        작업치료과 학생들의 임상실습 경험에 대한 포커스그룹 인터뷰

        The purpose of this study was to investigate about student’s experiences in occupational therapy through clinical fieldwork, such as the change in perception of occupational therapy, the stress that students experience, and the satisfaction of clinical fieldwork. We conducted a focus group interview with 10 participants to see what they experienced during the clinical fieldwork. The contents of the interview were recorded and conducted, and the meaning was analyzed according to the focus group interview procedure through repeated listening. Components were psychological experience, professional experience, environmental experience, changes of values, and behavioral changes. In psychological experience, he experienced anxiety and burden. Experiential experience was the connection of theory and clinical experience and realization of major knowledge. In the environmental experience, we experienced clinical guidance management system, and the change of values experienced change of perception of occupational therapy and change of perception of disabled person. Finally, behavioral changes have experienced an active response through clinical practice experience. Clinical fieldwork is a basic element that prepares students to become occupational therapists. It affects the role of occupational therapist and perception performance through various experiences in new environment. It is thought that the efforts of the institution are needed. The results of this study will help occupational therapists to direct clinical fieldwork in the right direction in clinical fieldwork.

        간호대학 임상실습강사의 역할 경험

        Purpose: The purpose of this study was to understand the clinical instructors' role experience in college of nursing. Method: Two focus group interviews were held with a total of 12 clinical instructors. All interviews were recorded and transcribed as they were spoken, and the collected data were analyzed using content analysis of Downe-Wamboldt(1992). Results: Three themes and six subthemes were extracted from the analysis: 1) Getting recognition of special roles as a clinical instructor: ‘Recognizing the role of clinical instructor for guiding student's individual practice goals’, ‘Recognizing helping and supporting roles for student's successful clinical practice’, 2) Having difficulty in performing the role of clinical practice instructor: ‘Difficulty from the lack of knowledge and experience as a clinical practice instructor’, ‘Difficulty from the gap between current clinical practice and purpose of clinical nursing practice’, 3) Making efforts to overcome the difficulties as a clinical instructor and accomplishing personal development: ‘Making efforts to acquire personal knowledge and experience’, ‘Making a chance to reflect on and grow up oneself’. Conclusion: Clinical instructors take an important part of clinical nursing education. To improve the quality of clinical nursing education, it is needed role reestablishment and institutional support for clinical instructors on the basis of the understanding of instructors’ experience.

        의료기관 유형별, 진료과별 의료서비스 경험 차이비교: 외래와 입원서비스를 중심으로

        The issue of medical service experience is considered to be more important than the quality of medical service, which is the essential product of medical service. The medical service experience is actually a consumer experience in the process of using the service. Medical service experience is different from patient satisfaction. Medical service experience is objective compared to patient satisfaction. From this point of view, this study compared the difference of experience of medical service according to type of medical institution and clinic centered on outpatient and inpatient services. The results of this study are summarized as follows. First, in outpatient clinics, the hospital has a good experience with respectful care of the doctor and experience of cleanliness of the medical institution. Also, it has been shown that they have good experience in the oriental hospital 's physician authenticity, nurse' s polite response and easy explanation, cleanliness and comfort. Second, outpatient patients have good experience in physician 's counseling, reflection of opinions and privacy protection in internal medicine. Also, it has been shown that they have good experience about comfort and administration service of oriental medicine department. Third, the hospitalized patients had a good experience in administrative services in hospitals and clinics. Fourth, hospitalized patients had good experience with hospitals. The implications of this study are summarized as follows. Efforts to improve patient response during the implementation of health care services are required. In addition, the hospital is required to continuously improve the treatment process, and the attitude of the doctor and the nurse in charge should be continuously improved.

        What is the Clinical Trial?

