        An Integrated Review of Evidence-based Healthcare Design for Healing Environments

        In preparation for the aging and aged society, long-term care facilities should develop healing environments to provide the elderly with physical, psychological, and social support for their quality of life. Therefore, this study aimed to suggest evidence-based guidelines by identifying factors affecting healing environments of long-term care facilities. An integrated review of the literature was conducted to discuss the concept of the healing environment and its attributes (evidence-based design, aging-friendly universal design, and evidence-based healthcare design). Factors for evidence-based design guidelines of long-term care facilities were identified from the literature including the model of the Evidence-based Design Wheel and the framework of the Evidence-based Design Process. Considering the general, physical, psychological, and social characteristics of the elderly, essential elements of the healing environment were classified into three categories such as environmental control, positive distraction, and social support. Factors affecting the healing environment for designing long-term care facilities, which also improve healthcare outcomes, were expanded including (1) human elements for individual needs and psychosocial support, (2) spatial elements for positive distraction, and (3) behavioral elements for controlling environment. As a result, the evidence-based guidelines for transformational changes in healthcare design were proposed in this study. To prepare for an aging and aged society, the guidelines can be served as a framework for evidence-based design interventions to improve healthcare outcomes of the elderly.

        Decision support system for evidence-based precautionary ecological planning

        While using precedent knowledge is critical especially for ecological planning to minimize permanent adverse effect on the natural environment, there have been only few cases that apply case-based design or decision support system to ecological designs. A possible reason might be because the design context varies dramatically for different sites with diverse variables to consider. Despite the difficulties, ecological wisdom – a relatively newly suggested ecological planning and design strategy – suggests using evidence-based precedents for new ecological designs to avoid ecologically harmful designs. For the past 5 years, there has been ongoing research on ecological wisdom by notable scholars; however, there remains a gap between the concept and its applicability as a planning or design methodology even though one of the goals of ecological wisdom is to be made practical. In this research, we propose a computational framework for the implementation of evidence-based Precautionary Ecological Planning Assistant and apply to a design case. To accomplish this goal, we found design patterns from vernacular garden designs in relation to environmental factors. By identifying statistically significant ecological design patterns, we generated a list of rules that were then incorporated into a decision support system alerting the users that the performed space planning strategy has never been observed before in previous ecological designs. From this research process, we were able to confirm that the proposed computational framework supports ecological design process to be more data-driven and precautionary providing second thoughts to planners when planning for unconfirmed ecological design.

        병원 진료 안내 디지털 사이니지 개선 효과 연구 - 근거기반디자인(EBD) 과정을 적용하여

        Purpose: Evidence-based design is the process of making design decisions based on reliable research to achieve the best results, so it is important to accumulate reliable evidence through research. Therefore, the purpose of this study is to present new evidence by applying the evidence-based design process to improve the digital signage for each room spaces in hospitals and verifying its effectiveness. Method: Through the 8-step process of evidence-based design, improved digital signage for each room spaces are installed. It conducted surveys and statistical analysis to prove hypotheses by linking design and research. Results: The hypotheses established in the study are: 1) improvement in the readability of digital signage is correlated with patient satisfaction; 2) Improving the intuitiveness of digital signage correlates with patient satisfaction; 3) Improving the sufficiency of digital signage correlates with patient satisfaction. As a result of satisfaction analysis and correlation analysis, all of the above hypotheses were proven. Implications: Although the hospital sign system is the element that patients rely on most intuitively in the long journey of reception, waiting, examination, and treatment, there is insufficient evidence to refer to or apply it when designing. It is necessary to expand future research to expand the evidence that can be applied to hospital sign design.

