        The purpose of this study was to provide date of muscular power evaluation of shoulder joint and difference of muscular power in shoulder joint isokinetic that external and internal rotator of university baseball players by pitcher and fielder, through above these experiment. This research was being progressed by 50 baseball players of college that is registered on KBA and made a selection players who were no injured of shoulder joint and were executed a experiment after a baseball season in 2006 at K-orthopedics, Gang Nam-Gu, Seoul City. We divided into two groups which were 30 fielders and 20 pitchers including catchers. The experiment of this study was to test strength of internal and external rotators at isokinetic shoulder joint, Biodex(system 3 U.S.A), through isokinetic muscular strength measure equipment. It is no significantly difference strength of internal rotators at 90˚/sec, 180˚/sec isokinetic shoulder joint. But, it is significantly difference strength of external rotators at 90˚/sec isokinetic shoulder joint between pitcher and fielder(p<.01). It is no significantly difference strength of external rotators at 180˚/sec isokinetic shoulder joint. It is no significantly difference peak toque of external rotators at 180˚/sec isokinetic shoulder joint. But, it is significantly difference peak toque % body weight at 90˚/sec isokinetic shoulder joint(p<.001). We came to the conclusion that they depend upon technical improvement in pitching motion, need to muscular strength weight training programs of shoulder, elbow and shoulder joint for gain in power performance. It is though that stretching and strengthening will necessary for the sensible correction of pitching form and improvement of ROM. It is though that Pitchers of college baseball wants a personal trainer of them for set up systematic a data of shoulder joint and strength in amateur baseball. In due time, it is need to study that intended about rotation strength of shoulder joint and ball speed.

        어깨관절 질환에 대한 관절가동술 유형의 연구 동향 : 주제범위 문헌고찰

        Purpose : This study sought to investigate research trends regarding joint mobilization type among patients with shoulder joint diseases. Methods : A scoping review was conducted according to the five steps outlined by Arskey and O’Malley and PRISMA-ScR. We searched six domestic databases (ScienceOn, DBpia, Riss, Kmbase, Kiss, KCI) and three international databases (CINAHL, Pubmed, Cochrane central) between 2013 and June 2023. The keyword terms used were ‘joint mobilization’, ‘Kaltenborn’, ‘Maitland’, ‘Mulligan’, and ‘shoulder joint’. Results : There were a total of 44 studies that investigated the topic, and these were divided into quantitative analysis and topic analysis. In terms of publication year, the number of studies within the last five years has increased more than compared to the previous five years, with most of them being randomized clinical trials. In shoulder joint diseases, it was found that the majority of joint movement studies focused on adhesive joint cystitis and shoulder collision syndrome. The Mulligan concept was the most commonly studied type of joint motion. The dependent variables used included pain, joint function (disability), and muscle function. The visual analog scale was the most commonly used for the pain variable, followed by the numeric rating scale. For joint function and disability variables, range of motion was the most commonly used, followed by shoulder pain and disability index, and disabilities of the arm, shoulder, and hand. For muscle function, variables such as muscle tone, strength, and activity were used. Conclusion : We believe that findings of this scoping review can serve as valuable mapping data for joint mobilization research on shoulder joint diseases. Further studies including systematic reviews and meta-analyses based on these results are recommended.

        견관절과 고관절의 장력 패턴 상관성 연구

        Objectives : Muscle tone is proportional to passive tension, so passive tension of antagonist can be measured by measuring passive ROM of a joint. Considering close correlation between movement and muscle tension, we hypothesized that tension patterns of upper and lower limbs would exit, and we tried to prove the existence of tension patterns of shoulder and hip joints by measuring of passive ROM of those joints. Methods : We collected 110 volunteers(75 men and 35 women), whose age was from 20 to 30, BMI was less than 23kg/m2 and who had no musculoskeletal disorder. we had passively horizontally adducted and abducted shoulder joint and passively adducted and abducted hip joint, then measured passive ROM of those joints. We decided which side was relatively dominant by comparing between left and right ROM. We analyzed the differences of tensions of shoulder and hip joints by the handedness, and the relationships between the tensions of left and right shoulder joints, between the tensions of left and right hip joints and between the tensions of shoulder joints and hip joints. Results : The right hip adduction and the left hip abduction were relatively significantly limited at right-handed persons. There was statistically significant relationship between the tensions of left and right shoulder joints, and between the tensions of left and right hip joints. There was no significant relationship between the tensions of shoulder and hip joint. Conclusions : Right-handed has the tension patterns especially at hip joints. There is also tension patterns between the homologous joints.

