        Multiple sclerosis and lupus erythematosus are chronic, potentially disabling diseases of unknown cause. It is a rare occasion that the finding of both diseases occurs in one patient. Because both diseases may affect the central nervous system, it may be difficult at times to differentiate symptoms of lupus erythematosus from those of multiple sclerosis. As far as we know, there was no case report of SLE in multilple sclerosis in Korea. We report a case of multiple sclerosis accompanied by SLE.

      • 부산지역 워터프런트의 기후특성에 관한 연구

        워트프런트는 도심 및 내륙지역과는 다른 독특한 기후특성을 가지고 있으며 이를 고려하지 않고 개발할 경우 시설물의 하자발생, 에너지 및 유지관리비용의 상승을 초래하게 됨은 물론 배후지, 도시기후에 악영향을 미치게 된다. 본 연구는 부산지역의 10개 기상관측지점을 해안선에서 거리에 따라 워터프런트 및 내륙지역으로 분류하고 5년간의 기상데이터를 이용하여 워터프런트의 기후특성에 대해 검토한다. 또한 원터프런트지역임에도 내륙지역과 유사한 기후특성이 나타나고 있는 대연지점을 대상으로 기후특성변화의 원인에 대해 검토한다. The waterfront has distinct climate characteristics different from urban or inland area. These may create not only the rise of energy and maintenance costs for facilities located at waterfront areas, but also negative effects on the climate of the nearby inland area, unless these are treated with particular care. For the present study, the climate characteristics of waterfront were examined with climate data of 10 observation points carefully selected in Busan area. Each weather observation point was classified into either waterfront area of inland area, based on the distance from the coastal line. Special considerations were given to the climate data gathered at the Dae-Yeon weather station because it shows the climate characteristics similar to those of inland area although it is located very near the waterfront area. Results indicates that this peculiar climate condition attributes, at least in part, to the reclamation of frontal coastal area.

        A Familial Case with Holt-Oram Syndrome with a Novel TBX5 Mutation

        Holt-Oram syndrome (HOS) is the most common heart-hand syndrome, which is inherited in an autosomal dominant manner, but most cases are sporadic. This condition is characterized by upper-extremity malformations involving radial-ray, thenar, and carpal bones, and congenital heart malformations including atrial septal defect and ventricular septal defect. It is caused by mutations in the TBX5 gene. In this report, a Korean case with HOS is described, which is inherited from her father. A novel nonsense mutation, p.Glu294*, was identified. This is the first Korean case with HOS confirmed by genetic testing.

        Prototype Development of a Three-wheel Riding Cultivator and Its Basic Performance

        Purpose: The aim of this study is to develop a three-wheel riding cultivator for improving the performance of the current four-wheel riding cultivators in the market. Methods: A prototype three-wheel riding cultivator with the rated power of 15.5-kW, a primary hydrostatic and a two-speed selective gear transmission shifts, front/rear three-wheel drive, a hydraulic wheel tread adjustment, and the mid-section attachment of the major implements was designed and constructed. Its specifications and basic performance are investigated. Results: The maximum speeds of the prototype at the low and high stages were measured to be approximately 7.31, and 11.29 km/h in forward travel, respectively, and approximately 3.60, and 6.37 km/h in rearward travel, respectively. The minimum ground clearance is shown to be 670 mm. The rotating speeds of the power takeoff (PTO) shaft at the low and high stages are shown to be approximately 795 and 1,140 rpm, respectively. The tread of the rear wheels, the minimum radius of turning, and the maximum lifting height of the parallel link device are measured to be within 1,320-1,720 mm, 2.80 m, and 390 mm, respectively. Approximately 25.3% and 74.7% of the total weight of the prototype are distributed in the front and rear wheels on flat ground, respectively. When the tread of rear wheels increased from 1,320 to 1,720 mm, the left and right static lateral overturning angles increased from 33.4° to 39.1° and from 29.0° to 36.1°, respectively. Conclusions: The prototype three-wheel riding cultivator showed a wide range of travel and PTO speeds, high minimum ground clearance, small minimum radius of turning, and easy control of the rear wheel tread. Further, the easy observation of cultivating operations by mid-mounting the implements can improve quality of work. Therefore, the prototype is expected to contribute to the riding mechanization of cultivating operations for various upland crops in Korea.

        Prototype Development of a Small Combine for Harvesting Miscellaneous Cereal Crops and its Basic Performance

        Purpose: The aim of this study is to develop a small combine for harvesting miscellaneous cereal crops. Methods: A prototype small combine was designed and constructed. Its specifications and basic performance were investigated. Results: The prototype small combine for harvesting miscellaneous cereal crops was designed and constructed to reflect similar specifications as those of the conventional combine. The prototype small combine comprises a diesel engine with the rated power/speed of 22.0 kW/2,600 rpm, three-stage primary and two-stage speed range transmission shifts, and a double acting threshing part. The maximum travel speeds of the prototype combine are approximately 0.72 m/s, 2.50 m/s, 0.30 m/s at the low, high speed range shifts in the forward direction, and while traversing in the reverse direction, respectively. The minimum radius of turning was approximately 1.50 m. In a static lateral overturning test, the prototype combine overturned neither to the right nor to left on a $30^{\circ}$ slope. The results of an oilseed rape harvesting test included the maximum operating speed of 0.32 m/s, the grain loss ratio of approximately 9.0%, and the effective field capacity of approximately 10.3 a/h. Additionally, among the outputs in grain outlet, the whole grains, damage grains, and materials other than grain (MOG) ratios accounted for 97.4%, 0.0%, and 2.6%, respectively. Conclusions: The prototype small combine for harvesting miscellaneous cereal crops indicates good driving ability and stability. The results of the oilseed rape harvesting test reveal that the harvesting performance must be enhanced such that the separating and cleaning parts are more suitable for each type of crop, thus reducing grain loss and foreign substances among the outputs in grain outlet. An improved small prototype combine could be used effectively to mechanize the harvesting of miscellaneous cereal crops in small family farms or semi-mountainous areas.

