      • Analysis of acne-related judicial precedents from 1997 to 2018 in South Korea

        Background: Medicolegal disputes are increasing in practical medicine. Acne is a common problem but is usually related to cosmetic problems such as acne scars. Medications or procedures related to acne could lead to medical malpractice. Objectives: To analyze medical litigation associated with acne or acne scar in South Korea. Methods: Acne-related judgements were searched using the Supreme Court of Korea’s Written Judgment Management System based on the keywords “acne” or “acne scar.” Results: Eleven cases were selected as litigated cases of acne or acne scar. Eight cases (72.7%) were related to acne scar and three (27.3%) were related to acne. Treatment modalities such as peeling (n = 6), laser treatment (n = 3), photodynamic therapy (n = 1), and antibiotic (n = 1) resulted in lawsuit. Claimed sequelae of the treatment were hyperpigmentation (n = 5), scar worsening (n = 5), erythema (n = 3), skin bumps (n = 1), and liver transplant (n = 1). Eight cases (72.7%) were awarded to the plaintiff, and the others were dismissed. The average awarded amount was 16,801,324± 24,452,486 (mean±standard deviation) Korean Won. Conclusion: Various treatments for acne or acne scar can cause medical disputes. Unnecessary litigation could be prevented if simple measures such as history taking, choosing proper procedure, and adequate management after the procedure along with sufficient informed consent were performed.

        목적: Demonstrate a new instrument, technique or procedure 방법: In this video, we showed how to remove huge ovarian tumors in sinlgel port laparoscopic surgery by SW KIM`s method. SW KIM`s method is the technique to put a huge ovarian tumor into a specially designed extremely large endopouch (XXXL: 30x30㎠sized) using two conventional laparoscopic needle holders and one laparoscopic grasper and gravity by changing the patient`s position. After identifying the infundibulopelvic and uteroovarian ligaments salpingooophorectomy including huge ovarian tumor was performed by LigaSure. And then ovarian tumor was located in the pelvic cavity by changing the patient`s position into reverse-trendelenberg position and a XXXL endopouch was inserted and un-rolled inside the abdominal cavity. To insert a huge tumor into the XXXL endopouch, the endopouch was opened in triangular shape. Bilateral apex of baseline of trianglular opening of the endopouch was hold by two needle holders and then the upper apex was hold by a grasper. After holding three point of endopouch opeing, the patient`s position was changed into deep trendelenberg postion, then the tumor came into the endopouch by gravity. While changing the patient`s position, 2 needle holders and a graspers were moved into the pelvic cavity from the abdominal cavity. Aftere identifying the tumor inside the endopouch, tumors could be removed through the single port opening site or transvaginally in laparoscopic hysterectomy case without spillage of ovarian tumor. 결과: The advantage of SW KIM`s method is to remove ovarian tumor without spillage in a single port laparoscopic surgery by putting it into the large endoscopic bag despite narrow space. 결론: Using a specially designed 30×30 cm sized XXXL Endopouch and SW Kim`s technique, huge ovarian tumors could be removed without spillage in single port laparoscopic surgery.

        Antiadhesive effect and safety of oxidized regenerated cellulose after thyroidectomy

        Purpose: To evaluate the antiadhesive effects and safety of an oxidized regenerated cellulose (Interceed) after thyroidectomy. Methods: Seventy-six thyroidectomized patients were prospectively randomized into two groups with regard to the use of Interceed. We evaluated each group for their adhesive symptoms using four subjective and four objective items at the 2nd week, 3rd and 6th month after thyroidectomy. All patients were examined for vocal cord motility by indirect laryngoscope at each period. Results: Total adhesion scores at each postoperative follow-up period decreased with time, but were not significantly different in each group. The median score for swallowing discomfort for liquid was significantly lower in the Interceed group than in the control group 2 weeks after surgery. In addition, the severity of skin adhesion to the trachea was reduced in the Interceed group compared with the control group 6 months after surgery. During the study, there were no adverse effects or significant differences in postoperative complications between the groups. Conclusion: Interceed appeared to be safe and effective in improving neck discomfort at early postoperative periods and preventing skin adhesion to the trachea 6 months after thyroidectomy.

