        타이치 운동이 노인 말초신경병증 환자의 발목 고유감각과 기능적 이동성에 미치는 영향

        본 연구의 목적은 노인 말초신경병증 환자의 발목 고유감각이 건강한 노인 집단과 어떠한 차이를 가지고 있으며, 타이치 운동 중재가 노인 말초신경병증 환자의 발목 고유감각과 기능적 이동성에 효과가 있는지를 검토하는 것이었다. 말초신경 병증을 가진 노인 10명과 건강한 노인 10명이 피험자로 참여하였다. 발목 고유감각은 Biodex 3 동력계를 이용하여 측정하였다. 기능적 이동성은 6분간 걷기와 TUG 검사를 이용하였다. 말초신경병증을 가진 노인 집단과 건강한 노인 집단의 비교를 위해 발목 고유감각은 세 가지 목표 위치 조건에서 능동적 재생성과 수동적 재생성 오류를 측정하였다. 이와 더불어, 각 집단별로 6분간 걷기 검사와 TUG 검사를 실시하였다. 능동적 재생성과 수동적 재생성 오류, 6분간 걷기 검사, 그리고 TUG 검사의 사전 자료를 확보 한 후 말초신경병증을 가진 실험집단은 주당 3회, 50분 동안 10주간의 타이치 운동 프로그램에 참여하였다. 발목 고유감각의 민감성 역치, 6분간 걷기 검사, 그리고 TUG 검사는 노인 말초신경병증 환자와 건강한 노인간에 차이가 있었으며, 10주간의 타이치 운동은 발목 고유감각의 수동적, 능동적 재생성, 6분간 걷기, 그리고 TUG 검사에서 개선을 보여주었다. 이러한 결과는 타이치 운동이 노인 말초신경병증 환자를 위한 안전하고, 효율적인 운동 중재가 될 수 있음을 시사한다. The aim of this study was to analyze differences in ankle proprioception between the elderly with peripheral neuropathy (PN) and the healthy elderly, and to examine whether a Tai Chi exercise impacts the ankle proprioception and functional mobility of the elderly with PN. Ten elderly subjects with PN and 10 healthy elderly subjects participated in this study. Ankle proprioception was measured using a Biodex 3 dynamometer. Functional mobility was assessed by 6 min walk and TUG tests. To compare the groups of the elderly with PN and the healthy elderly, ankle proprioception was examined by measuring the active and passive reproduction error at 3 target positions. In addition, the 6 min walk test and TUG test were carried out in each group. After obtaining data for active and passive reproduction error, 6 min walk, and TUG tests, the group with PN participated in a Tai Chi exercise program for 10 weeks (3 times per week, 50 min per session). The sensitivity threshold of ankle proprioception, 6 min walk test, and TUG test showed differences between the elderly with PN and the healthy elderly. The ten-week Tai Chi exercise program produced an improvement in active and passive reproduction in ankle proprioception, 6 min walk, and TUG tests. These results suggest that Tai Chi exercise can be a safe and efficient physical intervention for elderly individuals with PN.

        뇌졸중 환자들의 보행능력 평가를 위한 6분보행검사의 사용 : 문헌고찰

        The 6-min walk test (6MWT) are a measurement tool to evaluate a distance that subjects, including commonly elderly and patients with cardiopulmonary and neurological impairments, walks for 6 minutes. Outcome of the 6MWT indicates subject’s level of aerobic fitness to move their body for a longer time, and can be variously utilized to compare the effects of pre- and post-intervention for patients with cardiopulmonary diseases, predict the morbidity and mortality in some diseases, and evaluate physical endurance level of patients with neurological impairments such as stroke, multiple sclerosis, and incomplete ambulatory spinal cord injury. Previous studies have been recognized that the use of a straight 20-m or 30-m walk routes during the 6MWT is probably the best option to achieve the optimal outcome of the measurement. In clinical setting, the 10-min walk test (10MWT) has been frequently used to assess the walking velocity of numerous patients with orthopedic and neurological patients, with simple procedure and easy application; however, the walking velocity from the 10MWT can be overestimated as compared to that of the 6MWT. Accordingly, keep in mind that precaution needs to interpret the findings of the 10MWT for patients with neurological impairment such as stroke. In general, most of the walk tests for identifying walking performance of post-stroke hemiparesis are performed in the walk routes indoor, which contribute to misunderstanding patients’ capacity to walk indoor and outdoor because of the lack of environmental actuality. Therefore, research exploration should be focused on the technical goal of using the 6MWT to reflect the walk performance in various environments afterward rather than report the findings taken under simple environment controlled by their conveniences.

