        Development of a protocol for guidance in the pediatric nursing practicum in South Korea: a methodology study

        Purpose: Practical training in pediatric nursing gives students the opportunity to apply nursing knowledge to children in a realistic clinical context. Clinical practice faculty (CPF) and clinical nursing instructor (CNI) have played important roles in the pediatric nursing practicum. This study was conducted to develop a protocol to guide clinical practicum in pediatric nursing. Methods: A service design methodology was employed between August 2020 and May 2021 at four universities and four hospitals in South Korea. The participants were five CPFs, five CNIs, five nursing college graduates, and 60 nursing students. The service design process had four phases: discovery, definition, development, and delivery. Data were collected through self-report questionnaires, in-depth interviews, and observations. The data were analyzed using content analysis and descriptive statistics. Results: The participants reported needs for providing concrete guidance and explanation, nursing practice experience, and a link between school and the clinical field. A protocol was developed to fulfill the participants' needs. The protocol comprised detailed information, teaching methodology, and partnership to guide students in the pediatric nursing practicum. Conclusion: The protocol developed in this study can be used to provide guidance for students' clinical practice in the field of pediatric nursing.

        아동병동 간호사의 경력에 따른 아동 통증지식에 대한 연구

        Purpose : The purpose of this study was to identify pediatric nurses’s knowledge related to children's pain and pediatric nurses’ career including educational experience and to examine whether there is a difference of their knowledge according to their career. Method : Subjects of this study was 115 pediatric nurses of one university hospital who understood purpose of this study and accepted to participate in this study. Subjects responded questionnaire by self report. The data were gathered for 7 days during the period from October 20 to October 27, 2003. The pediatric nurses' knowledge and attitude survey regarding pain developed by Manworren(2001) was used to identify the knowledge related to children’s pain of pediatric nurses. Nurses’ career consisted of level of education, presently working ward, duration of experience at clinic, pediatric clinic and presently working ward and educational experience related to children's pain during undergraduate courses, ward orientation and continuing education. Educational experience regarding children’s pain was measured by whether they had education related to children’s pain during undergraduate courses, ward orientation and continuing education or not and their perception on the contents of education was measured by 5 point Likert type scale. SPSS Windows was used to analyze the data. Scores of the knowledge regarding children’s pain and career including educational experience of pediatric nurses were presented as mean and standard deviation. Mean percentage of correct answers responded by the subjects was presented under the category of introduction, assessment, pharmacological and nonpharmacological intervention for children’s pain. The difference of scores of the knowledge regarding children’s pain according to nurses’ career was analyzed using t-test, ANOVA and Tukey as a post hoc. Statistical significance was accepted at the level of p<.05. Result : Pediatric nurses had deficient knowledge related to children’s pain on the whole and did not trust pain complaint and underestimated the pain. They were short of concrete practical knowledge about pain assessment and pharmacological and nonpharmacological intervention for children’s pain. They were excessively concerned with side effect and addiction of analgesic drugs and had a negative view point on nonpharmacological pain intervention. About 50% of the subjects received education regarding children’s pain while their undergraduate courses. The number of nurses who received education regarding children’s pain while their undergraduate courses was found to be greater among the recently graduated nurses. In ward orientation and continuing education, only less than 10% of the nurses received education regarding children’s pain. Nurses perceived their experienced educational content deficient; experienced educational content consisted of pain introduction, assessment and pharmacological and nonpharmacological intervention for children’s pain while their undergraduate course, ward orientation and continuing education. There were no significant differences of the knowledge regarding children’s pain of pediatric nurses according to level of education, duration of experience at clinic, pediatric clinic and presently working ward. Scores of the knowledge regarding children’s pain of nurses at neonatal ward were significantly greater than those of nurses at pediatric intensive care unit. Scores of the knowledge regarding children’s pain of pediatric nurses who received education during their undergraduate course were significantly greater than those who did not receive it during their undergraduate courses. Also scores of the knowledge regarding children’s pain of pediatric nurses who received one kind of educational experience were significantly greater than those who had no educational experience. Conclusion : Pediatric nurses had deficient knowledge of children's pain and underestimated the pain of children. Also they were short of concrete practical knowledge on pain assessment and intervention for children’s pain.

        아동간호실습 운영형태에 따른 간호학생의 임상실습수행도 및 실습만족도

        Purpose: This study was conducted to explore clinical practice performance according to type of practice in students in pediatric nursing and the relationship with satisfaction of clinical practice. Methods: The subjects consisted of 531 nursing students (307 at a pediatric ward and 224 at a nursery) from 5 colleges of nursing. Data collected from March to June 2012 were analyzed using descriptive statistics, t-test, AVONA and Pearson correlation coefficients. Results: Clinical practice performance on direct nursing activity was significantly different according to the course on pediatric nursing, grade point average, and satisfaction of nursing major. Observation practice was different according to type of college, the course on pediatric nursing, grade point average, duration of practice and clinical educator in the pediatric ward. In the nursery, direct nursing activity was significantly different according to the type of college, the course on pediatric nursing, hospital type, and duration of practice. Observation practice was different according to type of college, satisfaction of a nursing major, and the clinical educator. In addition, significant correlations were found between clinical practice performance and satisfaction of clinical practice. Conclusion: Our research can assist effective pediatric nursing practice planning for nursing students.

