      • Development of a New Annual Dose Evaluation System Using On-Site Network Data

        A new annual dose evaluation system called E-DOSE has been developed. The system is based on the methodology of the previous version, K-DOSE60, which uses the dose evaluation methods of the International Commission on Radiological Protection (ICRP-60). However, E-DOSE is coded in ABAP to be compatible with the KHNP’s enterprise resource planning (ERP) system, SAP. This allows E-DOSE to use the real-time data from SAP, which minimizes the need for user intervention. The socio-environmental data, which was previously managed by the staff of each plant sites, can now managed in the system in a centralized manner. This is a significant improvement over the previous system, as it reduces the risk of errors and makes it easier to track and manage data. The system also automatically generates the reports required by regulations. EDOSE is expected to minimize the occurrence of human errors in preparing and managing the input data. This is because the system uses the data from SAP, which is less prone to errors than manually entered data. Additionally, the automatic generation of reports reduces the risk of errors in report preparation. E-DOSE is also expected to improve work efficiency. This is because the system automates many of the tasks involved in annual dose evaluation, such as data entry, calculation, and report generation. Overall, E-DOSE is a significant improvement over the previous annual dose evaluation system. It is more efficient, accurate, and user-friendly.

        비 상업용 3차원 치료계획시스템인 Plunc의 임상적용 가능성에 대한 연구

        목 적 : 비 상업용 3차원 컴퓨터치료계획시스템인 Plunc의 구축 사례를 소개하고 이의 임상적용 가능성에 대하여 검증하고자 한다. 대 상 및 방법 : 미국 Nortth Caolina 대학에서 개발된 3차원 치료계획시스템인 Plunc 의 소스코드를 제공받아, PC용 Unix인 Linux 환경의 Pentium Pro 200MHz(128MB RAM, Millennium VGA)에서 설치하였다 . 본과의 6MV 광자선(Siemens MXE 6740)에 대한 출력인자, 최대산란비, 최대산란인자, 쐐기의 모양 및 감쇄인자 등의 빔데이터를 입력한 후, 일반적인 치료조건인 10cm 깊이의 회전중심점에서의 심부선량백분율, 선량측면도, oblique 입사빔 및 공기간격 하에서의 선량계산 결과를 물팬톰에서의 측정치와 비교, 분석하였다. 결 과 : Plunc는 원래 CT 영상데이터를 이용한 모의치료기로써 개발되어, 빔 설계가 매우 편리하도록 사용자 인터페이스가 구성되어 있으며, BEV,DRR 및 영상합성 등의 기능을 갖추고 있다. 선량계산은 10초 정도가 소요되는 3차원 선량분포나 선량체적히스토그람을 제외하고는 거의 실시간으로 실행되었다. 결 론 : Plunc의 광자선량계산의 정밀도는 일반적인 치료조건하에서 약 2-5% 내외의 오차로써, 측정치에 대한 보정에 근거한 알고리즘을 사용하는 일반 치료계획시스템과 비슷한 수준이라 사료된다. 현재로서는 전자선에 대한 선량계산이 불가능하기 때문에 완전한 형태의 치료계획시스템이 되기 위해서는 향후, 전자선에 대한 계산모듈의 개발과 광자선 선량계산 또한 보다 정밀한 선량계산이 가능한 컨벌류션 방법과 같은 3차원 선량계산모듈의 개발도 필요하다. Plunc는 상업용 3차원 치료계획 시스템의 사용이 현실적으로 어려운 여건의 병원에서 2차원 치료계획시스템과 상호 보완적으로 사용한다면 2차원 치료계획시스템이 갖는 많은 제약을 극복할 수 있을 것으로 사료된다. Purpose : The objective of this study is to introduce our installation of a non-commercial 3D planning system, Plunc and confirm it's clinical applicability in various treatment situations. Materials and Methods : We obtained source codes of Plunc, offered by University of North Carolina and installed them on a Pentium Pro 200MHz (128MB RAM, Millenium VGA) with Linux operating system. To examine accuracy of dose distributions calculated by Plunc, we input beam data of 6MV photon of our linear accelerator(Siemens MXE 6740) including tissue- maximum ratio, scatter-maximum ratio, attenuation coefficients and shapes of wedge filters. After then, we compared values of dose distributions(Percent depth dose; PDD, dose profiles with and without wedge filters, oblique incident beam, and dose distributions under air-gap) calculated by Plunc with measured values. Results : Plunc operated in almost real time except spending about 10 seconds in full volume dose distribution and dose-volume histogram(DVH) on the PC described above. As compared with measurements for irradiations of 90-cm SSD and 10-cm depth isocenter, the PDD curves calculated by Plunc did not exceed 1% of inaccuracies except buildup region. For dose profiles with and without wedge filter, the calculated ones are accurate within 2% except low -dose region outside irradiations where Plunc showed 5% of dose reduction. For the oblique incident beam, it showed a good agreement except low dose region below 30 % of isocenter dose. In the case of dose distribution under air-gap, there was 5% errors of the central-axis dose. Conclusion : By comparing photon dose calculations using the Plunc with measurements, we confirmed that Plunc showed acceptable accuracies about 2-5% in typical treatment situations, which was comparable to commercial planning systems using correction-based algorithms. Plunc does not have a function for electron beam planning up to the present. However, it is possible to implement electron dose calculation modules or more accurate photon dose calculation into the Plunc system. Plunc is shown to be useful to clear many limitations of 2D planning systems in clinics where a commercial 3D planning system is not available.