      • 동물 및 기타 서울시 도봉구 산림녹지축 단절지 야생조류 이동을 위한 네크워크 방안 연구- 쌍문근린공워~초아산근린공원 사례로-

        본 연구는 서울시 도봉구 내 산림녹지축 단절지역의 도시생태계 현황분석과 야생조류 출현현황과의 관계를 조사·분석하여 야생조류 이동루트 설정과 비오톱 구조개선 방안을 제시하였다. 연구대상지 도시생태계 현황분석결과 토지이 용현황에서는 산림, 산지형공원과 저층 단독주거지가 대부분을 차지하였고 녹지율 분포에서는 녹지율 75% 이상인지역이 47.7%로 넓게 분포하였으며 토양피복 현황 분석결과 지붕이 평평한 건물과 녹지 및 오픈스페이스가 주요 유형이었다. 현존식생에서는 산림 중 자연림과 인공림 면적이 비슷하였으며 도시화지역이 41.3% 면적을 차지하였다. 건축물의 층고현황은 저층건물 지역에서는 대부분이 5층 이하의 단독주거지가 대부분이었고, 고층건물은 공동주택지가 대부분이었다. 야생조류 출현현황은 가을철 2회, 월동기인 겨울철 2회, 그리고 번식기인 봄철 1회 등 삼계절에 걸쳐 5회를 실시하여 월별, 계절별로 분석하였다. 가을철 출현 야생조류는 총26종 1,541개체로 다양하였고 겨울철에 출현한 야생조류는 총 22종 1,198개체로 가을철보다 비교적 적었고 봄철에 출현한 야생조류는 25종 643개체이었다. 봄철의 야생조류는 다른 계절에 비해 조사 횟수가 1회 적어서 개체수는 많이 줄었지만 여름철새 증가로 종수는 증가하였다. 전체적으로 시가화종과 텃새의 우점도 및 출현빈도가 높았다. 분석대상종은 연구 목적을 고려하여 연구대상지에 출현한 35종 중도심지에 적응한 종과 맹금류를 제외한 31종을 선정하였다. 선정된 종을 대상으로 각 비오톱 유형별 가을철, 겨울철, 봄철 출현종 및 개체수와 길드분석, 서식유형별, 내부종-장자리종, 종다양도를 분석하였다. 토지이용 유형, 식생의 자연성(인공림, 자연림), 식생의 층위구조, 토양피복과 종수간 분산분석 결과 고도의 유의성이 인정되어 토지이용 유형, 식생의 자연성 및 층위구조에 따라 종수가 차이가 있었고, 녹지율과 출현 종수간의 상관분석 결과도 고도의 양의 상관관계가 인정되어 녹지율이 증가함에 따라 야생조류 종수도 증가하였다.비오톱구조 개선방안 제시를 위해 이동목표종을 선정하였다. 이동목표종은 분석대상종으로 선정된 31종의 야생조류 중 텃새를 중심으로 단기적인 녹지개선으로 이동성이 증가 할 수 있는 종으로 대상지 내 출현빈도가 높은 직박구리, 쇠박새, 박새 등 3종을 선정하였다. 또한 녹지의 질적향상 및 장기적인 서식처 조성을 통해 중·장기적으로 도입가능한 쇠딱따구리, 오색딱따구리, 오목눈이, 붉은머리오목눈이, 동고비, 청딱따구리, 큰오색딱따구리 등 7종을 추가적으로 제안하였다. 녹지축 단절지에 이동목표종 출현특성 및 서식유형을 분석하여 야생조류 이동을 위한 루트를 단기, 중기, 장기로 구분하여 루트별로 현황을 분석하고 비오톱구조 개선방안을 제시하였다. 단기이동루트는 단절지내에서 비교적 녹지가 풍부하고 단기 이동목표종인 박새, 쇠박새, 직박구리가 다수 관찰된 루트다. 중기 및 장기 이동루트는 주택재건축예정지역이 포함된 단독주거지가 대부분으로 단기간에 녹지 확보가 어렵지만 중·장기적인 개선방안이 실현된다면 야생조류 유입이 가능한 지역으로 설정하였다. 단절녹지축에 대한 비오톱구조 개선방안으로 단독주택지는 생활가로조성, 담장 허물기 및 옥상녹화를, 공동주택지는 녹지구조 개선 및 관목식재 추가, 상업지역은 옥상녹화 및 가로수 수종 갱신, 학교는 운동장녹화, 옥상녹화 및 불투수포장 개선, 공공기관은 옥상녹화 녹지면적 증대를 개선방안으로 제시하였다. 노해로와 도봉로는 가로숲 조성을 제안하였다. 또한 녹지축단절지역을 생태축연결지구로 용도지구를 지정하여 도시관리계획 차원에서 장기적인 연결목표를 갖고 관리 할 것을 제안하였다.

        Phosphorylation and Reorganization of Keratin Networks: Implications for Carcinogenesis and Epithelial Mesenchymal Transition

        Metastasis is one of hallmarks of cancer and a major cause of cancer death. Combatting metastasis is highly challenging. To overcome these difficulties, researchers have focused on physical properties of metastatic cancer cells. Metastatic cancer cells from patients are softer than benign cancer or normal cells. Changes of viscoelasticity of cancer cells are related to the keratin network. Unexpectedly, keratin network is dynamic and regulation of keratin network is important to the metastasis of cancer. Keratin is composed of heteropolymer of type I and II. Keratin connects from the plasma membrane to nucleus. Several proteins including kinases, and protein phosphatases bind to keratin intermediate filaments. Several endogenous compounds or toxic compounds induce phosphorylation and reorganization of keratin network in cancer cells, leading to increased migration. Continuous phosphorylation of keratin results in loss of keratin, which is one of the features of epithelial mesenchymal transition (EMT). Therefore, several proteins involved in phosphorylation and reorganization of keratin also have a role in EMT. It is likely that compounds controlling phosphorylation and reorganization of keratin are potential candidates for combating EMT and metastasis.

