        Association between Heart Rate Variability Indices and Depressed Mood in Patients with Panic Disorder

        Objective: Heart rate variability (HRV) reflects the regulation of the autonomic nervous system. Panic disorder is highly associated with autonomic dysfunction, and is often accompanied by depression. The aim of this study is to determine the association between depression and HRV indices in patients with panic disorder. Methods: A total of 110 outpatients diagnosed with panic disorder participated in this study. The medical records of patients with panic disorder who visited the outpatient clinic of Konkuk University Hospital between December 2018 and March 2020 were retrospectively reviewed. Measurements used in this study include the Panic Disorder Severity Scale-Self Report, Beck Depression Inventory (BDI-II), Insomnia Severity Index, and HRV. Patients were divided into depressive and non-depressive groups based on their BDI-II scores. The association between HRV indices and depressive symptoms was statistically analyzed. Results: The low frequency/high frequency (LF/HF) ratio was reduced in patients with depression (mean = −0.095, p = 0.004 in the above moderate depressive group, mean = −0.120, p = 0.020 in the severe depressive group). Significant correlations were found between depressive symptoms and standard deviation of NN interval (SDNN) (ms) (−0.19, p = 0.044), very low frequency (VLF) (ms2/Hz) (−0.22, p = 0.021), LF (−0.25, p = 0.008), HF (−0.19, p = 0.043), and LF/HF (−0.25, p = 0.009). Multiple linear regression analysis showed that BDI predicted SDNN (ms), VLF (ms2/Hz), LF, HF, and LF/HF. Conclusion: We confirmed that the LF/HF ratio decreases when depression is accompanied by panic disorder. HRV indices may be useful markers for detecting depressive symptoms in patients with panic disorder.

        Network Analysis Revealed the Role of Helplessness as a Central Feature Among Late-Life Depressive Symptoms in Patients With Mild Cognitive Impairment and Early Stage Dementia

        Objective It has been reported that depressive symptoms in older adults are different from those in younger adults, especially when accompanied by cognitive decline. However, few studies have investigated the network structure of depressive symptoms in this population.Methods The participants consisted of 627 older adults (>60 yr) who were diagnosed with mild cognitive impairment (MCI) or early stage dementia. Among them, 36.7% were male and the mean age was 76.20±7.71 years. The Korean form of Geriatric Depression Scale (KGDS) was used to evaluate their depressive symptoms and network analyses were performed using bootnet R-package to identify the central features among depressive symptoms.Results Of all the KGDS items, we found that KGDS 2 (often feel helpless) had the highest node strength followed by KGDS 21 (in good spirits), KGDS 14 (not confident at all), and KGDS 15 (cheerful and happy). In terms of node betweenness, KGDS 2 also showed the highest value. The edge weights of edges connected to node KGDS 2 were strongest in KGDS 3 (restless and fidgety) and KGDS 28 (easily get tired).Conclusion In this study, we presented which symptoms are central among the elderly with MCI and early stage dementia. This result not only increases the understanding of depressive symptoms in this group but would also help determine target symptoms in the treatment program.

        The Mediating Effect of Insomnia on the Relationship between Panic Symptoms and Depression in Patients with Panic Disorder

        Background: This study aimed to determine if sleep disturbances may mediate the relationship between panic symptoms and depression in patients with panic disorder (PD). Methods: Electronic medical records were retrospectively reviewed for 110 consecutive patients with diagnosed PD in an outpatient clinic between October 2018 and December 2019. Measurements include the PD Severity Scale, Beck Depression Inventory-II (BDI-II) and Insomnia Severity Index (ISI). Statistical analyses were performed to assess any potential relationship between PD, insomnia and depression. Results: Of the PD patients, 88 (80%) and 89 (80.9%) had comorbid depression (BDI-II ≥ 14) and insomnia (Korean version of the ISI ≥ 8), respectively. In a mediation model using insomnia as the mediating variable, the total effect of panic symptom severity on depression was significant (t = 7.23, P < 0.001). There were significant effects of panic symptoms on insomnia (t = 4.62, P < 0.001) and of insomnia on depression (t = 6.69, P < 0.001). The main effect of panic symptom severity on depression was also significant, after controlling for the effect of insomnia (t = 5.10, P < 0.001), suggesting partial mediation. Conclusion: Both depressive symptoms and insomnia are common in patients with PD and depression was partially mediated by insomnia in these patients. These results suggest that an intervention for insomnia in patients with PD might help prevent the development of depression.

