Background/Aims: In distal malignant biliary obstruction, an antireflux metal stent (ARMS) with a funnel-shaped valve is effective as a reintervention for metal stent occlusion caused by reflux. This study sought to evaluate the feasibility of this ARMS as a first-line metal stent. Methods: Patients with nonresectable distal malignant biliary obstruction were identified between April and December 2014 at three Japanese tertiary centers. We retrospectively evaluated recurrent biliary obstruction and adverse events after ARMS placement. Results: In total, 20 consecutive patients were included. The most common cause of biliary obstruction was pancreatic cancer (75%). Overall, recurrent biliary obstruction was observed in seven patients (35%), with a median time to recurrent biliary obstruction of 246 days (range, 11 to 246 days). Stent occlusion occurred in five patients (25%), the causes of which were sludge and food impaction in three and two patients, respectively. Stent migration occurred in two patients (10%). The rate of adverse events associated with ARMS was 25%: pancreatitis occurred in three patients, cholecystitis in one and liver abscess in one. No patients experienced nonocclusion cholangitis. Conclusions: The ARMS as a first-line biliary drainage procedure was feasible. Because the ARMS did not fully prevent stent dysfunction due to reflux, further investigation is warranted. Background/Aims: Digital single-operator cholangioscopy (DSOC)-guided mapping biopsy (DMB) and tube-assisted mapping biopsy(TMB) are two techniques used for preoperative evaluation of biliary tract cancer (BTC). However, data regarding the diagnostic performanceof these techniques are limited. Methods: We retrospectively examined consecutive patients with BTC who underwent either technique at our institution between2018 and 2020. We evaluated the technical success rate, adequate tissue acquisition rate, and diagnostic performance of these techniquesfor the evaluation of lateral spread of BTC. Results: A total of 54 patients were included in the study. The technical success rate of reaching the target sites was 95% for DMB and100% for TMB. The adequate tissue acquisition rate was 61% for DMB and 69% for TMB. The adequate tissue acquisition rate was low,especially for target sites beyond the secondary biliary radicles. The sensitivity of DMB alone was 39%, which improved to 65% whencombined with visual impression. Experts demonstrated a higher negative predictive value and diagnostic accuracy with respect toboth DSOC visual impression and DMB for the evaluation of lateral spread of BTC compared to trainees. Conclusions: Adequate tissue acquisition rates were similar between the two techniques. Since DMB requires expertise, TMB may bean acceptable option when DSOC is unavailable or when DSOC expertise is limited.
Detecting Nonlinearity in Prediction Residuals of Snoring Sounds

This paper focuses on the nonlinear properties of snoring sounds for the purpose of obstructive sleep apnea diagnosis. Snoring sounds are convolutional sounds caused by wheezing of air way obstruction and oscillation of soft palate. Namely, it should be natural that the snoring sounds are generated from an onlinear dynamics, but the nonlinear properties of them have not yet been studied so far. In this paper, the nonlinearity is defined as the predictability using alinear AR prediction model, and the prediction residuals are analyzed by portmanteau test.
Analysis of Questionnaire Investigation on SNS Utilizing Bayesian Network

Social Networking Service (SNS) is prevailing rapidly in Japan in recent years. The most popular ones are Facebook, mixi, and Twitter, which are utilized in various fields of life together with the convenient tool such as smart-phone. In this work, a questionnaire investigation is carried out in order to clarify the current usage condition, issues and desired functions. More than 1,000 samples are gathered. Bayesian network is utilized for this analysis. After conducting the sensitivity analysis, useful results are obtained. Differences in usage objectives and SNS sitesare made clear by the attributes and preference of SNS users. They can be utilized effectively for marketing by clarifying the target customer through the sensitivity analysis.

The control of pancake vortices (PVs) caused by Josephson vortices (JVs) is demonstrated in the highly anisotropic superconductor Bi2Sr2CaCu2O8+y. The moderately strong attractive interactions between PVs and JVs in the crossing-lattices state under tilted fields are utilized to change the distribution of PVs. Micro-Hall probe magnetometry and differential magneto-optical imaging provide direct evidence for such a novel control. We have succeeded in visualizing the lensing effect of PVs by slowly ramping the in-plane field in the presence of a small c-axis field. How the lensing effect is affected by the history, the density of PVs, and the temperature is systematically studied.
