        경항통을 호소하는 환자들의 경추 만곡에 따른 통증 및 피로도 비교 연구

        Objectives :The purpose of this study was to compare about quantity of pain and fatigue according to cervical spine curvature of patient with neck pain Methods :Cervical spine curvature was measured using the sagittal radiography of the cervical spine, neck pain was evaluated using the VAS and neck fatigue was evaluated using fatigue symptom checklist. Based on four line Cobb`s method, 51 subjects were divided into hypolordosis group, normal group, hyperlordosis group. Window version SPSS 12K was used for statistical analysis about relation between pain and cervical spine curvature of each group, also about between fatigue and cervical spine curvature of each group. Results :1. A significant difference was not found between pain and cervical curvature of each group. 2. A significant difference was not found between fatigue and cervical curvature of each group. Conclusions :There was no relation between pain and cervical curvature of each group, also fatigue and cervical curvature.

      • 교통사고 후 한방병원에 입원한 일자목증후군 환자를 대상으로 한 진단방법의 비교 평가 및 경침사용의 유효성 연구

        Objectives : This study was designed to evaluate various tools including C-spine X-ray for estimating cervical curvature and identify the effect of wooden pillow on patients diagnosed with straightening of cervical curvature. Methods : This study was carried out on 51 subjects who were encountered traffic accident suffering cervical pain and diagnosed with straightening of cervical curvature by X-ray. 51 subjects were divided into wooden pillow(experimental) group and conservative therapy(control) group. Visual analogue scale, neck disability index and lateral pictures were used and compared after treatment. C-spine AP., Lat. X-ray were used on admission day to calculate cervical curvature and scoliosis by various ways. Results : 4 different measurements of cervical curvature didn`t show common results. Both wooden pillow(experimental) group and conservative therapy(control) group showed significant improvement in the visual analogue scale(VAS) and neck disability index(NDI) after hospital treatment. Comparison between center line pictured by lateral view and C-spine Lat. pictured by X-ray showed significant difference in cervical curvature. Lateral deviation of cervical vertebra showed lengthened distance between mastoid process and spinous process of C7. Conclusion : As a result of this research, I found out that various tools for calculating cervical lordosis could derive different results and C-spine AP., Lat X-ray could cause artificial cervical lordosis. According to visual analogue scale(VAS) and neck disability index(NDI) wooden pillow was not effective to reduce pain & disability caused by straightening of cervical curvature. Futhermore, correction effect of straightening of cervical curvature and scoliosis by wooden pillow was weak.

        척추 굴곡 반응형 의자등받이 개발 및 효과성 검증

        연구목적 : 본 연구는 착석시 의자등받이가 사용자의 허리를 감싸안고 척추형태에 반응하여 자연스러운 척 추 곡선을 유지 및 증진시켜 줌으로써 바른 자세 유지 및 허리 피로감 해소에 도움을 주는 척추 굴곡 반응형 의자등받이를 개발하고 이의 효과를 과학적으로 검증하는데 목적을 두었다. 연구방법 : 척추 굴곡 반응형 등받이는 일반 사무용 의자에 탈부착이 가능하며 사용자가 허리를 기댔을 때 등받이가 사용자의 척추라인에 따라 반응하도록 하여 허리 전체가 등받이에 밀착됨으로써 척추의 자연 스러운 굴곡 형태를 유지하고 증진시키도록 개발하였다. 개발된 등받이의 효과성 검증을 위해 표준체형 의 남자대학(원)생 10명을 피실험자로 선정하여 동작분석기와 근전계를 활용하여 착석시 척추 굴곡의 변화 측정과 척추 세움근의 활성도를 측정하였다. 결과 : 개발된 등받이와 비교 대상 의자에 착석시 척추 곡선 변화에 대한 동작분석기 평가결과, 등받이 각 도 105o부터 시작하여 리클라이닝 각도가 125o로 커짐에 따라 요추의 전만곡과 흉추의 후만곡이 커짐으 로써 척추 곡선이 증진됨을 보였다. 근전계를 활용한 척추 세움근(L5/S1)의 평가결과 개발된 의자를 사 용했을 때 모든 등받이 각도에서 사후 중앙주파수 값이 사전 중앙주파수 값보다 높게 측정됨으로써 피 로지수가 낮아졌음을 알 수 있었다. 결론 : 본 연구에서 개발한 척추 굴곡 반응형 등받이는 착석시 바른자세를 유지하게 하여 허리근육의 긴장완 화와 허리디스크에 가해지는 압력을 감소시켜 장시간 앉아서 업무를 보거나 공부하는 학생들의 피로감을 줄여줄 의자로써 활용이 가능하며 향후 이의 도입과 확산을 위한 추가적인 연구개발의 필요성을 제안한다. Objective: This study aimed at the development and validation of a spine curvature responsive chair backrest that helps maintain the correct posture and relieve back fatigue by facilitating and enhancing the natural spine curve in response to the shape of the spine while the chair back embraces the user’s back when seated. Methods: The spine curvature responsive backrest was designed to be attached to a general office chair. When the user leans on his or her back, the back responds according to the user’s spine curvature such that the entire back is closely stuck to the backrest, thereby maintaining and improving the natural curve of the spine. Ten male college students were selected as the subjects, and the change in their spinal curvature was measured using a motion analyzer and an EMG. Results: The results of a motion analyzer evaluation showed that the lordosis of the lumbar and the kyphosis of the thoracic vertebrae increased as the reclining angle increased to 125 degrees, starting from the backrest angle of 105 degrees when using the developed backrest. As the results of the EMG evaluation, the post-median frequency was higher than the pre-median frequency at all backrest angles. Conclusion: The spine curvature responsive backrest developed in this study helps to maintain a proper posture when seated, thereby relieving tension in the back muscles. It can be used as a chair for office workers and students who sit for a long period of time for reducing their back fatigue.

