Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

연구배경:2009년 4월 새로운 인플루엔자 A(H1N1) 바이러스에 의한 감염은 전세계적으로 대유행이 시작되었다. 본 연구에서는 대한민국의 한 대학병원에서 발생한 H1N1 인플루엔자 감염환자 중에서 청소년과 성인 입원 환자를 대상으로 임상적 특성과 예후에 대하여 2009년 9월부터 2010년 1월까지 조사하여 알아보고자 하였다. 방 법:대유행 H1N1 인플루엔자로 확진된 13세 이상의 환자의 의무기록을 후향적으로 분석하였으며 대유행 H1N1 인플루엔자는 real-time reverse transcriptase-polymerase chain reaction 시행하여 확진하였다. 결 과:43명의 대유행 인플루엔자 입원 환자 중에서 7명(16%)은 중환자실 치료를 받았으며 이중 1명(2%)은 사망하였다. 평균 연령은 45세였고 기저질환으로 천식, 당뇨, 허혈성 심장질환, 기관지 확장증, 암의 순서로 많은 빈도수를 보였다. 임상증상으로 발열이 가장 주된 증상이었으며 그 외에 기침, 호흡곤란, 콧물, 근육통 또는 관절통, 인후통을 호소하였다. 입원 시 42명 중 25명의 환자에서 흉부 방사선 검사 결과 폐렴을 동반하였고 만성호흡기 질환의 악화를 보인 경우가 11명의 환자에서 관찰 되었다. 또한 23명의 Fish환자에서 산소공급이 필요하였으며 이중 2명은 급성 호흡부전으로 인하여 기계 환기가 필요하였다. 중환자실 입원을 요한 7명의 환자들은 모두 기저질환을 가지고 있었으며 이중 5명은 천식, 3명은 허혈성 심장 질환이 있었고 임신 상태와 당뇨가 각각 1명으로 나타났다. 확진된 모든 환자들은 증상 발현으로부터 평균 2일 내에 항바이러스제를 처방 받았다. 결 론:천식은 입원치료를 요하는 대유행 인플루엔자 감염환자에서 가장 흔하게 동반된 내과적 질환이었으며 특히 허혈성 심장질환 동반하는 환자의 경우 중환자실 입원을 필요로 함을 알 수 있으며 연구 결과 대부분의 환자에서 항바이러스제 치료에 효과적인 것으로 나타났다.

Purpose: The purpose of this study is, in the situation where rapid response to the rapidly changing environment is required due to the development of the fourth industrial revolution such as artificial intelligence, virtual reality, and the internet of things, robotics, big data, additive manufacturing, bio-health, sharing economy and in the organizational culture aspiring toward the innovation of a major company, small business and a public institution, to analyze what influence a job-engagement and stress make, and what influence individual‟s self-efficacy as a moderator mediator makes, and to offer basic data for improving job-engagement and lowering job-stress. Research design, data, and methodology: For doing this, the literature and the empirical studies were combined. Deriving innovationoriented organizational culture as factors affecting the job engagement and job stress through the literature, and have established hypotheses to verify them. We have collected data of 281 from ex,ecutives and staff-members working in areas including major company, small business and officials (the central government, a local public service, the prosecution, the police, and school). And these data were analyzed by SPSS 23 version. Results: Based on these data, the results of analysis were as follows; First, the innovationoriented organizational culture which was recognized by organizational members had effect on job-stress. Second, the innovationoriented organizational culture which was recognized by organizational members influenced job-stress. Third, in the relationship between the innovation-oriented organizational culture and job-engagement, self-efficacy did not influenced job-engagement. Finally, in the relationship between the innovation-oriented organizational culture and job-stress, self-efficacy influenced job-stress. Conclusions: Innovation-oriented organizational culture places importance on the organization's adaptability and flexibility in the external environment, so companies need to establish an innovation-oriented organizational culture favorable to achieving survival and successful innovation, and to develop and disseminate programs of positive and continuous organizations to improve task enthusiasm, reduce task stress, and enhance organizational performance. In the future, it will be necessary to verify the effectiveness of various organizational culture types through comparative analysis with companies that actively maintain an innovation-oriented organizational culture (Google, Kakao, etc.) and companies that prefer hierarchy-oriented organizational culture, relationship-oriented organizational culture, and market-oriented organizational culture.

