The expression of proenkephalin (proENK) mRNA and Met-enkephalin (ME) secretion in C6 rat glioma cells and bovine adrenal medullary chromaffin (BAMC) cells were elucidated in the present study. The levels of proENK mRNA and ME secreted into the media in BAMC cells were measured in the presence of cycloheximide and 12-tetrade-canoylphorbol-13-acetate (TPA). Cycloheximide (20 nM) abolished the induction of proENK mRNA expression, protein synthesis and ME secretion by TPA (1nM), indicating that de novo protein synthesis was necessary for proENK gene expression and ME secretion.

The effects of nicotine, [Sarcosine]angiotensin II($[Sar^{1}]$AII; a degradation-resistant analogue of angiotensin II) and 12-0-tetradecanoylphorbol-13-acetate (TPA) on the time-course of the secretion of $[Met^{5}]$enkephalin (ME) in addition to proenkephalin A (proENK) mRNA levels were studied in bovine adrenal medullary chromaffin (BAMC) cells. Nicotine at a concentration of $10{\mu}M$ caused a rapid secretion (within 1 h) of ME followed by a long-term secretion (12~24h after treatment) into the medium. Short-term stimulation of BAMC cells with $[Sar^{1}]$AII had no significant effect on secretion of ME, while long-term stimulation enhanced a profound secretion. The treatment with TPA augmented the secretion of ME 3 h after the treatment and the magnitude of increase in ME secretion induced by $[Sar^{1}]$AII or TPA continued to increase with time. Intracellular level of ME in nicotine-, $[Sar^{1}]$AII-, and TPA-treated cells were not significantly different from that of controls. In a Western blot analysis, intracellular enkephalin precursor levels in TPA- and $[Sar^{1}]$AII-treated cells were elevated, while nicotine-treated group had no effect. Long-term exposure of BAMC cells with nicotine, $[Sar^{1}]$AII and TPA also increased proENK mRNA level. Pretreatment with a calcium blocker, nimodipine ($1{\mu}M$) or a calmodulin antagonist, calmidazolium ($1{\mu}M$), effectively inhibited ME secretion and proENK mRNA levels induced by nicotine and $[Sar^{1}]$ All but not TPA. Pretreatment with a protein synthesis inhibitor, cycloheximide effectively inhibited ME secretion and proENK mRNA levels induced by nicotine, $[Sar^{1}]$AII or TPA. In addition, increased proenkephalin protein level induced by $[Sar^{1}]$AII but not TPA was inhibited by nimodipine. The results indicate that differential mechanisms may be involved in the regulation of ME secretion, proENK mRNA level, and processing of proenkephalin protein when BAMC cells are continuously (up to 24 h) exposed to a neuronal regulator (nicotine), hormonal regulator ($[Sar^{1}]$AII) and protein kinase C activator (TPA).

<P><B>Purpose</B></P><P> Although combining aromatase inhibitors (AI) with gonadotropin-releasing hormone agonists (GnRHa) is becoming more common, it is still not clear if GnRHa is as effective as bilateral salpingo-oophorectomy (BSO).</P><P><B>Materials and Methods</B></P><P> We retrospectively analyzed data of 66 premenopausal patients with hormone receptor– positive, human epidermal growth factor receptor 2–negative recurrent and metastatic breast cancer who had been treated with AIs in combination with GnRHa or BSO between 2002 and 2015.</P><P><B>Results</B></P><P> The median patient age was 44 years. Overall, 24 (36%) received BSO and 42 (64%) received GnRHa. The clinical benefit rate was higher in the BSO group than in the GnRHa group (88% vs. 69%, p=0.092). Median progression-free survival (PFS) was longer in the BSO group, although statistical significance was not reached (17.2 months vs. 13.3 months, p=0.245). When propensity score matching was performed, the median PFS was 17.2 months for the BSO group and 8.2 months for the GnRHa group (p=0.137). Multivariate analyses revealed that the luminal B subtype (hazard ratio, 1.67; 95% confidence interval [CI], 1.08 to 2.60; p=0.022) and later-line treatment (≥ third line vs. first line; hazard ratio, 3.24; 95% CI, 1.59 to 6.59; p=0.001) were independent predictive factors for a shorter PFS. Incomplete ovarian suppression was observed in a subset of GnRHa-treated patients whose disease showed progression, with E2 levels higher than 21 pg/mL.</P><P><B>Conclusion</B></P><P> Both BSO and GnRHa were found to be effective in our AI-treated premenopausal metastatic breast cancer patient cohort. However, further studies in larger populations are needed to determine if BSO is superior to GnRHa.</P>

<P>Sasang typology is a traditional Korean medical classification scheme that combines medical management with general medicine and can be applied to chronic diseases. We aimed to analyze differences in Sasang Personality Questionnaire (SPQ) and Sasang Digestive Function Inventory (SDFI) results in patients with diabetes mellitus (DM), hypertension, functional dyspepsia, major depressive disorder (MDD), and adenomyosis. In this cross-sectional study, data were collected at a medical college hospital in South Korea. A total of 248 patients were included: 52 with DM, 47 with hypertension, 36 with functional dyspepsia, 26 with MDD, and 87 with adenomyosis. The subjects took both the SPQ and the SDFI. The outcome measures in this study were body mass index (BMI), SPQ score, and SDFI score. The DM and hypertension groups, which were expected to demonstrate many Taeeum-type traits, exhibited high BMI, high SPQ-Behavior and SDFI-Appetite scores, and low SPQ-Emotionality and SDFI-Digestion scores. The functional dyspepsia group showed low BMI and high scores for SPQ-Behavior, SPQ-Emotionality, SDFI-Digestion, and SDFI-Appetite. The MDD group demonstrated high SPQ-Emotionality and SDFI-Digestion scores, low BMI, and low SPQ-Behavior and SDFI-Appetite scores. The adenomyosis group demonstrated high scores for SPQ-Behavior, SDFI-Digestion, and SDFI-Appetite, low BMI, and low SPQ-Emotionality scores. Different characteristics distinguishing Sasang traits according to chronic diseases were ascertained, especially for Taeeum and Soeum types.</P>