        간호대학생의 아동간호학 실습 후 아동간호사와 간호직전망 인식에 대한 융합적 연구

        본 연구는 간호대학생들의 아동간호학 실습 후 아동간호사와 간호직전망에 대한 인식을 조사하여 교육프로그램의 기초자료를 얻고자, 간호학과 3학년 143명을 대상으로 시행된 서술적 융합조사연구이다. 수집된 자료를 SPSS 21.0으로 기술통계, 평균비교, 상관관계 및 위계적 회귀분석을 시행하였다. 아동간호사에 대한 전문적인식이 5점 만점에 4.09±0.61 점으로 전통적인식이나 사회적인식보다 높았고, 간호직전망은 5점 만점에 3.31±0.65점이었다. 성별, 연령, 간호학만족도, 실습병원유형에 따라 아동간호사 인식에 부분적으로 유의한 차이가 있었고, 간호학만족도가 보통인 대상자의 간호직전망 인식이 높았다(p<.002). 아동간호사에 대한 인식의 하위영역과 간호직전망 인식 간 양의 상관관계가 있었고(r=.287~.510, p<.01), 위계적 다중회귀분석 결과, 아동간호사에 대한 전통적인식, 사회적인식이 간호직전망에 영향을 미치는 요인이었고, 변수들의 설명력은 29.2%이었다(p<.001). 결론적으로 아동간호사에 대한 전통적 및 사회적 인식을 높임으로써 간호직전망 에 대한 인식을 높일 필요가 있다. This descriptive convergence research is to get basic data for educational program by investigating awareness on pediatric nurses and nursing prospects after pediatric clinical practicum among 143 junior nursing students. Using SPSS/Win 21.0, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression were carried out. The awareness of subjects on pediatric nurses was 4.09±0.61 on a scale of 5, which was higher than the traditional and social ones, and nursing prospects’ was 3.31±0.65 on a scale of 5. There’re significant differences partially in awareness on pediatric nurses according to sex, age, satisfaction in nursing and practium hospitals. Awareness on nursing prospects of those who had average satisfaction in nursing was higher than others(p<.002). There was a positive correlation between subvaribles of awareness on pediatric nurses and nursing prospects(r=.287~.510, p<.01), Traditional and social awareness were the significant factors on nursing prospects in hierarchical multiple regression, and the explannation rate was 29.2%(p<.001). It’s needed to raise awareness on nursing prospects by improving traditional and social awareness on pediatric nurses.

        Levels of Partnership between Nurses and Parents of Hospitalized Children and the Quality of Pediatric Nursing Care as Perceived by Nurses

        Purpose: This study investigated the relationship of the partnership between nurses and parents of hospitalized children with the quality of pediatric nursing care as perceived by pediatric nurses. Methods: A cross-sectional descriptive study was conducted among 90 nurses. The data collected were analyzed using the independent t-test and the Pearson correlation coefficient. Results: The level of partnership between nurses and parents showed significant differences by nurses' age (t=2.22, p=.029) and marital status (t=2.54, p=.013). The quality of pediatric nursing care was found to significantly differ by the nurses' age (t=2.62, p=.013), marital status (t=4.17, p<.001), whether nurses had children (t=2.53, p=.020), pediatric ward work experience (t=2.43, p=.020), and total work experience (t=2.51, p=.016). The level of partnership between nurses and parents was positively correlated with the quality of pediatric nursing care (r=.48, p<.001). Conclusion: To improve the quality of nursing care for hospitalized children and their parents, it is important to consider the specific characteristics of pediatric nurses, whose clinical skills should be improved.

        Pediatric nurses' grit and nursing intention during the COVID-19 pandemic: Mediating and moderating effects of mindset and psychological collectivism

        Purpose: This study aimed to identify the mediating and moderating effects of mindset and psychological collectivism, respectively, on the relationship between grit and nursing intention for children with emerging infectious diseases, including coronavirus disease 2019 (COVID-19), among pediatric nurses. Methods: We conducted a cross-sectional descriptive study with 230 pediatric nurses. Results: Grit directly impacted mindset (β=.27, p<.001) and nursing intention (β=.20, p=.001). The direct effect of mindset on nursing intension (β=.28, p<.001) and the indirect effect of grit on nursing intension via mindset (β=.08, p<.001) were significant. Accordingly, mindset mediated the effect of grit on nursing intention. The effects of grit and psychological collectivism (β=.19, p=.003), respectively, on nursing intention were significant. However, the interaction between grit and psychological collectivism was not significant. Thus, psychological collectivism did not moderate the effect of grit on nursing intention. Conclusion: The findings demonstrate the importance of pediatric nurses' grit and mindset on their intended care for patients in critical situations. Cultivating a gritty culture and developing interventions to enhance nurses' personality traits associated with their performance will be crucial, and such measures are especially salient for pediatric nurses to face the new adjustments required in the era of COVID-19.