        Shoulder MRArthography 검사 시 보조기구를 이용한 해부학적 회전 변화 영상에 대한 유용성 평가

        Shoulder MRArthrography was performed to get an accurate diagnosis about complex anatomical structure in shoulder joint. We carried out how the changes of anatomical rotations in shoulder joint could bring certain diagnosis effects on MRI images for various shoulder humerus positions; Neutral position, Internal rotation position and External rotation position. In addition, we prepared an aid tool in oder to maintain the right posture of a patient. This aid tool was made by adapting Modeling Design Program. By virtue of this aid, we obtained the following result. Shoulder MR Arthrography by the External rotation position for anatomical structure diagnosis was the most suitable in diagnostic evaluations of important anatomical structures in shoulder joint such as Biceps tendon, Supera-spiatus tendon, Sub-scapularis tendon, Labrum and Sub-acromial space. 자기공명 관절 조영술(Shoulder MR Arthrography)는 견관절(Shoulder joint) 의 복잡한 해부학적 구조에 관하여 정확한 영상평가를 진단하기 위해 시행 한다. 우리는 어께 상완골(Shoulder humerus) 의 다양한 position인 Neutral position, Internal rotation position, External rotation position 에 관하여 Shoulder joint의 해부학 적인 회전 변화가견관절 자기공명(Shoulder MRI) 영상에 어떤 진단적 결과를 가져오는가에 대해 상호 비교 하였다. 또한 환자의 정확한 촬영자세 유지를 위해 촬영 보조기구를 만들었다. 이 보조기구는 Modeling 설계에 의해 우리가 직접 제작한 자기공명 견관절 조영술 보조 기구이다. 이 보조기구를 사용하여 촬영한 결과 다음과 같은 결론을 도출해 내었다. External rotation position 에 의한 Shoulder MRArthrography 검사가 Shoulder joint의 중요 해부학적 구조인 Biceps tendon, Supera-spiatus tendon, Sub-scapularis tendon, Labrum, Sub-acromial space 의 해부학적 평가에서 제일 적합하다는 진단적 평가를 얻었다.

        Shoulder MRArthography 검사 시 보조기구를 이용한 해부학적 보조기구를 이용한 해부학적

        자기공명 관절 조영술(Shoulder MR Arthrography)는 견관절(Shoulder joint) 의 복잡한 해부학적 구조에 관하여 정확한 영상평가를 진단하기 위해 시행 한다. 우리는 어께 상완골(Shoulder humerus) 의 다양한 position인 Neutralposition, Internal rotation position, External rotation position 에 관하여 Shoulder joint의 해부학 적인 회전 변화가견관절 자기공명(Shoulder MRI) 영상에 어떤 진단적 결과를 가져오는가에 대해 상호 비교 하였다. 또한 환자의 정확한 촬영자세 유지를 위해 촬영 보조기구를 만들었다. 이 보조기구는 Modeling 설계에 의해 우리가 직접 제작한 자기공명 견관절 조영술 보조 기구이다. 이 보조기구를 사용하여 촬영한 결과 다음과 같은 결론을 도출해 내었다.         Supine 자세에서 Shoulder oblique촬영시 Tube angle 변화에 따른 Shoulder joint에 대한 고찰

