        Effect of local anesthesia on pulpal blood flow in mechanically stimulated teeth

        에피네프린을 함유한 국소마취제가 치수혈류량에 미치는 영향을 관찰하고, 이 국소마취가 와동 형성에 의해 야기되는 치수의 혈류변화에 미치는 영향을 평가하고자 전신마취된 아홉 마리의 고양이 견치에서 1 : 100,000 에피네프린이 함유된 2% 리도카인 용액으로의 국소 침윤마취 전후 및 와동형성 전후의 치수혈류를 laser Doppler flowmetry (Periflux4001, Perimed Co., Sweden)를 사용하여 측정하고 paired t-test로 통계분석하였다. 상아질 와동의 형성은 치수혈류의 현저한 증가를 초래하였다 (p < 0.05). 에피네트린을 함유한 리도카인의 침윤마취는 치수혈류를 유의하게 감소시켰다 (p < 0.05). 국소마취한 치아에서는 마취도지 않은 치아에 비해 와동형성시 유의하게 적은 치수혈류의 증가를 나타내었다 (p < 0.05). 따라서 본 연구에서는 혈관수축제를 포함한 국소마취가 와동 형성에 의해 초래되는 혈류량의 증가를 효과적으로 억제 할 수 있음을 보여주고 있다. The aims of the study were to evaluate the effect of epinephrine-containing local anesthetics on pulpal blood flow (PBF) and to investigate its effect on cavity preparation-induced PBF change. PBF was recorded using a laser Doppler flowmeter (Perimed Co., Sweden) from canines of nine cats under general anesthesia before and after injection of local anesthetics and after cavity preparation. 2% lidocaine hydrochloride with 1 : 100,000 epinephrine was administered by local infiltration given apical to the mandibular canine at the vestibular area and the same volume of isotonic saline was injected on the contralateral tooth as a control. A round carbide bur was operated at slow speed with isotonic saline flushing to grind spherical cavities with increasing depth through the enamel and into the dentin on both teeth. The obtained data was analyzed with paired t-test. Cavity preparation caused significant increase of PBF (n = 9, p < 0.05). Local infiltration of lidocaine with epinephrine resulted in decreases of PBF (n = 9, p < 0.05), whereas there was no significant change of PBF with the physiologic saline as a control. Cavity preparation on tooth anesthetized with lidocaine with epinephrine caused significantly less increase of PBF than in control tooth (p < 0.05). Therefore, the result of the present study demonstrates that local infiltration of 2% lidocaine with 1 : 100,000 epinephrine effectively reduces PBF increase caused by cavity preparation. 〔J Kor Acad Cons Dent 31(4):257-262, 2006〕

        경추 퇴행성 신경근증의 치료로 시행하는 경추 경추간공 경막외 신경 차단술에 있어서 스테로이드 사용 유무에 따른 임상 결과

        Study Design: A retrospective chart review. Objectives: To evaluate and compare the clinical outcomes of cervical transforaminal epidural block (CTEB) using local anesthetics with or without a steroid for cervical spondylotic radiculopathy (CSR). Summary of Literature Review: The typical mixture for a CTEB is a combination of local anesthetics with a non-particulate steroid. However, there are potential complications related to steroid injections such as steroid-induced osteoporosis, hypothalamus-pituitaryadrenal gland axis suppression, and hyperglycemia. Materials and Methods: From January 2018 to October 2019, 35 patients who underwent CTEB for CSR were enrolled in this study. Cases with arm pain over 4 on a visual analog scale (VAS) were included. In the first 19 cases, a combination of 1 mL of 1% lidocaine and 1 mL of dexamethasone was used (group A), and in the next 16 cases, 1 mL of 1% lidocaine mixed with 1 mL of normal saline was used (group B). Arm pain VAS and the Neck Disability Index (NDI) were obtained perioperatively. Results: Baseline characteristics were not significantly different between the two groups. In both groups, the arm pain VAS significantly decreased at 30 minutes, 2 weeks, and 6 weeks post-injection compared to pre-injection values. However, the arm pain aggravated 12 weeks post-injection. The NDI of both groups significantly improved 6 weeks post-injection compared to pre-injection. The clinical outcomes of arm pain VAS and NDI at 30 minutes, 2 weeks, and 6 weeks post-injection, as well as the amounts of change, were not significantly different between both groups. Conclusions: CTEB for CSR without a steroid improved symptoms by 6 weeks. The degree of improvement was similar to when CTEB was performed with a steroid in terms of VAS and NDI. 연구 계획: 후향적 의무 기록 연구목적: 경추 퇴행성 신경근증에 대해 국소 마취제를 이용한 경추 경추간공 경막외 신경 차단술을 시행하는 데 있어서 스테로이드 혼용 유무에 따른 임상결과의 비교 분석선행 연구문헌의 요약: 일반적인 경추 추간공 경막외 신경 차단술에 이용되는 약제는 국소 마취제와 비입자성 스테로이드이다. 하지만, 스테로이드는 이차성 골다공증, 시상하부-뇌하수체-부신 축 억제 및 고혈당증 등의 잠재적 부작용을 가지고 있다. 대상 및 방법: 2018년 1월부터 2019년 10월까지 경추 퇴행성 신경근증에 대해서 경추 경추간공 경막외 차단술을 시행받은 35예를 대상으로 연구를 진행하였다. 상지의 통증이 VAS 4점 초과인 환자들만 포함하였다. 첫 19예는 1% 리도카인 1 ml와 덱사메타손 1 ml를 혼용하였고(A군), 이후의 16예는 1% 리도카인 1 ml와 1 ml 생리 식염수를 혼용하였다(B군). 상지의 VAS (Visual Analog Scale) 점수와 Neck Disability Index (NDI)를 조사하였다. 결과: 기본 환자 정보는 두 군 사이에 차이가 없었다. VAS 점수는 두 군 모두에서 차단술 30분 후, 2주 후, 6주 후에서 술전에 비해 유의미한 감소를 보였다. 하지만, 두 군 모두 12주 후에는 6주 후에 비해 악화되었다. 차단술 6주 이후 NDI는 술전보다 유의미하게 호전되었다. 차단술 30분 후, 2주 후, 6주 후의 VAS 점수와 NDI는 두 군 사이에 유의미한 차이가 관찰되지 않았다. 결론: 경추 경추간공 경막외 차단술을 경추 퇴행성 신경근증에 시행한 경우 6주까지 호전시켰다. 하지만, 스테로이드 사용 유무는 VAS 점수와 NDI 등의임상적 결과에 영향을 끼치지 못하였다.약칭 제목: 경추 경추간공 신경 차단술 시 스테로이드 사용 유무의 임상적 차이