        Discrepancies in Dose-volume Histograms Generated from Different Treatment Planning Systems

        Background: We analyzed changes in the doses, structure volumes, and dose-volume histograms (DVHs) when data were transferred from one commercial treatment planning system (TPS) to another commercial TPS. Materials and Methods: A total of 22 volumetric modulated arc therapy (VMAT) plans for nasopharyngeal cancer were generated with the Eclipse system using 6-MV photon beams. The computed tomography (CT) images, dose distributions, and structure information, including the planning target volume (PTV) and organs at risk (OARs), were transferred from the Eclipse to the MRIdian system in digital imaging and communications in medicine (DICOM) format. Thereafter, DVHs of the OARs and PTVs were generated in the MRIdian system. The structure volumes, dose distributions, and DVHs were compared between the MRIdian and Eclipse systems. Results and Discussion: The dose differences between the two systems were negligible (average matching ratio for every voxel with a 0.1% dose difference criterion = $100.0{\pm}0.0%$). However, the structure volumes significantly differed between the MRIdian and Eclipse systems (volume differences of $743.21{\pm}461.91%$ for the optic chiasm and $8.98{\pm}1.98%$ for the PTV). Compared to the Eclipse system, the MRIdian system generally overestimated the structure volumes (all, p < 0.001). The DVHs that were plotted using the relative structure volumes exhibited small differences between the MRIdian and Eclipse systems. In contrast, the DVHs that were plotted using the absolute structure volumes showed large differences between the two TPSs. Conclusion: DVH interpretation between two TPSs should be performed using DVHs plotted with the absolute dose and absolute volume, rather than the relative values.

        The Development of a Korean Drug Dosing Database

        Objectives: This report describes the development process of a drug dosing database for ethical drugs approved by the Korea Food & Drug Administration (KFDA). The goal of this study was to develop a computerized system that supports physicians’prescribing decisions, particularly in regards to medication dosing. Methods: The advisory committee, comprised of doctors,pharmacists, and nurses from the Seoul National University Bundang Hospital, pharmacists familiar with drug databases,KFDA officials, and software developers from the BIT Computer Co. Ltd. analyzed approved KFDA drug dosing information,defined the fields and properties of the information structure, and designed a management program used to enter dosing information. The management program was developed using a web based system that allows multiple researchers to input drug dosing information in an organized manner. The whole process was improved by adding additional input fields and eliminating the unnecessary existing fields used when the dosing information was entered, resulting in an improved field structure. Results: A total of 16,994 drugs sold in the Korean market in July 2009, excluding the exclusion criteria (e.g., radioactivity drugs, X-ray contrast medium), usage and dosing information were made into a database. Conclusions: The drug dosing database was successfully developed and the dosing information for new drugs can be continually maintained through the management mode. This database will be used to develop the drug utilization review standards and to provide appropriate dosing information.

