Right lumbar lymph trunk injury after right laparoscopic donor nephrectomy: a case report

Laparoscopic donor nephrectomy (LDN) is increasingly popular because of its advantages over open surgery. Chyle leak after donor nephrectomy is a rare but potentially lethal complication if not treated appropriately. We describe a case of a 43-year-old female patient with no remarkable history who presented a chyle leak on day 2 after right transperitoneal LDN. Since conservative treatment failed, the patient underwent magnetic resonance imaging (MRI) and intranodal lipiodol lymphangiography, which confirmed the chyle leak from the right lumbar lymph trunk into the right renal fossa. The chyle leak was percutaneously embolized twice, on postoperative day (POD) 5 and POD 10, by a mixture of N-butyl-2-cyanoacrylate and lipiodol. The drainage fluid decreased significantly after the second embolization. The subhepatic drainage tube was withdrawn on POD 14, and the patient was discharged on POD 17. MRI lymphangiography and intranodal lipiodol lymphangiography effectively identified the chyle leak point. Percutaneous embolization seems to be a safe, effective method for treating high-output chyle leaks.
Positioning and Trajectory Tracking for Caterpillar Vehicles in Unknown Environment

This paper proposes positioning and trajectory tracking for Caterpillar Vehicles (CVs) in unknown environments. To do these tasks, the following are performed. Firstly, a system modeling of the Caterpillar Vehicle is presented. Secondly, solving the complicated tracking control problem in unknown environments is a challenging mission. Therefore, to guarantee the Caterpillar Vehicle system to be strong robustness against external disturbances in the unknown environments, a MIMO robust servo controller for tracking the desired trajectory is designed by using a Linear Shift Invariant Differential (LSID) operator. The CVs are able to accomplish various tasks in dangerous places where workers cannot enter. Thirdly, the positioning of the CV can be obtained using a Simultaneous Localization and Mapping (SLAM) method. This paper develops perfectly the SLAM algorithm for positioning of the CV based on laser sensor Lidar. The main advantage of this method is that it does not need to use more landmarks. Landmarks can be obtained from the unknown environment. Thus, the CV can work even in unknown environments and unsafe places. Finally, to verify the effectiveness of the proposed MIMO robust servo controller and the SLAM positioning algorithm, the experimental results are presented. The experimental results demonstrate the adequate tracking performance of the proposed MIMO robust servo controller in the unknown environment.
Mortality following transarterial embolization due to hemorrhage after liver venous deprivation

Liver venous deprivation (LVD) is considered to be a safe and effective method that induces more rapid and important liver hypertrophy before major hepatectomy in comparison with portal vein embolization (PVE) alone. Bleeding complications after LVD or PVE are rare, but can be a life-threatening event. Herein, we report a case of perihepatic and extended subcapsular hematoma of the liver after LVD. Transarterial embolization was performed using gelfoam to control the hemorrhage; however, the patient developed irreversible liver failure and passed away after 38 days of treatment. In patients with LVD or PVE, severe post-procedural bleeding poses a substantial challenge for treatment. Arterial embolization should be carefully considered to avoid liver failure and even death, regardless of whether temporary embolization is used.

Vascular injuries account for approximately 3% to 25% of all cases of liver trauma and are typically treated by transarterial embolization (TAE). We describe a case of American Association for the Surgery of Trauma grade V liver injury with a very large pseudoaneurysm and an arteriohepatic venous fistula (AHVF), which failed to respond to TAE. This case was successfully treated using a direct transhepatic glue injection without complications. AHVF is an exceedingly rare type of vascular injury in blunt trauma. Direct transhepatic glue injections could represent a reliable alternative to TAE for the treatment of visceral pseudoaneurysms, especially if TAE fails to eliminate the lesion.

In this paper, we mathematically investigate a downlink non-orthogonal multiple access (NOMA) system for short-packet communications (SPC) in which the near users are used as full-duplex (FD) relays to forward intended signals from the source to a far user. In addition, partial relay selection is employed to enhance the performance of the FD relays under the impact of imperfect interference cancellation. At the far user, selection combining (SC) or maximal ratio combining (MRC) is employed to combine the signals received from the source and the selected FD relay. The analytical expressions for the average block error rate (BLER) of two users over flat Rayleigh fading channels are derived. Furthermore, closed-form asymptotic expressions of the average BLERs at the near and far users in high signal-to-noise ratio (SNR) regimes are obtained. The numerical results show that the analytical BLERs of the near user and far user closely match the simulation results.