        No drug is a drug until it is proven safe and effective (safety first-emphasis added). All drugs in these days have been approved by government authority after a series of clinical trials. Today, clinical trials have begun in Korea only recently. Nevertheless Korea is fast becoming a popular destination for multinational clinical trials. Korea Food and Drug Administration has defined that clinical trial is any investigation in human subjects to study effects of investigational products. Clinical trial is a process that must strictly abide by rules, regulations and study plans. Clinical trial of new drug candidate has very high chance of failure and thus is very difficult. Finally, clinical trial is scientific experiment within frame of ethics and respect for human rights. Technical core of this last point is statistical principles of minimizing bias and design of experiment. Final product of clinical trials is statistical inference about effects of investigational products in human body. So, answer to "What is the clinical trial?" is "a statistically valid medical inference- making process for medicine."

        간호관리학실습 교과내용에 대한 중요도와 실습 경험에 대한 간호대학생과 임상현장지도자의 인식 비교 연구

        본 연구의 목적은 간호관리학실습 교과내용의 중요도와 실습 참여 경험에 대한 학생과 임상현장지도자의 인식을 비교·분석해 간호관리학실습 개선의 기초자료를 제공하고자 하였다. 3개 대학 4학년 학생과 임상현장지도자를 대상으로 Mann-Whitney test와 교차분석 등으로 최종 343부 분석하였다. 연구 결과 간호대학생과 임상현장지도자 모두 간호관리학실습의 평가항목 중 ‘의사소통’, ‘대인관계’, ‘책임’, ‘신뢰’, ‘도덕성’을 중요 요소로 인식하였고 간호관리학실습 교과내용 중 가장 중요하게 인식한 상위 10개 항목 중 6개 항목(‘감염관리’, ‘간호기록관리’, ‘환자간호관리’, ‘약품관리’, ‘환자안전’, ‘간호과오’)은 두 그룹이 일치했다. 또한 임상현장지도자는 대인관계 및 관계윤리를 중요하다고 인식한 반면 간호대 학생은 간호사의 ‘법적책임’을 중요하다고 인식했다. 두 그룹간의 실습경험 분석 결과, 간호대학생은 모두 간접 참여 경험이 높았으나 현장지도자는 직접 지도 경험이 높았다. 이와 같은 인식 차이가 확인된 교과내용은 학교와 실습기관의 유기적 파트너쉽 관계를 구축해 차이를 개선하고 간호대학생의 직접 실습 경험을 증대시키는 방안 마련이 필요하다. This study was conducted to provide basic data for improving nursing management practice by comparing and analyzing the perceptions of nursing students and clinical instructors about the importance of the learning contents of nursing management and their practical experience. The final 343 copies were analyzed using the Mann-Whitney test and cross-analysis, targeting 4th-grade students and clinical instructors at three universities. Both nursing students and clinical instructors recognized ‘communication’, ‘interpersonal relationship’, ‘responsibility’, ‘trust’, and ‘morality’ as important factors in the evaluation items of practice. Among the top 10 most importantly recognized items among nursing-management practice learning content, six items (’infection control’, ‘nursing record management’, ‘patient nursing management’, ‘drug management’, ‘patient safety’, and ‘nursing malpractice’) were matched in the two groups. Moreover, clinical instructors recognized that interpersonal relationships and relationship ethics were important, while nursing students chose legal responsibility. As a result of the practical experience analysis between the two groups, nursing students had high experience of indirect participation, but the clinical instructors had a high experience of direct instruction. A plan is needed to reduce the difference by establishing an organic partnership relationship between the university and practice institutions and increase the experience of direct practice of nursing students.