        디지털 방사선의학에서의 조사선량 설정과 인지에 대한 실태 - 대구 경북지역을 중심으로

          디지털 시스템이 가지는 장점인 영상판 검출기의 반응 범위(dynamic range)가 상당히 넓다는 것은 필름/증감지 시스템보다 더 높은 수준의 노광 관용도를 갖기에 재촬영이 줄어들고 영상관리에 효율적이지만, 조사조건의 설정범위가 상당히 넓어 필름/증감지 시스템의 엄격한 조사조건보다 더 많은 조사선량이 환자에게 노출될 수도 있다. 본 연구는 디지털 시스템 하에서 일반촬영 시 방사선사 개인 별 조사선량에 대한 인식과 행위실태를 파악하여 환자피폭선량을 감소시킬 수 있는 방안을 마련하고, 방사선 선량관리의 중요성을 새로이 인식하고자 하였다. 디지털 시스템 하에서 근무 중인 방사선사의 조사조건 설정과 환자피폭선량 인지 실태를 파악해 본 결과 환자의 체형이나 상태, 촬영부위에 따라 최적의 조사선량을 적용하기 보다는 영상의 농도와 업무의 편의성에 따라 조사조건이 설정되고 있었다. 디지털 시스템이 도입되며 검출기의 반응 범위가 필름/스크린 시스템보다 넓어짐에 따라 조사조건 설정에 대해 관심이 소홀한 경향이 있었다. 따라서 디지털 방사선 시스템 하에서 환자 피폭선량의 감소를 위해 최적의 조사조건으로 영상을 얻어야 할 것이다. 또한 조사선량을 최소로 하고 환자 피폭선량을 줄이기 위해 업무 습관과 인식을 새로이 할 필요성이 있고, 지속적인 관심과 주기적인 교육 및 점검, 다양한 교육 기회제공 등이 필요하다고 본다.   Digital imaging for general radiography has many advantages over the film/screen systems, including a wider dynamic range and the ability to manipulate the images produced. The wider range means that acceptable images may be acquired at a range of dose levels, and therefore repeat exposures can be reduced.<BR>  Digital imaging can result in the over use of radiation, however, because there is a tendency for images to be acquired at too high a dose.<BR>  We investigated the actual exposure dose conditions on general radiography and a questionnaire survey was conducted with radiotechnologist at medical institutions using digital radiology system.<BR>  As a results, the dose of exposure was not controlled with patient"s figure and dose optimization but was cmtrolled by worker s convenience and image quality. Radio-technologists often set up the exposure dose regardless of patient figure and body part to be examined.<BR>  Many organizations, such as the International Commission on Radiological Protection, recommend to keep the dose as low as possible. In addition, they strongly recommend to keep the optimal but minimal dosage by proper training programs and constant quality control, including frequent patient dose evaluations and education.

        KERMA-Based Radiation Dose Management System for Real-Time Patient Dose Measurement

        Because systems that reduce radiation exposure during diagnostic procedures must be developed, significant time and financial resources have been invested in constructing radiation dose management systems. In the present study, the characteristics of an existing ionization-based system were compared to those of a system based on the kinetic energy released per unit mass (KERMA). Furthermore, the feasibility of using the KERMA-based system for patient radiation dose management was verified. The ionization-based system corrected the effects resulting from radiation parameter perturbations in general radiography whereas the KERMA-based system did not. Because of this difference, the KERMA-based radiation dose management system might overestimate the patient’s radiation dose due to changes in the radiation conditions. Therefore, if a correction factor describing the correlation between the systems is applied to resolve this issue, then a radiation dose management system can be developed that will enable real-time measurement of the patient’s radiation exposure and acquisition of diagnostic images.

