean square (RMS) of electromyographic (EMG) also measured during maximal voluntary isometric contraction. Each group took one week resting period and then, performed BW and FW training program for 6 weeks, consisting of 3 sessions per week for a total of 18 sessions. The training intensities applied to individual subjects were at 65% of their VO2max respectively. Results The BW training group showed a significant increases in knee extension and flexion isokinetic PT (4.6%, 13.9%), isokinetic TW (17.34%), and isometric VM EMG RMS (48.5%). But FW training group did not show a significant increase in any of the items. Conclusion BW training has positive effectiveness on knee muscle strength, especially vastus medialis strength, which is acting as medial stabilizer of the patella.

        Comparisons of Vastus Medialis and Vastus Lateralis Muscle Activities according to Different Heights during Drop Landing in Flatfooted Adults

        Purpose: The purpose of this study was to investigate comparisons of vastus medialis (VMO) and vastus lateralis (VL) muscle activities according to different heights during drop landing in flatfooted adults. Methods: Fifteen subjects with a flat foot arch and 15 subjects with a normal feet arch were participated. Subjects performed a double limb drop landing task from 20, 40, and 60 cm heights. Surface electromyography was used to measure the muscle activities of the VMO and VL during drop landing. Results: There were significant differences of muscle activities in the VMO, VL, and the VMO and VL ratio between groups. The electromyography values of VMO, VL, and the VMO and VL ratio in the normal group were significantly greater than in the flat foot group, and muscle activities and the VMO and VL ratio significantly increased with landing heights in the both groups. Conclusion: Our results indicated that muscle activity patterns of VOM and VL in the flat foot group were lower at heights than in the normal group, so calf tightness was negative effects on balance and gait ability, so assessment of muscle activation patterns in the knee extensors should be considered during exercise and treatment of flat feet.

        와이드스쿼트 운동이 안굽이무릎 변형이 있는 성인의 안쪽넓은근과 가쪽넓은근의 근활성에 미치는 영향: 단일 사례 연구

        The purpose of this study is to investigate the effects of wide-squat exercise on muscle activities of vastus medialis and lateralis in a adult with Genu varum deformity. The subject, a 23 years-old male student with genu varum deformity, was performed the intervention, wide-squat exercise according to the knee joint angle, during three times a week for four weeks. After then, the changes of Q-angle and muscle activities were compared. The ratio of the vastus medialis and lateralis was changed from 30.70: 14.43 to 24.9: 30.63. Even though the wide squat exercise showed no significant change in Q-angle, that is the effective intervention for balancing the muscle activities of the vastus medialis and lateralis.

        This study was performed to investigate the effects of tibial rotation while going up stair on muscle activity of vastus medialis oblique and vastus lateralis, and on patellar displacement. The subjects included 30 people (male: 15; female: 15) who were randomly assigned to the tibial internal-rotation, neutral-rotation, and external-rotation groups, The subjects went up the stair while performing the assigned rotations, and the rotation of the hip and the displacement of the patella were measured using a 3D motion analyzer. In addition, the maximum voluntary isometric contraction (MVIC) of the vastus medialis oblique and vastus lateralis were measured using surface electromyogram. On the tibial internal rotation, the hip rotation significantly appeared in the same direction and so did on the tibial neutral and external rotations (p<.001). Although the MVIC of the vastus medialis oblique and vastus lateralis did not significantly differ by tibial rotation during the stair ascent, the MVIC of the vastus medialis oblique was higher than that of the vastus lateralis during the internal and neutral rotations (p<.05). In addition, during the stair ascent, the displacement of the patella was more significant during the tibial external rotation than during the tibial internal and neutral rotations (p<.001). Thus, patients with patellofemoral pain are required to be considered the effects of tibial rotation for their rehabilitation.