        보행 능력과 환경이 만성 뇌졸중 환자의 단거리 및 장거리 보행속도검사 결과에 미치는 영향

        Background: The method of measuring the walking function of patients with chronic stroke differs depending on patients walking capability and environmental conditions. Objects: This study aimed to demonstrate the influences of walking capacity and environmental conditions on the results of short- and long-distance walk tests in patients with chronic stroke. Methods: Forty patients with chronic stroke volunteered for this study, and allocated to group-1 (〈.4㎧, household walking, n 1 =13), group-2 (.4∼.8㎧, limited community ambulation, n 2 =16), and group-3 (〉.8㎧, community ambulation, n 3 =11) according to their walking capacity. The 10-meter walk test (10MWT) and 6-min walk tests, (6MWT) were used to compare the short- and long-distance walk tests results, which were randomly performed under indoor and outdoor environmental conditions. Results: The comparison of the results obtained under the indoor and outdoor conditions revealed statistically significant differences between the groups in the 6MWT and 10MWT (p〈.05). Post-hoc tests’ results showed significant differences between groups-1 and -2 and between groups-1 and -3 in the 10MWT, and between group-1 and -3 in the 6MWT. Furthermore, in group-2 the 10MWT and 6MWT results significantly differed between the indoor and outdoor conditions, and the values measured under the indoor and outdoor conditions significantly differed between 10MWT and 6MWT (p〈.05). Group-3 showed a significant difference in 10MWT results between the indoor and outdoor conditions (p〈.05). Conclusion: These findings suggest that the results of the short- and long-distance walk tests may differ depending on the walking capacity of patients with chronic stroke and the environmental condition under which the measurement is made, and these effects were greatest for the patients with the limited community ambulation capacity.

        여성노인 당뇨병 환자의 심폐체력 평가를 위한 Tecumseh Step Test의 타당성

        본 연구는 제2형 당뇨병을 가지고 있는 여성노인을 대상으로 최대산소섭취량(VO2max)을 평가하기 위한 Tecumseh Step Test(TST)의 타당도와 유용성을 알아보는데 그 목적이 있다. 제2형 당뇨병으로 진단받은 60대~70대(67.5±7.0세)의 여성노인 64명을 대상으로 신체구성성분, 혈액검사를 실시하고, 모든 대상자는 운동부하검사를 위해 cycle ergometer, 6-min walking test(6-MWT) 및 TST를 측정을 실시하였다. 에르고미터를 이용한 VO2max와 TST의 회복시 심박수, 6-MWT의 이동거리의 상관관계를 알아보고자 피어슨 상관계수와 회귀분석을 통해 다음과 같은 결과를 얻었다. 에르고미터를 통한 VO2max와 TST의 회복기 심박수는 유의한 음의 상관관계(r=-.490, p<.001), 6-MWT의 이동거리와는 양의 상관관계(r=.533, p<.001)를 나타냈었다. 다중회귀분석 결과 6-MWT와 TST는 각각 40%, 39%의 비슷한 설명력을 가지는 것으로 나타났으나, TST검사(p=.002)가 6-MWT(p=.006) 보다 다소 높은 타당도를 나타내었다. 이러한 결과를 통해 볼 때, TST 검사는 당뇨병을 가진 여성노인을 위해 최대산소섭취량을 설명하는 예측인자로 유용성이 있는 것으로 고려된다. The purpose of this study was to investigate validity of the Tecumseh Step Test(TST) for evaluation of cardiorespiratory fitness in elderly women patients with diabetes mellitus. Sixty four elderly women patients with type 2 diabetes mellitus aged 60-79 years volunteered for this study. All the subjects underwent incremental exercise test using a cycle-ergometer to estimate the rate of oxygen consumption(VO<sub>2</sub>), 6-min walking test(6-MWT), and TST. Data were analyzed using correlation coefficients and multiple regress analysis. The results of this study were as follows. The significant negative correlation(r=-.490, p<.001) between VO<sub>2</sub>max and the recovery heart rate of TST was observed, whereas VO<sub>2</sub>max was significantly positively correlated to the distance of 6-MWT(r=.533, p<.001). In regression analysis, both 6-MWT and TST were an important predict variation to VO2max. The recovery of TST(p=.002) rather than the distance of 6-MWT(p=.006) showed a higher test validity for evaluation of cardiorespiratory fitness. These results suggest that TST may be useful to test cardiorespiratory fitness in elderly women patients with type 2 diabetes mellitus.