          어깨의 병변을 관찰하는 단순 방사선촬영법들 중 견관절을 보기 위한 대표적인 검사법으로 Shoulder oblique법(Grashey법)이 있다. 이 검사는 견관절의 정면상을 볼 수 있는 검사로 누운 자세에서 검사할 때는 몸의 한쪽 면을 거상시키는 자세를 취하여 검사를 시행한다. 하지만 몸이 불편하거나 수술직후의 환자들의 경우에는 이러한 자세를 취하는 것이 힘들기 때문에 견관절이 잘 묘출되는 영상을 얻기가 어렵다. 이러한 어려움을 개선하면서 견관절이 잘 나타나는 영상을 얻을 수 있는 방법을 모색해 보고자 누운 자세에서 몸의 한쪽면을 거상시키는 자세를 취하는 대신에 누운 자세를 취한상태에서 X-선관의 각도를 Medio-Lateral(몸의 중심에서 바깥쪽으로) 방향으로 변화시켜 영상을 획득하였다.<BR>  총 15명(남: 9, 여: 6)을 대상으로 검사를 시행하였으며 획득한 영상은 Picture archiving and communication system(PACS) 모니터에서 전문가 4명(정형외과 전문의 2명, 방사선과 전문의 2명)이 각각 독립적으로 평가하여 점수를 부여하였다. 그 결과, X-선관의 각도를 Medio-Lateral로 변화시킨 정도가 25°인 영상에서는 상완골두와 견갑골이 겹쳐보였으며, 30°와 35°인 영상에서는 상완골두와 견갑골이 분리되어 보였고, 40°인 영상에서는 상완골두와 견갑골이 분리되어 보였으나 영상이 많이 왜곡되어 나타났다. 영상의 평가점수는 각각 25° 1.20±0.414(평균±표준편차), 30° 2.47±0.516, 35° 2.87±0.352, 40° 2.27±0.458로 나타났고 통계적으로 유의한 차이를 나타냈다(p<0.05). 왜곡의 정도는 30°에서 적게 나타났고 40°에서 심하게 나타났다.<BR>  누운 자세를 취한상태에서 X선관의 각도를 Medio-Lateral 방향으로 30~35° 주는 경우 가장 좋은 견관절 영상을 얻을 수 있었으나 이 방법이 Shoulder Oblique법보다 상완골두의 왜곡을 더 발생시키는 것도 확인할 수 있었다. 하지만 이 방법은 몸이 불편한 환자들의 견관절을 검사하는데 매우 유용하게 사용할 수 있고 특히 응급환자의 경우 환자의 체위변화 없이 사용가능하다는 장점이 있다. 뿐만 아니라 체위변화가 없는 이 촬영법은 환자의 추적 검사 시에도 최적의 동일 X선관의 각도를 적용하기 때문에 이전검사와 거의 같은 영상을 얻을 수 있어서 영상의 비교에도 유용할 것으로 사료된다.   There is a standard shoulder oblique method(Grashey method) available to view the shoulder joint. This method projects AP view of the shoulder joint so that the Humerus head"s subuxation or joint degeneration can be easily visualized. However, in this view, the patients, with supine or sitting or erect position, have to keep their body obliquely. Whereas, the patients who are not well or operated, usually feel very uncomfortable to keep their body in this position and hence, we need other persons" help and much efforts will be needed to get the good quality shoulder joint view. Therefore, we thought of examining a method which shows the joint well by angling the tube to Medio-Lateral direction and without keeping the patients" one side upward in supine position. For this study, total 15 subjects with no history of neurological or psychiatric illness, were recruited for examinations. They consisted of 9 males and 6 females. Statistic group analysis was performed with ANOVA test.<BR>  Scores of the evaluation of the experts were 1.10±0.54 at 25°, 2.50±0.50 at 30°, 2.85±0.36 at 35° and 2.33±0.47 at 40°, respectively, and they were significant(p<0.05, Table 1). Joint space of the Humerus head and Scapula were well distinguished at 35°, 30° and 40° with the almost same score. However, the degree of distortion at 40° was more severe than that at 30°. Ultimately, 30-35° views were shown to yield good quality shoulder oblique images.<BR>  In conclusion, this method may be very useful for the patients who are uncomfortable and for the emergency patients. In order to get similar or comparable view, the same X-tube angle is recommended to be used before and after the operation. Therefore, we hope that this new angled method seems to be efficient.

        프로야구선수의 견관절, 슬관절, 요부관절 등속성 근력 비교

        The purpose of this study was to provide data needed for comparison by position, scientific training, and exercise prescription by position in evaluation of shoulder joint, knee joint, and waist joint of professional baseball players. First, in body composition, there was difference in body mass index(BMI) and muscle mass among fielders(p<.05). Second, there was difference in peak torque and peak torque % body weight of external rotators at 90˚/sec shoulder joint among fielders(p<.05). There was difference in peak torque of external rotators at 300˚/sec shoulder joint among fielders(p<.05). Third, there was difference in torque % body weight of flexors and extensors at left knee joint among fielders(p<.05). In addition, there was difference in peak torque and peak torque % body weight of extensors at 60˚/sec right knee joint among pitchers(p<.05). We came to the conclusion that fielder showed higher than pitcher at external rotators of shoulder joint. the pitchers are need to set a pitch form for set up systematic a data of shoulder joint and improve the quantity of pitcher muscles and continuous icing operation after an pitching would an positive effect on improve the return of exhaustion of shoulder joint. It is necessary to changing recognition about number of pitches of field coach than number of pitches restriction and inning of pitches restriction.

        정구선수의 견관절 가동범위와 상·하지 근력 및 근지구력이 경기력에 미치는 영향

        The purpose of this study is to investigate the effects of movable range of shoulder joint and muscular strength and muscular endurance of upper·lower limbs on athletic performance by comparing and analyzing tennis players in business team and college team. 11 tennis players with no specific injury participated in this study as research subjects. Also, this study classified them into 2 groups of business team(n=5) and college team(n=6), and measured movable range of shoulder joint, maximum muscular strength of shoulder joint, and muscular function of knee joint. According to the result of measuring the movable range of shoulder joint, it was identified that external rotational angle was significantly high for the business team and college team(p<.05). According to the result of measuring the maximum muscular strength of shoulder joint, both extensor and flexor muscle were significantly high for the business team and college team (p<.05). To conclude, factors that can make positive effect on athletic performance of business team and college team were movable range of shoulder joint and maximum muscular strength of shoulder joint, and there was difference between business team and college team for muscular function of knee joint but it is not considered as a factor that makes effect on athletic performance.

