        The purpose of this study was to examine the state of the articles in the Journal of Korean Association for Disability and Oral Health. All the papers in the Journal of Korean Association for Disability and Oral Health of 2005 to 2018 were analyzed. A total of 181 papers were classified according to its type, field of the study, school, the number of authors, references, and written language. According to the type of the paper, 38 (21.0%) were original articles, 12 (6.6%) were review articles, 124 (68.5%) were case reports and 7 (3.9%) were others. In relation to the field, the most were pediatric dentistry. According to author’s school, some schools showed preponderances. As to the number of researchers, the most common number of researchers was four (39 articles, 21.5%), and the number of the articles written by six (37, 20.4%) was similar to that of the articles written by three and five (28, 15.5%). As for the number of references, this number ranged from minimum 0 to maximum 40, and the average was 14.8. Regarding the language of the articles, English was used in 5 articles (2.8%), and Korean was used in 176 articles (97.2%). Thirty-nine (21.5%) papers were done in collaboration with workers of the other field. The efforts of this study was to provide useful information of the journal and the development of research in Special care dentistry.

        장애인 치과 치료의 실제와 구강위생관리를 위한 고려사항

        Dental treatment for the disabled requires special consideration. Due to poor cooperation of the patient during dental procedures, behavioral management or general anesthesia is often necessary. We also need to consider the patient’s oral habits and ability to maintain oral hygiene. Since dental treatment may be difficult for the disabled, prevent is more important than treatment itself. In addition, secondary dental caries and periodontal disease after treatment are likely to occur when proper oral care is not sustained. Because the patient is difficult to maintain optimal oral hygiene by themselves, it is essential to educate the guardian about the importance of oral hygiene, as well as encourage them to participate in the patient’s daily oral care. Finally, it is necessary to regularly check the patient’s oral hygiene status and perform professional plaque control through frequent dental visit.

        장애인 구강보건의료서비스 현황과 구강보건정책 제안

        We reviewed the current oral health status, the use of dental health services, and the factors that impede access to dental care for disabled people in South Korea and aimed to suggest the future direction of the dental community for establishing the right to oral health for the disabled people. Disabled people experienced more and faster teeth loss than non-disabled people due to the lack of proper oral hygiene management. The rate of not receiving proper dental prosthodontic treatment for the lost teeth in disabled people was higher than in non-disabled people. Economic factors, patient cooperation, and communication issues were the most important constraints on dental accessibility for the disabled. Many people with disabilities used dental clinics nearby, and some severely disabled people used special dental clinics for the disabled even if they were far. The current dental policy for the disabled people was limited to the provision of dental treatment on the oral care center for the disabled and reduction of the treatment fee. It would be a much more economical and efficient way to supply the prevention-oriented oral health management rather than to provide dental treatment such as prosthetic fabrication, which requires long time and high cost. It is necessary to educate dentists, dental hygienists, and dental students in order to strengthen the competency for the special care dentistry. The whole dental community and the government should make an effort together to establish for the equal right to oral health of the people with disability.

      • 한국 치과대학 및 치의학 전문대학원의 장애인치과학 교육 현황

        Special care dentistry, also known as special needs dentistry, is concerned with the oral health of people who have intellectual disability, or who are affected by other medical, physical, or psychiatric issues. Dental schools must educate dental students for the competency in managing and treating individuals with special health care needs. The purpose of this study is to identify the current status of special care dentistry education in Korean dental schools. A questionnaire relating to the education of special care dentistry was sent to eleven dental schools in Korea via email. The result turned out that eight out of eleven schools had classroom teaching of special care dentistry, while only four had practical teaching programs. Nine schools had dental clinics for the disabled, in which two of them had special care clinics within the Pediatric dentistry clinic, and other seven in separate clinical sites. Despite the increased interest in special care dentistry, education including classroom teaching and practical teaching was still insufficient and needed more development. This study implicated a small but valuable understanding of special needs dentistry education in Korea.

      • Survey of the change in the recognition of dental college students for people with disabilities before and after the special care dentistry classes

        An opportunity for dental students to come into contact with individuals with disabilities is insufficient. Therefore, prejudice and negative attitudes towards persons with disabilities persist. Working under the assumption that educating dental students regarding people with disabilities will bring about positive awareness, we conducted this study. We investigated the changes in the recognition of the issues and needs of people with disabilities before and after special care dentistry class of students of dental college that will include an experience in a dental clinic with a disabled person. Seventy-two Kyung Hee University School of Dentistry third-year dental students attended the special care dentistry class in one semester and they took the same survey was before and after the class. In the questionnaire, we used the disability factor scale (DFS) that Siller has proposed. The authoritarian virtuousness factor getting lower to a significant level after receiving an education. This means that after the education, the respondents realize that how hard to adapt the disabilities. After receiving an education, the rejection of intimacy factor score significantly get higher than before they took the class. This means that after the training, positive attitudes towards those with disabilities were better than those towards the opposite sex. We were able to confirm a positive change in the recognition for persons with disabilities through the special care dentistry class. Our study shows that educating dental students on the how to take care of individuals with disabilities can bring about a positive change in the attitudes students may have regarding them.

        해외의 장애인치과 진료체계 및 건강보험 제도

        The purpose of this study is to look into the health insurance system of other countries and apply in Korea in order to effectively deliver dental health care services to special care needs patients. The countries being reviewed are followings: New York in the United States, Ontario in Canada, Japan, the United Kingdom, and Germany. Most of the countries studied allow dentist visit to facility and patient residence who has difficulty visiting dental office. In New York, protective immobilization is covered by medicaid insurance. In Japan, preventive dental care and behavior management are covered by national health insurance. Adopting and applying effective dental care systems can encourage local dentists treating more special needs patient in their offices.

      • 고령장애인의 정의와 일반적, 치과적 특징

        In Korea, the elderly is defined as the person aged over 65 and the disabled is defined as the person who is registered with Ministry of Health and Welfare. In 2008, the elderly with disabilities was about 770,000. Korea was already poised for an aging society in 2000 and is ahead of and aged society in 2018 and a super aged society in 2026. So, the number of the elderly with disabilities will continue to grow. Various oral diseases can be seen in the elderly with disabilities due to the systemic disease and impaired self oral care, in addition to the physiological aging. Particularly, we should be care for the aspiration pneumonia, which is the major cause of the death of the elderly. As a professional for the special care dentistry, we should understand the general and dental, individual characteristics of the elderly with disabilities and do proper oral care and treatment. Thereby, the quality of life of the patients might be improved.

        중증 지적 장애인을 위한 효과적인 치과치료

        Patients with severe mental challenges are often subjected to extensive dental problems. Their impaired communication and lack of cooperation unable to receive timely dental treatments, which deteriorates their adverse oral conditions. In spite of a limited time frame, well-planned comprehensive treatments can be delivered in an one-day operation setting under general anesthesia. This ultidisciplinary treatment regimen primarily comprises of single-visit endodontics, direct bonded restoration, periodontal treatment, and surgical extraction. Further, one or two additional visits for prosthetic procedures are able to accomplish functional reestablishment and esthetic enhancement. This article discusses on clinical managements for severely mentally challenged patients under a single-day operation.

        Considerations for Invasive Dental Treatment in Disabled Patients

        Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.