        고선량률 강내조사선원을 이용한 근접조사선량계획전산화 개발

        목적 : 고선량률의 Ir-192 선원을 이용한 근접조사의 모의촬영 영상을 개인컴퓨터(PC)에 입력하여 해부학적 영상에 선량분포를 구현하고 히스토그램, 선량-용적히스토그램 및 3차원 선량분포를 전산화하였다. 대상 및 방법 : 선량전산화에 이용된 선원은 원격근접조사장치(Buchler 3K, 독일)의 Co-60 대체선원으로 한국원자력 연구소와 공동으로 개발한 Ir-192이다. 선원 모양에 의존하는 선량분포의 비등방성은 선원을 미소 분할하여 구한 선량과 선원 중심에서 측방 기준점의 공중선량을 기준으로 규격화한 값을 이용하였다. 선원 주위의 조직선량은 선원 중심에서 측방으로 실측된 조직감쇠와 산란에 의한 보정계수와 에너지에 따른 공기 저지능에 대한 조직의 저지능 비로 공중-조직선량 변환계수를 적용하고 기준점에 대해 규격화한 선량률표를 검색하여 얻도록 하였다. 선량계획 전산화 과정에 모의촬영 영상입력, 선원입력과 선원의 축면 결정과 해부학적 영상을 이용한 선량분포와 점선량, 히스토그램 및 선량-용적 히스토그램을 구현하였다. 결과 : 저자들이 개발한 근접조사 선량계획시스템에는 선원모의촬영 영상을 스켄하여 비트맵 파일로 저장하고, 좌표원점과 확대율을 정해, 선원위치를 결정하고 선량분포와 선량분석 프로그램을 포함한 선량전산화를 구현하였다. 실험에 이용된 Ir-192 선원의 조직내 선량은 공중선량율과 조직에 의한 감쇠 및 산란에 의한 실험식을 이용하였다. 선원 중심에서 축상의 거리와 축에서 떨어진 거리에 따른 선량률표에서 행렬 검색하여 얻도록 하였다. 근접조사선량계획은 선원좌표 입력과 선원의 축면(principal plane)을 결정하여 선원이 포함된 평면상의 선량을 구현하였으며, 시뮬레이션 영상인 관상면과 시상면에 선량분포를 구현하였다. 선량-히스토그램에 의한 선량분포 분석은 임의의 해부학적 영상면 위에 커서가 놓인 위치의 선량 스켓치로 얻었다. 임상에 필요한 선량분석은 선원의 축에서 면의깊이를 이동하여 선량분포를 구할 수 있게 하였으며, 선량-용적 히스토그램과 3차원 선량분포를 구현하였다. 결론 : 고선량률 Ir-192를 이용하여 근접조사선량계획을 전산화하였으며, 선량분포의 분석에는 해부학적 영상의 선량분포와 선량-히스토그램, 선량-용적히스토그램을 구현하였으며, 선량분포의 면을 임의 선택할 수 있고 3차원 선량분포를 포함한 선량계획시스템을 준비하였다. Purpose : A PC based brachytherapy planning system was developed to display dose distributions on simulation images by 2D isodose curve including the dose profiles, dose-volume histogram and 30 dose distributions. Materials and Methods : Brachytherapy dose planning software was developed especially for the Ir-192 source, which had been developed by KAERI as a substitute for the Co-60 source. The dose computation was achieved by searching for a pre-computed dose matrix which was tabulated as a function of radial and axial distance from a source. In the computation process, the effects of the tissue scattering correction factor and anisotropic dose distributions were included. The computed dose distributions were displayed in 2D film image including the profile dose, 3D isodose curves with wire frame forms and dosevolume histogram. Results : The brachytherapy dose plan was initiated by obtaining source positions on the principal plane of the source axis. The dose distributions in tissue were computed on a 200 times 200(m㎡) plane on which the source axis was located at the center of the plane. The point doses along the longitudinal axis of the source were 4.5~9.0% smaller than those on the radial axis of the plane, due to the anisotropy created by the cylindrical shape of the source. When compared to manual calculation, the point doses showed 1~5% discrepancies from the benchmarking plan. The 2D dose distributions of different planes were matched to the same administered isodose level in order to analyze the shape of the optimized dose level. The accumulated dose-volume histogram, displayed as a function of the percentage volume of administered minimum dose level, was used to guide the volume analysis. Conclusion : This study evaluated the developed computerized dose planning system of brachytherapy. The dose distribution was displayed on the coronal, sagittal and axial planes with the dose histogram. The accumulated DVH and 3D dose distributions provided by the developed system may be useful tools for dose analysis in comparison with orthogonal dose planning.