최근 우리나라를 포함한 여러 나라의 과학, 기술 관련 교육 분야에서 많은 관심을 받고 있는 STEAM(science, technology, engineering, arts, mathematics) 교육은 미래 과학기술 사회가 요구하는 과학, 기술, 공학, 예술, 수학 등 다양한 분야의 융합적 지식을 기반으로 학생들의 과학기술에 대한 이해·흥미·잠재력을 제고하여 창의성, 직관력, 감성과 예술적 감각의 신장을 포함하는 총체적인 접근에 기초하고 있다. 본 연구자들은 우리나라의 교육 환경과 실정에 맞는 STEAM 교육의 방향에 대해서 제언하고자 연구를 수행하였고, 우리나라 과학기술교육의 문제점을 최소화 하고 학생들의 과학기술공학에 대한 흥미와 동기 유발, 효능감 등의 극대화 및 창의·인성 교육을 포함한 한국형 융합인재교육의 유형으로 4C-STEAM 유형을 제안한다. 4C-STEAM 교육은 융합적 지식 및 개념 형성과 창의성을 가지고 다른 사람과의 소통과 배려를 추구하기 때문에 우리나라 교육에 시사점을 줄 수 있을 것으로 기대된다. STEAM Education refers enhancing science, technology, engineering, arts and mathematics by interdisciplinary approach. STEAM Education fosters integrative STEM education with artistic perspectives. Korean students have high academic achievement in math and science, but show low self-efficacy and interest in them. We highlight the affective aspect of science-technology-mathematics education on STEAM, 4C-STEAM stands for convergence, creativity, communication, and caring. The key elements of STEAM are creative design and emotional touch. STEAM education will help students: To possess integrative understanding and problem-solving ability in STEAM subject areas to become competent citizens in future society; to have interest and curiosity in STEAM subject areas to bring high self-efficacy and strong motivation; and to be considerate of others feelings.

Biological and molecular characterization of a Korean isolate of Orthotospovirus chrysanthinecrocaulis (formerly known as chrysanthemum stem necrosis virus, CSNV) isolated from Chrysanthemum morifolium was determined using host range and sequence analysis in this study. Twenty-three species of indicator plants inoculated mechanically CSNV-Kr was investigated for determination of host range. CSNV-Kr induced various local and systemic symptoms in the inoculated plant species. CSNV-Kr could not infect three plant species and induced symptomless in systemic leaves in Nicotiana tabacum cultivars, though the plant samples reacted positively with the antiserum to CSNV by double-antibody sandwich–enzyme-linked immunosorbent assay. The complete genome sequence of CSNV-Kr was determined. The L RNA of CSNV-Kr consists of 8,959 nucleotides (nt) and encodes a putative RNA-dependent RNA polymerase. The M RNA of CSNV-Kr consists of 4,835 nt and encodes the movement protein (NSm) and the glycoprotein precursor (Gn/Gc protein). The S RNA of CNSV-Kr consists of 2,836 nt and encodes NSs protein and N protein. The Gn/Gc and N sequence of CSNV-Kr were compared with those of previously published CSNV isolates originating from different countries at nucleotide and amino acid levels. The Gn/GC sequence of CSNV-Kr shared 98.8−99.5% identity with CSNV isolated from other countries and the N sequence of CSNV-Kr shared 98.8−99.6% identity. No particular region of variability could be found in either grouping of viruses. All of the CSNV isolates did not show any relationship according to geographical origins and isolation hosts, suggesting no distinct segregation of the CSNV isolates.

<P>In the field of bone research, various natural derivatives have emerged as candidates for osteoporosis treatment by targeting abnormally elevated osteoclastic activity. Methyl gallate, a plant-derived phenolic compound, is known to have numerous pharmacological effects against inflammation, oxidation, and cancer. Our purpose was to explore the relation between methyl gallate and bone metabolism. Herein, we performed screening using methyl gallate by tartrate resistant acid phosphatase (TRAP) staining and revealed intracellular mechanisms responsible for methyl gallate-mediated regulation of osteoclastogenesis by Western blotting and quantitative reverse transcription polymerase chain reaction (RT-PCR). Furthermore, we assessed the effects of methyl gallate on the characteristics of mature osteoclasts. We found that methyl gallate significantly suppressed osteoclast formation through Akt and Btk-PLCγ2-Ca<SUP>2+</SUP> signaling. The blockade of these pathways was confirmed through transduction of cells with a CA-Akt retrovirus and evaluation of Ca<SUP>2+</SUP> influx intensity (staining with Fluo-3/AM). Indeed, methyl gallate downregulated the formation of actin ring-positive osteoclasts and resorption pit areas. In agreement with in vitro results, we found that administration of methyl gallate restored osteoporotic phenotype stimulated by acute systemic injection of lipopolysaccharide in vivo according to micro-computed tomography and histological analysis. Our data strongly indicate that methyl gallate may be useful for the development of a plant-based antiosteoporotic agent.</P>
