        건강포인트제도의 효과와 참여 예측 인자 : 인천 만성질환관리사업의 고혈압 환자를 중심으로

        This study describes the hypertensive patients characteristics associated with the health incentive point program, and develops and analyzes a simple predictive model of participation in the program. Based on the Incheon Chronic Disease Management System(iCDMS), a model program of community partnership for hypertensive or diabetic patients detection and follow-up since 2005 in Incheon metropolitan city, a cross-sectional design was used in this study. An effective 10.844 adults sample was divided into groups according to participation in the health incentive point program and continuing treatment, and individual and health characteristics among groups were compared. Furthermore, the predictors associated with participation in the program were identified by the logistic regression analysis. After the health incentive point program in iCDMS was introduced, the number of hypertensive patients participation in the program increased 23.9 times which is vastly high given the various programs were provided. There were statistically significant differences among the groups: age(0.000), treatment compliance(0.000), and blood pressure control at the last measurement(0.000), in particular, between participation group(GroupⅠ, =246) and non-participation group(GroupⅢ, =10,408). Furthermore, age over 60 years(OR: 0.33), treatment compliance(OR: 3.49∼3.78) and blood pressure controls(OR: 2.13∼2.30) were statistically significant predictors associated with participation in the program, based on the logistic regression analysis with GroupⅠ and GroupⅢ. To increase participation in the health incentive point program, variables such as age, treatment compliance and blood pressure controls are more concerned. And, high-risk patients and family members need targeted health incentive programs.

        건강 인센티브 프로그램이 고혈압 환자의 혈압 조절에 미치는 영향

        만성질환관리사업에서 건강 인센티브 프로그램에 따른 고혈압 환자의 특성을 분석하고, 고혈압 환자의 혈압 조절에 미치는 영향을 고찰하였다. 인천광역시의 웹기반 만성질환등록관리체계(iCDMS)에 등록관리된 고혈압 환자의 진료정보에서 고혈압 환자의 성, 연령, 건강포인트 적립 수준, 의원 방문 횟수 등의 변수가 혈압 조절이 이루어지고 있는 환자((n=17,278)와 그렇지 않은 환자(n=5,020)에서 통계적으로 유의한 차이를 나타냈으며, 성, 연령, 건강포인 트 적립 수준이 고혈압 환자의 혈압 조절에 통계적으로 유의하게 영향을 주었다. 본 연구는 고혈압 환자의 자가 관리 역량을 강화하고 지속치료를 유도하기 위한 다양한 만성질환관리사업의 개발에 활용될 수 있으며, 특히 향후 만성질 환관리사업에서 건강포인트제도와 유사한 인센티브 시스템의 체계적인 개발에 유용한 자료가 될 것으로 기대된다. This study described hypertensive patients characteristics associated with a health incentive program, and analyzed the effects of the health incentive program on blood pressure control. Based on the Incheon Chronic Disease Management System (iCDMS), a community partnership program for hypertensive patients, a cross-sectional design was used. There were statistically significant differences between the control group (n= 17,278) and the experimental group (n=5,020) in gender, age, points earned, and number of hospital visits. Furthermore, being male, being over 60 years of age, and points earned were statistically significant factors associated with blood pressure control. To increase the effectiveness of chronic diseases management systems, various health incentive programs will be developed and introduced.

          Purpose: This study is to analyse the community-based hypertension control program of public health centers in Daegu and Gyeongbuk. Method: It analyzed purpose, goal, management activity and strategy, educational media and educational materials for hypertension control from 17 public health centers in Daegu and Gyeongbuk that had been doing program on hypertension, through a questionnaire. Result: The purposes of hypertension control program of public health centers are complication prevention 88.2% , enhancement of self care ability 88.2%, enhancement of quality of life 82.4%, reduction of mortality 52.9%, in order. The goals of hypertension control program are reduction of risk factors and mobility 82.4%, improving of recognition 76.5%, maintenance of normal blood pressure, 64.7%, detection of patient, 58.8%, monitoring 41.2%, in order. The control activities are registration and follow up 88.2% , early detective program 76.5%, risk factor control 47.0% and staff training 29.4% , in order. The control strategies are lecture 88.2%, mass media and web 41.2%, in order The educational medias are reflet, pocket book, panel and video for adolescent and middle aged women. The educational medias are reflet, panel, pocket book and video for elderly and public population, in order. The educational contents are drinking , smoking, exercise for adolescent, and nutrition, exercise, stress for middle aged women and complication, nutrition, exercise for elderly and nutrition, exercise, complication for public population, in order. Conclusion: Educational materials should be developed focusing on prevention and observance of risk factors through daily living.