        A Prospective, Single-Center, Single-Blind, Randomized, Confirmatory, Controlled Trial to Assess the Efficacy and Safety of PF-72 Combined with 0.75% Ropivacaine Hydrochloride in Patients Undergoing Single-Level Lumbar Discectomy

        Background: We conducted this study to assess the efficacy and safety of PF-72 (TGel Bio Co. Ltd., Seoul, Korea) combined with 0.75% ropivacaine hydrochloride in relieving the postoperative pain in patients undergoing single-level lumbar discectomy. Methods: A total of 95 eligible patients were randomized to either the trial group (n = 47; PF-72 combined with 0.75% ropivacaine hydrochloride) or the control group (n = 48; ramosetron hydrochloride 0.3 mg). The patients were monitored at 3, 6, 24, 48 and 72 hours postoperatively, for which the cumulative area under the curve (AUC0-72) of NRS pain scores was plotted and the amount of rescue analgesics used at 6, 24, 48 and 72 hours or 24, 48 and 72 hours postoperatively. Moreover, incidences of treatment-emergent adverse events (TEAEs) and serious adverse events (SAEs) were analyzed. Results: The amount of analgesics used and the AUC0-72 of the NRS pain scores was significantly smaller and the NRS pain scores were significantly lower in the trial group as compared with the control group (P < 0.05). In addition, there was no significant difference in the number of the patients presenting with no pain (NRS pain scores ≤ 3) between the two groups at 48 and 72 hours postoperatively (P > 0.05). There were no significant differences in the incidences of TEAEs and SAEs between the two groups (P > 0.05). Conclusions: PF-72 combined with 0.75% ropivacaine hydrochloride is an effective, safe drug delivery system.

        Comparison of Clinico-Physiologic and CT Imaging Risk Factors for COPD Exacerbation

        <P>To date, clinico-physiologic indices have not been compared with quantitative CT imaging indices in determining the risk of chronic obstructive pulmonary disease (COPD) exacerbation. We therefore compared clinico-physiologic and CT imaging indices as risk factors for COPD exacerbation in patients with COPD. We retrospectively analyzed 260 COPD patients from pulmonary clinics at 11 hospitals in Korea from June 2005 to November 2009 and followed-up for at least one year. At the time of enrollment, none of these patients had COPD exacerbations for at least 2 months. All underwent clinico-physiologic and radiological evaluation for risk factors of COPD exacerbation. After 1 yr, 106 of the 260 patients had at least one exacerbation of COPD. Multiple logistic regression analysis showed that old age, high Charlson Index, and low FEV<SUB>1</SUB> were significant in a clinico-physiologic model, with C-statistics of 0.69, and that increased age and emphysema index were significant in a radiologic model, with C-statistics of 0.64. The difference between the two models was statistically significant (<I>P</I> = 0.04 by bootstrap analysis). Combinations of clinico-physiologic risk factors may be better than those of imaging risk factors in predicting COPD exacerbation.</P>

        An agonistic anti-Tie2 antibody suppresses the normal-to-tumor vascular transition in the glioblastoma invasion zone

        Tumor progression is intimately associated with the vasculature, as tumor proliferation induces angiogenesis and tumor cells metastasize to distant organs via blood vessels. However, whether tumor invasion is associated with blood vessels remains unknown. As glioblastoma (GBM) is featured by aggressive invasion and vascular abnormalities, we characterized the onset of vascular remodeling in the diffuse tumor infiltrating zone by establishing new spontaneous GBM models with robust invasion capacity. Normal brain vessels underwent a gradual transition to severely impaired tumor vessels at the GBM periphery over several days. Increasing vasodilation from the tumor periphery to the tumor core was also found in human GBM. The levels of vascular endothelial growth factor (VEGF) and VEGF receptor 2 (VEGFR2) showed a spatial correlation with the extent of vascular abnormalities spanning the tumor-invading zone. Blockade of VEGFR2 suppressed vascular remodeling at the tumor periphery, confirming the role of VEGF-VEGFR2 signaling in the invasion-associated vascular transition. As angiopoietin-2 (ANGPT2) was expressed in only a portion of the central tumor vessels, we developed a ligand-independent tunica interna endothelial cell kinase 2 (Tie2)-activating antibody that can result in Tie2 phosphorylation in vivo. This agonistic anti-Tie2 antibody effectively normalized the vasculature in both the tumor periphery and tumor center, similar to the effects of VEGFR2 blockade. Mechanistically, this antibody-based Tie2 activation induced VE-PTP-mediated VEGFR2 dephosphorylation in vivo. Thus, our study reveals that the normal-to-tumor vascular transition is spatiotemporally associated with GBM invasion and may be controlled by Tie2 activation via a novel mechanism of action.