        교통사고로 유발된 경항통 환자의 경추 만곡도에 따른 통증 및 기능 호전도 비교연구

        Objectives : The aim of this study was to compare improvement of pain and physical function according to cervical spine curvature on neck pain patients induced in traffic accident. Methods : We investigated patients of 78 cases with neck pain induced in traffic accident. Based on Cobb`s method[C1-C7], 78 subjects were divided into hyperlordosis group, normal group, hypolordosis group. We treated each group by the same therapies acupuncture, pharmacopuncture, herbal medicine. To compare the treatment effects of three groups, we used numeric rating scale(NRS), neck disability index(NDI). Results : Improvement in NRS of neck pain and NDI, there were no significant difference among the groups. Conclusions : There was no relation between improvement of neck pain and physical function and cervical curvature.

        Prognostic Factor Analysis for Management of Chronic Neck Pain : Can We Predict the Severity of Neck Pain with Lateral Cervical Curvature?

        Objective : Although little is known about its origins, neck pain may be related to several associated anatomical pathologies. We aimed to characterize the incidence and features of chronic neck pain and analyze the relationship between neck pain severity and its affecting factors. Methods : Between March 2012 and July 2013, we studied 216 patients with chronic neck pain. Initially, combined tramadol (37.5 mg) plus acetaminophen (325 mg) was administered orally twice daily (b.i.d.) to all patients over a 2-week period. After two weeks, patients were evaluated for neck pain during an outpatient clinic visit. If the numeric rating scale of the patient had not decreased to 5 or lower, a cervical medial branch block (MBB) was recommended after double-dosed previous medication trial. We classified all patients into two groups (mild vs. severe neck pain group), based on medication efficacy. Logistic regression tests were used to evaluate the factors associated with neck pain severity. Results : A total of 198 patients were included in the analyses, due to follow-up loss in 18 patients. While medication was successful in reducing pain in 68.2% patients with chronic neck pain, the remaining patients required cervical MBB. Lateral cervical curvature, such as a straight or sigmoid type curve, was found to be significantly associated with the severity of neck pain. Conclusion : We managed chronic neck pain with a simple pharmacological management protocol followed by MBB. We should keep in mind that it may be difficult to manage the patient with straight or sigmoid lateral curvature only with oral medication.

        교통사고로 인한 편타손상 후 경추 만곡의 변화 연구

        Objectives: To investigate the change of the cervical spine curvature after whiplash injury. Methods: Clinical study carried out in 46 patients who already had neck pain and took a cervical lateral X-Ray in Jaseng Oriental hospital. these patients had whiplash injury whitin one year. We followed up the cervical lateral X-Ray after that. Cervical spine curvature was measured using four measuring method. Type of cervical spine curvature was analyzed by Jochumsen method, Ishihara Index. T-test was used. Results: By Jackson's Angle and Cobb's angle, it is tend to be more Iordotic curvature after whiplash injury. In Jochumsen method, Ishihara Index the cervical spine curvature was significantly increased their Iordotic angle(P<0.05). Conclusion: The results suggest that after whiplash injury, patients cervical curvature change more Iordortic curve.