        치유환경 연구문헌 고찰 및 근거중심디자인(Evidence-Based Design) 활성화에 관한 연구 -미국 치유환경 연구사례를 중심으로-

        This paper assesses the evidence of the built environments` impact on patients` and staff`s well-being. It also introduces evidence-based design (EBD) and discusses barriers and solutions for promoting EBD. EBD is an informed approach to design by improving design decisions based on best available knowledge. Designers should provide psychologically and physically supportive environments to promote occupants` well-being. There is increasing evidence that a poorly designed environment elicits negative feelings and stress. Better design will lead to better outcomes especially when it applied to a vulnerable population such as patients, children, and the elderly. Due to high demands of promoting patients` health, healthcare providers and professional designers have been searching for evidence to create healing environments. EBD has been well received in healthcare facilities design and is expanding to other types of environmental design such as educational and commercial design. Design firms should consider EBD as a value-added component of design to meet current and future challenges. It is important for designers to realize that engaging in EBD is not a rejection of creativity, but a means by which to scrutinize their design projects. With critical literature reviews on healing environments, there is sufficient evidence that well-designed environments can promote patients` and medical staff`s health outcomes.

      • 근거기반 공공공간 디자인 교육사례 연구

        The purpose of this study is to introduce an evidence-based capstone design project and to examine students’ perception on the evidence-based design method. The project topic was public space design which lasted for thirty-six weeks. Students were asked to conduct a literature review, observation, and interview to collect design evidence. Students’ survey results regarding the three types of research showed that conducting research during the programming phase enhances overall design quality. Students presented positive perception on the three research methods and the interview was the most frequently mentioned method that improved design quality. It is needed to integrate research-based design methods in the interior design studio curriculum and to develop systematic teaching methods for evidence-based design.

        다중목적 최적화 알고리즘을 활용한 주거복합커뮤니티공간 디자인 연구

        (Background and Purpose) Recently, a residential complex community development that provides medium/high-density housing and living SOC using the blighted low-rise residential area in the downtown area is being developed. Architecturally, the high-density integration of complex programs is essential to simultaneously satisfying different requirement for environmental performance and each use. As the existing black box design method has a limit to its response, this design focuses on the design method using a multi objective optimization algorithm and considers the impact of the adjacent context including the own building within the site. We propose a design for a residential complex community space that reflects the optimal architectural composition that satisfies the different design conditions that conflict with each other. (Method) For design example, it is set as a residential complex community space including residential, commercial, and community facilities that respond to community problems for blighted area located on a slope site in the Jeongneung Redevelopment Area in Seoul. Based on the relationship between the adjacent context and the group of buildings on the site, the design is drawn based on the site plan and shape that simultaneously satisfy the environmental optimization based on insolation and the view and perception requirements for each use. To do this, it utilizes Octopus, a multi objective optimization add-on based on Rhino Grasshopper. The optimal solution that meets the complex requirements of the example is selected and applied to the final design through the review of performance and planning for multiple solution groups derived from Octopus. (Results) The derived design includes not only site plan and shape of the most importantly, but also urban and architectural aspects that respond to various requirements for the community. First, from an urban point of view, the social integration of the community was strengthened by providing functions required to respond to the physical and socio-economic problems of the community cut off from the surrounding. From the architectural point of view, through the calculation results derived from the optimization, a number of complex conditions including the environmental performance of complex including local facilities were satisfied. In addition, from a technical point of view, evidence-based design based on objectivity that goes beyond the limits of subjective arbitrariness inherent in the existing architectural design process was applied. (Conclusions) This design confirmed the applicability and potential use of an optimization genetic algorithm linked with a new digital tool in architectural design by deriving alternatives to the set examples. The effectiveness of multi objective optimization to overcome the limitations of traditional design was confirmed. Furthermore, it has meaning in expanding the versatility of practical grafting of advanced new technology and architectural design.