        리도카인 패치 적용이 당뇨노인환자의 혈당검사 시 통증과 두려움에 미치는 효과

        Purpose: This study was done to examine the effects of lidocaine patch application to decrease pain and fear during blood sugar testing in elderly patients with DM. Methods: The participants were 56 elderly patients admitted to J geriatric hospital in G city, Korea. Of the elderly patient, 27 were assigned to the experimental group and 29 to the control group. Participants in the experimental group applied a lidocaine patch on the fingertip for 30 minutes before a blood sugar test. The control group applied a plaster on the fingertip. Pain was evaluated using a visual analogue scale and fear using the Procedure Behaviour Check List. The results were compared using paired t-test and t-test. Results: The scores for pain and fear were significantly lower in the experimental group compared to the control group. Conclusion: The lidocaine patch was found to be an effective local anesthetic to relieve pain and fear during blood sugar test in elderly patients with DM without any severe adverse events.

        10% 리도카인 분무와 아로마 손 마사지가 혈액투석 동정맥루 천자 시의 통증, 불안, 혈압 및 맥박에 미치는 효과

        Purpose: To determine the effects of 10 % lidocaine spray and aroma hand massage on pain, anxiety, blood pressure, and pulse during arteriovenous (AV) fistula needling in hemodialysis (HD) patients. Methods: This study used a quasi-experimental design. Forty HD patients were assigned to either 10 % lidocaine spray group (n=21) or aroma massage group (n=19). 10 % lidocaine was sprayed 3 times around AV fistula 10 minutes before. Aroma hand massage was performed for 5 minutes with fluids containing 2 % of lavender, peppermint, and geranium concentrate mixture. Pain, anxiety, blood pressure, and pulse were measured during AV fistula needling without any intervention on the first week and during interventions on the second week. Data were analyzed with x2 test, Fisher’s exact test, t-test, paired t-test and MANCOVA. Results: Pain and anxiety were significantly decreased in both the lidocaine spray group and aroma massage group. Aroma hand massage was more effective to reduce pulse during AV fistula needling. Conclusion: The results suggest that 10% lidocaine spray and aroma hand massage may be effective to reduce pain, anxiety, and pulse during AV fistula needling in HD patients.

