        간세포암의 십이지장침윤에 의한 상부위장관 출혈 1 예

        Recent advances in both the diagnosis and treatment of hepatocellular carcinoma have improved the prognosis and changed the clinical significance of the recently increasing distant metastases. Distant metastases found after successful treament of the primary lesions are of great clinical significance for the treatment of hepatocellular carcinoma. The duodenum is a rare site of hematogenous metastases or direct invasion from hepatocellular carcinoma. A 23 year old man was admitted with upper gastrointestinal bleeding. He had been diagnosed with hepatocellular carcinoma and treated by a left lobectomy and chemoembolization. The patient was admittted for 12 months after the treatment of the primary tumor. Endoscopic examination revealed a mass in the duodenal bulb that protruded into the lumen. He died due to massive hematemesis. We report on a rare case of hepatocellular carcinoma with duodenal invasion in a 23-year-old male patient.

        전자선과 불가사리 분말을 이용한 하수슬러지 탈수능 향상

        This study was focused on the manufacturing method of a dewatering aid, which would reduce the water content of the sludge cake by enhancing the dewaterability of sewage sludge. The pretreatment technology for sludge by using radiation and among diverse discarded resources were starfish selected as the material to manufacture the dewatering aid. Starfish went through the process of washing, drying, and pulverizing. The starfish powder made in this process was applied to the digested sludge generated at the sewage treatment plant of D City, and its effects were investigated. The starfish powder that was 300 ㎛ in particle size was added to the irradiated digested sludge. After the application of the condensation process, the sludge with the starfish powder added was dewatered using the belt press and centrifuge, which were the traditional pressure dewatering devices. As the result, it reduced the water content of the sludge 20% higher than the dewatered cake with no dewatering aid added and irradiation. When the powder was added, it contributed to less use of the coagulant added. The more irradiation dose, the lower water content did the dewatered cake have and the more coagulant was needed for condensation, which seems to be a disadvantage that can be compensated for by the starfish dewatering aid. A small-scaled treatment of the study to a radiation technology and dewatering aid using a discarded resource confirmed the potential of dewaterability. Based on the results saying that the dewatering aid and radiation technology can improve dewatering effects using the traditional dewatering devices, this pretreatment technology will be expected to be applied to sewage treatment plants.

      • 우리나라의 전통적 授乳 및 養兒 방식에 비추어 본 Donald Winnicott의 모성적 돌봄 환경

        This study is to investigate the implication of Donald Winnicott's theoritical concept, "marternal caring environment" through the traditional method of infant feeding and caring in Korea. The method of this study is literature research. First, to examine the traditional method of i7ant feeding and caring in Korea, it refered to korean documents in the Chosen dynasty involving 「Dong Eu Bo Gam」 「Gyu Hab Chong Seo」, and 「Traditional Child Rearing Method」 which was united Korean sundry records about child rearing. Second, to study Donald Winnicott's "maternal caring environment", it refered recent volumes about Donald Winnicott and his theory. They are 「The boundart and space」, 「Play and reality」, 「Psychoanalytic object relation theory」, 「Maturational Processes and the facilitating environmen」. The results of this study are as follows. It was found that the traditional method of infant feeding and caring in Korea is connected with D. Winnicott's "maternal caring environment". First, traditional method of infant feeding is connected with "Holding", "Handling", Object offer and omnipotence experience" and "Mirror-role of Mother (or Mirroring)". Second, traditional method of infant caring is connected with "handling" "Mirror-role of Mother". Conclusively, It was confirmed that our traditional infant raring method is rational and precursive on a viewpoint of modern developmental psychology.

      • 십이지장 궤양 환자와 만성 위염 환자의 염증 분포의 차이 및 H. pylori 집락도와 염증도의 상관관계

        An inflammatory difference between the gastric antrum, body and fundus was studied in 39 H.pylroi positive duodenal ulcer and 29 H. pylori chronic gastritis patients. Also we studied correlation of H. pylori colony count and inflammatory severity. Patients with newly diagnosed duodenal ulcer and chronic gastritis were endoscoped and two biopsy secimens each taken from the antrum, body and fundus. Although intestinal metaplasia in antrum and inflammatory activity in fundus were more severe in chronic gastritis patients. another inflammatory difference was equally likely to be found between two groups. There was no definite histological inflammatory difference between duodenal ulcer and chronic gastritis patients. A positive correlation was found between the number of H. pylori and the severity of inflammation.