      • 근거기반디자인을 활용한 스마트 홈서비스 디자인 방법론에 관한 연구

        The medicine field maintains that evidence-based medicine be utilized to enhance heath care efficiency(EBM). This enabled design in healthcare environments to establish qualified design strategy to guarantee the effect of treatment(EBD). As human life is much affected by living environment, it should be very carefully planned. Therefore, for planning housing environment, it is very appropriate to utilize decision-making (EBD) based on qualitative and quantitative data. This research herein aims to derive measures that enable evidence-based design to be applied to smart-home service. Detailed research problems are as follows; (1) Define concepts of evidence-based design, (2) Summarize design methodology of evidence-based design, (3) Summarize information collection methods for evidence suggestion, (4) Collect housing environment information for smart-home service, (5) Discuss smart-home service process development measures. In conclusion, 4 steps of evidence-based design methodology (i.e. information collection, research goal set-up, strategic analysis on research hypothesis-based information, and conclusion inducement) were used to develop a housing smart service design process. This will enable us to suggest service for one-to-one customized, pleasant housing environment and healthy building management.

        과학기술혁신정책의 과학화: 근거기반 정책 설계에 대한 인식 연구

        이 연구는 과학기술혁신정책의 과학화를 위한 지원에서 활용되는 정책적 근거에 대하여, 정책담당자와 정책 이해관계자인 현장연구자의 인식을 비교・분석했다. 분석결과 세 가지를 발견했다. 첫째, 과학기술혁신정책의 과학화를 위한 근거의 활용에 있어 차이가 나타났다. 경험적근거의 활용이 가장 높고, 과학적 근거, 휴리스틱 근거 순으로 활용되고 있었다. 둘째, 과학기술혁신정책의 목적에 따라 주로 활용되는 근거 유형이 다르게 나타났다. 과학기술패권 경쟁을지원하는데 있어서는 과학적 근거, 디지털 전환은 휴리스틱 근거, 사회적 난제 해결은 경험적근거의 영향이 상대적으로 크게 나타났다. 마지막으로 정책집행집단인 정책담당자가 정책대상집단인 현장연구자보다 근거의 활용 수준을 높게 인식하는 것으로 나타나, 행위자-관찰자 비대칭성을 확인하였다. This study identified the perception of government officials and researchers on evidence-based policy for science of science, technology and innovation[STI] policy. Specifically, 127 policy managers and 223 field researchers were surveyed on the use of evidence. First, policy managers recognized the level of utilization of all scientific, empirical, and heuristic grounds higher than field researchers. Regarding the obstacle to the use of evidence, the policy manager recognized that it was difficult to collect evidence, which was a situational factor, while the field researcher recognized the lack of competence of the person in charge, which was a characteristic factor of the actor. Through this, it was confirmed that the actor-observer asymmetry exists in the perception of evidence-based policies in the field of science and technology innovation. Second, it was confirmed that empirical grounds, scientific grounds, and heuristic grounds were highly usable in the order of science and technology innovation policies. Policy managers use empirical evidence the most, use scientific evidence, and use heuristic evidence relatively low. Field researchers were similar in that the use of heuristic evidence was relatively low, but field researchers recognized that empirical and scientific evidence were used to a similar extent, unlike policy makers.

      • Pervasive misclassification and misconception of study designs in Asian dermatology journals listed in science citation index-expanded

        Background: Misclassification of study designs of journals can hinder the readers from assessing the strengths and weaknesses of the study and evaluating the applicability of the study in the real-world setting. However, it seems that it is common for authors to neglect to classify the study design. Objectives: We aimed to evaluate the accuracy of the classification of study designs and examine the common errors. Methods: Among Asian dermatology journals listed in the science citation index expanded from January 2018 to December 2018, we investigated discrepancies between author-reported and actual study designs. Design Algorithm for Medical Literature on Intervention (DAMI) was used to determine the actual study design. Results: Of the 177 papers analyzed, only 72 articles (40.7%) revealed their study design and among them, 23 articles (32.0%) showed discrepancies between the author-reported and the actual study designs. Case-control studies were the most commonly misclassified study designs by authors. Conclusion: There were considerable differences between the author-reported study design and the actual study design in Asian dermatology journals. Proper classification of study designs by the authors is essential to strengthen evidence-based medicine.