        소아물리치료사의 치료경험 및 전문성에 대한 인식연구 - 포커스그룹인터뷰를 중심으로

        이 연구는 국내 소아물리치료의 역사가 반세기를 넘은 시점에서 치료주체인 소아물리치료사 고유의 치료경험과 삶의 의미를 탐색하여 이들이 전문가로 성장하는 과정을 이해하고, 나아가 소아물리치료사 직무에 대한 이해와 전문성 향상에 기여하려는 의도로 진행하였다. 이를 위해 소아물리치료사의 경력에 따른 치료경험과 인식의 차이가 존재함을 간주하여 포커스그룹인터뷰 방법을 적용하여 연구를 진행하였다. 서울, 경기소재 다양한 근무지에 종사하는, 소아물리치료경험 3년 이상의 치료사 9명을 연구참여자로 선정하여 경력별 2집단으로 분할하여 인터뷰를 실시하고, 수집된 자료를 반복비교 분석한 결과 9개의 상위범주와 19개의 하위범주가 도출되었다. 치료경험 및 전문성 인식은 두 집단에서 비슷한 맥락으로 해석되었으나, 소아물리치료 직업 선택 동기와 이들이 처한 어려움 등에서는 두 집단의 경험이 차이가 있음을 알 수 있었다. 끝으로 참여자의 공통된 치료경험과 경력별 경험이 의미하는 바를 시사점을 중심으로 논의하였다. The purpose of this study was to explore the unique treatment experience and the meaning of life of pediatric physiotherapist to understand their growth process as experts, and to contribute to the understanding of the job of pediatric physiotherapist and the improvement of professionalism, at a time when the domestic history of pediatric physiotherapy has exceeded half century. For this, the research was conducted by using the focus group interview method, considering the differences in treatment experience and the awareness depending on the work experience of pediatric physiotherapist. The subjects of the interview were 9 physiotherapist who worked in Seoul and Gyeonggi area with more than 3 years of experience in pediatric physiotherapy. The interviewees were divided into 2 groups based on their experience. As a result of constant comparison method, 9 high-level and 19 sub-categories were derived from participants by 3 research subjects. The two groups' treatment experiences and the recognition of professionalism were all interpreted in a similar context, but experiences of two groups were differed in the career choice motivation, and the difficulties they faced. Finally, the implications of participant’s common treatment experience and career experience were discussed.

        간호대학생의 성인 중환자실 임상실습 경험에 관한 내용분석

        본 연구의 목적은 간호대학생의 성인 중환자실 임상실습 경험의 의미와 구조를 탐색하고 심층적으로 이해하기 위함이다. 참여자는 S시 일개 대학 4학년에 재학 중인 간호학과 학생 45명을 대상으로 하였다. 자료수집기간은 2015년 6월부터 11월까지였으며, 자료수집을 위하여 개방형 자기보고서식 성찰일지를 작성하였다. 수집된 자료는 Krippendorff(2003)의 내용분석에 따라 분석하였다. 성인 중환자실 임상실습 경험의 의미를 분석한 결과 32개의 주제, 14개의 주제모음, 6개의 범주가 확인되었다. 6개의 범주는 ‘다양한 임상상황 경험’, ‘배움을 통한 성취감’, ‘실습에 대한 아쉬움’, ‘전문화된 간호에 대한 자긍심’, ‘미래 간호사 역할에 대한 두려움’, ‘중환자실 근무환경에 대한 양가감정’ 이 도출되었다. 이러한 결과는 이론교육과 연계성을 강화하여 임상실습의 질을 높이며, 추후 임상실습운영의 중요한 기초자료가 될 것이다. The purpose of this study is to explore and better understand the meaning and structure regarding undergraduate nursing students clinical experiences in an adult intensive care unit. The participants of this study were 45 senior nursing students at a university in S city. Data were collected from June to November 2015 and an open self - report form reflection journal was prepared for data collection. The data were retrieved through nursing student’s daily clinical log including their feelings and thoughts on their clinical experience. Collected information was analyzed through Krippendorff’s(2003)’s content analysis. In present study, we found that 6 categories, 14 subjects, and 32 subjects regarding clinical experiences of nursing students in intensive care units. Six categories are, ‘Experience of various clinical situation’, ‘Achievement through learning’, ‘Regret for practice’, ‘Pride in specialized nursing’, ‘Fear of future nurse role’, and ‘Ambivalence about working environment of intensive care unit’. It is needed to enhance the quality of clinical practice by strengthening linkage between theory and practice, Further studies are required to develop for critical care nursing curriculum for undergraduate students based on evidence based practice and critical thinking.