        A Study on Electron Dose Distribution of Cones for Intraoperative Radiation Therapy

        전자선을 이용한 수술중 방사선치료를 위해서는 수술중에 드러내 놓은 종양에 고선량의 방사선을 조사하되 주변 정상 조직을 보존하기 위하여 cone 장치를 개발할 필요가 있으며, 임상에 필요한 자료를 얻는 것 뿐만아니라 콜리메이터에 의한 적정 X-선 창 크기도 정할 필요가 있다. holder와 cone으로 이루어진 알루미늄제 결합형 cone장치를 개발하였다. 광자선의 SSD 100cm에서 조사면 크기가 직경 4~9cm이면서 lcm씩 차이가 있는 28cm 길이의 원통형 cone을 만들였으며, holder는 cone의 직경이 7cm이상인 것과 미만인을 것을 접속시키기 위해 따로 두 개를 만들었다. holder의 측면에는 조사부위를 관찰하기 위한 거울과 조명등을 삽입할 수 있는 개구부를 두었다. cone에 의한 조사면 크기와 콜리메이터에 의한 X선 창의 크기의 여러 가지 결합에 대하여 수중의 전자선의 깊이선량분포독선 및 측방선량분포곡선, 선량분포를 1차원 물팬톰 장치로 조종하는 p-형 실리콘 검출기로 측정하였다. 출력계수도 p-형 실리콘 검출기로 수중에서 측정하였다. 전자선의 에너지와 cone의 크기의 결합이 일정할지라도 콜리메이터에 의한 X-선 창의 크기는 표면선량 및 최대신량점의 깊이, 80% 선량점의 깊이, 측방선량분포, 출력계수에 영향을 미쳤다. 그중, 출력계수의 변화가 가장 현저하였다. 예로서 9 MeV 전자선의 출력계수는 0.637과 1.549의 범위에 있었다. 콜리메이터에 의한 X-선 창의 크기는 수술중 전자선치료용 cone 장치의 벽으로 향하는 산란 전자의 양에 영향을 미치고, cone장치에서 다시 산란된 전자는 출력계수 뿐만 아니라 선량분포도 바꿀 것으로 생각된다. 따라서 수술중 전자선치료용 cone장치에 대한 선량분포 측정은 임상에서 선량의 불확정도를 최소화하기 위해 필수적이다.능적으로 평가되어야 하므로 앞으로 이에 대한 연구가 요구된다.수 있었다. 즉, $a^{*}$ / $b^{*}$ 의 비값이 1이상 흑은 1에 가까울 때는 미숙기, 0.8 부근 일때는 적숙기, 0.8보다 작아 질수록 적숙기에서 과숙기로 점차 옮아감을 알 수 있었다.지해줄 수 있는 문헌들이 충분히 고찰되지 못하였고, 이론적배경 또한 횡문화 이론과의 관련성이 적었다. 또한 횡 문화 연구에 기초가 되는 연구대상자의 사회 인구학적 특성과 역사적 배경은 잘 나타났으나, 이론적 연구와 경험적 연구 간에 괴리가 있었다. 3. 표본추출방법은 문화에 기반을 둔 대상자를 선정한다는 점에서 한계성 이 있었다. 4. 방법론적 이유로는 대상자와의 면담시간이 구체적으로 기술되지 않았으며, 고유한 언어를 통역하는 과정에서 의미론적 문제에 대한 고려가 부족하였다. 면접과 기록과정에서 보면 자료의 기록과정과 분류 및 분석과정이 명시되어 있지 않았다. 참여관찰과 면접방법을 사용시 이에 대한 자세한 기술이 되어 있지 않았다. 5. 연구결과의 적용 및 이에 대한 논의는 상당히 제한되어 있었는데, 수편의 연구만이 방법론 문제점과 앞으로의 연구분야에 대한 전망을 제시하였으며, 특이한 것은 어 떤 연구자도 이른 개발을 위한 적용 및 임상실무적 차원에서 간호에 대한 제언을 하지 않았다.유모델변수들은 유입-유출 자료들로부터 평가할 수 있으며, 이를 위해서 본 논문에서는 Gauss-Newton 방법을 이용한 Bard 알고리즘을 사용하였다. 서울 구로구 시흥동 산사태 발생 지역의 산사면에 대하여 개발된 모델을 적용하여 예제 해석을 수행함으로써, 지하수 흐름 모델이 산사태 For intraoperative radiation therapy using electron beams, a cone system to deliver a large dose to the tumor during surgical operation and to save the surrounding normal tissue should be developed and dosimetry for the cone system is necessary to find proper X-ray collimator setting as well as to get useful data for clinical use. We developed a docking type of a cone system consisting of two parts made of aluminum: holder and cone. The cones which range from 4cm to 9cm with 1cm step at 100cm SSD of photon beam are 28cm long circular tubular cylinders. The system has two 26cm long holders: one for the cones larger than or equal to 7cm diamter and another for the smaller ones than 7cm. On the side of the holder is an aperture for insertion of a lamp and mirror to observe treatment field. Depth dose curve. dose profile and output factor at dept of dose maximum. and dose distribution in water for each cone size were measured with a p-type silicone detector controlled by a linear scanner for several extra opening of X-ray collimators. For a combination of electron energy and cone size, the opening of the X-ray collimator was caused to the surface dose, depths of dose maximum and 80%, dose profile and output factor. The variation of the output factor was the most remarkable. The output factors of 9MeV electron, as an example, range from 0.637 to 1.549. The opening of X-ray collimators would cause the quantity of scattered electrons coming to the IORT cone system. which in turn would change the dose distribution as well as the output factor. Dosimetry for an IORT cone system is inevitable to minimize uncertainty in the clinical use.