        The packet communication protocol(X.25, TCP/IP etc.) was designed to ensure the efficient data transmission in the current public packet communication network. The packet communication network has various time of transmission delay because packet data doesn't use same path in the packet network. The retransmission setting time of the timer is also various. This paper proposes new method which decides retransmission timer based fuzzy theory in the packet communication protocol. The timer using the fuzzy Petri-net is modeled to implement the packet communication protocol. The fuzzy model of the retransmission timer is to infer the smoothed parameter αby constructing the fuzzy antecedent resultant from the errors between the elapsed time of one packet from the sender to the receiver and the estimated time. The fuzzy model of the timer is employed to adaptively produce the next packet timer value with the elapsed time and smoothed parameter α. We propose an effective method of network using packet mode.

        The objective of this study is to develop the operation model of the double-skin facade for energy conservation and user comfort. This study proposes development process of the operation model in three steps. A case study is used to investigate the feasibility of the process. The G building in Kungkido is chosen. The main contents of this study are as follows. 1) The operation objectives and standards are established. And the operation factors classified according to the objectives and standards. 2) Correlation of the operation factors are clarified. And Individual models. The operation model is essential to building which has the double-skin facade and logical operation would be improved by this model. For the practical application of this operation model, it will be needed to connect the building management system.

      • 발포폴리스티렌비드를 사용한 경량콘크리트의 역학적 특성에 관한 실험적 연구

        The purpose of this study is to obtain basic data on the properties of the lightweight concrete within expanded polystyrene beads for various applications in the field. Inquiring into the change of the properties as altering cement matrix mixture and the capacity mixture ratio of expanded polystyrene beads, the mixture plane of this study is set up as W/C is fixed 50%, expanded polystyrene beads is increased 10% from 0% to 80% per 1m³ and the ratio of cement:sand in the cement matrix is 1:0, 1:0.5, 1:1. The results of this study are as follow. The slump as changing the mixture ratio of expanded polystyrene beads decrease smoothly until mixture ratio 50% but decrease rapidly being in excess of 50%. The density decrease regularly as the mixture ratio of expanded polystyrene beads increase but the ratio of strength decrease. The difference of cement matrix. The strength of cement matrix having more sand mixture ratio relatively decrease rapidly as increasing the mixture ratio of expanded polystyrene beads. It is judged decreased strength phenomenon as the quantity of coherent material created in the mixture decrease rapidly.

        플라이 애쉬가 콘크리트 구조물의 철근 부식에 미치는 영향

        Addition of fly ash has beneficial effects on some mechanical properties of concrete, as well as the corrosion process induced by the chloride ion. The aim of this study was to investigate the effect of fly ash on the steel corrosion in concrete with various water-to-binder ratios. The corrosion process of steel bars embedded in concrete without fly ash and with 20% fly ash was tested under complete immersion, in 3.5 wt.% NaCl solution. Monitoring of open-circuit potential and linear polarization method and electrochemical impedance spectroscopy(EIS) were used to follow the corrosion process. Also, ASTM C1202 test method was used to estimate the concrete's resistivity of chloride penetration in concrete. The results obtained from electrochemical tests show that the use of fly ash has led to the improvement of corrosion resistance. Partial cement replacement by fly ash caused significant pore refinement, reduced permeability to chloride ions, and increased corrosion resistance.

        It is important to represent limit state function in reliability-based design optimization. Since it is very difficult to explicitly represent a limit state function in topology optimization, it is necessary to make the limit state function an approximate function. In case of implementing RSM, the limit state function can easily approximated. In this study, the limit state function of structures was approximated using ADAPRES to obtain the effective response surface modeling as reducing the experiment points. From the result of carrying out the BESO using the ADAPRES, it is concluded that the ADAPRES can be applied to RETO and is more effective than RSM in computing cost.