        Apoptosis of Human Islet Cells by Cytokines

        FasL, perforin, $TNF{\alpha}$, IL-1 and NO have been considered as effector molecule(s) leading to ${\beta}$-cell death in autoimmune diabetes. However, the real culprit(s) of ${\beta}$-cell destruction have long been elusive despite intense investigation. Previously we have suggested $IFN{\gamma}/TNF{\alpha}$ synergism as the final effector molecules in autoimmune diabetes of NOD mice. A combination of $IFN{\gamma}$ and $TNF{\alpha}$ but neither cytokine alone, induced classical caspase-dependent apoptosis in murine insulinoma and pancreatic islet cells. $IFN{\gamma}$ treatment conferred susceptibility to $TNF{\alpha}$-induced apoptosis on otherwise resistant murine insulinoma cells by STAT1 activation followed by IRF-1 induction. Here we report that $IFN{\gamma}/TNF{\alpha}$ synergism induces apoptosis of human pancreatic islet cells. We also observed STAT1 activation followed by IRF-1 induction by $IFN{\gamma}$ treatment in human islet cells. Taken together, we suggest that $IFN{\gamma}/TNF{\alpha}$ synergism could be involved in human islet cell death in type 1 diabetes, similar to murine type 1 diabetes.

        <i>ABCG2</i>Q141K polymorphism is associated with chemotherapy-induced diarrhea in patients with diffuse large B-cell lymphoma who received frontline rituximab plus cyclophosphamide/doxorubicin/ vincristine/prednisone chemotherapy

        <P>ATP-binding cassette transporter G2 (ABCG2) is the most recently described transporter of the multidrug-resistance pump and it promotes resistance to anticancer drugs such as doxorubicin, mitoxantrone, topotecan, and SN-38. Of the ABCG2 polymorphisms, V12M and Q141K alter the functional activity of the ABCG2 transporter and influence the drug response and various toxicities to chemotherapeutic agents. We therefore evaluated the impact of the ABCG2 V12M and Q141K polymorphisms on the therapeutic outcomes and toxicities of primary rituximab plus cyclophosphamide/doxorubicin/vincristine/prednisone (R-CHOP) therapy in 145 Korean patients with diffuse large B-cell lymphoma (DLBCL). ABCG2 V12M and Q141K genotyping was carried out by pyrosequencing of polymerase chain reaction products. The clinical characteristics, treatment outcomes, toxicities of the patients, and the predictive value of the polymorphisms on response, survival, and adverse events to R-CHOP for 145 patients were analyzed according to the ABCG2 V12M and Q141K polymorphisms. No differences were observed according to ABCG2 Q141K and V12M genotype in patient characteristics, disease characteristics, response, survival, or hematology toxicity profiles in patients with DLBCL who received frontline R-CHOP chemotherapy. On multivariate analysis, grade I-IV diarrhea was statistically significant according to ABCG2 Q141K polymorphism (the QQ genotype vs the QK or KK genotypes; hazard ratio 2.835; 95% confidence interval 1.432-5.613; P = 0.003). This study demonstrates that the ABCG2 Q141K polymorphism may correlate with chemotherapy-induced diarrhea in patients with DLBCL who have received frontline R-CHOP chemotherapy, and this has implications for optimizing treatment with such agents.</P>

        Crystal structure of Tpa1 from <i>Saccharomyces cerevisiae</i> , a component of the messenger ribonucleoprotein complex

        <P>Tpa1 (for termination and polyadenylation) from <I>Saccharomyces cerevisiae</I> is a component of a messenger ribonucleoprotein (mRNP) complex at the 3′ untranslated region of mRNAs. It comprises an N-terminal Fe(II)- and 2-oxoglutarate (2OG) dependent dioxygenase domain and a C-terminal domain. The N-terminal dioxygenase domain of a homologous Ofd1 protein from <I>Schizosaccharomyces pombe</I> was proposed to serve as an oxygen sensor that regulates the activity of the C-terminal degradation domain. Members of the Tpa1 family are also present in higher eukaryotes including humans. Here we report the crystal structure of <I>S. cerevisiae</I> Tpa1 as a representative member of the Tpa1 family. Structures have been determined as a binary complex with Fe(III) and as a ternary complex with Fe(III) and 2OG. The structures reveal that both domains of Tpa1 have the double-stranded β-helix fold and are similar to prolyl 4-hydroxylases. However, the binding of Fe(III) and 2OG is observed in the N-terminal domain only. We also show that Tpa1 binds to poly(rA), suggesting its direct interaction with mRNA in the mRNP complex. The structural and functional data reported in this study support a role of the Tpa1 family as a hydroxylase in the mRNP complex and as an oxygen sensor.</P>