      • Comparison of Holistic Approach with Progressive Dysarthria according to Clinical Experience : a Multi-Institutional Survey in Korea

        Recently a more holistic approach that includes impairment, activation, and participation is recommended. This study investigated the difference between speech language pathologists (SLPs) ' awareness and preference of the assessment according to the clinical experience after adjustment of the medical center by regional groups. Sixty-two SLPs who work in 51 medical centers have completed the survey, and the data was analyzed via Cochran-Mantel-Haenszel Test. As a result, there is a significant difference between the SLPs surveyed who have less than 5 years experience and SLP's who have more than 5 years experience (p<0.05). Particularly in the preferences of assessment methods such as observation of intelligibility in conversation with SLPs, observation of AAC ability in the both technical ability and interaction, hierarchy of difficult communication situations, observation of interaction in conversation with others, and recording of communication in non-clinical situations. In the awareness of assessment, there are noticeable differences between the two aforementioned groups concerning the importance of assessing a communication partner's abilities (p<0.05).The results showed that the two groups of SLPs have different views regarding the awareness and preferences of assessment with progressive dysarthria according to their related clinical experience. Thus, it is necessary to develop and nurture guidelines with a more holistic approach to progressive dysarthria.

        다양한 진로모색을 위한 치위생학과 학생과 선배 치과위생사의 심층면담

        The aim of this study was to provide practical information regarding career exploration to dental hygiene students. This study interviewed 15 senior dental hygienists (graduates of Y University Dental Hygiene program) who have worked in non-clinical fields for about three months from January 14, 2014 to April 28. The most frequently mentioned pieces of information that the senior dental hygienists wanted to convey to dental hygiene students were as followed. Contrary to popular belief, clinical experience was also required in non-clinical fields as the importance of clinical experience was quite often and widely emphasized and applied in such fields. High academic excellence and clinical performance were expected of junior dental hygienists and the senior dental hygienists expressed their hope for their juniors to pioneer new fields and areas of work that a dental hygienist can do. Most of the senior dental hygienists pointed out that to develop technical tools for career exploration as early as possible, such as during undergraduate years and to think about which career path to take, whether it is clinical or academic were significantly important factors for desirable career outcomes. Based on the results of this study, undergraduate dental hygiene students should be provided with opportunities to consider and explore various career paths during their undergraduate years and to communicate with their seniors for hands-on experience-based advices. This would in turn broaden each student`s perspective beyond their knowledge or thought about the professional filed of dental hygiene. It was found that the majority of the senior dental hygienists agreed that their clinical experience was the source of their self-competence as a dental hygienist. Our study could be utilized as a valuable resource for future dental hygienists who wish to work in non-clinical fields.

        조산원 분만 산모의 분만통증, 분만통제감이분만경험지각에 미치는 영향

        Purpose: This study intends to identify the relations among labor pain and labor agentry of puerperas giving birth in midwifery clinic and perception of delivery experience, and to establish factors influencing on their perception of delivery experience. Methods: Descriptive survey research. Data were collected from puerperas giving birth through vaginal delivery in six midwifery clinics, and they were analyzed through t-test, ANOVA, pearson's correlation coefficients, and stepwise multiple regression. Results: Figures of labor pain of puerperas giving birth in midwifery clinic by stage were 4.12, 6.80 and 8.11 in average in latent, active and transitional stage, respectively, while labor agentry and perception of delivery experience showed upper-middling figures, namely 3.70 and 3.94. It was revealed that labor agentry of puerperas giving birth in midwifery clinic had negative correlation with labor pain in latent stage (r=-0.176, p=0.021). Perception of delivery experience had the same with labor pain in latent stage (r=-0.177, p=0.020) and labor pain in active stage (r=-0.159, p=0.037), whereas perception of delivery experience had positive correlation with labor agentry (r=0.750, p<0.001). In addition, factors influencing on perception of delivery experience of puerperas giving birth in midwifery clinic were labor agentry, educational background and marital satisfaction, accounting for 58.8%. Conclusion: The results suggest that developing and applying nursing intervention program that increases labor agentry so that puerperas giving birth in midwifery clinic may perceive childbirth experience positively.