      • A Pediatric Oncology Diverse Learning Course on Caring for nursing students’ reflection

        Aim(s): This study aimed to determine if a pediatric oncology diverse learning course could enhance nursing students’ theoretical knowledge and understanding of clinical care for children with leukemia and their families. Method(s): This was a qualitative retrospective study. Data were collected in July 2016. Participants were college nursing students (N = 33) enrolled in a diverse learning course in pediatric oncology, which was taught between November 2014 and January 2015 at a science and technology university in Taiwan. The three-part course included reading picture books, participating in role-playing, and watching a cancer-related family movie. Focus group discussions occurred during each part. Students maintained written reflections during the course, which were submitted to an e-learning website. Reflections were analyzed using content analysis. Result(s): Students’ reflections resulted in five main themes: (1) Pediatric oncology nursing knowledge and skills learned; (2) Understanding children with leukemia and their families suffering; (3) Empathy parents’ and children’s reactions to the effects of leukemia; (4) Nursing skills necessary for caring for children with leukemia; and (5) Expect future role as a pediatric oncology nurse. Conclusion(s): The pediatric oncology simulation course employed multiple teaching strategies. The improvements in student cognition, empathy, and understanding of care requirements of children with leukemia and emotional needs of families as pediatric oncology nurses suggest the teaching objectives of enhancing student learning were achieved. The knowledge and skills acquired could assist students in developing professional competencies and values in nursing.

        Development of a virtual reality program in South Korea for the measurement of vital signs in children: a methodological study

        Purpose: We developed a virtual reality (VR) program for use in pediatric nursing practicums to help nursing students learn to measure vital signs in children. Methods: The analysis, design, development, implementation, and evaluation model was employed between July 2021 and December 2021 at a university in South Korea. In the analysis phase, in-depth interviews were conducted with four nursing students, two nursing school graduates, and four experts. The topic and contents of the VR program were settled in the design phase. The VR program was developed and subsequently used and evaluated by 20 nursing students and four experts. Results: The contents of the VR program for pediatric nursing practicums included the measurement of vital signs in a newborn baby and a young child, as well as an evaluation system. The mean score for the nursing students' satisfaction with practice was 4.02 out of 5 points. The mean scores for overall satisfaction with the VR program were 4.15 and 4.79 out of 5 points for nursing students and experts, respectively. Conclusion: The VR program developed in this study allows nursing students to practice measuring vital signs in children, thus improving the students' clinical performance in pediatric nursing.

        문제중심 학습방법 경험에 대한 간호학생의 인식유형 - 아동간호학 학습경험을 중심으로 -

        Purpose: This research was done to provide fundamental data to improve learning methods in Pediatric nursing and meet the needs of the students in actual nursing by analyzing nurse student experiences with problem-based learning in Pediatric nursing. Method: Using the 31 Q-samples selected, 20 nursing students from J college were selected as p-samples. The students were personally interviewed in January or February 2008. Result: The result of the study showed 3 types. The first type was the “negative resister”, who failed to adapt to the problem-based learning and resists negatively. The second type was the “active receiver”, who participated in the process of the problem-based learning and received it actively. The third type was the “passive accepters”, who accepted problem-based learning but worried because they were familiar only with traditional learning. Conclusions: In this study, problem-based learning was used for classes in the science of pediatric nursing. The findings indicate that preparation for learning and details should be considered when developing and using modules for pediatric nursing. Further study on the development of problem-based learning modules is also indicated.

        아동병동 간호사와 입원아동 어머니가 인식한 간호의 질 비교

        Purpose: This study aimed to compare the quality of nursing care as perceived by registered nurses and mothers of hospitalized children in South Korea. Methods: This was a descriptive study that recruited 70 mothers of hospitalized children and 70 nurses in pediatric units in university hospitals as participants. The quality of pediatric nursing care was measured using importance and performance scores for 19 items describing various elements of nursing care. The collected data were analyzed using descriptive statistics and the independent t-test. Results: The hospitalized children mothers’mean importance scores were significantly higher than those of the nurses (t=2.94, p=.004). However, there were no significant differences in the mean performance scores of nurses and mothers (t=0.91, p=.363) or between nurses’and mothers’perceptions of quality of nursing care, with the exception of a significant difference for the quality of explanations (t=2.78, p=.006). The quality of explanations was assessed more positively by nurses than by mothers. Conclusion: This study suggests that when developing strategies to improve the quality of nursing care in pediatric wards, ensuring that pediatric nurses provide detailed explanations should be considered as a way to improve the quality of nursing care in pediatric units.