      • Deformable DETR 을 활용한 서적 손상 탐지 기법 구현

        본 논문은 서적의 앞/뒤표지와 옆면을 촬영한 이미지를 바탕으로 서적의 손상 정도를 인식하기 위한 프레임워크를 제안한다. 도서 표면의 손상 탐지를 위하여 Deformable DETR 모델을 학습하고 다양한 실험을 통해 성능 평가를 진행하였다. 실험 결과, Mosaic 및 MixUp 증강 기법을 적용한 모델이 mAP 0.48 로 가장 높은 성능을 보여 제안 시스템의 활용 가능성을 확인할 수 있었다.

        한의과대학 재학생의 한의사 역량에 대한 교육요구도 및 우선순위 분석

        목적 본 연구는 한의사 핵심 역량에 대한 예비 한의사의 교육요구도와 우선순위를 분석하고 미래 한의사에게 필요한 역량을 도출하는 것을 목적으로 한다. 방법 한의과 학생 44명을 대상으로 한의사 핵심 역량에 대한 중요도와 현 교육과정 실행도에 대한 설문을 실시한 후, Borich 요구도 분석과 The Locus Focus 모델 분석을 실시하여 한의사 역량에 대한 교육요구도와 우선순위를 분석하였다. 또한, 미래 사회에 한의사에게 필요한 역량에 대하여 설문을 실시하였다. 결과 교육 요구도를 확인한 결과, 모든 역량에서 중요도가 실행도보다 높게 나타났으며 특히, ‘보건의료 정책 개발 및 참여’, ‘재무 및 인력 관리’, ‘의약품 및 의료기기 관리’ 등에 대한 요구도가 높은 것을 확인할 수 있었다. 반면, ‘국제 협력 활동’ 및 ‘지역 사회 및 국내 공공 보건 활동’, ‘교육과 연구 활동’에 대한 요구도는 낮게 나타났다. 예비 한의사가 인식하는 미래 한의사에게 필요한 역량으로는 ‘전자 진료기록 활용 역량’, ‘원격진료 기술 이해 및 적용력’, ‘인공지능 및 빅데이터 분석 능력 전자 의료 기록 활용’ 등이 높은 순위로 나타났다. 결론 모든 역량에 대하여 중요도가 실행도보다 높게 나타나 현 교육과정에 대한 검토가 필요함을 확인하였다. 특히, 한의학과 관련된 법과 정책, 제도에 참여할 수 있는 방안과 경영자로서 한의사 역량을 함양하는 것이 필요하며 미래의 한의사에게 필요한 디지털 테크놀로지 활용 역량 또한 필요함을 확인하였다. 이러한 예비 한의사의 교육 요구도와 인식을 반영하여 한의과 교육과정을 재설계 할 필요가 있다. Objectives In 2015, the Institute of Korean Medicine Education & Evaluation undertook a study to develop a competency model for Doctors of Korean medicine, with the aim of transitioning the educational system of Korean medical colleges to a competency-based approach. Anticipating future societal changes, the research identifies educational needs for core competencies in Korean medicine, intending to enhance a competency-based curriculum for prospective doctors. Methods The study involved a survey of 44 students in Korean traditional medicine to investigate the importance and current implementation of core competencies. It utilized the Borich Needs Assessment and The Locus for Focus model to analyze the educational needs. Additionally, a survey was conducted to assess the competencies needed for traditional Korean medicine doctors in future. Results Results highlighted the highest educational needs in ‘Participating in Developing Policies for Public Health,’ ‘Finance & Human Resources Management,’ and ‘Manage Medications & Supplements.’ Conversely, lower needs were observed for ‘Participating in Public Health Events Locally & Nationally’ and ‘Learning & Research Activities.’ Essential competencies for future Korean medicine doctors included utilizing electronic medical records, understanding telemedicine technology, and proficiency in artificial intelligence and big data analysis. Conclusions The results showed a higher importance than implementation for all competencies, highlighting the need to review the current educational curriculum. Specifically, there is a need to cultivate competencies in traditional Korean medicine related to law, policy, and system involvement, and to enhance management skills as practitioners. The study also underscored the necessity of incorporating digital technology competencies for future Korean medicine doctors.The findings underscore the necessity for a curriculum redesign.