        남녀 노인의 6분 걷기 검사와 강도별 신체활동량 수준과의 관련성

        The cardiovascular and musculoskeletal systems have received most attention as they are involved with most basic functions of everyday life. Maximal aerobic performance is reduced with age. The 6 minute walk test (6 MWT) is an useful assessment instrument for the exercise capacity of elderly. The impact of the physical activity status on the 6MWT-distance, however, remains unclear in the Korean elderly. The purpose of this study was to investigate whether the magnitude of regular exercise/physical activity is related to 6 MWT distance and to suggest a new evaluation standard for exercise capacity in older adults. Elderly people (206 men, 265 women) in a local community performed an IPAQ-7 questionnaire for the assessment of physical activity and participated in 6 MWT. The distance in 6 MWT was significantly reduced with advancing age in both men and women, consistent with a marked decrease in physical activity over aging. In contrast, physically active elderly individuals (more than 150 −300 min/week) showed a considerably longer distance in 6 MWT than that of less active individuals (less than 150 min/week). In conclusion, it is suggested that older adults who participate in regular physical activity with moderate/vigorous intensity for 150−300 min/week show greater aerobic endurance capacity than that of less active older adults. The innovative standard established in the present study contributes to the assessment of cardio-respiratory fitness level and would be utilized for exercise prescription of elderly individuals.

        Exercise-induced desaturation during a six-minute walk test is associated with poor clinical outcomes in patients with pulmonary arterial hypertension

        Background The six-minute walk test (6MWT) is an established exercise test for patients with pulmonary arterial hypertension (PAH), afording insight into both exercise intolerance and overall prognosis. Despite the widespread application of the 6MWT, the prognostic implications of exercise-induced desaturation (EID) during this test has been inadequately studied in PAH patients. Thus, we evaluated the occurrence of EID and its prognostic signifcance in PAH patients. Methods We analyzed PAH patients in a single-center cohort from April 2016 to March 2021. EID was defned as a reduction in oxygen saturation exceeding 4% from the baseline or to below 90% at any point during the test. Results We analyzed 20 PAH patients in this cohort, primarily consisting of 16 females with an average age of 48.4±13.3 years. Among them, ten exhibited EID. Baseline characteristics, echocardiographic data and right heart catheterization data were similar between the two groups. However, total distance (354.3±124.4 m vs. 485.4±41.4 m, P=0.019) and peak oxygen uptake (12.9±3.2 mL/kg⋅min vs. 16.4±3.6 mL/kg⋅min, P=0.019) were signifcantly lower in the EID group. During the total follow-up duration of 51.9±25.7 months, 17 patients had at least one adverse clini‑ cal event (2 deaths, 1 lung transplantation, and 13 hospital admissions). The presence of EID was associated with poor clinical outcome (hazard ratio=6.099, 95% confdence interval=1.783–20.869, P=0.004). Conclusions During the 6MWT, EID was observed in a half of PAH patients and emerged as a signifcant prognostic marker for adverse clinical events.

        환경조건이 만성 뇌졸중 환자들의 10미터보행검사와 6분보행검사 결과에 미치는 영향

        This study aimed to identify the effects of environmental conditions on the results of the 10-m walk test (10MWT) and 6-min walk test (6MWT) in people with chronic stroke. Thirty subjects were recruited for this study, and were assigned to one of three groups depending on walking velocity: (1) group 1 (G1: <0.4 m/s), (2) group 2 (G2: 0.4-0.8 m/s), and (3) group 3 (G3: >0.8 m/s). Subjects of all groups randomly performed The 10MWT and 6MWT in indoor and outdoor environments. Significant differences were not found for subjects of the G1 and G3 in comparison of indoor and outdoor environmental conditions (p<0.05); however, subjects of the G2 showed statistically significant difference in values of the 10MWT and 6MWT between the environmental conditions (p<0.05). In addition, there was found significant interaction effect between measurement condition (10MWT and 6MWT) and environmental condition (indoor and outdoor) (p<0.05). These findings suggest that the measurement value of walking velocity can be different in environmental conditions, that is, indoor and outdoor conditions. Also, keep in mind that value of walking velocity can be different depending on the use of short-term distance measure (10MWT) or long-term distance measure (6MWT). Further studies need to warrant the findings of this study.