      • 편타 손상 경항통 환자와 잠행성 경항통 환자의 경추 만곡 비교

        Objectives : To investigate and compare the curvature of the cervical spine of the patients with whiplash and insidious onset neck pain. Method : Clinical study carried out in 33 insidious onset neck pain outpatients and 34 whiplash onset neck pain inpatients in Conmaul Oriental Hospital. Cervical spine curvature was measured using five measuring Methods. Type of cervical spine curvature was analyzed by Jochumsen method, Ishihara Index. T-test was used to compare the cervical spine angle of the two groups. Results : The prevalence of straight and kyphotic cervical spines was 46.5% in the insidious onset cases and 26.47% in the whiplash onset cases. In Jackson s angle, Jochumsen method, Ishihara Index, and Park s method, angle of the Cervical spine curvature was significantly lower in the insidious onset cases. (P<0.01) Conclusion : The results suggest that the cervical spine of neck pain patients is straight and kyphotic and more significant in insidious onset cases.

        교통사고 후 응급실에 내원한 경항통 환자의 경추 만곡 연구

        Objectives : The purpose of this study is to investigate the cervical spine curvature after whiplash injury by traffic accident. Methods : The cervical lateral radiographs of 102 outpatients who visited emergency room in Kyung-Hee university hospital at Gangdong after whiplash injury by road traffic accident were reviewed to measure the cervical spine angle using C1-7, C2-7 Cobb method, Sagittal tangent method, Jochumsen method and the Ishihara index by two oriental medical doctors. For statistics, we used SPSS version 17.0 for windows. Results : Means of cervical angle are $37.63{\pm}11.34^{\circ}$, $12.92{\pm}9.13^{\circ}$, $16.19{\pm}10.62^{\circ}$, $1.78{\pm}3.37$ and $8.51{\pm}9.78$ by C1-C7 Cobb Method, C2-C7 Cobb Method, Sagittal tangent method, Jochumsen method and Ishihara index, respectively. Hypolordosis is most numeral in patients by C1-C7 Cobb Method(n=40; 39.22%), Sagittal tangent method(n=68; 66.67%). And Straight is the most by Jochumsen method(n=54; 52.94%), but Normal is the most by Ishihara index(n=53; 51.96%). And Female has smaller curvature in cervical spine than male significantly by C2-C7 Cobb method and Sagittal tangent method(P<0.05). Conclusions : Whiplash injury tends to make hypolordosis or straight. And female has more vulnerable curvature than male in whiplash injury.

        30대 성인의 골반, 척추 및 견갑대 정렬의 패턴 분석 -Full Spine AP X-ray 분석에 따른-

        Objectives: This study was designed to analyze the pattern of asymmetrical alignment. Methods :This study was carried out with the data from comprehensive medical testing. 91 subjects aged 30-39 were evaluated by full spine AP X-ray. For pelvis, innominate measurement(IM), off centering measurement(OCM), ilium shadow measurement(ISM), major axis of obturator foramen(MaF), minor axis of obturator foramen(MiF) were analyzed. Spinal curvature and height of shoulder girdle were analyzed. Results :1. In pelvis, lt. posterior-inferior and lt. inflare combination pattern was 38 cases(42.8%). 2. In spinal curvature, "reverse S" curve was 45 cases(49.4%) and "reverse C" curve was 30 cases(33%). 3. In shoulder girdle, rt. superior pattern was 42 cases(46.1%) and lt. superior pattern was 39 cases(42.9%). 4. In whole body analysis, lt. posterior-inferior and lt. inflare pelvis, "reverse S" spinal curvature and lt. superior shoulder girdle combination patten was 11 cases(12.1%). This pattern is similar to Kendall`s right handedness pattern and Zink`s common compensatory pattern. Conclusions :Results from this investigation showed asymmetrical alignment in 30-39 years-old adults. This results are expected to contribute to classifying the alignment pattern in clinic and systemic treatment.