        고교야구선수의 투수와 야수 견관절 등속성 최대회전력 및 근력비율 비교

        The purpose of this study was to provide date of injury precaution of high school baseball player and decision of pitching motion through above these experiment, difference of muscular strength ratio in shoulder joint isokinetic that external and internal rotator of high school baseball players by pitcher and fielder. This research was being progressed by 30 baseball players of high school that is registered on KBA and divided into two groups which were 15 fielders and 15 pitchers. There were playing for more than 7 years in the field and the experiment of this study was to test muscular strength, muscular power and muscular strength ratio of internal and external rotators at isokinetic shoulder joint. First, It is significantly difference peak toque and peak toque % body weight of external rotators at 90˚/sec isokinetic shoulder joint of pitcher and fielder(p<.01). Second, It is significantly difference peak toque and peak toque % body weight of external rotators at 180˚/sec isokinetic shoulder joint of pitcher and fielder(p<.05). It is significantly difference muscular power at internal rotators of pitcher and fielder(p<.05). It is no significantly difference muscular strength ratio of external rotators and internal rotators at 180˚/sec isokinetic shoulder joint of pitcher and fielder.         상완골 간부 골절의 교합성 골수강 내 금속정을 이용한 치료 결과 및 술 후 견관절 기능 평가

        목 적: 교합성 골수강 내 금속정을 이용한 상완골 간부 골절의 치료 결과 및 특히 단점으로지적되어 온 술 후 견관절 기능 감소를 평가하고, 영향을 미치는 요소를 평가하여 술 후 견관절 기능 향상에 도움을 주고자 한다. 대상 및 방법: 1999년 4월부터 2004년 10월까지 상완골 간부 골절로 교합성 골수강 내 금속정을 이용하여 치료받은 35예를 대상으로 하였고, 술 전 견관절 통증 유무, 퇴행성 소견 유무를 평가하고, 술 후 1년에 American Shoulder Elbow Surgery Scale (ASES)방법을 이용하여 견관절 기능과의 상관관계를 평가하였다. 결과: 33예에서 유합을 보였으며 평균 유합 시기는 12주였고, 합병증으로 불유합 2예, 감염 1예, 윈위 고정 나사못 해리 1예, 요골신경 마비 1예, 액와신경 마비 1예가 있었으며, 견관절 기능은 술 후 3개월에 ASES 점수 평균 78.2이었고, 술 후 12개월에서는 89.6이었다. 술 후 견관절 기능에 영향을 미칠 것으로 가정한 요소 중 술 전 견관절 통증유무, 금속정 근위 돌출은 통계적으로 유의한 상관관계를 보였다. 결 론: 교합성 골수강 내 금속정을 이용한 상완골 간부 골절의 치료는 다른 치료 방법과 비슷한 양호한 결과를 보였으며, 견관절 기능장해는 금속정 돌출이 없도록 유의하여 수술한다면, 최종 추시에서는 견관절 기능 감소 없이 비교적 안전하며, 유용한 치료방법으로 판단된다. Purpose: To evaluate the post-operative functional reduction of the shoulder joint and the impacting factors to post-operative shoulder joint function in interlocking IM nailing treatment of humeral shaft fracture. Materials and Methods: From April 1999 to August 2004, 35 patients (35 cases) whom admitted to hospital for humeral shaft fracture and treated using interlocking intramedullary nail were followed up for more than 1 year. 1 year post-operative shoulder joint function were evaluated using American Shoulder Elbow Surgery Scale (ASES). Pre-operative shoulder joint pain, radiologically degenerative change and extent of nail protrusion were evaluated, and each factor was correlated with function of the shoulder joint. Results: 33 cases out of 35 cases showed union and average union period was 12 weeks. Complications consisted of 2 cases of nonunion, 1 case of infection, 1 case of loosening of distal fixing screw, 1 case of radial nerve palsy and 1 case of axillary nerve palsy. Shoulder joint function 3 months after operation :mean ASES score 78.2, 12 months after operation :mean ASES score 89.6. Pre-operative shoulder joint pain and nail protrusion showed to be statistically related to shoulder joint function. Conclusion: If the operation leaves no protrusion of intramedullary nail, it can be concluded to be relatively safe and effective.