        기니픽 와우에 대한 리도카인 미세관류의 청각적 효과

        Objectives:Lidocaine is one of the therapeutic trials to treat tinnitus. However, the exact mechanism of the efect of lidocaine remains unclear. The aim of this study was to elucidate the action site of lidocaine in the cochlea by measuring compound action potential (CAP) and transient evoked otoacoustic emission (TEOAE) amplitude in guinea pigs. Method:Artificial perilymph was perfused into the scala tympani of the guinea pig cochlea in the control group, and lidocaine diluted with artificial perilymph was perfused into the scala tympani of the experimental groups. Electrocochleogram (ECoG) and TEOAE were measured in each groups both before and after lidocaine perfusion. Results:Artificial perilymph perfused to the scala tympani of the guinea the guinea pig cochleae produced a dose-dependent increase in CAP threshold, but did not affect TEOAE response. Conclusion:This study revealed that lidocaine perfused into the scala tympani of the guinea pig cochlea affects the CAPthreshold but not the TEOAE amplitude and reproducibility. It means that the localy perfused lidocaine affects the cochlear nerve greater than the outer hair cells. (Korean J Otolaryngol 2004 ;47 :9-14)

        10 Topical Lidocaine과 EMLA 크림을 이용한 고막 국소마취 효과 비교

        Background and Objectives:Minor otologic surgeries, such as tympanic injection, myringotomy and ventilating tube insertion are a common otologic procedure. Effective, safe and comfortable anesthetic method is very useful alternative to lidocaine infiltration anesthesia. This study was designed to assess the efficacy of topical lidocaine as compared with eutectic mixture of local anesthetics (EMLA) for pain relief during minor otologic surgery. Materials and Methods:One hundred eight patients who were between the age of 4 and 71 years were allocated randomly into 4 groups. Group I (n=30) received 1 spary of topical lidocaine;group II (n=34), 2 sprays of topical lidocaine;group III (n=23), 1 cc of EMLA cream;and group IV (n=21), 2 cc of EMLA cream to the ear canal. Groups were randomized to perform one of otologic minor surgery at anesthetic application time of either 10 or 20 minutes respectively. Patients rated pain and satisfaction during surgery on the comfort scale score. Results:All surgeries were successfully without additional anesthesia. At 10 minute application, group I and II showed significantly lower comfort scores of pain compared with group III (p<0.05). Group II showed significantly lower scores than group III at 20 minute application (p<0.05). In lidocaine applied group I and II comfort scores of pain were significantly lower at 20 minute application than 10 minute application (p<0.05). There was one complication of severe vertigo after one hour of procedure in group II. Conclusion:This study demonstrates that 10 minute application of topical lidocaine is more effective for relieving pain associated with minor otologic surgery in office than 10 minute application of EMLA cream and 20 minute application of lidocaine is more effective than 10 minute application.         Effect of Manufacturing and Absorption Promotion of Lidocaine Hydrogel Using Ultrasonic Waves during Cosmetic Procedures

        목적: 본 연구에서는 안티에이징의 대중화에 따른 미용 시술 시 사용되는 마취제품을 보다 효과적으로 사용하기 위해 연구하였다. Lidocaine을 함유한 hydrogel을 초음파로 미용시술 시에 사용한다면 통증완화에 보다 효과적일 것으로 판단되어 hydrogel을 제조하 였다. 방법: 제조한 하이드로겔과 시판 중인 겔을 비교하여 초음파 치료에 사용할 수 있는지 평가하였으며, 초음파 적용가능성에 따 라 피부를 통한 침투와 초음파 한도에 따른 피부 침투에 미치는 영향을 평가하였다. 초음파 처리 시 사용성 및 최적의 초음파 매개 변수를 알아보았다. 결과: 제조한 하이드로겔의 점도측정, 겔화율, 팽윤도, 피부투과실험, HPLC 분석한 결과 모두 적합한 특성을 보였으며, 주파수 1MHz, 순환주기 Low 200, High 50 일 때가 가장 피부투과율이 높아지는 초음파 조건임을 확인 할 수 있었다. 결 론: 결론적으로 Lidocaine을 함유한 hydrogel은 초음파 치료 시 피부투과율을 높여 더 빠르게 목표하는 양을 경피에 투과시킬 수 있 으며 이는 초음파의 매개변수에 따라서 더 효과적으로 투과시킬 수 있었다. 이에 미용 시술에 사용 시 적용이 가능한 것으로 판단된다. Purpose: In this study, we study to more effectively use anesthesia products used in beauty procedures following the popularization of anti-aging. Hydrogel, which contains lidocaine, is believed to be more effective in relieving pain if used in cosmetic procedures with ultrasonic waves. Methods: The availability of manufactured hydrogels and commercial gels for ultrasonic treatment was compared, and the effect on skin penetration and skin penetration due to ultrasonic limitations was evaluated based on their applicability. Usability and optimal ultrasound parameters were identified during ultrasound treatment. Results: Viscosity measurement, gelation rate, swelling, skin permeability experiment, and HPLC analysis of manufactured hydrogels all revealed properties, with skin permeability being highest at frequency 1 MHz, cycle low 200, and high 50. Conclusions: Finally, hydrogels containing lidocaine increased skin permeability during ultrasound treatment, allowing for faster targeted transdermal transmission that was more effective depending on the ultrasound parameters. As a result, it is determined that it can be used in cosmetic procedures.