        OBJECTIVES: This report describes the development process of a drug dosing database for ethical drugs approved by the Korea Food & Drug Administration (KFDA). The goal of this study was to develop a computerized system that supports physicians' prescribing decisions, particularly in regards to medication dosing. METHODS: The advisory committee, comprised of doctors, pharmacists, and nurses from the Seoul National University Bundang Hospital, pharmacists familiar with drug databases, KFDA officials, and software developers from the BIT Computer Co. Ltd. analyzed approved KFDA drug dosing information, defined the fields and properties of the information structure, and designed a management program used to enter dosing information. The management program was developed using a web based system that allows multiple researchers to input drug dosing information in an organized manner. The whole process was improved by adding additional input fields and eliminating the unnecessary existing fields used when the dosing information was entered, resulting in an improved field structure. RESULTS: A total of 16,994 drugs sold in the Korean market in July 2009, excluding the exclusion criteria (e.g., radioactivity drugs, X-ray contrast medium), usage and dosing information were made into a database. CONCLUSIONS: The drug dosing database was successfully developed and the dosing information for new drugs can be continually maintained through the management mode. This database will be used to develop the drug utilization review standards and to provide appropriate dosing information.

      • Development of Criteria Evaluation System of Leakage Rate Test for Spent Fuel Dry Storage Systems

        It is very important that the confinement of a spent fuel storage systems is maintained because if the confinement is damaged, the gaseous radioactive material inside the storage cask can leak out and have a radiological impact on the surrounding public. For this reason, leakage rate tests using helium are required for certificate of compliance (CoC) and fabrication inspections of spent fuel storage cask. For transport cask, the allowable leakage rate can be calculated according to the standardized scenario presented by the IAEA. However, for storage cask, the allowable leakage rate is determined by the canister, facility, and site specific information, so it is difficult to establish a standardized leakage rate criterion. Therefore, this study aims to establish a system that can derive system-specific leakage test criteria that can be used for leakage test of actual storage systems. First, the variables that can affect the allowable leakage rate for normal and accident conditions were derived. Unlike transportation systems, for storage systems, the dose from the shielding analysis and the dose from the confinement analysis are summed up to determine whether the dose standard is satisfied, and even the dose from the existing nuclear facilities is summed up during normal operation condition. For this reason, the target dose is used as an input variable when calculating the allowable leakage rate for the storage system. In addition, the main variables are the distance from the boundary of the exclusive area, the number of cask, the inventory of nuclide material in the cask, the free volume, and the internal and external pressure. Utilizing domestic and US NRC guidelines, we derived basic recommended values for the selected variables. The GASPARII computer code that can evaluate the dose to the public under normal operating conditions was utilized. Using the above variables, the allowable leakage rate is calculated and converted to the allowable criteria for helium leakage rate test. The developed system was used to calculate the allowable leakage rate for normal and accident conditions for a hypothetical storage system. The leakage rate criteria calculation system developed in this study can be useful for CoC and fabrication inspections of storage systems in the future, and a GUI-based program will be built for user convenience.