        포자모형을 적용한 교권보호 정책의 정책형성과정 분석 : 학생인권 정책과의 관계를 중심으로

        우리나라에서 ‘교권’에 관한 논의는 제한적으로 이루어져 왔다. 교권에 대한 현재의 논의는 교권의 가장 소극적인 차원, 교사의 인권을 보호하거나 교육활동 침해에 대한 법적인 방지 조치를 다루는 수준에 머물러 있으며, 교권을 바탕으로 우리 교육이 어떻게 이루어져야 하는지에 대한 실제적이고 적극적인 탐색이 되지 못하고 있다. 따라서 본 연구에서는 현재 교권과 관련하여 가장 활발하게 논의되는 ‘교권보호’라는 의제와 관련 정책이 설정되는 과정을 분석하며 그 이유를 밝혀보고자하였다. 특히 정책형성과정 모형 중 ‘포자모형’을 통해 교권보호 정책의 형성과정을 분석함으로써교권보호의 의제 설정 과정을 살피고 교권보호 정책이 학생인권 정책과 가지는 연관성과 이를 통한 변화의 과정을 분석하였다. 포자모형은 민주주의 사회에서 ‘상이하고 작은 힘을 가진 개인들’이 ‘하나의 영향력’을 행사하는과정을 설명하기에 적합한 모델이다. 따라서 포자모형을 통해 교권과 관련된 의제가 형성되어가는과정을 분석하는 것은 교권 관련 정책으로서 제시되는 교원지위법이나 교권보호조례와 같은 정책형성의 원인과 과정을 더 세밀하게 드러내어준다. 본 연구에서는 이와 같은 분석의 결과 현재 우리나라에서 시도되는 제한적인 교권에 대한 논의와 정책의 원인은 바로, 교권이 교육과 교사의 본질적이고 적극적 역할을 수반하는 개념임에도 불구하고 그에 대한 의제설정과정이 ‘학생인권조례’와같은 학생인권 정책에 대한 반동으로 이루어졌기 때문임을 드러내고자 하였다. Discussions on ‘teachers’ rights have been limited in our country. The current discussion of teacher’s rights remains at the most passive level, focused on the legal measure to protect teachers human rights or infringement of educational activities and lacks a practical and active search on how education should be developed based on teacher s rights. In this regard, this study highlights ‘protection of teachers’ rights and the relevant policy making process focusing on the reason of the current passive discussion regarding teacher s rights. In particular, it studies the agenda building process for the protection of teachers rights by analyzing the relevant policy making process applying the Spore Model and analyzes the connection between the student s human rights policies and the teacher s rights protection policies. The spore model is suitable for explaining the process how individuals exercise their influence in a democratic society. Therefore, analyzing the agenda building process regarding teachers rights through a spore model further reveals the causes and the process of the policy making such as the Teachers Status Act and the Teacher s Rights Protection Ordinance. In this light, this study addresses the reason for the current limited discussion on teacher s rights in the Korean society, suggesting that even though teacher s rights entails active and fundamental role of education and teachers, the agenda was built as a reaction to the student s human rights policies such as Students’ Human Rights Ordinance .
